目的：探讨应用负压封闭引流技术治疗难愈性创面的临床效果。方法：40例难愈性创面患者,男23例,女17例,平均年龄41.13岁(17岁-78岁),致伤原因：交通事故、高处坠落伤、机械性损伤、褥疮、糖尿病足。随机分为2组。A组为治疗组,n=20,治疗中应用负压封闭引流技术;B组为对照组,n=20,治疗中应用传统的换药疗法。观察2组创面愈合时间,评价负压封闭引流技术疗难愈性创面的临床效果。结果：A组创面平均愈合时间是23.3±3.2天,B组创面平均愈合时间是32.3±5.1天,：A组平均愈合时间比B组明显加快,P<0.05。同时观察到A组创面能促进肉芽组织生长,加速坏死组织清除,提高植皮的成活率。结论：负压封闭引流术有助于改善创面微循环,引流坏死液化的组织和渗出液、有效减少各种有害代谢产物进入血循环,同时促进肉芽组织生长,促进创面愈合,缩短住院时间,降低患者住院费用,提高救治率,为临床提供了一种创面封闭或修复的简单、经济、安全有效的治疗方法,值得在临床上推广应用。

Performance measurement(PM)generates useful data for process control,facilitates communication between different sectors,and helps to align efforts on the most important aspects of the ***,PM plays a key role in the management of projects and *** is also important in the implementation of lean production principles and methods,such as reducing the share of nonvalue-adding activities,increasing process transparency,building continuous improvement into the process,and ***,the adoption of the lean production philosophy requires changes in *** its importance,limited studies have been conducted on the use of PM systems for assessing the impact of lean production programs in construction *** addition,studies on how lean companies(or projects)use performance measurement and to what extent the indicators adopted reflect the result of actions that have been undertaken are *** study proposes a set of requirements in PM systems of construction projects from the perspective of lean production and a taxonomy of performance metrics for lean production *** empirical studies have been carried out on construction companies from South America involved in the implementation of lean production *** scope of this investigation is limited to the construction projects as production systems rather than PM at the level of construction organizations.

Candida albicans (C. albicans) and Aspergillus fumigatus (A. fumigatus) are the two main pathogens in the clinical setting to cause serious, sometimes, lethal fungal infections. Immunocompromised patients fall victims to these fungi, with a mortality rate rising drastically over the past decades. This is in correlation with the fact that conventional antifungals are no longer capable of completely eradicating the disease, or if so, high doses are usually required to do so, leading to eventual resistance to those drugs and severe side effects. High drug resistance is in association with the discovery that these opportunistic pathogens have the ability to develop a multicellular complex, known as biofilm. Biofilms prevent drugs from reaching the fungal cells by sequestering them in their extracellular matrix. Other factors such as extracellular DNA, persister cells or heat shock protein 90 (Hsp90) also play a role in biofilm and contribute to drug recalcitrance. With the discovery of new antifungals lagging behind, scientists focused on other more profitable ways to counteract this phenomenon. Combination of two or more antifungals was found effective but came with serious drawbacks. Natural plant extracts, such as traditional Chinese medicine have also been demonstrated in vitro to possess antimicrobial actions. Great interest was directed towards their use with conventional antifungal agents with a possibility of lowering the necessary concentration required to inhibit the growth of fungi. This review aims in understanding the different factors contributing to clinical drug resistance and evaluating the effect of combination therapy and natural products on those cases difficult to treat.

Healthcare Staff Perceptions of Patient Safety Culture in Nursing Home Settings—A Cross-Sectional Study

In nursing homes, knowledge about patient safety culture is still limited. This study investigates staff perceptions of patient safety culture in Norwegian nursing homes, measured with the Nursing Home Survey on Patient Safety Culture (NHSOPSC). 466 (69%) staff from 12 different nursing homes participated. The total percentages of positive responses for each patient safety culture dimension and differences in perceptions according to staff’s educational background and position were calculated. Multiple linear regression analysis was used to test if the NHSOPSC dimensions predicted participants’ ratings of the question “Please give this nursing home an overall rating on patient safety”. The proportion of positive responses was high, with six of ten dimensions having an average percentage above 70%. “Supervisor expectations and actions promoting patient safety” (88%), “feedback and communication about incidents” (87%), and a “non-punitive response to mistakes” (78%) had high average scores, while “staffing” (46%) and “training and skills” (56%) had the lowest average scores. Managers reported higher scores on all dimensions, except for “compliance with procedures” compared with other staff groups. Educational level had less influence on staff’s perceptions of patient safety culture than management position. The ten NHSOPSC dimensions explained 47.2% of the variance for the overall rating question “Please give this nursing home an overall rating on patient safety” (F [10, 384] = 34.39, p < 0.001). “Management and organizational learning” had the strongest unique contribution (28.1%). This study suggests that staff working at the bedside have confidence in their nursing managers’ attention to patient safety issues and that a non-punitive environment is prevalent in Norwegian nursing homes.

电子商务通常涵盖范围很广的商业和贸易活动。在基于浏览器/服务器应用方法的开放互联网环境中,买卖双方从事各种各样的活动以实现网上购物、网上交易、网上结算以及其它各种各样的商业活动都属于电子商务。在电子商务领域,中国近年全面贯彻落实供给侧结构性改革,聚焦扶贫攻坚和“一带一路”建设,在稳就业、稳外贸、稳投资、稳预期等方面发挥了积极作用,加快形成高质量发展新局面。在全球信息革命与工业革命的双重时代背景下,从推动中哈产能合作,到借助跨境电商实现本国经济的转型突破,哈萨克斯坦正在着力推动“光明之路”的新经济政策,这将为哈萨克斯坦的经济发展呈现出欣欣向荣的合作前景。电子商务是一个非常广泛的概念,它也随着技术创新而不断发展。本文通过比较分析中国和哈萨克斯坦电子商务发展的过程、成就、特色、存在的问题及采取的一系列措施,找出哈萨克斯坦电子商务发展较慢的原因,并借鉴中国电子商务发展的经验,提出加快哈萨克斯坦电子商务发展的对策。本文运用比较分析法,文献研究法与数据分析等研究方法,针对哈萨克斯坦与中国的电子商务现状、运营管理特点、存在的问题进行对比研究,寻找对哈萨克斯坦最适用的措施促进哈萨克斯坦电子商务加快发展。通过网上调查法、对比分析与PEST分析相结合法对中国与哈萨克斯坦电子商务进行了分析。最后对分析所存在的问题提出了相应的对策建议。

Surface runoff and soil erosion in a natural regeneration area of the Brazilian Cerrado

The Brazilian Cerrado has been converted to farmland,and there is little evidence that this expansion will decrease,mainly because agriculture is the country's main economic ***,the impacts of intense modification of land use and land cover on surface runoff and soil erosion are still poorly un-derstood in this ***,we assessed surface runoff and soil loss in a woodland Cerrado area under a former pasture area,which was abandoned and has undergone a natural regeneration process for 7 years(RC).Its results were compared with that found in an undisturbed area of woodland Cerrado(CE),40-month-old eucalyptus(3.0×1.8 m)(EU),and pasture under rotational grazing(PA).The study was conducted on Red Acrisol located in the Brazilian *** performed rainfall simulations on a plot of 0.7 m2 and using three constant rainfall intensities of 60,90,and 120 mm h-1 for 1 *** each rainfall intensity,we carried out four repetitions using different plots in each treatment,i.e.12 plots per treat-ment studied and 48 plots in *** noted that the soil physical properties were improved in RC and,consequently,water infiltration and soil erosion control;RC presented surface runoff and soil loss different from EU and PA(a=0.05).The macroporosity and soil bulk density affected surface runoff in RC and PA because the RC was used as pasture and is currently regenerating back to the cerrado *** the rainfall intensity increased,EU became more similar to PA,which showed the highest surface runoff and soil *** findings indicate that natural regeneration processes(pasture to the cerrado vegetation)tend to improve the soil ecosystem services,improving infiltration and reducing surface runoff and soil erosion.

Compounds with dual action on cyclic adenosine monophosphate （cAMP） and cyclic guanosine monophosphate （cGMP） may be a treatment option for erectile dysfunction, as they not only promote penile erection but also prevent the upregulation of phosphodiesterase-5. In this study, we examined the possible relaxant effect and mechanism of 17-nor-subincanadine E （SEC, 0.2-200 pmol I^-1）, a plant-derived alkaloid, in rabbit corpus cavernosum （RbCC） strips that had been precontracted by exposure to phenylephrine （10 pmol I^- 1） or a high concentration of K＋（60 mmol I^- 1） in vitro. In addition to SEC＇s effect on cAMP and cGMP levels, electrical field stimulation （EFS） in phenylephrine-precontracted RbCC and calcium chloride （1-100 mmol I^-1） evoked responses in depolarized RbCC were analysed. SEC relaxed the phenylephrine-precontracted RbCCs in a concentration-dependent manner. Atropine, guanethidine and N-co-nitro-L-arginine methyl ester （L-NAME） did not have any effect on the relaxation of RBCCs. When 1H-1,2,4oxadiazole[4,3-a] quinoxalin-1-one （ODQ） was added, it effectively blocked the relaxant response of SEC. Although SEC enhanced the maximal relaxation produced by sodium nitroprusside （SNP） and forskolin in phenylephrine-precontracted cavernosal smooth muscle, it caused a decrease in the maximal contractile response induced by calcium chloride in depolarized RbCCs. The relaxant effect of SEC was paralleled by an increase in the tissue levels of the Cyclic nucleotides cAMP and cGMP. We conclude that SEC promotes the relaxation of RbCC, possibly favouring cAMP and cGMP accumulation and calcium blockade. This novel mechanism could be useful for patients who do not benefit from phosphodiesterase inhibitors and for those with endothelial and nitrergic dysfunction, such as patients with diabetes, hypertension and dyslipidaemias.

AIM:To evaluate the effect of autoclaved diet on the jejunum neurons of the myenteric plexus of rats during their ***:The experimental groups were made up of rats going through weaning whose mothers received either an autoclaved or a non-autoclaved diet during gestation and lactation,and rats that were fed the same diet as their mothers during the post-weaning *** order to measure the neurons'body profile and to quantify the number of neurons per area,preparations were stained by the nicotinamide adenine dinucleotide-diaphorase ***:No significant changes were observed in rats'body weight or in the number of neurons regardless of the diet used(P>0.05) .There was a decrease in the jejunum-ileum length in rats treated with an autoclaved diet(P<0.05) .An increase in the neuronal cross-sectional area was seen in rats that had received the autoclaved diet,an effect that was significant for animals undergoing *** addition,all observed factors showed significant differences when related to the age of the ***:The autoclaved diet did not alter the quantity of neurons,but increased their cell body area,suggesting changes similar to those observed in protein deficiency.

We are entering an exciting epoch in livestock biotechnology during which the fundamental approaches(such as transgenesis, spermatozoa cryopreservation and artificial insemination) will be enhanced based on the modern understanding of the biology of spermatogonial stem cells(SSCs) combined with the outstanding recent advances in genomic editing technologies and in vitro cell culture systems. The general aim of this review is to outline comprehensively the promising applications of SSC manipulation that could in the nearest future find practical application in livestock breeding. Here, we will focus on 1) the basics of mammalian SSC biology;2) the approaches for SSC isolation and purification;3) the available in vitro systems for the stable expansion of isolated SSCs;4) a discussion of how the manipulation of SSCs can accelerate livestock transgenesis;5) a thorough overview of the techniques of SSC transplantation in livestock species(including the preparation of recipients for SSC transplantation,the ultrasonographic-guided SSC transplantation technique in large farm animals, and the perspectives to improve further the SSC transplantation efficiency), and finally, 6) why SSC transplantation is valuable to extend the techniques of spermatozoa cryopreservation and/or artificial insemination. For situations where no reliable data have yet been obtained for a particular livestock species, we will rely on the data obtained from studies conducted in rodents because the knowledge gained from rodent research is translatable to livestock species to a great extent. On the other hand, we will draw special attention to situations where such translation is not possible.
