Palliative care service in patients with childhood cancer from a tertiary pediatric oncology center

Importance: Pediatric palliative care (PPC) has gained great attention in western countries, however data on hong Kong children receiving PPC are limited. There are gaps in knowledge about the PPC needs in local children with cancer. Objective: To assess the current situation of PPC service of hong Kong children with cancer. Methods: We performed a 10-year retrospective review in a tertiary pediatric oncology unit and studied the referral pattern, clinical characteristics and services provided. results: Totally 117 children were referred to PPC Team which constituted 65% deceased children within the study period. The commonest diagnoses were central nervous system tumour (32.5%), leukaemia (25.6%) and neuroblastoma (9.4%). Ninety-one percent of children were referred after the last relapse or stopping curative treatment. The median time of referral to death was 77 days (interquartile range (IQR): 35, 182)The median number of hospital admission after referral was 2 (IQR: 1, 5), with a median of 27 days total hospital stay (IQR: 10, 60). The reasons for admission were palliative chemotherapy (16.1%), pain control (12.1%) and platelet transfusion (11.2%). For the death episode, the median duration of hospital stay was 15 days (IQR: 2, 46). Most patients received oxygen (82.0%), intravenous fluid (81.2%) and intravenous analgesic (52.1%). A total of 111 (94.9%) children died in hospital, 15 had been admitted into intensive care unit (ICU) but only 5 (4.5%) died in ICU. Interpretation: Pain control and platelet transfusion were common reasons of readmission. Death in ICU and at home was uncommon in hong Kong situation.

To the Editor : Barcelona Clinic Liver Cancer(BCLC) Stage C hepatocellular carcinoma(HCC) with vascular invasion has been considered unresectable. however, the curative rate from systemic treatment is rather dismal. Even with the use of immunotherapy, complete remission is less than 10% [ 1 ]. On the contrary, recent advances in transplant oncology, especially in the application of liver transplant-related surgical techniques in complex liver resections, make the operation possible for patients with locally advanced HCC who were previously considered to be unresectable [ 2 ]. We hereby present the surgical management of a patient with giant HCC in right liver lobe with inferior vena cava(IVC) invasion.

The continued evolution of bacterial and fungal species poses a significant difficulty for the treatment of disease of microbial origin. Given this situation, the prevention of transmission of such microbial diseases becomes of increasing importance. Efforts of this laboratory have been directed toward the destruction of microbial species on environmental surfaces as a prophylaxis toward infection, and we herein report on the efficacy of a system that demonstrates activity against both Gram-positive and Gram-negative bacteria, as well as fungi. We report specifically herein on the use of fabric materials so activated for the destruction of these microbial species, useful for a variety of surfaces within hospital and related settings wherein transmission of microbial disease is a major problem, while these approaches are also applicable for a variety of other types of surfaces.

Background:Extensive loss of donor neural stem cell(NSCs)due to ischemic stress and low rate of differentiation at the site of cell graft are two of the major issues that hamper optimal outcome in NSCs transplantation *** that histone deacetylases(HDACs)modulate various cellular processes by deacetylating histones and non-histone proteins,we hypothesized that combined treatment with small molecules,sodium butyrate(NaB;a known HDAC inhibitor)and nicorandil,will enhance the rate neuronal differentiation of NSCs besides their preconditioning to resist oxidative ***:NSCs derived from 14-day old Sprague Dawley rat ganglion eminence were characterized for tri-lineage *** with 1 mM NaB significantly changed their culture characteristics while continuous treatment for 10 days enhanced their neural *** treatment also preconditioned the cells for their resistance to oxidative ***:The highest rate of neural differentiation and preconditioning effect was achieved when the NSCs were treated concomitantly with NaB and *** proliferation assay showed that concomitant treatment with NaB and nicorandil retarded their rate of ***:These data conclude that preconditioning of NSCs with NaB and nicorandil effectively enhances their differentiation capacity besides preconditioning the cells to support their survival under ischemic conditions.

Achieving high strength,deformability and toughness in polymers is important for practical industrial *** has remained challenging because of the mutually opposing effects of improvements to each of these ***,a self-assembling nacre-like polymer composite is designed to achieve ex-tremely tough with increasing *** special design significantly improved polymer’s mechanical properties,including an ultra-high fracture strain of 1180%,a tensile strength of 55.4 MPa and a toughness of 506.9 MJ/m^(3),which far exceed the highest values previously reported for polymer *** ex-cellent combination of properties can be attributed to a novel toughening mechanism,achieved by the synergy of the domain-limiting effect of metallic glass fragments with the strain-gradient-induced orien-tation and crystallisation within the polymer during *** approach opens a promising avenue for designing robust polymer materials in armour and aerospace engineering for a range of innovative applications.

解纤维热酸菌SK1.009发酵产生合成塔格糖所需的L-阿拉伯糖异构酶,通过单因素与正交实验优化确定产酶培养基条件。结果表明,发酵最佳条件(g/L):可溶性淀粉8、蛋白胨鱼粉1、L-阿拉伯糖1.5、NH4Cl1、kh2PO41、Na2HPO4·12H2O0.135、MgSO4.7H2O0.2、CaCl20.15,发酵初始pH4.5,培养温度55℃,接种量4%,培养时间60h。在该最佳培养基下发酵产生酶活为0.184U/mL。

1950年,意大利藏学家图齐(***)发表了他在尼泊尔西部考古发现的梵文题记。他证明尼泊尔西部的一个古老王国正是西藏史书记载的雅济国(Ya-tshe),两者的国王系谱名字完全一样。自此,印度、尼泊尔学者跟藏学家展开长久争论:雅济国的王族是藏人还是印度雅利安裔?王国如何形成的?数十年来这些问题在学界仍未找到共识。中国学界一直没有参与讨论。本文列出仅见的三条雅济国的中文史料,希望协助研究雅济国。本文扩展视野,探究雅济国跟西藏萨迦政权与元朝帝国的关系,也探究雅济国跟印度德里锁鲁檀王朝的关系。本文推断,雅济国跟萨迦与元朝维持友好关系,以维持自身在喜马拉雅山的霸权,同时用以对抗印度大陆的伊斯兰帝国德里锁鲁檀王朝。

考证征伐女真、高丽的札刺亦儿台与也速迭儿——兼论《蒙古秘史》1252年成书之说

《蒙古秘史》记载窝阔台可汗委派札剌亦儿台与也速迭儿出征女真与高丽.学界一向认定这条文是后来蒙哥可汗年代侵略高丽的史事(1253-1259),是被错误移进窝阔台年代(1229-1241).本文论证以上条文的确是窝阔台年代的史事.波斯文《史集》记载札剌亦儿台是成吉思汗的一百廿九千户之一,属于左翼.汉文《元史·货食志三·岁赐》记载也速迭儿是左翼千户之一.二人确是窝阔台年代的将领.窝阔台时代的战役主要是攻伐女真/满洲地区,高丽是次要的.二人应该是镇守女真地区的探马赤军主将,从未到过高丽战场.因此二人不见于窝阔台年代高丽国的文史记录.十多年后女真地区统治巩固,二人就被蒙哥可汗委派攻伐高丽.两件史事完全不同.《蒙古秘史》的作者为了歌颂窝阔台派出探马赤军平定四方,就举例写了札剌亦儿台与也速迭儿出镇女真、高丽.《秘史》这段的重点是歌颂窝阔台,因为并举了绰儿马罕出镇伊朗、长子西征平定东欧,以及出镇女真、高丽,以探马赤军为第274节的主轴,将各个件时间不同的史事贯串在同一章节.学者不解《秘史》作者的用心,不了其书写习惯,就简单判断说《秘史》的史事错乱,草率否定《秘史》的史实.《蒙古秘史》记载窝阔台可汗征伐女真的内容没有错误.《秘史》的内容没有超过1252年.另外,程矩夫《雪楼集》收录的常家神道碑文证明了1252年夏天的确举行过宗亲大聚会,正是《蒙古秘史》第282节跋文记录的时间鼠儿年.本文认同《蒙古秘史》成书于1252年.
