Frequency of HER2/neu overexpression in adenocarcinoma of the gastrointestinal system

AIM:To determine the frequency of HER2/neu protein overexpression in gastric(group A),small intestine(group B),and colorectal(group C)***:A descriptive,cross-sectional study was performed on 50 cases of gastrointestinal adenocarcinoma(stomach,small intestine,and colorectal);11from group A,8 from group B,and 31 from group *** samples were grossed and processed in the pathology department,and sections were stained with HE(hematoxylin and eosin stain)for histopathological confirmation of malignancy(well-differentiated,moderately-differentiated,and poorly-differentiated).The confirmed samples were processed for immunomarker study of HER2/***:HER2/neu protein overexpression was found in 33(66%)patients overall(P=0.000).Out of 33HER2/neu positive subjects,23(69.6%)were from group C,while the remaining 10(30%)were from group *** of the patients from group B had positive HER2/neu protein *** protein overexpression or membrane staining in<10%tumor cells was observed in 17(34%)patients,which were labeled as score"0"and considered negative for HER2/neu protein ***/weak staining(in≥10%of tumors cells)were observed in 8(16%)patients and given the"1+"*** 13(26%)patients reported moderate staining(in≥10%tumor cells)and were thus labeled as"2+",and strong staining(in≥10%tumors cells),labeled as"3+",was observed in 12(24%)*** of 50 patients,26(52%)were suffering from grade-Ⅱmalignancy,16(32%)from grade-Ⅰ,and 8(16%)from grade-Ⅲ.There was highly significant association between tumor grades and HER2/neu protein overexpression(P=0.0000).CONCLUSION:HER2/neu protein is credibly overexpressed in colon and gastric adenocarcinomas in *** is significant association between grade of tumor and HER2/neu protein overexpression.

Diagnostic accuracy of color Doppler in diagnosis of Hepatocellular carcinoma taking histopathology as gold standard

Hepatocellular carcinoma (HCC) is the fifth most common cancer in the world and the third most common cause of cancer mortality. Despite the high numbers of patients diagnosed worldwide, HCC continues to pose challenging clinical problems. Good-quality ultra sound with careful evaluation of the entire liver can be a screening examination for HCC in patients at risk. The aim of the study was to determine the diagnostic accuracy of the Doppler sonography for differentiation of hepatocellular carcinoma (HCC) from other focal liver lesions by taking histopathology as a gold standard. It was a cross-sectional survey, conducted in the Department of Diagnostic Radiology, King Edward Medical University/Mayo Hospital, Lahore in one year from 13-12-2010 to 12-12-2011. A total of 70 patients were included in this study with focal lesions on ultrasound are included while pregnant patients are excluded. After evaluating by Doppler sonography and doing core biopsy, the patients were sent to the Department of Pathology, Mayo Hospital for histopathological examination. Histopathology was assessed for confirmation of positive and negative cases. Mean age of the patients was 60.9 ± 4.7 years. Out of 70 patients, 47 patients (67.1%) were male while remaining 23 patients (32.9%) were female. On Doppler sonography 37 cases were positive for hepatocellular carcinoma and 33 cases were negative while 36 cases were positive and 34 negative on histopathology. On Doppler sonography, sensitivity (94.4%), specificity (91.1%), diagnostic accuracy (92.8%), positive predictive value (91.8%) and negative predictive value was 93.9%. We concluded that Doppler sonography is a useful method for differen- tiation of Hepatocellular carcinoma from other focal liver lesions, detection and characterization of hepatocellular carcinoma.

Effects of different sleeping patterns on academic performance in medical school students

In the past, efforts have been made to determine the influence of sleep quantity and its deprivation, on functioning efficiency of human beings. However, determination of sleeping patterns that could improve intellectual performance has been largely neglected. This study is designed to discover the effects of different sleeping patterns on academic performance among medical students. A descriptive study was carried out in King Edward Medical University in Lahore, Pakistan during a six-month time span from May 11th, 2011 to September 30th, 2011. Of the total population of 1350 students in King Edward Medical University, 591 undergraduates were included in the study. A questionnaire designed on sleeping patterns and academic performance was distributed in May 2011. What was described as outstanding students were greater in number in group 4 (7/19) 36.8% and group 6 (6/19) 31.6%. Above average students with sleeping patterns were in group 4 (13/37) 35.1% and group 6 (10/37) 27%. Average students were shown to have sleeping patterns of group 4 (11/25) 44% and group 6 (7/25) 28%. Below average students were shown to have sleeping patterns of group 4 (3/3) 100%. Most of our students had a reduction in the total amount of sleeping hours throughout the years. Midnight to 6 o’clock in the morning with an afternoon nap was the sleeping pattern that was most commonly seen in all groups. We concluded that different sleeping patterns do not affect the performance of medical students in the academic prospective. Many other factors may be involved in the lack of significant achievement, in order to prove that the sleeping patterns are not related to the academic performance, and more data would need to be collected.

Primary Isolated Case of Drug Induced Pancreatitis Misdiagnosed as Idiopathic Pancreatitis

Drug Induced Pancreatitis is a very common cause of acute pancreatitis in older patients and is often treated by just symptomatic treatment. An old hypertensive lady was admitted repeatedly to emergency department and treated as Idiopathic Pancreatitis. Initial labs showed elevated pancreatic enzymes. Detailed history showed that increase in thiazides dosage was the cause of her pancreatitis. It was later diagnosed as a case of thiazide induced pancreatitis. It is important to consider medication as a cause of acute pancreatitis in especially in older people presenting with acute pancreatitis without common risk factors.

Evaluation of non-palpable thyroid nodules by ultra sound guided fine needle aspiration cytology

The aim of this study was to see the usefulness of fine needle aspiration by “theBethesdasystem for reporting Thyroid Cytopathology” (TBS- RTC) for non palpable thyroid nodules through ultrasound-guidance for the evaluation and treatment planning of nonpalpable thyroid lesions. This study was conducted on 200 patients with non palpable thyroid nodules which are very low lying or felt on swallowing;in Department of Pathology and Radiology since January 2011 to June 2012. The patients were scanned and USG- FNAC was performed and reporting was done by “TBSRTC”. Of the 200 specimens 17 samples were nondiagnostic or unsatisfactory (Class I), 145 samples were benign (Class II), 20 samples were showing Atypical of Undetermined Significance (AUS) or Follicular Lesion of Undetermined Significance (FLUS);(Class III), 6 were showing follicular neoplasm or suspicious for a follicular neoplasm (Class IV), 7 samples were suspicious for malignancy (Class V) and 5 samples were positive for malignancy (Class VI). On comparison of ultrasound guided FNAC with histopathology the sensitivity for correct diagnosis was 93%, specificity was 86%, positive predictive value was 37%, negative predictive value was 99% and accuracy was 86%. USG-FNAC is a useful modality for the evaluation and treatment planning of nonpalpable thyroid lesions smaller than5 mmin the maximum diameter. TBSRTC is the best method of reporting but class III and IV are the main pitfall of this system for reporting Thyroid Cytopathology and show high sensitivity, specificity and accuracy.

Integrated Effects of Rhizobial Inoculum and Inorganic Fertilizers on Wheat Yield and Yield Components

An experiment was conducted in pots under natural condition with two factor factorial completely randomized design (CRD) (CRD) to investigate the integrated effect of rhizobia inoculums and inorganic fertilizers on growth and yield of wheat crop at The University of Agriculture Peshawar, during 2012-2013. The experiment was comprised of four inoculums (no, lentil, peas and chickpeas) and two NPK levels (recommended 120:90:60 kg·ha-1 & 20% less of recommended). It was observed that inoculation of wheat by rhizobia significantly increased tillers per plant by a maximum of 42%, plant height by 13%, grain per spike by 16%, 100 grain weight by 10%, biological and grain yield by 10% over un-inoculated (control) treatment. Among inoculums, peas inoculum was found to be the most efficient for all traits except plant height where chickpea inoculum performed better. Similarly recommended NPK significantly increased tillers per plant by 33%, plant height by 19%, grain per spike by 9%, 100 grain weight by 10%, biological yield by 8% and grain yield by 10% compared with 20% less of recommended NPK. Interactive effect of inoculum x NPK was significant for tillers per plant, grain per spike, grain yield and non-significant for plant height, 100 grain weight and biological yield. However, it was evident from the results that inoculation improved all traits both under recommended and 20% less of recommended NPK. The persistent good performance of peas and lentil rhizobial inoculation in wheat growth exhibited that this could be used as a plant growth promoting rhizobacteria for wheat and other cereal crops in prevailing soil and climatic conditions.

Assessment of Heritable Variation and Best Combining Genotypes for Grain Yield and Its Attributes in Bread Wheat

Grain yield in wheat is the resultant of several plant attributes. It is very important to assess heritable variation involved in the inheritance of these attributes in addition to find the best combining genotypes. For this purpose, the present study involving 5 × 5 full diallel analysis was performed. Twenty F1 hybrids along with their parents (9797, 9801, 9802, Chakwal-50 and Chakwal-86) were planted in field using randomized complete block design (RCBD) with three replications in the research area of Department of Plant Breeding and Genetics, University of Agriculture, Faisalabad during 2015-2016. Plant characters like plant height, flag leaf area, spike length, No. of fertile tillers per plant, No. of grains per spike, No. of spikelets per spike, 1000 grain weight and grain yield per plant were studied in this experiment. Mean squares due to general combining ability (GCA) were highly significant for all the traits except for spike length for which GCA effects were significant. Mean squares due to specific combining ability (SCA) and reciprocal combining ability (RCA) were highly significant for all the characters studied. GCA variance was higher than the SCA variance for spike length and No. of grains per spike exhibiting the predominant role of additive genetic variation in the inheritance of these traits. However, for the characters like plant height, flag leaf area, No. of fertile tillers per plant, No. of spikelets per spike, 1000 grain weight and grain yield per plant, the value of SCA variance was higher than the value of GCA variance showing non additive gene action for these traits. The variety Chakwal-50 proved to be the best general combiner for plant height, spike length, No. of spikelets per spike, No. of grains per spike and grain yield per plant. The cross, 9802 × Chakwal-86 was the best specific combination for grain yield and most of the yield attributes.
