Association of risky driving behavior with psychiatric disorders among Iranian drivers:A case-control study

Purpose:This study aimed to investigate the possible association between psychological disorders and riskydriving behavior(RDB)in Iran.Methods:This case-control study conducted in Shiraz,Iran in 2021.The case group included drivers with psychological disorders and the control group included those without any disorders.The inclusion criteria for selecting patients were:active driving at the time of the study,being 18-65 years old,having a driving license,having a psychological disorder including depression,bipolar disorder,anxiety spectrum disorder,or psychotic disorder spectrum confirmed by a psychiatrist,and completing an informed consent form.The exclusion criterion was the existence of conditions that interfered with answering and understanding the questions.The inclusion criteria for selecting the healthy cases were:active driving at the time of the study,being 18-65 years old,having a driving license,lack of any past or present history of psychiatric problems,and completing an informed consent form.The data were gathered using a researcher-made checklist and Manchester driving behavior questionnaire.First,partition around medoids method was used to extract clusters of RDB.Then,backward logistic regression was applied to investigate the association between the independent variables and the clusters of RDB.Results:The sample comprised of 344(153 with psychological disorder and 191 without confirmed psychological disorder)drivers.Backward elimination logistic regression on total data revealed that share of medical expenditure≤10%of total household expenditure(OR=3.27,95%Cl:1.48-7.24),psychological disorder(OR=3.08,95%Cl:1.67-5.70),and substance abuse class(OR=6.38,95%CI:3.55-11.48)wereassociatedwithhighlevelof RDB.Conclusion:Substance abuse,psychological illnesses,and share of medical costs from total household expenditure were found to be main predictors of RDB.Further investigations are necessary to explain the impact of different psychological illnesses on driving behavior.

It is hypothesized that liver impairment caused by coronavirus disease 2019(COVID-19)infection might play a central role in severe clinical presentations.Liver injury is closely associated with severe disease and,even with antiviral drugs,have a poor prognosis in COVID-19 patients.In addition to the common hepatobiliary disorders caused by COVID-19,patients with pre-existing liver diseases demand special considerations during the current pandemic.Thus,it is vital that upon clinical presentation,patients with concurrent pre-existing liver disease associated with metabolic dysfunction and COVID-19 be managed properly to prevent liver failure.Careful monitoring and early detection of liver damage through biomarkers after hospitalization for COVID-19 is underscored in all cases,particularly in those with pre-existing metabolic liver injury.The purpose of this study was to determine most recent evidence regarding causality,potential risk factors,and challenges,therapeutic options,and management of COVID-19 infection in vulnerable patients with pre-existing liver injury.This review aims to highlight the current frontier of COVID-19 infection and liver injury and the direction of liver injury in these patients.

Invasive Fungal Sinusitis in Immunocompromised Patients: A Multicenter, University Hospital Experience in Shiraz

Objective: It is to determine the causes of invasive fungal sinusitis in patients of Shiraz University hospitals, Iran. Methods: This cross-sectional study was conducted during 18 months (from 21 March 2009 till 22 September 2010) in three Shiraz University Hospitals. Thirty-six patients with sings of invasive fungal sinusitis were enrolled, and tissue samples were investigated for histopathology, culture and antifungal susceptibility test. The laboratory results with host factor and sinus computed tomography scan were evaluated for classification of patients as proven, probable and possible invasive fungal sinusitis. Results: Thirty-five patients have involved with at least one risk factor (immune compromised disease, diabetes mellitus, or use of immune suppressed drugs). Radiological findings of parasinus invasion or necrosis were present in 20 patients. By histopathology, 21 patients were considered as proven, from these, 17 samples had positive growth. The culture aetiology agents were 4 Candida, 8 Aspergillus, and 5 Mucor. All positive culture samples were matched with histopathology findings. Significant associations were considered for radiologic finding and histopathology and culture (p 0.05). From 8 patients with mucormycosis histopathology, 6 suffered from diabetes mellitus. None of the antifungal agents were effective on these three types of infections. Conclusion: DM is the most common predisposing factor for IFS followed by ALL and AML. The most common aetiology of IFS was found to be Aspergillus fumigant followed by Mucormycosis and Candida. None of antifungal agents could successfully cover all the species.

AIM: To evaluate the gastric emptying inhibitory effects of sugar and levodopa on H pylori eradication period. METHODS: A total of 139 consecutive patients were randomized into 6 groups. The participants with peptic ulcer disease or non-ulcer dyspepsia non-responding to other medications who were also H pylori-positive patients either with positive rapid urease test (RUT) or positive histology were included. All groups were pretreated with omeprazole for 2 d and then treated with quadruple therapy regimen (omeprazole, bismuth, tetracycline and metronidazole); all drugs were given twice daily. Groups 1 and 2 were treated for 3 d, groups 3, 4 and 5 for 7 d, and group 6 for 14 d. Groups 1 to 4 received sugar in the form of 10% sucrose syrup. Levodopa was prescribed for groups 1 and 3. Patients in groups 2 and 4 were given placebo for levodopa and groups 5 and 6 received placebos for both sugar and levodopa. Upper endoscopy and biopsies were carried out before treatment and two months after treatment. Eradication of H pylori was assessed by RUT and histology 8 wk later.RESULTS: Thirty patients were excluded. Per-protocol analysis showed successful eradication in 53% in group 1, 56% in group 2, 58% in group 3, 33.3% in group 4, 28% in group 5, and 53% in group 6. Eradication rate, patient compliance and satisfaction were not significantly different between the groups. CONCLUSION: It seems that adding sugar or levodopa or both to anti H pylori eradication regimens may lead to shorter duration of treatment.

Cholangiocarcinoma(CCA)is a type of cancer with increasing prevalence around the world that originates from cholangiocytes,the epithelial cells of the bile duct.The tumor begins insidiously and is distinguished by high grade neoplasm,poor outcome,and high risk for recurrence.Liver transplantation has become broadly accepted as a treatment option for CCA.Liver transplantation is expected to play a crucial role as palliative and curative therapy for unresectable hilar CCA and intrahepatic CCA.The purpose of this study was to determine which cases with CCA should be subjected to liver transplantation instead of resection,although reported post-transplant recurrence rate averages approximately 20%.This review also aims to highlight the molecular current frontiers of CCA and directions of liver transplantation for CCA.

AIM: To investigate the co-incidence of apoptosis, autophagy, and unfolded protein response(UPR) in hepatitis B(HBV) and C(HCV) infected hepatocytes.METHODS: We performed immunofluorescence confocal microscopy on 10 liver biopsies from HBV and HCV patients and tissue microarrays of HBV positive liver samples. We used specific antibodies for LC3β, cleaved caspase-3, BIP(GRP78), and XBP1 to detect autophagy, apoptosis and UPR, respectively. AntiHCV NS3 and anti-HBs antibodies were also used to confirm infection. We performed triple blind counting of events to determine the co-incidence of autophagy(LC3β punctuate), apoptosis(cleaved caspase-3), and unfolded protein response(GRP78) with HBV and HCV infection in hepatocytes. All statistical analyses were performed using SPSS software for Windows(Version 16 SPSS Inc, Chicago, IL, United States). P-values 0.05). Process of becoming a mother in women receiving donated egg: Based on the grounded theory

Objective:To elucidate theoretically the safety and identity of mothers receiving egg and the process of becoming a mother.Methods:This study was conducted during 7 months from July 2018 to January 2019 by using a grounded theory approach.Infertile women at the gestational age who received oocyte were selected with purposive sampling method and then entered the study using theoretical sampling.Mothers who had not decided definitely to participate and had not started the treatment course yet were excluded from the study.The participating mothers were 28-44 years old with a mean age of(37.00±2.49)years.A total of 30 interviews were performed.Data were collected by unstructured deep interviews and field notes.The interview duration ranged between 19 to 74 min with a mean time of 40 min and they were performed individually.Results:Data analysis showed that"feeling of insecurity in personal and familial identity"formed in the context of"exposure to socio-cultural constraints"was the main problem of mothers receiving donated oocyte.This led to a set of coping strategies as"gradual acceptance","attempts to maintain the marital life","sensitivity in selecting donor","sensitivity in fetal care","seeking information and consultation","challenging the internal restlessness",and"treatment follow-up",all indicating the concept of"protection of personal and familial identity"as the core variable.Conclusions:The challenge faced by Iranian mothers receiving donated oocyte in the constrained sociocultural context forms a multilateral and all-inclusive insecurity.
