Protection of inactivated vaccine against SARS-CoV-2 infections in patients with comorbidities:a prospective cohort study

Protection against severe acute respiratory syndrome Coronavirus 2(SARS-CoV-2)infection of inactivated vaccines is not well characterized in people with comorbidities,who are at high risk of severe infection.We compared the risk of SARS-CoV-2 infection after complete vaccination with Sinopharm/BBIBP in people with comorbidities(e.g.,autoimmune diseases,cardiovascular disease,chronic lung disease,and diabetes)with healthy individuals using a Cox-proportional hazard model.In July-September 2021,a total of 10548 people(comorbidities,2143;healthy,8405)receiving the complete primary series of vaccination with Sinopharm/BBIBP in Bangkok,Thailand were prospectively followed for SARS-CoV-2 infection through text messaging and telephone interviewing for 6 months.A total of 295 infections from 284 participants were found.HRs(95%CI)of individuals with any comorbidities did not increase(unadjusted,1.02(0.77–1.36),P=0.89;adjusted,1.04(0.78–1.38),P=0.81).HRs significantly increased in the subgroup of autoimmune diseases(unadjusted,2.64(1.09–6.38),P=0.032;adjusted,4.45(1.83–10.83),P=0.001)but not in cardiovascular disease,chronic lung disease,or diabetes.The protection against SARS-CoV-2 infection of the Sinopharm vaccine was similar in participants with any comorbidities vs.healthy individuals.However,the protection appeared lower in the subgroup of autoimmune diseases,which may reflect suboptimal immune responses among these people.

The Quest for Understanding Cutaneous Leishmaniasis in Northern Province,Sri Lanka:An Analysis of Clinical Data From the District General Hospital,Vavuniya

The study aimed to examine the clinical and epidemiological patterns of cutaneous leishmaniasis(CL)in patients attending the Dermatology Unit,District General Hospital in Vavuniya,Sri Lanka.A total of 77 patients clinically suspected for CL were interviewed through a structured questionnaire,and skin-lesion samples were obtained between January 2016 and January 2017.The definitive diagnosis of CL was made through microscopic identification of smears,histopathological examination of biopsies and kDNA PCR.Treatment modalities were chosen based on the location of the lesions on the body and complexity of the lesions.Of 77 suspected patients,68 were confirmed for CL,with a mean age of 34.6(±12.7)years,and included 54 males(79.4%)and 14 females(20.6%).Being a male was a significant risk factor(P=0.032,OR=4.82)associated with CL.Lesions were observed mainly on the exposed areas of the body,of which the forearm(22.1%)was the most commonly affected site.Single lesions(75.0%)and ulcerated nodules with central crust(39.7%)were the prominent features among the infected group.The age group of 21–40 years was significantly associated with healing lesions(P=0.028,marginal effect=0.55).However,a significant negative relationship was detected between ulcerated nodular lesions and lesion healing(P=0.0436,marginal effect=−0.375).Males are at higher risk of CL.Early diagnosis and specific treatment,along with preventive measures such as protective clothing and sand fly repellents,can limit the spread of the disease.There is a need for a comprehensive approach to prevent and control the transmission of CL.

Human Facial expressions exhibits the inner personality.Evaluating the inner personality is performed through questionnaires during recruitment process.However,the evaluation through questionnaires performs less due to anxiety,and stress during interview and prediction of leadership quality becomes a challenging problem.To the above problem,Temporal sequence based SENet architecture(TSSA)is proposed for accurate evaluation of personality trait for employing the correct person for leadership position.Moreover,SENet is integration with modern architectures for performance evaluation.In Proposed TSSA,face book facial images of a particular person for a period of one month and face images collect from different social environments and forms the sequential facial image database are analysed for personality trait estimation.Now a days,Facebook plays a vital role,where people express their emotions by posting images and updating their proﬁle pictures.In TSSA method,50 Facebook temporal sequence of images of person with answered questionaries during the face image collection forms as a Temporal sequence image(TSI)database for prediction of the Big Five personal-ity trait.In order to get precise prediction,we have analysed the face images that were posted in a period of one month and validated the result with the next month face images from face book.Face images for predicting the personality,where asked toﬁll the Questionnaires through Google Forms increase the accuracy in prediction.The TSSA prediction results are utilized for assessment of a person’s conscientiousness for leadership quality suitability.The study implements Deep Learning algorithm with SENet architecture and compares with traditional algo-rithms.From the validation results the proposed TSSA method performs 96%of accuracy in conscientiousness prediction.

Aphasia is an acquired neurogenic language disorder that makes patients unable to comprehend or formulate language.Along with language impairment,patients suffering from aphasia face difficulties in reading,writing,and expressing themselves.The causes of aphasia are stroke,trauma,brain tumors,infections,and injury,particularly to the left hemisphere.About 20%of acute stroke cases in the world account for aphasia.Aphasia is categorized into:non-fluent and fluent forms.Non-fluent is further classified based on language and comprehension into:1)Broca’s aphasia,2)transcortical motor aphasia,3)global aphasia,and 4)transcortical mixed aphasia.

Introduction Intussusception happens when one part of the intestine invaginates into another adjacent part[1].Transanal protrusion of intussusception,which is also known as intussusception prolapse,is described as the protrusion of the top of the intussusception from the anus.This presentation accounts for 8%–29%of intussusceptions[2–4].Intussusception is rare in adults and commonly occurs as a complication for patients with organic diseases.Our case is a transanal protrusion of intussusception,which has occurred in the ileorectal anastomosis site.This makes the case unique.

AIM:To determine the long-term postoperative outcomes of deep sclerectomy-trabeculectomy(DST)with mitomycin C(MMC)in the treatment of glaucoma.METHODS:Patients who underwent DST with MMC between 2010 and 2017 were included in this retrospective observational study.Complete success was defined as postoperative intraocular pressure(IOP)≤21 mm Hg or 30%reduction of IOP from baseline without any topical IOP-lowering agent,and qualified success defined as IOP≤21 mm Hg or 30%reduction of IOP from baseline with/without single topical agent.We evaluated the surgical success rates and complication rates of this procedure,as well as described the IOP profiles,best corrected visual acuity(BCVA)profiles and mean deviations(MD)of Humphrey visual field(HVF)24-2 performance at each follow-up time point.Mixed linear regression models were constructed to determine estimated predictive values of demographic data,use of topical IOPlowering agents,baseline and postoperative IOP and optical profiles(e.g.,BCVA and MD).RESULTS:Totally 98 eyes(mean postoperative followup 67.5mo)showed mean IOP reduction at every followup interval.Both median BCVA and MD of visual fields were maintained throughout the follow-up intervals when comparing to baseline.The number of IOP-lowering medications decreased from 2.8±0.8 to 0.3±0.7(P=0.068).Totally 84(85.7%)eyes achieved complete success at final follow-up.Transient hyphaema and transient choroidal effusion developed in 15 eyes(15.3%)and 11 eyes(11.2%)respectively.Other complications included shallow anterior chamber in 5 eyes(5.1%),bleb leak in 4 eyes(4.1%),bleb revision in 7 eyes(7.1%),bleb needling in 9 eyes(9.2%)and repeat trabeculectomy in 1 eye(1.0%).There was no endophthalmitis,blebitis or macular oedema.There was no significant correlation between postoperative IOP control and postoperative BCVA.CONCLUSION:DST with MMC demonstrates effective and sustained long-term outcomes in the treatment of glaucoma with no major complication.
