Introduction: This study evaluated the difference in operative and clinical outcomes for patients with advanced ovarian cancer after primary debulking surgery (PDS) versus neoadjuvant chemotherapy (NACT) followed by interval debulking surgery (IDS) in Bangladesh. Methods: Sixty patients with advanced epithelial ovarian cancer presenting to the department of Gynaecological Oncology at the National Institute of Cancer Research and Hospital were prospectively enrolled. Thirty patients underwent primary debulking surgery and thirty patients received NACT followed by IDS. Commelina diffusa Burm. F. is a herbaceous tropical plant with different traditional medicinal uses. Present study is aimed to isolate the bioactive compounds from DCM-Methanol extract of the powdered whole plant of Commelina diffusa and to investigate the cytotoxic, antimicrobial and antioxidant activities of the crude extract and its twelve vacuum liquid chromatographic fraction (CD1-12). Vacuum Liquid Chromatography (VLC) was employed to isolate bioactive metabolites. The physicochemical properties of the isolated compound were examined and the molecular structure was elucidated by NMR spectroscopy.?Cytotoxic, antibacterial, antioxidant activities were evaluated following brine shrimp lethality bioassay, disc diffusion method and DPPH free radical scavenging assay respectively. The isolated compound was identified as steroid (stigmasterol) which had significant cytotoxic effect on vero cell line. The crude extract and its fractions (CD8-CD12) exhibited strong cytotoxicity in brine shrimp lethality bioassay having LC50 values 3.79, 9.19, 29.49, 16.60, 19.36, 44.58 μg/mL respectively. The crude extract showed mild to strong antibacterial activity. Fractions (CD5-CD12) showed mild to strong antibacterial activity against Escherichia coli in comparison with Kanamycin (standard). Strong antioxidant activities were found in crude extract (IC50 value 30.52 μg/ml), CD11 (IC50 value 39.27 μg/ml) and CD12 (IC50 value 19.50 μg/ml). The study suggests Commelina diffusa plant extract to have strong antioxidant and cytotoxic activity which is indicative of presence of compounds with broad spectrum of curative applications. One compound namely stigmasterol has been isolated from the plant.

A study was conducted to explore the traditional utilization pattern and indigenous management practices of wild date palm （Phoenix sylvestris Roxb） in the rural agrarian regions of Bangladesh. A multistage sampling method with 10% intensity and a semi-structured questionnaire were used for the study. The farmers manage the palm mainly for sap production with which sugar based secondary goods are manufactured. The sap is either used fresh as drink or after some sort of processing as molasses and/or alcoholic beverage. Seven diversified sites support the palm as its habitat and most palms （20.40%） occur in orchards. Besides growing naturally, the palm is also established in orchards using the wildings as the staple planting material. Although the medium category farmers own most of the palms （33%）, a considerable portion （28.68%） of it is managed by the landless farmers, who earn a substantial livelihood from the palms. The farmers practice their own indigenous wisdom in every stage of the palm＇s maintenance from planting through tapping for sap collec- tion to the processing of products. If managed more scientifically on a sustainable basis with the collaboration of farmers＇ indigenou~ knowledge, this familiar palm could be able to support the rural economy of the country to a great extent. Side by side, it would also be able to contribute to the richness of biodiversity in the region.

The Perhentian Islands located in the East coast of Peninsular Malaysia. They are well-known for their rich coral reef ecosystems. Marine resources of Malaysia have been overexploited in general due to overfishing and other development activities. Such no-take marine protected areas (MPAs) were established in Malaysia including Perhentian Islands Marine Park to enable overexploited marine resources to recover and to conserve coral reef ecosystems. This paper examines the current level of activities causing damages to the coral reef habitats in the Perhentian MPA. This study used paired comparison method to elicit the perception of the local stakeholders on the activities harmful to the marine habitats. The results of the analysis showed that various respondent groups had similar preference rankings on the harmful activities: littering, discarding fishing equipments, excess fishing and too many divers that cause damage to the habitats in the MPA area. The findings suggest that policy makers should take cognizance of the local stakeholders’ concern in planning and designing of marine protected areas in Malaysia.

The diagnosis of breast cancer can cause a significant anxiety in someone’s life. The care of the breast cancer patient requires in many centres. The care of breast cancer patient is undertaken as a joint venture between the surgeon, medical oncologist, radiotherapist, pathologist and allied health professionals such as the clinical nurse specialist as well as counsellors. This is a case of a 39-years-old Malay woman presented to HUSM with a history of ulcerating breast lump with bloody pus discharge on the right breast. An initial mammogram study showed lesion suspicious of malignancy, and histopathological examination was consistent with invasive ductal carcinoma. She received neoadjuvant chemotherapy before proceeding to right modified radical mastectomy with axillary clearance and immediate chest wall reconstruction. Detailed discussion on the diagnosis, treatment, surgical intervention and progress of the patient with reference to available literature reviews are presented.

Isolated Endophytic Fungi from the Plant <i>Curcuma longa</i>and Their Potential Bioactivity—A Review

Endophytes (both fungi and bacteria) represent a treasure house for bioactive compounds such as anticancer, immunomodulatory, antioxidant, antiparasitic, insecticidal etc. for use in the pharmaceutical and agrochemical industries. Endophytic population is greatly influenced by climatic conditions and location where the host plant grows. Curcuma longa L. (Turmeric) is a medicinal plant belonging to the Zingiberaceae family that is usually used as spice and preservative. Traditionally it is used to treat various conditions and symptoms such as digestive disorders, arthritis, cardiovascular conditions, cancer, bacterial infections etc. In recent times, there have been different discoveries on endophytic fungi and their biological activity from C. longa. But there is no comprehensive review on endophytic fungi of C. longa. The aim of this review is to analyze the published report based on the endophytic fungi to provide for the first time an updated information about their isolation from different tissues, their biological activities including the phytochemistry of C. longa. This will lead the way to forecast the missing link for future research work.

Racial disparity in colorectal cancer: Gut microbiome and cancer stem cells

Over the past two decades there has been remarkable progress in cancer diagnosis, treatment and screening. The basic mechanisms leading to pathogenesis of various types of cancers are also understood better and some patients, if diagnosed at a particular stage go on to lead a normal pre-diagnosis life. Despite these achievements, racial disparity in some cancers remains a mystery. The higher incidence, aggressiveness and mortality of breast, prostate and colorectal cancers(CRCs) in AfricanAmericans as compared to Caucasian-Americans are now well documented. The polyp-carcinoma sequence in CRC and easy access to colonic epithelia or colonic epithelial cells through colonoscopy/colonic effluent provides the opportunity to study colonic stem cells early in course of natural history of the disease. With the advent of metagenomic sequencing, uncultivable organisms can now be identified in stool and their numbers correlated with the effects on colonic epithelia. It would be expected that these techniques would revolutionize our understanding of the racial disparity in CRC and pave a way for the same in other cancers as well. Unfortunately, this has not happened. Our understanding of the underlying factors responsible in African-Americans for higher incidence and mortality from colorectal carcinoma remains minimal. In this review, we aim to summarize the available data on role of microbiome and cancer stem cells in racial disparity in CRC. This will provide a platform for further research on this topic.
