AIM:To undertake a metasynthesis of qualitative studies to understand the health and social needs of people living with inflammatory bowel disease (IBD). METHODS:A systematic search strategy identified qualitative studies exploring the phenomenon of living with inflammatory bowel disease. Databases included MEDLINE, PsychInfo, EMBASE, CINAHL and the British Nursing Index via the OVID platform. Qualitative search filters were adapted from Hedges database (http://***/hslt/miner/ digital_library/tip_sheets/Cinahl_eb_***). Qualitative empirical studies exploring the health and social needs of people living with inflammatory bowel disease were selected. Study eligibility and data extraction were independently completed using the Critical Appraisal Skills Programme for qualitative studies. The studies were analysed and synthesised using metasynthesis methodology. The themes from the studies allowed for common translations into a new interpretation of the impact of living with inflammatory bowel disease. RESULTS:Of 1395 studies, six published studies and one unpublished thesis fulfilled the inclusion criteria. First iteration of synthesis identified 16 themes, 2nd iteration synthesised these into three main 2nd order constructs:"detained by the disease"; "living in a world of disease" and "wrestling with life". "Detained by the disease" is the fear of incontinence, the behaviour the patients display due to the fear, and the impact this has on the individual, such as social isolation and missing out on life events. All of these serve to "pull" the patient back from normal living. "Living in a world of disease" is the long term effects of living with a long term condition and the fear of these effects. "Wrestling with life" is the continued fight to thrive, the "push" to continue normal living. CONCLUSION:The metasynthesis provides a comprehensive representation of living with IBD. Coronavirus disease 2019(COVID-19)is a disease produced by severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)and it is currently causing a catastrophic pandemic affecting humans *** disease has been lethal for approximately 3.12 million people around the world since January ***,among the most affected countries,Mexico ranks third in deaths after the United States of America and *** the high number of deceased people might also be explained by social aspects and lifestyle customs in Mexico,there is a relationship between this high proportion of deaths and comorbidities such as high blood pressure(HBP),type 2 diabetes,obesity,and metabolic *** official epidemiological figures reported by the Mexican government have indicated that 18.4%of the population suffers from HBP,close to 10.3%of adults suffer from type 2 diabetes,and approximately 36.1%of the population suffers from *** in the gut microbiota(GM)have been associated with these diseases and with COVID-19 severity,presumably due to inflammatory *** data about the association between GM dysbiosis and metabolic diseases could suggest that the high levels of susceptibility to SARSCoV-2 infection and COVID-19 morbidity in the Mexican population are primarily due to the prevalence of type 2 diabetes,obesity,and metabolic syndrome.

Surgical chylothorax in neonates:management and outcomes

Background Postoperative chylothorax occurs due to trauma to lymphatic vessels and can occur after any thoracic *** study reviewed recent literature to evaluate the management and outcomes of surgical chylothorax in *** PubMed database was searched for articles in English,Portuguese and Spanish from 2000 to *** were collected for surgery,chylothorax management,complications,mortality and length of hospital stay(LOS).Results Twenty studies offered 107 neonates:congenital diaphragmatic hernia(CDH)(n=76,71%),cardiac malforma-tions(n=25,23.4%),esophageal atresia(n=5,4.7%)and CDH+extralobar sequestration(n=1,0.9%).Medium-chain tri-glycerides(MCT)was the initial treatment in 52 neonates(48.6%),prednisolone+MCT in one(0.9%),total parenteral nutri-tion in 51 patients(47.7%),and three patients(2.8%)did not require any *** and somatostatin were used as second or third line treatment in 25 neonates(23.4%),and 15 neonates(14%)underwent 17 surgeries,including thoracic duct ligation(TDL)(n=9);pleurodesis(n=3)(2 patients required TDL);TDL+pleurodesis(n=2),and TDL+placement of hemostat(n=1).Complications due to the chylothorax were reported in 27 neonates(25.2%):hypoalbuminemia+hypona-tremia(n=18),hypoalbuminemia(n=4),hypoalbuminemia with cutaneous flushing as colateral effect of somatostatin(n=1),loose stool after somatostatin use(n=1),pneumonia(n=1),congestive heart failure+hypernatremia(n=1),and hyponatremia(n=1).There were 21 deaths(19.6%)and median LOS was 53.4 days(30-93.1 days).Conclusions Conservative management is appropriate as initial treatment for neonatal postsurgical *** and somatostatin are safe in neonates and surgical approach should be considered in prolonged leaks.

Behavioural lateralization,which reflects the functional specializations of the two brain hemispheres,is assumed to play an important role in cooperative intraspecific ***,there are few studies focused on the lateralization in cooperative behaviours of individuals,especially in a natural *** the present study,we investigated Iateralized spatial interactions between the partners in life-long monogamous *** male-female pairs of two geese species(barnacle,Branta leucopsis,and white-fronted,Anser albifrons geese),were observed during different stages of the annual cycle in a variety of *** geese flocks,we recorded which visual hemifield(left/right)the following partner used to monitor the leading partner relevant to the type of behaviour and the disturbance *** a significant majority of pairs,the following bird viewed the leading partner with the left eye during routine behaviours such as resting and feeding in undisturbed *** behavioural lateralization,implicating the right hemisphere processing,was consistent across the different aggregation sites and years of the *** contrast,no significant bias was found in a variety of geese behaviours associated with enhanced disturbance(when alert on water,flying or fleeing away when disturbed,feeding during the hunting period,in urban area feeding and during moulting).We hypothesize that the increased demands for right hemisphere processing to deal with stressful and emergency situations may interfere with the manifestation of lateralization in social interactions.

The corrosion behaviour of mill-scaled X65 pipeline steel with and without a primer layer was studied in a simulated near-neutral pH soil *** revealed a three-stage corrosion process of the mill-scaled pipeline steel *** first stage included an initial preferential dissolution of goethite(γ-FeOOH)and lepidocrocite(?-FeOOH)in mill *** second stage was marked by enhanced localized corrosion and pit-formation because of either galvanic corrosion or acidic dissolution in areas enclosed by mill *** final stage was general corrosion after the mill scale flaked off the steel *** the primer layer was applied,localized corrosion was significantly enhanced on the steel surface and persisted for an extended period as compared to the mill-scaled *** precipitation of siderite(FeCO3)was observed at flawed locations of mill scale,although the bulk chemistry is not favorable for its formation on the steel surface free of mill *** local precipitation of siderite formed a capped mill scale enclosure where localized corrosion can be further enhanced.

The increasing prevalence of obesity is alarming because it is a risk factor for cardiovascular and metabolic diseases(such as type 2 diabetes). The occurrence of these comorbidities in obese patients can arise from white adipose tissue(WAT) dysfunctions, which affect metabolism, insulin sensitivity and promote local and systemic inflammation. In mammals, WAT depots at different anatomical locations(subcutaneous, preperitoneal and visceral) are highly heterogeneous in their morpho-phenotypic profiles and contribute differently to homeostasis and obesity development, depending on their ability to trigger and modulate WAT inflammation. This heterogeneity is likely due to the differential behavior of cells from each depot. Numerous studies suggest that adiposederived stem/stromal cells(ASC; referred to as adipose progenitor cells, in vivo)with depot-specific gene expression profiles and adipogenic and immunomodulatory potentials are keys for the establishment of the morphofunctional heterogeneity between WAT depots, as well as for the development of depot-specific responses to metabolic challenges. In this review, we discuss depot-specific ASC properties and how they can contribute to the pathophysiology of obesity and metabolic disorders, to provide guidance for researchers and clinicians in the development of ASC-based therapeutic approaches.
