Agricultural Soil Fertilizing Potential of Dry Faecal Sludge from Treatment Plants in Burkina Faso

The dry faecal sludge (DFS) are potential sources of organic fertilizers because of their high content in nutrients and organic matter, critical for plants growth and soil health maintaining. In Burkina Faso, the DFS are processed in faecal treatment plants. However, after drying, the DFS are most often dumped in the nature without any control or directly used as fertilizer without any idea of their potential risks for human health and the environment. This investigation aimed at physico-chemical and toxicological characterization of the DFS from faecal treatment plants according to the duration of their storage. For this purpose, DFS samples were collected in three (3) faecal treatment plants in Ouagadougou and one in Bobo Dioulasso, in Burkina Faso. The measurements were carried out on pH (H2O), organic matter content, major nutrients (N, P, K), trace elements (Na, Ca and Mg) and metallic trace elements. Indifferently to the faecal treatment plants and the duration of the storage, the DFS showed strong acidity (4.85 ± 0.13 et 6.53 ± 0.10) and low content in total elements (Na à40.97% ± 9.99%). Values recorded of the trace metallic elements showed there were no risk of contamination when used as fertilizers: (103.9 ± 2.00 mg/kg < Cu < 137 ± 25.69 mg/kg);(710.13 ± 18.97 mg/kg < Zn < 922.30 ± 7.04 mg/kg);(33.03 ± 1.65 mg/kg < Pb < 152.40 ± 19.40 mg/kg);(1.34 ± 0.17 mg/kg < Cd < 1.76 ± 0.04 mg/kg);(34.34 ± 0.27 mg/kg < Ni < 52.32 ± 3.60 mg/kg) et (771.15 ± 18.36 mg/kg < Cr < 1697.83 ± 55.11 mg/kg). The results indicated a high fertilizer potential of the DFS after addressing the issue of their acidity.

On-Station and On-Farm Assessment of the Effects of Soil Cover on Conservation Agriculture Performances in Western Burkina Faso

Conservation agriculture (CA) is one option for dealing with such a challenge, but its main difficulty in West Africa is in maintaining permanent soil cover, particularly with crop residues, due to their preferential use for livestock fodder. The aim of our study was to determine the effect of crop residue mulching on the efficiency of cropping systems based on the principles of conservation agriculture. The experimental design was based on on-station experiments, intended to assess the specific effect of different levels of crop residue mulching on the biological and chemical soil fertility parameters, while in on-farm experiments we mainly assessed the ability of farmers to actually collect crop residues for soil cover and the associated effects on weeds and yields. The on-station experimental design was in fully randomized factorial blocks comprising five treatments and three replicates. The treatments compared the conventional system, tillage and cropping without mulching, and CA systems with 1 ton, 2 tons, and 4 tons of straw per hectare in plots of 10 m2. The effects of CA on the macrofauna, respiratory activity, and soil chemical parameters were evaluated in the 2014 and 2015 growing seasons. For the on-farm experiments, the conventional and CA practices of 15 farmers were compared to conventional practices in 2013 and 2014 in plots of 626 m2. The on-station results showed that the presence of crop residue mulching induced an increase in the density of termites. A significant release of carbon dioxide (CO2) in the CA treatments compared to the conventional treatment was also observed. For the chemical parameters (pH, N, P and K) were significantly and positively affected by mulch in the top 5 centimeters of soil. Double Arthrodesis, Postero-Medial Release and Posterior Tibial Transfer in One Step in Paralytic Inveterate Equine Varus Foot

Introduction: Varus equine foot deformity is common in developing countries. The management of these deformities is surgical in adults. Several surgical techniques have been described with more or less satisfactory results. To our knowledge, no study has been performed on the simultaneous association of double arthrodesis, posteromedial release, and posterior tibial transfer in a single operation in inveterate paralytic varus equines feet. The purpose of this work was to evaluate the results obtained. Patients and Method: This was a retrospective descriptive study from January 01, 2018 to December 31, 2021. It concerned inveterate paralytic varus equines feet operated on by the simultaneous association in a single operative time of double arthrodesis of the foot, posteromedial release of the back foot and transfer of the posterior tibial muscle to the lateral cuneiform. We identified seven patients with a mean age of 22.1 years with extremes of 11 years and 36 years. There were three males and four females. The cause of the deformity was neurological in all cases. All patients had painful walking discomfort and shoeing difficulties. The average time to management was 13.3 years with extremes of 4 and 25 years. The chronology of the interventional steps was posteromedial release, arthrodesis, and transfer of the posterior tibial muscle to the lateral cuneiform. The average postoperative follow-up was 21.7 months with extremes of 6 and 48 months. The parameters studied were the duration of the procedure, complications related to the procedure, muscle strength at the last recoil, consolidation of the arthrodesis, residual pain, patient activity, gait perimeter, stepping, ankle mobility, residual deformity, footwear, protrusion of the transferred tendon, and the possibility of walking on the heel. Effects of Direct Sowing under Mulch-Based Cropping System (DMC) on Cotton and Maize Yield and Chemical Characteristics of Ferruginous Soil (Lixisoil) in the South Sudan Area of Burkina Faso

To better understand the effects of direct sowing under mulch-based cropping system (DMC) in Burkina Faso’s cotton production systems, randomized blocks of Fisher experimental design were implemented at Farako-Ba research station from 2010 to 2019. The study was conducted on lixisoil to evaluate DMC effects on biomass production, crops yields and soil chemical properties in a maize and cotton rotation system associated with cover crop. Conventional tillage and direct seeding without cover crop were compared to DMC under B. ruziziensis (GERM. & EVRARD), DMC under B. ruziziensis + M. cochinchinensis mulch and DMC under C. juncea (L.) mulch used in association with maize. Biomass production, crop yields and soil chemistry were evaluated. Results showed that over 10 years, no-till with or without a cover crop provided cotton seed and maize yields that were statistically equivalent to the tillage commonly practiced by farmers. Cover crop has allowed increasing the biomass production compared to Conventional Tillage and Direct Seeding. Maize yield has not varied significantly with the cover crop. After 10 years of maize and cotton rotation, the improvement raised from +27% to +38% for organic matter and from +15% to +29% for nitrogen with DMC including legumes such as M. cochinchinensis and C. juncea compared to Conventional Tillage on 0 - 5 cm depth. No significant differences were found on soil pH like P2O5 and K2O content. Although DMC with C. juncea used as cover crop did not provide the best biomass production, it contributes to increase nitrogen and organic matter and presents better mineral balances in 10 years of rotation. The 5 - 10 cm and 10 - 20 cm were little influenced by DMC systems.

Olecranon Bone Tumor Complicated by Ulnar Nerve Paresis Revealing Tuberculous Osteitis—About a Case

Introduction: The localization of bone tuberculosis at the level of the olecranon is rare and can pose a problem of differential diagnosis with a tumoral affection. Only the biopsy allows the diagnosis of certainty. The goal was to report our first case to do a review of the literature. Clinical Observation: This was a 64-year-old patient who consulted 2 years after the onset of symptoms for swelling and mechanical pain in the left elbow. On clinical examination there was a firm consistent mass measuring 6/4 cm in diameter, not very painful and hot on the posterior surface of the left elbow, accompanied by paresthesias in the territory of the ulnar nerve associated with partial functional impotence of the left forearm with little altered general condition. The X-rays of the left elbow showed extensive bone lysis of the olecranon with fracture of the base of the olecranon. The CT Scan of the elbow performed showed osteolysis of the olecranon with extensive bone reactions at the distal end of the humerus. The biopsy carried out with histological examination concluded with an aspect of tuberculous osteitis and the culture came back sterile. A curettage was performed associated with anti-tuberculosis treatment for 12 months as well as the placement of a posterior splint. The evolution at 9 months was unsatisfactory with persistence of paresis and stiffness of the elbow. Conclusion: Tuberculous osteitis of the olecranon can simulate a malignant tumor with non-specific signs of bone tuberculosis. Only the anatomopathological examination is definitive for the diagnosis. The treatment is multidisciplinary. Orthopedic evolution is difficult to predict.

Management of Spinal Injuries Secondary to Rockfall in Traditional Mines

Introduction: Spinal injuries represent the whole of the mechanisms responsible for vertebral, disco-ligamentary and spinal cord injuries. Spinal cord injury is life-threatening and is responsible for functional consequences that make the subsequent socio-economic integration of victims difficult. In developing countries, spinal injuries in traditional gold mining are becoming more and more frequent. Purpose: The purpose of this study was to describe the anatomopathological aspects and the results of surgical treatment of spinal injuries secondary to traditional mine collapses at the University Hospital of Kati. Patients and Methods: This was a prospective, descriptive study over 18 months, from November 2017 to May 2019. Included in this study were patients operated on for spinal trauma secondary to rockfall in traditional mines. The neurological status was assessed in each patient since admission to the department using the Fränkel score. Decompression by laminectomy combined with titanium device placement was preferred in lumbar, thoracic and thoraco-lumbar surgery. We performed an anterior cervical spine approach with SENEGAS plate placement in cervical injuries. Motor physiotherapy was associated whenever recovery was not complete. At a minimum follow-up of 6 months, all our patients were re-evaluated by the Fränkel score. Results: The mean age was 31 years with extremes of 10 and 49 years. The majority of patients were men, 95%. Twenty patients underwent surgery. According to the spinal segments, the cervical spine was affected in 05 cases, the thoracic spine in 08 cases and the lumbar spine in 17 cases. The neurological examination revealed tetraplegia (n = 5), paraplegia (n = 12) and paraparesis (n = 3). According to the types of lesions, we collected 12 cases of fractures, 4 cases of fracture-luxations and 04 cases of pure luxations. Tonsillitis and Their Complications: Epidemiological, Clinical and Therapeutic Profiles

Tonsillitis or angina, is considered as an inflammation of the palatine tonsils most often of infectious origin. It can be acute or chronic. The germs in question are mostly viruses and bacteria. Objectives: The objective of this work was to study the epidemiological and therapeutic profile of tonsillitis and their complications in the ENT Department and Head and Neck Surgery of the Mother-Child University Hospital in Luxembourg. Patients and Methods: This is a prospective study over a 12-month period from January 2018 to December 2018, including all patients who presented with tonsillitis and/or their complications. Results: Three hundred and fifteen (315) patients were collected during this period. The average age in our study was 14.25 years with extremes ranging from 2 years to 61 years. The sex ratio was 0.65 in favor of the female sex. During this period we recorded 80.95% of cases of uncomplicated tonsillitis including 60.50% for acute tonsillitis, 24.5% for chronic tonsillitis 5% for adenoid tonsillitis and 19.05% for complications. Complications included peritonsillar phlegmon 42.22%, heart disease 33.33%, cervical cellulitis 8.89%, adeno-phlegmon 6.67%, para-pharyngeal abscess 4.44%, and sepsis 4.44%. The exclusive medical treatment was performed in 44.31%. Surgical treatment (drainage incision and tonsillectomy) was performed in 55.69%. Conclusion: Tonsillitis and its complications represent an important pathology in ENT. Feynophagy febrile is the pair-origin symptom that can lead to the diagnosis. The prevention of complications is the guarantor of the reduction of its morbidity. We recommend that in the absence of RDT (Rapid Diagnostic Test), it is necessary to systematize antibiotic therapy to prevent serious complications.

Introduction: Post-intubation tracheal stenosis (STPI) is a complication of ventilatory assistance by intubation and/or tracheotomy. Her frequency is estimated between 10% to 20%. The treatment of choice remains surgery. It is based on tracheal resection-anastomosis which guarantees satisfactory and reliable long-term results. Objectives: To determine the frequency of post-intubation tracheal stenosis, to specify the diagnostic and therapeutic methods, to determine the complications and the evolution. Material and methods: This was a study carried out in the ENT and head and neck surgery department of the University Hospital Center Gabriel TOURE. Our study focused on two (02) cases of post-intubation tracheal stenosis (STPI) collected during a period of 1 year (January 2021 to December 2021). Results: They are all acquired, secondary to an intubation. The two (02) cases benefited from a tracheal anastomosis resection, the indications of which were specified and the results and complications evaluated. Conclusion: Our results show that resection anastomosis remains the reference treatment for STPI.

Fracture of the Humeral Palette in Adult Patient: Therapeutic and Evolutive Aspect at Gabriel Touré Hospital: About 35 Cases

Introduction: Fractures of the humeral pallet occur between the distal insertion of the anterior brachial muscle and the joint space of the elbow. It represents 2% of all elbow fractures. These lower humeral fractures present a therapeutic challenge to trauma. These fractures are in the majority of joint cases and can be accompanied by loss of substance, which makes their functional prognosis random despite the application of the main therapeutic principles: exact reduction with possible bone graft;stable osteosynthesis, usually allowing early rehabilitation. This work focused on identifying therapeutic aspects and assessing treatment outcomes. Material and Methods: This was a descriptive cross-sectional study of patients with a fracture of the humeral palette from January 2015 to December 2019 at the Orthopaedic Trauma Department CHU Gabriel Touré Bamako. Results: We collected 35 cases of fractures of the humeral palette. The average age was 39.25 years with extremes of 22 and 74 years. The male gender predominated (71.4%) with a sex ratio of 2.5. Etiologies were dominated by falls with 19 cases (54.3%) and stroke with 13 cases (37.1%). The standard X-ray of the face and profile of the elbow was performed in all our patients. The most frequent types of anatomoradiology were supra condylar fractures type A in 16 cases (45.7%), supra and inter condylar fractures type C in 15 cases (42.9%);4 cases of uni condylar fractures type B (11.4%). Associated lesions were observed in 42.85%. Surgical treatment was performed in 80% of cases. Complications were dominated by elbow stiffness in 18 cases (51.42%), secondary displacement in 5 cases (14.28%), ossifications in 4 cases (11.42%). At an average decrease of 3.2 years our results were considered good in 45.7%. Conclusion: Fractures of the humeral palette are relatively common with associated lesions. The complexity of the fracture with associated lesions makes treatment difficult with unpredictable evolution.

Surgical Management of Unstable Superior Femoral Epiphysiolysis with Great Displacement

Introduction: Superior femoral epiphysiolysis (SFE) is a most progressive but sometimes abrupt displacement of the femoral head relative to the superior femoral metaphysis through the cervicocerebral growth plate of the femur. The displacement of the femoral head is most often downward and backward. It is a typical pathology of the pubescent adolescent, most often overweight. It occurs on average at the age of 12 years in girls and 14 years in boys. Unstable forms with large displacements have a high risk of femoral head necrosis and chondrolysis. Although in situ fixation is the generally accepted treatment for minor SFE, the treatment of more severe cases remains controversial. When the extent of the displacement makes it impossible to pass a screw between the femoral neck and the femoral head, the only option is to reduce the displacement. This reduction must be gentle, progressive and limited to the minimum necessary for osteosynthesis. Objective: The aim of the work was to evaluate the results of the technique of progressive reduction of epiphyseal displacement by transtibial traction followed by percutaneous fixation of the femoral head by screw. Patients and Method: This was a retrospective descriptive study over a 10-years period from January 1, 2013 to December 31, 2022. It focused on the surgical treatment of unstable upper femoral epiphysiolysis with large displacement in the orthopaedic trauma department of the University Hospital of Kati. All patients operated on in our department for large displacement SFE were included in this study. Cases of secondary large displacement upper femoral epiphysiolysis and patients who had already undergone surgery on the proximal femur were excluded. Continuous progressive traction on the Boppe splint through a pin under the anterior tibial tuberosity was performed for 15 days in all patients. The hip was flexed to 45° and the knee to 35°.
