Non-compliant and compliant balloons for endovascular rescue therapy of cerebral vasospasm after spontaneous subarachnoid haemorrhage: experiences of a single- centre institution with radiological follow- up of the treated vessel segments

Background For endovascular rescue therapy(ERT)of cerebral vasospasm(CVS)due to spontaneous subarachnoid haemorrhage(sSAH),non-compliant(NCB)and compliant(CB)balloons are used with both balloon types bearing the risk of vessel injury due to specific mechanical properties.Although severe delayed arterial narrowing after transluminal balloon angioplasty(TBA)for CVS has sporadically been described,valid data concerning incidence and relevance are missing.Our aim was to analyse the radiological follow-up(RFU)of differently TBA-treated arteries(CB or NCB).Methods Twelve patients with utilisation of either NCB or CB for CVS were retrospectively analysed for clinical characteristics,ERT,functional outcome after 3 months and RFU.Compared with the initial angiogram,we classified delayed arterial narrowing as mild,moderate and severe(60% calibre reduction).Results Twenty-three arteries were treated with CB,seven with NCB.The median first RFU was 11 months after TBA with CB and 10 after NCB.RFU was performed with catheter angiography in 18 arteries(78%)treated with CB and in five(71%)after NCB;magnetic resonance angiography was acquired in five vessels(22%)treated with CB and in two(29%)after NCB.Mild arterial narrowing was detected in three arteries(13%)after CB and in one(14%)after NCB.Moderate or severe findings were neither detected after use of CB nor NCB.Conclusion We found no relevant delayed arterial narrowing after TBA for CVS after sSAH.Despite previous assumptions that CB provides for more dilatation in segments adjacent to CVS,we observed no disadvantages concerning long-term adverse effects.Our data support TBA as a low-risk treatment option.

Physical activity(PA)epidemiology first emerged as a new field of study around the 1950s with publication of seminal investigations on London civil servants1 and has continued to evolve as a scientific discipline to present time.This subdiscipline of epidemiology studies the frequencies,distributions,and dynamics of PA and its impact on human health,morbidity,and mortality.Research over the past 75 years has demonstrated numerous health benefits of PA.Based on these research findings,the World Health Organization recommends adults aged 18-64 years engage in at least 150-300 min of moderate-intensity aerobic PA per week,or at least 75-150 min of vigorous-intensity aerobic PA per week,or a combination of both intensities.

BACKGROUND Peripheral nerve injury can result in significant clinical complications that have uncertain prognoses.Currently,there is a lack of effective pharmacological interventions for nerve damage,despite the existence of several small compounds,Despite the objective of achieving full functional restoration by surgical intervention,the persistent challenge of inadequate functional recovery remains a significant concern in the context of peripheral nerve injuries.AIM To examine the impact of exosomes on the process of functional recovery following a complete radial nerve damage.METHODS A male individual,aged 24,who is right-hand dominant and an immigrant,arrived with an injury caused by a knife assault.The cut is located on the left arm,specifically below the elbow.The neurological examination and electrodiagnostic testing reveal evidence of left radial nerve damage.The sural autograft was utilized for repair,followed by the application of 1 mL of mesenchymal stem cell-derived exosome,comprising 5 billion microvesicles.This exosome was split into four equal volumes of 0.25 mL each and delivered microsurgically to both the proximal and distal stumps using the subepineural pathway.The patient was subjected to a period of 180 d during which they had neurological examination and electrodiagnostic testing.RESULTS The duration of the patient’s follow-up period was 180 d.An increasing Tinel’s sign and sensory-motor recovery were detected even at the 10th wk following nerve grafting.Upon the conclusion of the 6-mo post-treatment period,an evaluation was conducted to measure the extent of improvement in motor and sensory functions of the nerve.This assessment was based on the British Medical Research Council scale and the Mackinnon-Dellon scale.The results indicated that the level of improvement in motor function was classified as M5,denoting an excellent outcome.Additionally,the level of improvement in sensory function was classified as S3+,indicating a good outcome.It is noteworthy that these assessments were conducte

Oil blending is the method of choice used worldwide to improve oxidative stability and nutritional value.There is no such edible oil/fat that meets all the recommendations from the health point of view.The fatty acid composition of vegetable oils decides the fate of the oil.Pure single oil is unable to provide a balanced amount of fatty acids(FAs)required/recommended on a daily intake basis.Blending oils/fats is an appropriate procedure of physically mixing multiple oils in suitable proportions which may provide functional lipids with improved antioxidant potential and desirable physical and chemical properties.This review piled up the accessible data on the blending of diverse oils/fats in the combination of binary,ternary,quaternary,or other types of oils into a single blended oil.Blending can be found very convincing towards appropriate FA profile,enhancement in physicochemical characteristics,and augmented stability for the period of storage or when used as cooking/frying processes which could ultimately serve as an effectual dietary intervention towards the health protectiveness.

BACKGROUND Cerebral palsy(CP)describes a group of disorders affecting movement,balance,and posture.Disturbances in motor functions constitute the main body of CP symptoms.These symptoms surface in early childhood and patients are affected for the rest of their lives.Currently,treatment involves various pharmacotherapies for different types of CP,including antiepileptics for epilepsy and Botox A for focal spasticity.However,none of these methods can provide full symptom relief.This has prompted researchers to look for new treatment modalities,one of which is mesenchymal stem cell therapy(MSCT).Despite being a promising tool and offering a wide array of possibilities,mesenchymal stem cells(MSCs)still need to be investigated for their efficacy and safety.AIM To analyze the efficacy and safety of MSCT in CP patients.METHODS Our sample consists of four CP patients who cannot stand or walk without external support.All of these cases received allogeneic MSCT six times as 1×106/kg intrathecally,intravenously,and intramuscularly using umbilical cord-derived MSCs(UC-MSC).We monitored and assessed the patients pre-and post-treatment using the Wee Functional Independence Measure(WeeFIM),Gross Motor Function Classification System(GMFCS),and Manual Ability Classification Scale(MACS)instruments.We utilized the Modified Ashworth Scale(MAS)to measure spasticity.RESULTS We found significant improvements in MAS scores after the intervention on both sides.Two months:Rightχ^(2)=4000,P=0.046,leftχ^(2)=4000,P=0.046;four months:Rightχ^(2)=4000,P=0.046,leftχ^(2)=4000,P=0.046;12 months:Rightχ^(2)=4000,P=0.046,leftχ^(2)=4000,P=0.046.However,there was no significant difference in motor functions based on WeeFIM results(P>0.05).GMFCS and MACS scores differed significantly at 12 months after the intervention(P=0.046,P=0.046).Finally,there was no significant change in cognitive functions(P>0.05).CONCLUSION In light of our findings,we believe that UC-MSC therapy has a positive effect on spasticity,and it partially improves motor functions.

BACKGROUND Traumatic brain injury(TBI)is characterized by a disruption in the normal function of the brain due to an injury following a trauma,which can potentially cause severe physical,cognitive,and emotional impairment.Stem cell transplantation has evolved as a novel treatment modality in the management of TBI,as it has the potential to arrest the degeneration and promote regeneration of new cells in the brain.Wharton’s Jelly-derived mesenchymal stem cells(WJ-MSCs)have recently shown beneficial effects in the functional recovery of neurological deficits.AIM To evaluate the safety and efficiency of MSC therapy in TBI.METHODS We present 6 patients,4 male and 2 female aged between 21 and 27 years who suffered a TBI.These 6 patients underwent 6 doses of intrathecal,intramuscular(i.m.)and intravenous transplantation of WJ-MSCs at a target dose of 1×106/kg for each application route.Spasticity was assessed using the Modified Ashworth scale(MAS),motor function according to the Medical Research Council Muscle Strength Scale,quality of life was assessed by the Functional Independence Measure(FIM)scale and Karnofsky Performance Status scale.RESULTS Our patients showed only early,transient complications,such as subfebrile fever,mild headache,and muscle pain due to i.***.injection,which resolved within 24 h.During the one year follow-up,no other safety issues or adverse events were reported.These 6 patients showed improvements in their cognitive abilities,muscle spasticity,muscle strength,performance scores and fine motor skills when compared before and after the intervention.MAS values,which we used to assess spasticity,were observed to statistically significantly decrease for both left and right sides(P<0.001).The FIM scale includes both motor scores(P<0.05)and cognitive scores(P<0.001)and showed a significant increase in pretest posttest analyses.The difference observed in the participants’Karnofsky Performance Scale values pre and post the intervention was statistically significant(P<0.001).CONCLUSION This study showed

Evaluation of Interferon-Gamma Release Assay Testing and Tuberculin Skin Test for Early Diagnosis of Tuberculosis in Children and Adolescents

Background: This study aimed to evaluate the diagnostic value of interferon-γ release assay (IGRA), a sensitive microbiological diagnostic method, in children and adolescents with suspected tuberculosis in a country with a high burden of tuberculosis. Method: This study included 581 children and adolescents aged 4 - 19 years who were suspected of having tuberculosis, were latently infected with Mycobacterium tuberculosis, and had received at least one dose of BCG vaccine between April 17, 2019, and February 24, 2021. The study evaluated the TST results of 106 patients who had a positive Quantiferon test and were suspected of having tuberculosis. Results: The study included 581 patients aged between 4 and 19 years. Of these, 106 patients tested positive for the Quantiferon test, while 19 were indeterminate and 456 were negative. The Quantiferon test positivity rate was 18.24%. Among the 106 QFT-Plus-positive cases, 23 patients also tested positive for TST. The difference in distribution was found to be statistically significant. Conclusion: The QFT-Plus test is considered an alternative to TST and other microbiological diagnostic methods for early tuberculosis diagnosis, particularly in children and adolescents.

The relationship between gunshot-residue particle size and Boltzmann distribution

Gunshot-residue(GSR)particles usually form spherical structures,have special dimensions,and a composition that consists of burned and partially unburned matter with a 0.5–50-mm diameter(sometimes larger).The GSR particle-size distributions have been argued to be caused by the effects of equilibrium-surface distributions during formation and have not yet been correlated with a theoretical analysis or probability distribution.This study proposes a model to explain the GSR particle-size distribution quantitatively.Based on the data,and,as predicted by our model,the number of GSR particles decreases proportionally to the inverse square of the GSR particle size as the particle size increases.This result occurs because of the abundance of microstructures that are encountered in the GSR particles.

Tribological and Electrochemical Corrosion Properties of CNT-Incorporated Plasma Electrolytic Oxidation(PEO)Coatings on AZ80 Magnesium Alloy

Plasma electrolytic oxidation(PEO)coatings were carried out in silicate-based electrolyte embedded with various amounts of carbon nanotube(CNT)additives(0,0.5,1,2 and 4 g/L)on AZ80 magnesium alloy substrate.Microstructure,tribological and electrochemical corrosion properties of the coated specimens were investigated.The results demonstrated that the increasing CNT additions into the electrolyte resulted in a gradual increase in the thickness of PEO-coating layer.The CNT addition to the electrolyte by 0.5 g/L resulted in a slight decrease in the roughness of PEO-coating above which it continually increased.Wear resistance of the PEO-coated specimens showed a gradual improvement with increasing CNT-incorporation within the coating.The electrochemical corrosion tests revealed that the best corrosion resistance was found after the CNT addition into the electrolyte by 0.5 g/L due to the better roughness values,more homogenous coating layers and less pore formation.

Investigation of hearing aid users'speech understanding in noise and their spectral-temporal resolution skills

Purpose:Our study aims to compare speech understanding in noise and spectral-temporal resolution skills with regard to the degree of hearing loss,age,hearing aid use experience and gender of hearing aid users.Methods:Our study included sixty-eight hearing aid users aged between 40-70 years,with bilateral mild and moderate symmetrical sensorineural hearing loss.Random gap detection test,Turkish matrix test and spectral-temporally modulated ripple test were implemented on the participants with bilateral hearing aids.The test results acquired were compared statistically according to different variables and the correlations were examined.Results:No statistically significant differences were observed for speech-in-noise recognition,spectraltemporal resolution among older and younger adults in hearing aid users(p>0.05).There wasn’t found a statistically significant difference among test outcomes as regards different hearing loss degrees(p>0.05).Higher performances were obtained in terms of temporal resolution in male participants and participants with more hearing aid use experience(p<0.05).Significant correlations were obtained between the results of speech-in-noise recognition,temporal resolution and spectral resolution tests performed with hearing aids(p<0.05).Conclusion:Our study findings emphasized the importance of regular hearing aid use and it showed that some auditory skills can be improved with hearing aids.Observation of correlations among the speechin-noise recognition,temporal resolution and spectral resolution tests have revealed that these skills should be evaluated as a whole to maximize the patient’s communication abilities.
