Unexpected Diagnosis of an Intraoperative Riedel’s Thyroiditis and Analysis of Current Data

Background: Riedel’s thyroiditis is a rare form of chronic inflammatory disease affecting the thyroid gland. It is characterised by the replacement of the thyroid parenchyma by fibrous tissue. It also affects the adjacent vital structures. Due to its characteristic presentations, Riedel’s thyroiditis may not be immediately diagnosed. Objectives: Through this clinical case, we aimed to describe the pathology and clinical characteristics of Riedel’s thyroiditis and the diagnostic, therapeutic, and progressive modalities. Methods: A 62-year-old woman with no particular pathological history is consulted at the outpatient clinic for anterior cervical swelling that has been present for more than 5 years and is associated with respiratory discomfort and dysphagia to solids. Clinical examination, ultrasound, lab tests and injected cervical computed tomography (CT) were performed. These tests were completed with pathological examination. Results: General conditions were normal. Anterior cervical swelling, marked on the right, of hard consistency, causing cervical shielding with healthy surrounding skin, was observed at the examination. A compressive goitre was first suspected. Thyroid tests showed hypothyroidism. The CT revealed a hypodense goitre with micro-calcifications developed at the expense of the right lobe and exerting a mass effect on the trachea and oesophagus. Aspiration thyroid was not conclusive given the suspicion of the malignant nature of the goitre, and the decision to perform a right loboisthmectomy was taken. Pathological examination of the biopsy showed atrophy of the thyroid parenchyma with dense septal fibrosis punctuated by lymphocytes and extending throughout the tissue associated with inflammation—an appearance in favour of Riedel’s thyroiditis. The patient was treated with Levothyroxine and corticoids. AIM:To compare objective dry retinoscopy and subjective refraction measurements in patients with mild keratoconus(KCN)and quantify any ***:This cross-sectional study was done on 68 eyes of 68 patients diagnosed with mild *** dry retinoscopy using autorefractometer and subjective refraction measurements were ***,cylinder,J0,J45,and spherical equivalent values were compared between the two ***:The mean age of 68 patients with mild KCN was 21.32±5.03y(12–35y).There were 37(54.4%)*** refraction yielded significantly more myopic sphere(-1.44 D vs-0.57 D),higher cylinder magnitude(-2.24 D vs-1.48 D),and more myopic spherical equivalent(-2.56 D vs-1.31 D)compared to subjective refraction(all P0.05).CONCLUSION:In patients with mild KCN,objective dry retinoscopy overestimates the degree of myopia and astigmatism compared to subjective *** irregular cornea in KCN likely impacts objective *** refraction allows compensation for irregularity,providing a more accurate *** determining refractive targets,the tendency of objective methods to overcorrect should be considered.

In the paper we propose a general, abstract framework for Automatic Secret Generation and Sharing (ASGS) that should be independent of underlying Secret Sharing Scheme (SSS). ASGS allows to prevent the Dealer from knowing the secret. The Basic Property Conjecture (BPC) forms the base of the framework. Due to the level of abstraction, results are portable into the realm of quantum computing. Two situations are discussed. First concerns simultaneous generation and sharing of the random, prior nonexistent secret. Such a secret remains unknown until it is reconstructed. Next, we propose the framework for automatic sharing of a known secret. In this case the Dealer does not know the secret and the secret Owner does not know the shares. We present opportunities for joining ASGS with other extended capabilities, with special emphasis on PVSS and pre-positioned secret sharing. Finally, we illustrate framework with practical implementation.

In this paper, we study the performance of a distributed power adjustment algorithm for shared and split spectrum allocation setups. The theoretical analysis reveals that the convergence of the power control algorithm is guaranteed under different spectrum allocation schemes and the convergence rate is exponential. The performance analysis is also carried out via simulations which demonstrate the algorithm fairness under Jain＇s and Atkinkons＇ fairness indices.

Malonylcarnitine in Newborns with Non-syndromic Cleft Lip with or without Cleft Palate

Aim Malonyl-CoA is regarded as a key signaling molecule in mammalian ceils. It is converted to acetyl-CoA, and to a lesser extent, to malonyl acid and malonylcamitine （C3DC）. Availability of carnitine has been reported to be essential for the developing fetus. The objectives of the present study were to analyze associations of malonylcarnitine, acetylcarnitine （C2）, and free carnitine （CO） in subjects with orofacial clefts. Methodology We performed a retrospective analysis of carnitine concentration obtained from a newborn screening program carried out in our institution. Concentrations of whole blood malonylcarnitine, acetylcarnitine, and free carnitine were measured using tandem mass spectrometry. The study group consisted of 51 children with nonsyndromic cleft lip with or without cleft palate. In total, 106 healthy children without congenital anomalies served as controls. Cut-off points were established using likeli-hood ratio values. Results The mean concentration of malonylcarnitine in the cleft group was lower than that of the control group, 0.048 μmol.L^1 vs. 0.058 μmol.L^-1, respectively （P=-0.009）. In patients with orofacial cleft, low malonylcarnitine levels （〈0.047 μmol.L-1） were 1.7 times more predominant than in healthy individuals （P=-0.03）. The mean concentration of acetylcarnitine was also lower in affected newborns in comparison to controls, 33.8 μol.L^-1 vs. 37.8 μmol·L^-1, respectively （P=-0.026）. After analysis of acetylearnitine and free carnitine concentrations, the likelihood ratio test did not indicate valuable cut-offpoints. Conclusion The study provides initial data indicating a potential association between decreased malonylcarnitine and abnormal palatogenesis.

Magnesia（MgO） is widely used in the production of refractory materials due to its high melting point,high thermal shock,and excellent slag *** properties of refractory materials depend upon magnesia sources and processing *** this work,three different magnesium sources,namely,magnesium hydroxide concentrate,magnesium carbonate concentrate,and intermediate flotation concentrate,were obtained from the Zinelbulak talc-magnesite,Uzbekistan,by causticization-hydration and flotation processes,respectively.A series of refractory materials were prepared on the basis of these magnesium sources,and their effects on physico-mechanical properties and microstructures were investigated as a function of sintering temperature,molding pressure,and the particle size of magnesium *** experimental results showed that a refractory material obtained from the magnesium hydroxide concentrate at 1600℃ for 4 h demonstrated favorable parameters due mainly to a higher degree of contact among fine *** results obtained from X-ray diffraction and optical microscopy confirmed the presence of periclase and forsterite as the predominant phases in refractory *** prepared refractory materials meet the requirements of the State Standards（Nos.4689-94 and 14832-96） for magnesia and forsterite refractories,*** is,therefore,suggested herein that the use of different magnesium sources derived from the Zinelbulak talc-magnesite will offer the potential to provide economic benefits in the refractory industry.
