A detailed mathematical procedure of the optimization of the fluid flow in a tundish water model with and without flow control devices （weir and dam） was carried out using the commercial CFD eode FLUENT 6.0. The （k- ε） two-equation model was used to model turbulence. The residence time distribution （RTD） curves were used to analyze the behavior of the flow in tundish. The location of flow control devices in the tundish was studied. The results show that the flow modifiers play an important role in promoting the floatation of nonmetallic inclusions in steel. Comparing the three geometric configurations that are considered （bare tundish, weir, weir＋dam）, the tundish equipped with the arrangement （weir＋dam） is a best and optimal geometric configuration of tundish.

Background: Metformin (M) is an effective first-line hypoglycemic agent in obese type 2 diabetes mellitus due to its low cost and safety profile. The Case: A 66-year-man presented with shock due to lactic acidosis induced by M-supersaturation subsequent to acute renal failure following infective diarrhea. The drug has been used, by this patient, for >10 years without complication. Physical examination, laboratory tests, radiological investigations and blood cultures did not show evidence of new cardiac, hepatic and septic insult. Despite discontinuation of M and 2-days of aggressive hydration, bicarbonate infusions and pressors;toxic levels of the drug persisted and shock-state culminated in severe and oliguric renal failure with serum urea and creatinine up to 50 mmol/L and 1270 umol/L, respectively. Hence, continuous venovenous hemodiafiltration (CVVHDF) was used, for 16-hours, to remove the drug, correct his acidosis and support his severe renal complications. Hours after the procedure;drug level, lactic acidosis and its associated shock improved followed by gradual renal recovery. The patient was discharged after 6 days and serum creatinine reached his base line (180 umol/L) 2 weeks later. The drug was not recommended for his future use. Conclusion: M-induced lactic acidosis, should be considered in assessment of shock in M-treated patients and management of unstable patients indicates early-use of CVVHDF.

The viscous dissipation effect on forced convection in a porous saturated circular tube with an isoflux wall is investigated on the basis of the Brinkman flow model. For the thermally developing region, a numerical study is reported while a perturbation analysis is presented to find expressions for the temperature profile and the Nusselt number for the fully developed region. The fully developed Nusselt number found by numerical solution for the developing region is compared with that of asymptotic analysis and a good degree of agreement is observed.

This paper introduces a recursive identification methods toolbox(called RIM) running under Matlab environment for dynamic system identification from available data. The RIM includes many methods which are generally used. The RIM helps users to validate the theoretical results and to carry out comparison between identifications methods without the need of algorithms programming. Furthermore, the RIM can be used as an education platform to study the identification parameters effect on model validity and results accuracy. To show its performance and capability, the RIM is evaluated through many application examples.

Background: Diabetes insipidus (DI) is a rare disorder characterized by inappropriate polyuria and hypo-osmolar urine. It is caused by inadequate production of antidiuretic hormone, in response to hypothalamic osmoreceptor-stimulation, from the pituitary gland (central DI) or resistance to its action at terminal distal convoluted tubules and collecting ducts (nephrogenic DI). Most cases of nephrogenic DI are caused by drugs, especially chronic lithium use. The Case: A 46-year-old man manifested such a disorder 8 months following d-Penicillamine (d-P) therapy for cystinuria. The drug was discontinued and the patient was managed conservatively with high fluid intake, diet low in protein and salt as well as alkalization of urine with Urolyte U to a pH > 7.5. Six weeks later, such side effect disappeared. Our patient had developed such phenomenon: a) without significant liver or renal disease to account for cumulative toxicity, and b) with a conventional dosage range of d-P. Such isolated toxicity indicates inherited a predisposition to this side effect. Conclusion: DI is a potential side effect of d-P therapy that is nephrogenic in site, transient in prognosis and an isolated phenomenon likely to reflect genetic predisposition.

Background: Hypertension (HTN) is present in up to 90% of end stage kidney disease (ESRD) patients irrespective of the etiology of their kidney disease. Moreover, it is an important modifiable risk factor for progression to ESRD and its overall cardiovascular morbidity and mortality. Objective: to evaluate, prospectively, the role of Renin-Angiotensin-Aldosterone System blockade (RAAS) in HTN, resistant to 3 conventional antihypertensives, in patients on maintenance hemodialysis (MHD). Patients and methods: A total of 52 such patients were treated with Ramipril and 5 with Losartan after intolerable cough/shortness of breath following Ramipril-use. None of the patients had fluid depletion, renal artery stenosis and primary endocrinopathy. The study group was compared to a matched control group of MHD patients with normal blood pressure following 3 drugs-combination therapies. Results: All patients, with resistant HTN, had significant activation of RAAS system prior to treatment compared to inactive one in the control group. In those with resistant HTN, control of HTN, was established within 2 weeks of therapy and was associated with suppression of the RAAS. Such therapy was associated with minor side effects. Conclusion: Our study has shown that RAAS blockade is safe and effective in controlling such resistant HTN in MHD patients.

This paper presents a novel adaptive mode switching scheme for hypersonic morphing aircraft with retracted winglets based on type-2 Takagi-Sugeno-Kang(TSK) fuzzy sliding mode control. For each of retracting and stretching modes, a specific sliding mode controller has been adopted. Drawing upon input/output linearization to globally linearize the nonlinear model of the hypersonic aircraft at first, a type-2 TSK fuzzy logic system is devised for robust mode switching between these sliding mode controllers. For rapid stabilization of the system, the adaptive law for mode switching is designed using a direct constructive Lyapunov *** results demonstrate the stability and smooth transition using the proposed switched control scheme.
