Effects of progressive muscular relaxation and stretching exercises combination on blood pressure among farmers in rural areas of Indonesia:a randomized study

Objective:Maintaining blood pressure(BP)could improve the quality of life among farmers in agricultural *** study aims to evaluate the effects of progressive muscular relaxation and stretching exercises(SEs)for BP in farmer subjects in rural ***:A randomized controlled design was applied for this *** performed a method,which is the combination of progressive muscle relaxation(PMR)and SEs for participants(30 in the control group and 60 in the intervention group).The intervention group self-practiced PMR and SEs through a video that providing instructions for 15 *** practiced before going to sleeping in the night,and SEs practiced before going to farms in the morning per day for *** signed-rank test was performed to measure the difference between systolic blood pressure(SBP)and diastolic blood pressure(DBP)as one pre-and post-test comparison of baseline and 3 months data in control and intervention ***:There were no significant differences between SBP and DBP pre-and post-test in control group(P>0.050).Meanwhile,there were significant differences in reducing SBP(M=126.67;SD=18.07;95%CI=120-147.5 mmHg)and DBP(M=80.67;SD=6.91;95%CI=80-90 mmHg)pre-and post-test combination of PMR and SEs in intervention group(P<0.001).After 3-months of follow-up data,number type SBP and DBP still remained at the same levels of baseline and 3-month data in control ***,there was an increased number of normal and prehypertension for SBP and DBP(10%vs.10%and 20%vs.31.6%)and reduced of hypertension stage I for SBP and DBP(30%vs.41.6%).Conclusions:This pilot study demonstrated effectively to reduce SBP and DBP among farmers using the combination of PMR and SEs in the agricultural health setting.

Development and Evaluation of Disease Management Program and Service Framework for Patients with Chronic Disease

We developed a disease management program and service framework to enhance the self-management skills of patients with chronic diseases for the first time in Japan. In this study, we analyzed the efficacy of this program. A single-group pre-test and post-test design was used. In the program, disease management nurses provided self-management education to patients that were specific for their disease and risk factors in cooperation with their primary physicians. The duration of the program was 6 or 12 months. A total of 1258 patients were enrolled. After excluding discontinued subjects, 987 patients in the diabetes program and 11 patients (except those with diabetic nephropathy) in the chronic kidney disease (CKD) program were analyzed. The diabetic patients showed improvement of HbA1c and blood pressure after intervention, as well as maintaining renal function. CKD patients showed no change of renal function after intervention, but there was improvement of non high density lipoprotein-cholesterol (HDL-c). The patients acquired the ability to detect at an early stage and respond to abnormal symptoms and data, which enabled them to avoid progression of their diseases and emergency hospitalization. The program was effective at preventing disease progression in patients with chronic illnesses and may contribute to the containment of medical costs.

Health Resource Utilization and Comorbidities in Patients with Mental Disorders: Analysis Based on Health Insurance Claim Data

The number of patients with mental disorders is increasing all over the world and they have a high prevalence of physical complications. To analyze and compare the cost of mental disorders with the total medical costs such as mental disorders and physical disorders, we analyzed the cost that patients with mental disorders incurred. We investigated the distribution of medical costs and the characteristics of diseases by using the health insurance claims of people in City A, Japan from March 2013 to February 2014. The subjects had one or more of 4 mental disorders: schizophrenia, alcohol related disorders, bipolar disorder, and depressive episode. As a result, the total number of patients who met inclusion criteria per year was 7403 (6522 outpatients and 881 inpatients). It was revealed that the hospitalization rate of patients with mental disorders increased with age, and many inpatients stayed in hospital for a long time. Also, it was revealed that many patients with mental disorders were in complicated condition with more than one mental or physical disorders and incurred medical costs for these comorbidities. In conclusion, this analysis indicated that many patients with mental disorders switch from attending the outpatient department to hospitalization as they become older. Further, they incurred a lot of medical expenses for complication management. To improve their QOL, comprehensive assessment of their mental/ physical health, self-management education, coordination of services, and support for decision making regarding treatment are necessary.

Efficacy and Usability of an E-Learning Program for Fostering Qualified Disease Management Nurses

In order to train nurses to perform disease management and telenursing, we developed an e-learning education program, and assessed the efficacy. A single-group pre-test and post-test design was used. Nurses who worked at a medical institution or a disease management company were included, and the duration of the program was set 2 months. We developed the program so that it could grow attitude and improve knowledge and skills in disease management and patient education. Of 55 subjects, 48 who completed the program were analyzed. After the program, subjects increased knowledge and interests in disease management and patient education. Almost of the subjects answered that e-learning was a good learning method. Our program was effective at enhancing subject’s interests in disease management and patient education, and considered to improve their skills in the future.

Prediction of Low Heating Value of Sugar Cane Bagasse as a Fuel for Industrial Boilers in the High Relative Humidity Region: Case of Cameroon

Many attempts have been made to estimate calorific value of bagasse using mathematical equations, which were created based on data from proximate, ultimate, physical and chemical analysis. Questions have been raised on the applicability of these equations in different parts of the globe. This study was initiated to tackle these problems and also check the most suited mathematical models for the Law Heating Value of Cameroonian bagasse. Data and bagasse samples were collected at the Cameroonian sugarcane factory. The effects of cane variety, age of harvesting, source, moisture content, and sucrose on the LHV of Cameroon bagasse have been tested. It was shown that humidity does not change within a variety, but changes from the dry season to the rainy season;the sugar in the rainy season is significantly different from that collected in the dry season. Samples of the same variety have identical LHV. LHV in the dry season is significantly different from LHV in the rainy season. According to the fact that this study was done for cane with different ages of harvesting, the maturity of Cameroonian sugarcane does not affect LHV of bagasse. Tree selected models are much superior tool for the prediction of the LHV for bagasse in Cameroon compared to others. The standard deviation of these validated models is around 200 kJ/kg compared to the experimental. Thus, the models determined in foreign countries, are not necessarily applicable in predicting the LHV of bagasse in other countries with the same accuracy as that in their native country. There was linear relationship between humidity, ash and sugar content in the bagasse. It is possible to build models based on data from physical composition of bagasse using regression analysis.

Fruit Powder of Tetrapleura tetraptera in Local Feed on Some Zootechnical Performances of Juveniles of Clarias gariepinus in Concrete Tank

This study focused on the supplementation of four-sided spice fruit powder (Tetrapleura tetraptera) on some zootechnical performances of juveniles of Clarias gariepinus and was carried out from February 14 to July 20 at the Massoma Fish Farm of Bojongo Mbeidi located in the Littoral Region, Department of Wouri, Douala IV district. The farm benefits from a climate favorable to aquaculture activity. For this purpose, five (5) treatments were tested including T0+ (imported feed), T0-, T0.3, T0.4, and T0.5 respectively for the treatments with 0% supplemented food;0.3%;0.4% and 0.5% of Tetrapleura tetraptera powder. A total of 450 Clarias gariepinus fry with an initial average weight of 7 ± 1.5 g, were distributed in happas of 0.49 m2 each placed in Triplicate in a completely randomized device. Four isoprotein foods with 38% crude protein were formulated to feed these fry at 7% of their biomass per day for 15 days then to their fullness until the end of the experiment. Intermediate fishing was carried out every fortnight. 75 days later, the results recorded show that the highest average survival rate (96.44% ± 3.5%) was recorded with the subjects fed feed supplemented with 0.4% T. tetraptera powder. The average weight (120.93 ± 67.20), weight gain (113.93 ± 67.20), specific growth rate (3.64 ± 0.79% g/d) and total lengths and standard (26.43 ± 1.9 and 23.66 ± 1.76) the highest were recorded with the T0+ treatment (imported feed). The highest conditioning factor K (2.14 ± 0.15) was recorded with the treatment supplemented with 0.4% T. tetraptera. The lowest conversion ratio (1.28) was recorded with the treatment fed with imported blue crown food (T0+). Furthermore, the lowest production cost was recorded with the treatment supplemented with 0.4% T. tetraptera. 以氯化胆碱/尿素/微生物联合预处理秸秆纤维为原料，依次通过填充不同添加量的杨木纤维，调控湿强剂组合种类以及利用自主研发的纳米木质素聚合物作表面涂层，制备改性秸秆纤维基地膜，分析改性对可降解秸秆纤维基地膜力学性能、韧性、疏水性、降解性及水稻生长等的影响。研究结果表明：杨木纤维的填充可有效降低细小秸秆纤维流失率；优选杨木纤维与秸秆纤维质量占比4:6，硫酸铝添加量1%、阳离子分散松香胶（CDR）1.5%，纳米木质素聚合物表层涂布量2～2.5g/m2时，获得的改性秸秆纤维基地膜（MSFM）抗张指数为30.33N·m/g、湿抗张指数9.23N·m/g、撕裂指数5.16mN·m2/g、耐破指数2.36kPa·m2/g，应变由0.48%提高到1.13%，增幅达135.42%，且表现出优异的疏水性和湿稳定性。地膜降解试验结果表明：第9周时MSFM 失重率达62%，同时盆栽试验期杂草抑制率达81.20%。且具有良好的土壤保温及水稻促生作用。生物降解秸秆纤维地膜能够从源头减少污染物和化石资源投入，在农田土壤覆盖应用领域前景广阔。

Effects of Fresh Cupressus sempervirens Leaves Infusion on Growth Performance, Intestinal Microbiota and Haemato-Biochemical Parameters in Broilers

The accumulation of growth-promoting antibiotic residues in animal products and the resistance developed by bacteria in poultry farms has led to a search for natural compounds derived from plants. This study was designed to promote the production performance of broiler chickens using fresh Cupressus sempervirens leaves infusion. Fresh Cupressus sempervirens leaves were harvested, washed, chopped and ground to a paste using a blender and fermented for three days in a closed container at a rate of 500 g/L of water. The solution obtained was filtered and added at the rate of 2, 4, 6, 8 and 10 ml/L of drinking water. The chickens fed on the graded level of the solution were compared to a control ration without an additive and positive control group supplemented with 1 g antibiotic/kg feed. At the finisher phase and throughout the study period, water intake increased significantly (P 0.05) affected by the inclusion of Cupressus sempervirens infusion. Haematological parameters significantly (P < 0.05) increase independently of the rate of incorporation of the infusion into the drinking water, with the exception of RBCs, MCHT and PCT. Serum content in total protein, globulins, LDL cholesterol and triglycerides were significantly (P < 0.05) high with 8 and 10 ml Cupressus sempervirens infusion/litre drinking water as compared to all other treatments. AST, ALT, urea, creatine, albumin, total cholesterol and HDL-cholesterol were not significantly affected. The lactic acid bacteria load increased significantly (P E. coli and salmonella counts decreased significantly (P < 0.05)

An Effective Runge-Kutta Optimizer Based on Adaptive Population Size and Search Step Size

A newly proposed competent population-based optimization algorithm called RUN,which uses the principle of slope variations calculated by applying the Runge Kutta method as the key search mechanism,has gained wider interest in solving optimization ***,in high-dimensional problems,the search capabilities,convergence speed,and runtime of RUN *** work aims at filling this gap by proposing an improved variant of the RUN algorithm called the *** size plays a vital role in both runtime efficiency and optimization effectiveness of metaheuristic *** the original RUN where population size is fixed throughout the search process,Adaptive-RUN automatically adjusts population size according to two population size adaptation techniques,which are linear staircase reduction and iterative halving,during the search process to achieve a good balance between exploration and exploitation *** addition,the proposed methodology employs an adaptive search step size technique to determine a better solution in the early stages of evolution to improve the solution quality,fitness,and convergence speed of the original ***-RUN performance is analyzed over 23 IEEE CEC-2017 benchmark functions for two cases,where the first one applies linear staircase reduction with adaptive search step size(LSRUN),and the second one applies iterative halving with adaptive search step size(HRUN),with the original *** promote green computing,the carbon footprint metric is included in the performance evaluation in addition to runtime and *** results based on the Friedman andWilcoxon tests revealed that Adaptive-RUN can produce high-quality solutions with lower runtime and carbon footprint values as compared to the original RUN and three recent ***,with its higher computation efficiency,Adaptive-RUN is a much more favorable choice as compared to RUN in time stringent applications.
