Surgical management of clinoidal meningiomas: 10 cases analysis

Aim: The purpose of this article is to advocate standard skull base technique for removing the clinoidal meningioma and to delineate the technique's advantages that aid in achieving an improved extent of tumor resection and enhancing the patients' overall outcome, specially their visual outcome. Methods: A retrospective analysis was performed on 10 consecutive patients with clinoidal meningiomas who underwent surgical resection at the Bangabandhu Sheikh Mujib Medical University and other private clinics between May 2013 and July 2016. A standard pterional craniotomy technique consisting of extradural anterior clinoidectomy, coupled with optic canal unroofing and optic nerve sheath opening was used in all patients. All patients had thorough preoperative and postoperative ophthalmological evaluations. The follow-up period ranged from 6 to 42 months. Results: Total resection was achieved in 5 (50.0%) of the 10 patients in this series. The majority of the patients with preoperative visual impairment experienced significant visual improvement 7 of 10 patients;70.0%). Conclusion: In the majority of patients with clinoidal meningiomas, total resection may be achieved with minimal complications. For large tumors encasing the optic nerve and internal carotid artery, or for those tumors causing preoperative visual impairment, use of the cranial base technique delineated in this study may lead to significant improvement in the patients' visual and overall outcomes.

The present era has witnessed tremendous growth of the Internet and various applications that are supported by it. There is an enormous pressure on Internet Service Providers (ISPs) to make available adequate services for the traffics like VoIP and Video on demand. Since the resources like computing power, bandwidth etc. are limited, the traffic needs to be engineered to properly exploit them. Due to these limitations, terms like Traffic Engineering, Quality of Service (QoS) came into existence. Traffic Engineering broadly includes techniques like multipath routing & traffic splitting to balance the load among different paths. In this document, we survey various techniques proposed for load balancing that are available on the Internet. We here try not to be exhaustive but analyze the important techniques in the literature. Present survey would help to give a new direction to the research in this realm.

Drugs are traditionally been identified on basis of chromatographic-spectroscopic hyphenated techniques in instrumental analysis. Gas chromatography (GC) and Liquid chromatography (LC) hyphenated with mass spectroscopy (MS) i.e. GC-MS and LC-MS give reliable and confirmatory results in drugs identification. In the present work the novel hyphenated technique High Performance Thin Layer Chromatography-Mass Spectroscopy (HPTLC-MS) has been used. This technique provides efficient, quick and simple method for identification and separation of Narcotic drugs and psychotropic substances. The drugs under study are Papaverine, Methadone, Cocaine, Ketamine, Caffeine, Codeine, Diazepam, Thebaine, Heroin, Methamphetamine, Carbamazepine, Morphine, Narcotine and Ephedrine. The present study comprising of sixteen drugs has been carried out on CAMAG HPTLC instrument with automatic sampling. Thin layer chromatography (TLC) plates were developed in various solvent systems, scanned under TLC scanner and the results in terms of Retention Factor (Rf value) and UV spectrum (λmax) are presented in the manuscript. Using hyphenated technique of HPTLC-MS (MS 2020 SHIMADZU) spots of these drugs from TLC plate was lifted with CAMAG TLC-MS interface and confirmed by the mass spectrum of the individual drugs by their m/z values thus delivering fast and accurate confirmatory result on the TLC plate.

Many applications do not fit well with the traditional best effort packet delivery policy of the Internet. These include applications such as Internet telephony and video conferencing which require voice and bulky graphical images transfer. Therefore, the policies of assigning traffic to various service classes and providing service as per the service level agreement of the user with the network provider came into existence. Multi-protocol Label Switching is the backbone of fast switching technology that helps the network service providers to implement these policies. It provides Quality of service oriented reserved paths from the source to the destination for the user’s traffic. Selection of these paths is a cumbersome task, especially when the traffic forecast is totally unknown. Furthermore, nodes and link failures in the Internet worsen the situation. This paper addresses the issue of selecting Label Switched Paths (LSPs) for various traffic demands in the network so that the resultant network has the characteristics like high failure resistance, low LSP demand blocking probability, low impact from the node or link failure, load balancing and low over-all resource utilization. By extensive simulations, the proposed cost function has been compared with the various cost functions mentioned in the literature and it was found to score over them in major aspects.

The present study highlights the sustainability issues Indian carpet industry should be addressing in the current situation. India has been traditionally producing carpets since a very long time. It is one of the major cottage industry in *** carpets are exported world over and are retailed by world's leading retailers. It brings home foreign exchange and provides employment to millions of people. However, Indian carpet industry is also one of the biggest pollutants. The process of carpet production involves use of water and dyes which are often harmful for nature and human beings. Further, there have been concerns regarding unethical practices being used in carpet industry. Child labour is one of the most disturbing concern apart from differentiated wages and gender bias. This study analyses world's best carpet producer's commitment to the nature and society. It lists their efforts towards, reuse and recycle. It concludes that in order to remain on top among carpet exporters,Indian carpet manufacturers have a long way to go and it is high time that they start addressing all sustainability issues. In order to take the legacy of producing the world class carpets, Indian manufacturers have to strive hard to become more responsible towards our society and nature. They need to work towards efficient use of resources and raw materials and aim at zero wastage.

Because of the intimate association of obesity with type 2 diabetes mellitus(T2DM), during the last two decades, extensive research work is being conducted to find out whether the coexistence of the two is a simple association or there is a positive correlating link between the two. In this article, an attempt has been made to collect and analyse the recent developments in this field and to arrive at a conclusion on the subject. The possible role of several important factors(obtained from adipocytes/not of adipocyte origin) in linking the two has been discussed in detail. Some of the agents, specifically adiponectin, are beneficial(i.e., reduce the incidence of both), while others are harmful(i.e., increase their incidence). From the analysis, it appears that obesity and T2 DM are intimately linked.

Due to the grave pathological role of obesity, worldwide research is being continued to find out the causative factors involved in it. Recent advances in this field reveal a possible relationship between the compositional pattern of gut microbiota and genesis of obesity. Several study results have shown that short-chain fatty acids(SCFAs, microbiota-induced fermentation products) and lipopolysaccharides(LPS, an integral component of Gram negative microorganisms) play the key role in linking the two. Though several SCFAs are produced as microbiota-fermentation products, three of them, i.e., butyrate, propionate and acetate have been found to be definitely involved in obesity; though individually they are neither purely obesogenic nor antiobesogenic. Out of these, butyrate and propionate are predominantly antiobesogenic. Butyrate, though a major energy source for colonocytes, has been found to increase mitochondrial activity, prevent metabolic endotoxemia, improve insulin sensitivity, possess antiinflammatory potential, increase intestinal barrier function and protect against diet-induced obesity without causing hypophagia. Propionate has been found to inhibit cholesterol synthesis, thereby antagonizing the cholesterol increasing action of acetate, and to inhibit the expression of resistin in adipocytes. Moreover, both these SCFAs have been found to cause weight regulation through their stimulatory effect on anorexigenic gut hormones and to increase the synthesis of leptin. Unlike butyrate and propionate, acetate, which is substantially absorbed, shows more obesogenic potential, as it acts as a substrate for hepatic and adipocyte lipogenesis. High fat diet increases the absorption of LPS, which, in turn, has been found to be associated with metabolic endotoxemia and to induce inflammation resulting in obesity.
