Characteristics of Nursing Care for Terminally Ill Patients in Hospice/Palliative Care Unit

The purpose of this study was to clarify the characteristics of nursing care for patient with terminally ill in the hospice/palliative care units. Semi-structured interviews on “communication, care, spiritual pain care and prediction of worsening of symptoms” were conducted, incorporating items indicated as important principles of palliative care by Lugton et al. Sixteen nurses at five hospice/ palliative care facilities in urban areas of Japan were surveyed in 2013, and results were analyzed qualitatively. Following characteristics by hospice/palliative care nurses (HPN) were categorized as [HPN sharing meaningful time with the patient] and [HPN’s continual attempts to understand the world in which the patient lives] in the communication;as [HPN providing comfortable care so that patients can value their last moments] and [HPN’s efforts to attend to patients so they can die as they hope to] in the care;and as [HPN’s observation in a range that does not interfere with the patient’s comfort] and [HPN senses that something is different from before] in the prediction of worsening of symptoms. Common characteristic was . In this study, spiritual pain care was included in the communication and care, and could not be extracted alone. It was suggested that an HPN provides communication, care and prediction of worsening of symptoms with excellent judgment and technological competency, while placing importance on offering support for the patient’s natural death.

AIM: To compare rabeprazole (RPZ; 10 mg) with Lansoprazole orally disintegrating tablets (LPZ; 30 mg OD) in terms of antisecretory activity and blood drug concentration after a single dose. METHODS: Eight H pylori-negative cytochrome P450 (CYP) 2C19 extensive metabolizers were assigned to receive a single oral dose of RPZ 10 mg or LPZ 30 mg OD. Twelve hour intragastric pH monitoring was perform- ed on the day of treatment. Blood samples were also collected after the administration of each drug. RESULTS: LPZ 30 mg OD induced a significantly earlier rise in blood drug concentration than RPZ 10 mg; consequently, LPZ 30 mg OD induced a significantly earlier rise in median pH in the third and fourth hours of the study. CONCLUSION: In H pylori-negative CYP2C19 extensive metabolizers, LPZ 30 mg OD induced a significantly faster inhibition of gastric acid secretion than RPZ 10 mg.

These days, some manufacturing enterprises which are forced to diversify management enter Agri- Business and plant factory business, make an Innovation utilizing high research and development ability and enough capital. As Agri-Business Innovation in Japan, this study examinedNK Agri, a subsidiary company of Noritsu Koki, which is a motion picture producer and machinery maker, based on Industrial Cluster Theory and Diamond Model by Michael E. Porter. The important points to enter Agri-Business of manufacturing enterprises were summed up as below. 1) New coming manufacture enterprises can form Industrial Cluster with farmhouse and foods makers, and enhance productivity and competitiveness. 2) New coming manufacture enterprises can enhance productive efficiency by collaborating with orthodox farmhouses as important partners. 3) Re- search institutions such as universities and local government should support for forming and development of Agri Industrial Cluster by manufacture enterprises. Agri Industrial Cluster by manufacture enterprises can improve local economy, so local government should prepare for the system to expand support as a policy.

OBJECTIVE: To examine the effects of Yokukansan(YKS) extract on two endogenous modulators of anxiety, brain-derived neurotrophic factor(BDNF)and serotonin (5-HT)_(2A) receptors pharmacologically, in the ischemic rat model of ***: The cerebral ischemia(CI) was induced by bilateral occlusion of the vertebral and common carotid arteries(4-vessel occlusion ischemia). The CI was combined with the amyloid-β42 peptide(A42) injected intracerebroventricularly, and referreβdto as CI+Aβ. Anxiety-like behaviors were assessed by elevated plus maze(enclosed arm), light/dark transition test(dark chamber), and open-field ***-dog shakes were induced by the 5-HT_(2A) receptor agonist 2, 5-dimethoxy-4-iodoamphetamine(DOI). The concentration of BDNF in serum was determined by enzyme-linked immuno sorbent ***: CI + Aβ increased anxiety, as demonstrated by the increase of time spent in the enclosed arms and dark chambers, and locomotion in the outer zone of the open field(thigmotaxis). CI + Adecreased the serum concentration of BDNF. YKS reβ-duced the anxiety-like behaviors, suppressed the DOI-induced wet-dog shakes and increased serum BDNF ***: Our findings suggest that YKS extract improves CI + Aβ-induced anxiety by antagonizing 5-HT_(2A) receptors and increasing BDNF.

A Ring-Cavity Diode Laser

In this article, we report on a ring-cavity diode laser configuration, which has more features compared to the standing-wave external-cavity diode lasers. First, a transmission grating will be used, and the ring-cavity laser output will be the zero-order output from the transmission grating. Therefore, unlike the standing-wave external cavity diode lasers, the output direction of this ring-cavity laser does not depend on the laser's frequency. Second, since it is a ring-cavity laser, it can avoid the local heating caused by the antinodes of the standing-wave lasers. Moreover, because this ring-cavity will allow the laser beam to pass the transmission grating two times, it is expected that this will ultimately result in a narrower linewidth compared to the external-cavity standing-wave lasers with the same cavity length.

Sources of Sexual Knowledge among Vietnamese High School Students

This study determined the sources from which Vietnamese high school students currently acquire sexual knowledge as well as the sources from which they believe they should acquire it. Participants were eleventh-grade students at three public high schools in Hanoi, Vietnam. We conducted a questionnaire-based survey from September to October 2012, with 1672 students participating. Responses were received from all 1672 students, a response rate of 100%. Complete data were received from 1653 respondents (98.9%), including 896 males (54.2%) and 757 females (45.8%). The survey was a self-administrated questionnaire. Although many participants believed that they were knowledgeable about sex, only a small number of them actually possessed accurate sexual knowledge. Few participants in this study, targeted to a specific age (eleventh grade), had used the Internet as a major source for obtaining sexual knowledge, in contrast to results from previous studies which covered a wider age demographic. Future research should include comparative studies between eleventh and twelfth graders as well as between those of the same age who are and who are not enrolled in high school. Additionally, it might be beneficial to consider the relationship between adolescent age and Internet use to study the influence of the Internet on sexual knowledge. Our results suggest that parents and schoolteachers should shoulder more responsibility in providing sexual education, particularly by improving their own knowledge and ability to provide such education, because many participants indicated that sexual knowledge should be acquired from parents and schools.

The Association between Awareness of Sexual Behavior and Cognitive Social Capital among High School Students in Vietnam

Background: Globally, adolescent sexual behavior is a major health concern and the focus of many researches. However, negative attitude towards premarital sex and sense of community belonging have not been examined in relation to adolescent sexual behavior. The purpose of this study was to examine whether these two factors are associated with adolescent sexual behavior. Methods: We conducted a cross-sectional survey of 1672 eleventh grade students from three public high schools in Hanoi, Vietnam. Participants’ sense of community belonging and negative attitude towards premarital sex were assessed through a self-administered questionnaire. We performed multiple logistic regression analyses to examine associations between the cognitive social capital concepts, such as a sense of community belonging. Results: 92.3% of students agreed with a sense of community belonging, but only 14.1% of students accepted premarital sex. A high sense of community belonging was significantly associated with negative attitude towards premarital sex OR (95% CI), male 3.16 (2.3-4.34), female 8.77 (5.38-14.29). After adjusting for a sense of community belonging, the association disappeared between acceptance of premarital sex and sexual experience of peers, proper time to wear a condom, and Rosenberg Self-Esteem Scale score. Conclusion: Cognitive social capital, such as sense of community belonging, was associated with negative attitude towards premarital sex. Therefore, cognitive social capital concepts should be included in sexual behavior and health promotions, especially with adolescents.

Gender Effects on Statin-Associated Muscular Adverse Events: An Analysis of the FDA AERS Database

Objective: The reports submitted to the US Food and Drug Administration (FDA) Adverse Event Reporting System (FAERS) from 1997 to 2011 were reviewed to assess the gender effects on muscular adverse events induced by the administration of 3-hydroxy-3-methylglutaryl coenzyme A (HMG-CoA) reductase inhibitors (statins). Methods: After the deletion of duplicated submissions and the revision of arbitrary drug names, the reports involving pravastatin, simvastatin, atorvastatin, rosuvastatin, and cerivastatin were analyzed. Data mining algorithms were applied for the quantitative detection of signals, where a signal means a drug-associated adverse event, including the proportional reporting ratio, the reporting odds ratio, the information component, and the empirical Bayes geometric mean. Myopathy, myalgia, myositis, rhabdomyolysis, and an increase in creatine phosphokinase level were focused on as the muscular adverse events. Results: The total number of reports was 3,472,494. The signal scores suggested that all 5 statins were associated with 5 muscular adverse events in both male and female patients. The scores varied among statins, but were more noteworthy for cerivastatin. Conclusion: The data strongly suggested the necessity of well-organized clinical studies on statin-associated muscular adverse events.
