Portal vein stenting as a significant risk factor for biliary stricture in adult living donor liver transplantation

BACKGROUND: Although perioperative portal vein (PV) stent implantation is an effective treatment for steno-occlusive disease in adult living donor liver transplantation (LDLT) re-cipients, we experienced high incidence of biliary anastomotic strictures (BAS) after PV stenting. In this study, we sought to clarify the relation between BAS and PV stenting and to suggest the possible mechanism of BAS and measures to reduce its in-cidence. METhoDS: We retrospectively analyzed 44 LDLT recipients who underwent PV stent implantation across the line of PV anastomosis regardless of the location of steno-occlusion (stent group) and their matched controls (non-stented LDLT recipients, n=131). RESULTS: The incidence of BAS was higher in patients in the stent group than that in the control group (43.2% vs 17.6%, P=0.001). Cumulative 6-month and 1-, 2- and 5-year BAS rates were 31.8%, 34.1%, 41.4% and 43.2%, respectively, in the stent group and 13.0%, 13.8%, 16.1% and 17.8%, respectively, in the control group (P=0.001). Multivariate analysis revealed that PV stenting was an independent risk factor for BAS. CONCLUSIONS: Although PV stent implantation is a reliable treatment modality for steno-occlusive PV in adult LDLT recipients, innovative methods to prevent the PV stent from crossing the line of PV anastomosis may be necessary to reduce the incidence of postoperative BAS.

Genevestigator analysis has indicated heat shock induction of transcripts for NADPH-thioredoxin reduc-tase, type C （NTRC） in the light. Here we show overexpression of NTRC in Arabidopsis （NTRC°E） resulting in enhanced tolerance to heat shock, whereas NTRC knockout mutant plants （ntrcl） exhibit a temperature sensitive phenotype. To investigate the underlying mechanism of this phenotype, we analyzed the protein＇s biochemical properties and protein structure. NTRC assembles into homopolymeric structures of varying complexity with functions as a disulfide reductase, a foldase chaperone, and as a holdase chaperone. The multiple functions of NTRC are closely correlated with protein structure.. Complexes of higher molecular weight （HMW） showed stronger activity as a holdase chaperone, while low molecular weight （LMW） species exhibited weaker holdase chaperone activity but stronger disulfide reductase and fol-dase chaperone activities. Heat shock converted LMW proteins into HMW complexes. Mutations of the two active site Cys residues of NTRC into Ser （C217/454S-NTRC） led to a complete inactivation of its disulfide reductase and foldase chaperone functions, but conferred only a slight decrease in its holdase chaperone function. The overexpression of the mutated C217/454S-NTRC provided Arabidopsis with a similar degree of thermotolerance compared with that of NTRC°E plants. however, after prolonged incubation under heat shock, NTRC°E plants tolerated the stress to a higher degree than C217/454S-NTRC°E plants. The results suggest that the heat shock-mediated holdase chaperone function of NTRC is responsible for the increased thermotolerance of Arabidopsis and the activity is significantly supported by NADPH.

Multimodal imaging evaluation in staging of rectal cancer

Rectal cancer is a common cancer and a major cause of mortality in Western countries. Accurate staging is essential for determining the optimal treatment strategies and planning appropriate surgical procedures to control rectal cancer. Endorectal ultrasonography (EUS) is suitable for assessing the extent of tumor invasion, particularly in early-stage or superficial rectal cancer cases. In advanced cases with distant metastases, computed tomography (CT) is the primary approach used to evaluate the disease. Magnetic resonance imaging (MRI) is often used to assess preoperative staging and the circumferential resection margin involvement, which assists in evaluating a patient’s risk of recurrence and their optimal therapeutic strategy. Positron emission tomography (PET)-CT may be useful in detecting occult synchronous tumors or metastases at the time of initial presentation. Restaging after neoadjuvant chemoradiotherapy (CRT) remains a challenge with all modalities because it is difficult to reliably differentiate between the tumor mass and other radiation-induced changes in the images. EUS does not appear to have a useful role in post-therapeutic response assessments. Although CT is most commonly used to evaluate treatment responses, its utility for identifying and following-up metastatic lesions is limited. Preoperative high-resolution MRI in combination with diffusion-weighted imaging, and/or PET-CT could provide valuable prognostic information for rectal cancer patients with locally advanced disease receiving preoperative CRT. Based on these results, we conclude that a combination of multimodal imaging methods should be used to precisely assess the restaging of rectal cancer following CRT.

Background: Understanding the composition of the microbial community and its functional capacity during weaning is important for pig production as bacteria play important roles in the pig’s health and growth performance. however,limited information is available regarding the composition and function of the gut microbiome of piglets in ***, we performed 16 S rRNA gene and whole metagenome shotgun sequencing of DNA from fecal samples from healthy piglets during weaning to measure microbiome shifts, and to identify the potential contribution of the early-life microbiota in shaping piglet health with a focus on microbial stress responses, carbohydrate and amino acid ***: The analysis of 16 S rR NA genes and whole metagenome shotgun sequencing revealed significant compositional and functional differences between the fecal microbiome in nursing and weaned piglets. The fecal microbiome of the nursing piglets showed higher relative abundance of bacteria in the genus Bacteroides with abundant gene families related to the utilization of lactose and galactose. Prevotel a and Lactobacil us were enriched in weaned piglets with an enrichment for the gene families associated with carbohydrate and amino acid metabolism. In addition, an analysis of the functional capacity of the fecal microbiome showed higher abundances of genes associated with heat shock and oxidative stress in the metagenome of weaned piglets compared to nursing ***: Overal, our data show that microbial shifts and changes in functional capacities of the piglet fecal microbiome resulted in potential reductions in the effects of stress, including dietary changes that occur during *** results provide us with new insights into the piglet gut microbiome that contributes to the growth of the animal.

Reciprocal inhibition of expression between RAV1 and BES1 modulates plant growth and development in Arabidopsis

RAV1(Related to ABI3/VP1)is a plant-specific B3 and AP2 domain-containing transcription factor that acts as a negative regulator of growth in many plant *** expression of RAV1 is downregulated by brassinosteroids(BRs);large-scale transcriptome analyses have shown that the expression of RAV1 was previously targeted by BRI1-EMS-SUPPRESOR1(BES1)and BRASSINAZOLE-RESISTANT1(BZR1),which are critical transcription factors for the BR-signaling *** RAV1-overexpressing transgenic plants,we showed that RAV1 overexpression reduced the BR signaling capacity,resulting in the downregulation of BR biosynthetic genes and BES1 ***,we demonstrated that BES1,not BZR1,is directly bound to the RAV1 promoter and repressed RAV1 expression,and vice versa;RAV1 is also bound to the BES1 promoter and repressed BES1 *** mutual inhibition was specific to RAV1 and BES1 because RAV1 exhibited binding activity to the BZR1 promoter but did not repress BZR1 *** observed that constitutively activated BR signaling phenotypes in bes1-D were attenuated by the repression of endogenous BES1 expression in transgenic bes1-D plants overexpressing ***-sequencing analysis of RAV1-overexpressing transgenic plants and bes1-D mutant plants revealed differentially expressed genes by RAV1 and BES1 and genes that were oppositely co-regulated by RAV1 and ***1 and BES1 regulated different transcriptomes but co-regulated a specific set of genes responsible for the balance between growth and *** results suggested that the mutual inhibitory transcriptional activities of RAV1 and BES1 provide fine regulatory mechanisms for plant growth and development.

Stabilizing a frequency comb to an ultra-stable optical frequency reference requires a multitude of optoelectronic peripherals that have to operate under strict ambient ***,the frequency comb-to-comb stabilization aims to synchronize a slave comb to a well-established master comb with a substantial saving in required equipment and ***,we report an utmost case of frequency comb-to-comb stabilization made through a 1.3 km free-space optical(FSO)link by coherent transfer of two separate comb lines along with a feedback suppression control of atmospheric phase *** FSO link offers a transfer stability of 1.7×10-15 at 0.1 s averaging,while transporting the master comb,s stability of 1.2×10-15 at 1.0 s over the entire spectrum of the slave *** remote comb-to-comb stabilization is intended to expedite diverse long-distance ground-to-ground or ground-to-satellite applications;as demonstrated here for broadband molecular spectroscopy over a 6THz bandwidth as well as ultra-stable microwaves generation with phase noise of-80dBc Hz-1 at 1 Hz.

Adjuvant Effects of Fermented Red Ginseng Extract on Advanced Non-Small Cell Lung Cancer Patients Treated with Chemotherapy

Objective： To investigate the adjuvant therapeutic effects of fermented red ginseng（FRG） extract on non-small cell lung cancer（NSCLC） patients treated with chemotherapy. Methods： A total of 60 patients with advanced NSCLC were assigned to two groups using a random number table, i.e., the gemcitabine plus cisplatin（GP） chemotherapy alone group（26 patients） and the FRG ＋ GP chemotherapy group（34 patients）, for 60-day treatment. Patients were then assessed according to the Fatigue Symptom Inventory, Chinese medicine symptoms score, Self-Rating Anxiety Scale, Self-Rating Depression Scale, Karnofsky Performance Status Scale, and Functional Assessment of Cancer Therapy-Lung. In addition, chemotherapy toxicity and tumor biomarkers were measured. Results： For NSCLC patients after chemotherapy, FRG extract significantly improved the FSI score, CM symptoms score, psychological status, physical conditions, and quality of life and reduced chemotherapy toxicity（P〈0.01）, but the expression levels of carcinoembryonic antigen, cytokeratin-19 fragments, and neuron-specific enolase were not significantly different between the chemotherapy alone and the FRG ＋ chemotherapy groups or between pre-and post-treatments（P〉0.05）. Conclusions： This study demonstrated that FRG extract had an adjuvant effect on advanced NSCLC patients treated with chemotherapy. Further studies with a larger sample size will verify the current findings.

The sizes and concentrations of 21 atmospheric polycyclic aromatic hydrocarbons (PAHs) were measured at Jhu-Shan (a rural site) and Sin-Gang (a town site) in central Taiwan in October and December 2005. Air samples were collected using semi-volatile sampling trains (PS-1 sampler) over 16 days for rice straw burning and non-burning periods. These samples were then analyzed using a gas chromatograph with a flame-ionization detector (GC/FID). Particle-size distributions in the particulate phase show a bi-mode, peaking at 0.32-0.56 μm and 3.2-5.6 μm at the two sites during non-burning period. The concentrations of total PAHs (sum of 21 gases + particles) at the Jhu-Shan site (Sin-Gang site) were 522.9 ± 111.4 ng/m3 (571.8 ± 90.9 ng/m3) and 330.1 ± 17.0 ng/m3 (or 427.5 ± 108.0 ng/m3) during burning and non-burning periods, respectively, accounting for a roughly 58% (or 34%) increase in the concentrations of total PAHs due to rice-straw burning. On average, low-weight PAHs (about 87.0%) represent the largest proportion of total PAHs, followed by medium-weight PAHs (7.1%) and high-weight PAHs (5.9%). Combustion-related PAHs during burning periods were 1.54-2.57 times higher than those during non-burning periods.
