Pattern of bony injuries among civilian gunshot victims at tertiary care hospital in Karachi, Pakistan

Purpose： Firearm injuries impose a continuous economic burden on society and hospital resources. The aim of this study was to assess the pattern of bony injuries among victims of gunshots. Methods： A retrospective study was conducted in the Department of Orthopedics, Dow University of Health Sciences and Civil Hospital l〈arachi from January 2011 to December 2012. Patients with isolated bony injuries were included while patients with other systemic injuries were excluded. Results： There were 90 cases and the majority of them were male （84.4%）. Mean age was （32.52 ± 10.27） years. Most of the patients （72.2%） belong to the younger age group. A low velocity weapon was used in 61 （67.8%） cases and a high velocity weapon was used in 29 （32.2%） cases. Armed robbery （64.4%） was the cause of conflict in more than half of the cases. Lower limb was involved in 72.2%. Fifty eight （64.4%） patients remained hospitalized for 15-20 days and others for more than 20 days. Internal fixation with intramedullary nailing was done in 35 patients while K-wire was used in 5 patients. Fifty patients were managed with external fixation, either uniplanar or multiplanar ilizarov. Deep wound infection and nonunion were observed more often in high velocity injuries. Conclusion： Armed robbery was the leading cause of gunshot bony injuries in our hospital. Young males were victimized in a majority of cases. High velocity injuries were associated with more complications.

A consensus meeting of national experts from all major national hepatobiliary centres in the country was held on May 26,2023,at the Pakistan Kidney and Liver Institute&Research Centre(PKLI&RC)after initial consultations with the *** Pakistan Society for the Study of Liver Diseases(PSSLD)and PKLI&RC jointly organised this *** effort was based on a comprehensive literature review to establish national practice guidelines for hilar cholangiocarcinoma(hCCA).The consensus was that hCCA is a complex disease and requires a multidisciplinary team approach to best manage these *** coordinated effort can minimise delays and give patients a chance for curative treatment and effective *** diagnostic and staging workup includes high-quality computed tomography,magnetic resonance imaging,and magnetic resonance *** cytology or biopsy utilizing endoscopic retrograde cholangiopancreatography is a mainstay for ***,histopathologic confirmation is not always required before *** ultrasound with fine needle aspiration of regional lymph nodes and positron emission tomography scan are valuable adjuncts for *** only curative treatment is the surgical resection of the biliary tree based on the Bismuth-Corlette *** patients with unresectable hCCA can be considered for liver *** chemotherapy should be offered to patients with a high risk of *** use of preoperative biliary drainage and the need for portal vein embolisation should be based on local multidisciplinary *** with acute cholangitis can be drained with endoscopic or percutaneous biliary *** chemotherapy with cisplatin and gemcitabine has shown improved survival in patients with irresectable and recurrent hCCA.

Influence of Seed Priming on Performance and Water Productivity of Direct Seeded Rice in Alternating Wetting and Drying

Direct seeded rice is promising alternative to traditional transplanting, but requires appropriate crop and water management to maintain yield performance and achieve high water productivity. Present study evaluated the effect of seed priming and irrigation on crop establishment, tillering, agronomic traits, paddy yield, grain quality and water productivity of direct seeded rice in alternate wetting and drying （DSR-AWD） in comparison with direct seeded rice at field capacity （DSR- FC）. Seed priming treatments were osmo-priming with KCI （2.2%）, CaCI2 （2.2%） and moringa leaf extracts （MLE, 3.3%） including hydro-priming as control. Among the treatments, seed osmo-primed with MLE emerged earlier and had higher final emergence, followed by osmo-priming with CaCI2. Tillering emergence rate and number of tillers per plant were the highest for seed priming with CaCI2 in DSR- AWD. Total productive and non-productive tillers, panicle length, biological and grain yields, harvest index were highest for seed priming with MLE or CaCI2 in DSR-AWD. Similarly, grain quality, estimated in terms of normal grains, abortive and chalky grains, was also the highest in DSR-AWD with MLE osmo-priming. Benefit cost ratio and water productivity was also the highest in DSR-AWD for seed priming with MLE. In conclusion, seed priming with MLE or CaCI2 can be successfully employed to improve the direct seeded rice performance when practiced with alternate wetting and drying irrigation.
