Objectives: 1) To calculate the ratio of maternal mortality. 2) To describe the socio-demographic characteristics of deceased patients. 3) To identify the main causes of maternal deaths. Methodology: This was a retrospective study of the 12-month period from January 1st to December 31st, 2015 performed at the Gynaecology Obstetrics Department of the Ignace Deen National Hospital, Conakry, Guinea. The study included women who died during pregnancy, childbirth, and in its peripheries according to WHO’s maternal death report. Results: We collected 38 cases of maternal deaths out of 4404 live births, accounting a ratio of 863 per 100,000 live births. The socio-demographic characteristics of these 38 patients were: 20 - 24 years of age (26%), married (78%), housewives (37%), students (44%), and nulliparous (29%), no prenatal follow-up (47%), and home-birth (49%). The 1st and 3rd type of delay amounted for 40% and 53%, respectively. Patients consulted after 12 hours after symptom-onsets accounted 47%, whereas those before 6 hours accounted for 19%, suggesting the delay of first medication. The final diagnosis and diagnosis at admission coincided in 69% of cases. The emergency kit was available for all. The opinion of a specialist was available in 16 patients. Blood was available in 40% of the patients who required it. Death caused by conditions directly related to pregnancy/delivery accounted for 71%. Haemorrhage was the most frequent cause of death. Death occurred within the first 24 hours of admission in 73% of cases. Conclusion: We here shed light on the maternal death in this area. Although we did not demonstrate the method/procedure to reduce this high rate of maternal mortality, the present study may provide a fundamental data to reduce maternal death in this area.

Indications and Results of Thyroidectomies in Northern Senegal

Goiters account for a very high prevalence disease. In Senegal, surgery is a very important part of the management. Our aim is to determine indications and results of thyroidectomies at the regional hospital of St. Louis. Patient and Method: We conducted a descriptive retrospective study over a period of 6 years on 81 records. All patients who underwent thyroidectomy during the study period were included. Computer software EPI INFO and Excel were used for collection and processing of data. Results: The average age of our patients was 40 years with a sex ratio at 0.05. The determination of TSH found 88.9% in euthyroid, 1.2% in hypothyroidism and 9.9% in hyperthyroidism. Thyroid ultrasound was performed in 97.5% of patients. Surgically, gestures performed were divided between the lobo-isthmectomies (43.1%), subtotal thyroidectomy (9.8%), total thyroidectomy (46.91%). The postoperative course was uneventful in all patients. The histology was benign in 88.9% of surgical specimens and found adenocarcinoma in 11.1%. Conclusion: Our study confirms the predominance of female thyroid pathology. The thyroidectomy which is getting better codified occupies an important place in the management. It offers the advantage around the problem of therapeutic non-compliance and also stockouts in our region.

Factors of Low Birth Weight Risk in the Department of Gynecology-Obstetrics of the Ignace Deen National University Teaching Hospital in Conakry, Guinea

Introduction: The underweight at birth is recognized as a major cause of morbidity and mortality in childhood. Objective: To identify maternal and obstetric sociodemographic factors associated with low birth weight. Patients and Methods: This is a retrospective study of analytical type that examined the records of women who gave birth in the Obstetrics and Gynecology department of Ignace Deen National University Teaching Hospital from 1st December 2016 to 30th April 2017. The analysis was made with the R version 3.3.1 software. We did a univariate and multivariate analysis. Outcomes: Out of the 1633 live births of single pregnancies that occurred during the study period, 109 children were born with a low weight (<2500 g) corresponding to a rate of 6.7%. In univariate analysis, we found a significant association between low birth weight and maternal single status (p = 0.019), maternal weight less than 60 kg (p = 0.038), primary parity (p = 0.018), maternal history of abortion (p = 0.001), history of preterm birth (p < 0.001), arterial hypertension (p < 0.001), anemia (p < 0.001) and malaria (p < 0.001). In multivariate analysis, the variables associated with low birth weight were: history of preterm delivery with OR of 8.5 [1.8 - 40.1], history of abortion (OR = 4.4 [1.4 - 13.9]), malaria (OR = 23.8 [6.1 - 92.5]), anemia (OR = 11.8 [3.7 - 38.2]) and high blood pressure (OR = 5.4 [1.6 - 17.9]). Conclusion: The decrease in frequency of low birth weight in Guinea will be done by improving the quality of prenatal care with an emphasis on screening, prevention and treatment of malaria, anemia and high blood pressure during pregnancy, prevention of abortion and premature birth.

Tele-Analytical Characterization of the Eburnean Tectonic Structures within the Birimian Formations of Mako Area, Eastern Senegal

The tectonic structures of the Mako area in the Kédougou-Kéniéba inlier (KKI) were mapped with tele-analytical investigation which is validated by field data. This study is based on different images processing in particular: 1) the colored combinations (1 to 5) and panchromatic (8) thematic mapping bands of the Landsat-8 (ETM+) satellite;2) Digital Elevation Model (DEM) image of Space Shuttle Radar Topography Mission (SRTM);3) airborne geophysics (aeromagnetic and radiometric) images. In these images, four major directions of lineaments NS, NNE-SSW, NW-SE and EW would be related to the tectonic structures, have been identified and mapped. Field data confirm these four structural directions and show that most of these lineaments are related to faults, shear and/or thrust zones, or even basic rock dykes. N-S to NE-SW oriented lineaments are more frequent followed by those NW-SE oriented. These two directions of lineaments are generally in correlation with trajectories of major sinistral shear zones in the Mako area. They would be associated with the S2a and S2b schistosities relative to the D2 Eburnean major deformation. Objectives: The objectives of this work were to calculate the frequency of obstetrical emergencies, to describe the socio-demographic profile of women admitted for obstetric emergencies, to identify the main emergencies, to describe the care taking of emergencies and to establish the maternal foetal prognosis of obstetric emergencies. Methodology: It was a 6-month descriptive prospective study conducted in the Obstetrics and Gynaecology Department of the Donka National Hospital, CHU Conakry, Guinea. The study took place from July 1st to December 31st, 2005. The data collected were entered and corrected using the Word and Excel 2010 software and then transferred to the Epi Info software version 7 for analysis. The results are presented in the form of tables, figures and texts using Word and Excel software, commented on, discussed and compared to current literature data. The limitations of the study: The poor filling of the partograph has been the main problem of our study. Results: The frequency of obstetric emergencies was 19% in the Department. The socio-demographic profile was that of a woman aged 15 to 24 (46.4%), married (92%), housewives (38.1%), out of school (49.5%), nulliparous (34.3%), without prenatal follow-up (47.37%), coming from home (56%), evacuated (44%). The main emergencies are dominated by haemorrhage (34.5%) followed by HTA Arterial hypertension and eclampsia (25.7%). The therapeutic attitude was based on clinical data and was dominated by caesarean section (70%). General anaesthesia was performed in 75% of cases and 1.6% benefited from local anaesthesia. The demand for blood was honoured in 19% of the cases. The maternal morbidity was dominated by anaemia (66.7%) and a lethality of 4%. After the 5th minute, 47% of the newborns had APGAR greater than 7. The neonatal mortality rate was 21%. Objectives: The objectives of this work were to calculate the frequency of the uterine rupture, to describe the epidemiological profile, to identify the etiologies and to establish the maternal prognosis and foetal. Methodology: It was about a descriptive survey with compilation of the data in two phases: a retrospective spreading on one period of 6 years and the other forecasting of 1 one year achieved to the service of Obstetric Gynecology of the hospital National Donka, Fallen from Conakry, Guinea. Results: We recorded 24.030 childbirths of which 188 cases of uterine rupture either a frequency of 0.78, which represents an uterine rupture for 128 childbirths. The epidemiological profile was the one of a woman of 24 to 28 years (31.91%), housewives (69.14%), without prenatal follow-up (47.87%), big multipare (37.76%) and évacuées (78.78%). The motives of consultation have been dominated by the hémorragie (95.74%). The rupture was of transverse type in the majority of the case (63.82%). The hysterorraphy was the most performed surgical procedure which is 85.10% followed by the total sub hysterectomy in 10.63%. The newborns of birth weight superior or equal to 4000 g represent 25.53%. The maternal morbidity has been dominated by the anemia of the postpartum (60%). We recorded a rate of maternal létalité of 12.76%. The maternal deaths were due to the hemorrhage in 78.57%. The living newborns endured a respiratory distress in 9.57% and those stillborn represent 87.23%. The etiologies of uterine rupture were dominated by fetal-pelvic disproportions 48.93% followed by an iatrogenic uterine rupture 22.33%. Conclusion: The reduction of this uterine rupture rate would pass by the recentered prenatal consultation offered, the one of obstetric cares and complete néonataux of emergency, the discount to level of the beneficiaries of the basic structures so that they can discover the cases in time susceptible to drag a rupture to evacuate better in time and the promptness in the

Study of the Biochemical Composition of Senegalese Mango Varieties Intended for Export and Local Consumation

Biochemical characterization was carried out on five mango varieties grown in Senegal, two of which are intended for export (Kent and Keitt) and three to local consumption, on the one hand, and to national marketing, on the other. It can be seen that the flavanols composition is between “0.74 ± 0.03 mg/100 g” to “4.70 ± 0.33 mg/100 g” and the total polyphenols composition is “265.83 ± 33.70 mg gallic acid/100 g” to “834.29 ± 25.78 mg gallic acid/100 g”. This analysis shows that the variety Bk is richer in polyphenols followed by the variety Dr then Knt and finally the lowest proportions are noted with the varieties Kt and SL. The analysis of antioxidant activity shows us percentages ranging from “4.16% ± 2.68% inhibition” to “50.21% ± 3.91% inhibition”. It was noted that the SL and Knt varieties give the highest inhibition percentages of “50.21 ± 3.91” and “33.97 ± 0.36” respectively. Variety Bk gives the lowest percentage followed by variety Kt. The Dr variety is the richest in flavonoids “11.75 ± 0.27” followed by the SL variety “11.27 ± 0.10” and the lowest composition is noted in the Knt variety “1.91 ± 0.04”. For total sugars, the most important contents are found in the varieties: Bk “12.71 ± 0.32 mg/100 g” followed by Dr “11.5 ± 0.33 mg/100 g” and SL “9.86 ± 0.06 mg/100 g”. This time the variety Kt stands out with also important proportions. For quantification, there is a slight difference in the values obtained for the different varieties studied. However, the highest values are observed in local varieties: “0.36 ± 0.06 g/100 g” for Bk and Dr followed by SL “0.31 ± 0.01 g/100 g”. The study of the mineral composition shows that the local varieties are the richest and the Knt variety is the poorest. The difference in composition noted within the same variety can be explained by the fact that some varieties have an advanced maturity compared to others.
