Idiopathic hirsutism:Is it really idiopathic or is it misnomer?

Hirsutism,which is characterized by excessive growth of terminal hair in a male pattern,may result from various causes including polycystic ovary syndrome(PCOS),non-classic congenital adrenal hyperplasia,adrenal or ovarian tumors or it may be *** hirsutism is currently defined as hirsutism associated with normal ovulatory function,normal serum androgen levels and normal ovarian morphology,however,the pathogenesis of idiopathic hirsutism is not *** androgens are the main hormones to stimulate growth of body hair,therefore,there should be any form of increased androgen effect irrespective of normal serum androgen levels in any patient with *** accordance to this scientific truth,we have previously shown that,although within normal limits,patients with idiopathic hirsutism have relatively higher serum androgen levels(relative hyperandrogenemia)in comparison to healthy subjects which let as to think that is idiopathic hirsutism really idiopathic?In addition to relative hyperandrogenemia,we have previously shown that,in comparison to healthy subjects,women with idiopathic hirsutism demonstrated higher expression of steroid sulphatase and 17-beta hydroxysteroid dehydrogenase mRNA both in the subumbilical region and arm skin,which contributes to local androgen *** results support the idea that,in some patients,although the adrenals or ovaries do not secrete increased amount of androgens leading to hyperandrogenemia,pilocebaceous unit locally produce increased amount of androgens leading to hirsutism without ovulatory *** the demonstration of relative hyperandrogenemia and possible increase in local androgen synthesis in patients with idiopathic hirsutism,we think that idiopathic hirsutism is not idiopathic and it may be named as“normoandrogenic hirsutism”.Furthermore,it may not be a different entity but may be an early stage of hyperandrogenic disorders such as ***,this can be find out by following-up p

Predictors of poor outcome in gastrointestinal bleeding in emergency department

AIM: To determine the prognostic risk factors of gastrointestinal bleeding in emergency department ***: The trial was a retrospective single-center study involving 600 patients over 18-years-old and carried out with approval by the Institutional Ethics Committee. Patient data included demographic characteristics, symptoms at admission, past medical history, vital signs, laboratory results, endoscopy and colonoscopy results, length of hospital stay, need of intensive care unit(ICU) admission, and mortality. Mortality rate was the principal endpoint of the study, while duration of hospital stay, required interventional treatment, and admission to the ICU were secondary ***: The mean age of patients was 61.92-yearsold. Among the 600 total patients, 363(60.5%) underwent upper gastrointestinal endoscopy and the most frequent diagnoses were duodenal ulcer(19.2%) and gastric ulcer(12.8%). One-hundred-and-fifteen(19.2%) patients required endoscopic treatment, 20(3.3%) required surgical treatment, and 5(0.8%) required angiographic embolization. The mean length of hospital stay was 5.21 ± 5.85 d. The mortality rate was 6.3%. The ICU admission rate was 5.3%. Patients with syncope, higher blood glucose levels, and coronary artery disease had significantly higher ICU admission rates(P = 0.029, P = 0.043, and P = 0.002, respectively). Patients with low thrombocyte levels, high creatinine, high international normalized ratio, and high serum transaminase levels had significantly longer hospital stay(P = 0.02, P = 0.001, P = 0.019, and P = 0.005, respectively). Patients who died had significantly higher serum blood urea nitrogen and creatinine levels(P = 0.016 and P = 0.038), and significantly lower mean blood pressure and oxygen saturation(P = 0.004 and P = 0.049). Dear Editor, We want to present the first reported Propionobacterium acnes(P. acnes) endophthalmitis case after sutureless fixation of posterior chamber intraocular lens(IOL) to the sclera for the traetment of aphakia to the best of our knowledge. A 69-year-old man with aphakia due to complicated cataract surgery underwent sutureless intrascleral fixation of threepiece posterior chamber intraocular lens(PCIOL) with Yamane technique.

Apoptosis in the iris and trabecular meshwork of medically treated and untreated primary open angle glaucoma patients

Changes in retinal vessel diameters in migraine patients during attack-free period

AIM：To evaluate the retinal vessel diameters in patients with migraine by optical coherence tomography（OCT）.METHODS：In this cross-sectional study,124 eyes of 62 patients with a diagnosis of unilateral migraine during attack-free period and 42 age-and sex-matched control subjects were included. Migraine patients were divided into the ≤2 migraine attacks per month group and the ≥5 migraine attacks per month group. All subjects underwent complete ophthalmological and neurological examinations before measurements. Retinal vessel diameters and choroidal thickness were examined with the Spectralis ***：The mean diameters of the arteries in the eyes on the headache side of control group,≥5 migraine attacks per month and ≤2 migraine attacks per month group at 480 μm from the optic disk（Raster 3）were 119.54±46.69,136.68±25.93 and 119.34±31.75 μm respectively with a steady decline to 105.57±32.15,118.18±31.87 and 108.05±38.77 μm at 1440 μm（Raster 7）,the last measurement point,respectively. The retinal artery diameter measurements were significantly increased in ≥5 migraine attacks per month patients at four out of five measured points compared to control group（P〈0.05）. There were no statistical differences at any of the points of vein measurements. The choroidal thickness measurements were significantly decreased in ≥5 migraine attacks per month patients at all measured points compared to control group（P〈0.05）.CONCLUSION：The retinal artery diameter is found to increase significantly and the choroidal thickness is found to decrease in the eyes on the headache side in ≥5 migraine attacks per month patients compared to control group.

Fourier analysis of corneal Scheimpflug imaging:clinical use in keratoconus

AIM:To evaluate the clinical use of Fourier analysis of videokeratography data in the diagnosis and follow-up of keratoconus(KC).METHODS:We conducted a chart review of consecutive patients presented to our cornea clinic.A team of two experienced cornea specialists divided the patients into three groups:normal cornea,forme fruste KC(FFKC),and clinical *** exclusion criteria were a history of previous ocular surgery,any accompanying corneal pathology other than KC,high myopia(>6.00 diopters),amblyopia,pregnancy,breastfeeding,or any current autoimmune *** data of Fourier series harmonic analysis were evaluated for their diagnostic capacity using the receiver operating characteristic(ROC)curve.A binary logistic regression analysis was also conducted to construct a diagnostic model.A total of 259 eyes showed progression in the clinical KC group and underwent a combination of accelerated corneal collagen cross-linking and topographyguided customized treatment with an excimer ***:The study included 1262 eyes(618 normal,530 KC,and 114 FFKC)of 1262 *** observed that maximum decentration(MaxDec)was almost as good as maximum keratometry(Kmax)in detecting progressive *** area under the curve(AUC)was 0.95 for KC[95%confidence interval(CI):0.93-0.96]and 0.84 for FFKC(95%CI:0.79-0.88).Higher predictive accuracy was obtained using a model combining the spherical component,MaxDec,irregularity,and regular astigmatism in the center of the cornea(AUC:0.97;sensitivity:89%,and specificity:96%).CONCLUSION:Decentration,Kmax,and posterior radii of curvatures from a 3.0-mm optical zone centered on the thinnest point of the cornea provide the highest accuracy with low reproducibility of Kmax.

All-optical image denoising using a diffractive visual processor

Image denoising,one of the essential inverse problems,targets to remove noise/artifacts from input *** general,digital image denoising algorithms,executed on computers,present latency due to several iterations implemented in,e.g.,graphics processing units(GPUs).While deep learning-enabled methods can operate non-iteratively,they also introduce latency and impose a significant computational burden,leading to increased power ***,we introduce an analog diffractive image denoiser to all-optically and non-iteratively clean various forms of noise and artifacts from input images–implemented at the speed of light propagation within a thin diffractive visual processor that axially spans<250×λ,whereλis the wavelength of *** all-optical image denoiser comprises passive transmissive layers optimized using deep learning to physically scatter the optical modes that represent various noise features,causing them to miss the output image Field-of-View(FoV)while retaining the object features of *** results show that these diffractive denoisers can efficiently remove salt and pepper noise and image rendering-related spatial artifacts from input phase or intensity images while achieving an output power efficiency of~30–40%.We experimentally demonstrated the effectiveness of this analog denoiser architecture using a 3D-printed diffractive visual processor operating at the terahertz *** to their speed,power-efficiency,and minimal computational overhead,all-optical diffractive denoisers can be transformative for various image display and projection systems,including,e.g.,holographic displays.

Emergency admissions due to swallowed foreign bodies in adults

AIM:To study a retrospective analysis of patients who presented to the emergency departments(ED)with complaints related to foreign body ***:Patients older than 16 years of age who presented to the ED between January 1stand December 31stof 2010 with complaints related to swallowed foreign bodies were identified from electronic health records and patient ***:A total of 100 patients presented with a complaint of foreign body ingestion during the study ***,an X-ray was performed on 75 patients,and a fiberoptic evaluation was performed on45 patients.A foreign body was detected in 46(46%)*** diagnostic yield of the X-ray was 27(36%)out of 75 patients,while the diagnostic yield of the fiberoptic evaluations was 21(47%)out of 45 *** detected foreign bodies were mostly located in the esophagus(17 out of 46 foreign bodies detected).When the types of ingested foreign bodies were evaluated,52(52%)patients reported ingesting food,and19(19%)patients reported swallowing *** X-ray was performed on 33 patients with accidental food ingestions but yielded a positive result in only two *** 12 out of 21 patients with accidental food ingestion who underwent fiberoptic evaluation,the foreign material was detected and ***:Plain radiography is helpful in the localization of radiopaque swollen foreign bodies,while fiberoptic methods are useful as both diagnostic and therapeutic tools,regardless of radiopacity.

Postoperative intestinal obstruction caused by Meckel’s diverticulitis after appendectomy

Surgical treatment of the complications associated with Meckel’s diverticulum is widely accepted, but surgical management of an asymptomatic diverticulum found incidentally during laparotomy for other reasons is controversial. Appendectomy is the most common reason for laparotomy in many pediatric surgery clinics. The most common causes of an intestinal obstruction in the early period after appendectomy are an intra-abdominal abscess, adhesive small bowel obstruction and invagination. Obstruction due to Meckel’s diverticulum is reported rarely. In this study, we present 2 cases, five and eleven years old, with an asymptomatic Meckel’s diverticulum where appendectomy was performed due to acute and perforated appendicitis and obstruction developed due to Meckel’s diverticulitis in the postoperative early stage. Meckel’s diverticulitis must be considered in intestinal obstruction cases developing in the early stages after appendectomy although quite rare.
