Safety and Efficacy of Ultra-Thin, Biodegradable Polymer Coated Sirolimus-Eluting Supralimus-Core Stents in Real-World Patients: Outcomes at 24-Month Follow-Up

Aim: The purpose of this registry was to establish long-term safety and efficacy through implantation of Supralimus-Core sirolimus-eluting stents (SES) in real-world patients with coronary artery disease (CAD). Methods:The present registry was a retrospective, singe-arm, single-centre, investigator-initiated registry. A total of 372 consecutive patients were implanted with Supralimus-Core SES between January 2015 and November 2016. The primary endpoints were major adverse cardiac events (MACE), a composite of cardiac death, myocardial infarction (MI), target lesion revascularization (TLR) and target vessel revascularization (TVR) at 24 months. The secondary endpoints were all-cause death and all separate components of the primary endpoint. Additional endpoints included events of stent thrombosis classified as definite, probable, and possible stent thrombosis. Follow-ups were conducted at 30-days, 6-months, 12-months and 24-months after the index procedure. Results: The mean age of the registry population was 56.3 ± 11.1 years. Males constituted 276 (74.2%) patients. Hypertensives, diabetics, alcoholics, tobacco chewers and smokers comprised 198 (53.2%), 160 (43.0%), 93 (25.0%), 91 (24.5%) and 88 (23.7%) of the registry population, respectively. The mean length and diameter of stents implanted was 19.3 ± 8.8 mm and 2.9 ± 0.3 mm, respectively. At the 24-month follow-up, MACE was reported in 14 (3.8%) patients. Cardiac death, MI, TLR and TVR was reported in 7 (1.9%), 4 (1.1%), 3 (0.8%) and 4 (1.1%) patients, respectively. Overall stent thrombosis occurred in 4 (1.1%) patients. Conclusions: The low MACE rate of 3.8% at the 24-month follow-up indicates favorable long-term results after implantation of the ultra-thin strut Supralimus-Core SES in all-comer, real-world patients.

The mountains of Southwest China comprise a significant large mountain range and biodiversity hotspot imperiled by global climate *** high species diversity in this mountain system has long been attributed to a complex set of factors,and recent large-scale macroevolutionary investigations have placed a broad timeline on plant diversification that stretches from 10 million years ago(Mya)to the *** our increasing understanding of the temporal mode of speciation,finer-scale populationlevel investigations are lacking to better refine these temporal trends and illuminate the abiotic and biotic influences of cryptic *** is largely due to the dearth of organismal sampling among closely related species and populations,spanning the incredible size and topological heterogeneity of this *** study dives into these evolutionary dynamics of speciation using genomic and ecomorphological data of Stellera chamaejasme *** identified four previously unrecognized cryptic species having indistinct morphological traits and large metapopulation of evolving lineages,suggesting a more recent diversification(~2.67-0.90 Mya),largely influenced by Pleistocene glaciation and biotic *** factors likely influenced allopatric speciation and advocated cyclical warming–cooling episodes along elevational gradients during the *** study refines the evolutionary timeline to be much younger than previously implicated and raises the concern that projected future warming may influence the alpine species diversity,necessitating increased conservation efforts.

Mitigating People-Park Conflicts on Resource Use through Ecotourism:A Case of the Nanda Devi Biosphere Reserve,Indian Himalaya

Eeotourism aims to deliver the philosophy of sustainable development through environmental conservation as well as socio-economic development. Areas with rich biodiversity and scenic beauty, like the Indian Himalayan region, have been one of the major destination centers for nature lovers from historical time. In view of conserving the rich biodiversity and natural heritage of the Himalayan region 92 protected areas have been set aside, of these Nanda Devi Biosphere Reserve （NDBR） is a well known world heritage site. Over a period of time establishing NDBR has invited conficts between the local people and reserve authorities on the management approaches and utilization criteria of the natural resources, especially banning the traditional bio-resource collection rights of local community, expeditions, trekking, and other anthropogenic activities in the core zones. The present study aims to understand and analyze the various people - park conflicts, and also to suggest ways to mitigate such conflicts in view of biodiversity conservation and improvement of economic status of local people by promoting eeotourism. With this view the study further discusses the various aspects of eeotourism, its pros and cons, and the avenues of developing appropriate strategies for promotion of ecotourism in buffer zone areas of NDBR.

Determining bioclimatic space of Himalayan alder for agroforestry systems in Nepal

Himalayan alder species are proven to be very useful in traditional as well as contemporary agroforestry practice. These nitrogen-fixing trees are also useful in the land restoration. Therefore, understanding the distribution of Himalayan alder and the potential zone for plantation is meaningful in the agroforestry sector. Suitable climatic zones of Alnus spp. were modelled in Max Ent software using a subset of least correlated bioclimatic variables for current conditions(1950 -2000), topographic variables(DEM derived) and Landuse Landcover(LULC) data. We generated several models and selected the best model against random models using ANOVA and t-test. The environmental variables that best explained the current distribution of the species were identified and used to project into the future. For future projections, ensemble scenarios of climate change projection derived from the results of 19 Earth System Models(ESM) were used. Our model revealed that the most favorable conditions for Alnus nepalensis are in central Nepal in the moist north-west facing slope, whereas for Alnus nitida they are in western *** major climatic factor that contributes to Alnus species distribution in Nepal appears to be precipitation during the warmest quarter for A. nepalensis and precipitation during the driest quarter for A. nitida. Future projections revealed changes in the probability distribution of these species, as well as where they need conservation and where they can be planted. Also, our model predicts that the distribution of Alnus spp. in hilly regions will remain unchanged, and therefore may represent sites that can be used to revitalize traditional agroforestry systems and extract source material for land restoration.

Predicting the impact of climate change on the distribution of two threatened Himalayan medicinal plants of Liliaceae in Nepal

Predicting the potential distribution of medicinal plants in response to climate change is essential for their conservation and management. Contributing to the management program, this study aimed to predict the distribution of two threatened medicinal plants, Fritillaria cirrhosa and Lilium nepalense. The location of focal species gathered from herbarium specimen housed in different herbaria and online databases were geo-referenced and checked for spatial autocorrelation. The predictive environmental variables were selected, and MaxEnt software was used to model the current and future distributions of focal species. Four Representative Concentration Pathway （RCP） trajectories of the BCC-CSMI.1 model were used as the future （2050） projection layer. The MaxEnt modelling delineated the potential distribution of F. cirrhosa and L. nepalense. The current suitability is projected towards Central and Eastern Hilly/Mountainous regions. Both species gain maximum suitability in RCP 4.5 which decline towards other trajectories for L. nepalense. Overall, both the focal species shift towards the north-west, losing their potential habitat in hilly and lower mountainous regions by 2050 across all trajectories. Our results highlight the impact of future climate change on two threatened and valuable species. The results can be further useful to initiate farming of these medicinally and economically important species based on climatically suitable zone and for designing a germplasm conservation strategy.

Molecular phylogeny,biogeography and character evolution of the montane genus Incarvillea Juss.(Bignoniaceae)

The complex orogeny of the Himalaya and the Qinghai-Tibet Plateau(QTP)fosters habitat fragmentation that drives morphological differentiation of mountain plant ***,determining phylogenetic relationships between plant subgenera using morphological characters is ***,we used both molecular phylogeny and historical biogeographic analysis to infer the ancestral states of several vegetative and reproductive characters of the montane genus *** determined the taxonomic position of the genus Incarvillea within its family and inferred the biogeographical origin of taxa through Bayesian inference(BI),maximum likelihood(ML)and maximum parsimony(MP)analyses using three molecular data sets(trnL-trnF sequences,nr ITS sequences,and a data set of combined sequences)derived from 81%of the total species of the genus *** the genus-level phylogenetic framework,we examined the character evolution of 10 key morphological characters,and inferred the ancestral area and biogeographical history of the *** analyses revealed that the genus Incarvillea is monophyletic and originated in Central Asia during mid-Oligocene ca.29.42 *** earliest diverging lineages were subsequently split into theWestern Himalaya and Sino-Himalaya during the early Miocene ca.21.12 *** lineages resulted in five re-circumscribed subgenera(Amphicome,Olgaea,Niedzwedzkia,Incarvillea,and Pteroscleris).Moreover,character mapping revealed the ancestral character states of the genus Incarvillea(e.g.,suffruticose habit,cylindrical capsule shape,subligneous capsule texture,absence of capsule wing,and loculicidal capsule dehiscence)that are retained at the earliest diverging ancestral nodes across the *** phylogenetic tree of the genus Incarvillea differs from previously proposed phylogenies,thereby recommending the placement of the subgenus Niedzwedzkia close to the subgenus Incarvillea and maintaining two main divergent lineages.

GLP inhibits heterochromatin clustering and myogenic differentiation by repressing MeCP2

Myogenic differentiation is accompanied by alterations in the chromatin states, which permit or restrict the transcriptional machinery and thus impact distinctive gene expression profiles. The mechanisms by which higher-order chromatin remodeling is associated with gene activation and silencing during differentiation is not fully understood. In this study, we provide evidence that the euchromatic lysine metbyltransferase GLP regulates heterocbromatin organization and myogenic differentiation. Interestingly, GLP represses expression of the methyl-binding protein MeCP2 that induces heterochromatin clustering during differentiation. Consequently, MeCP2 and HP1γ localization at major satellites are altered upon modulation of GLP expression. In GLP knockdown cells, depletion of MeCP2 restored both chromatin organization and myogenic differentiation. These results identify a novel regulatory axis between a histone methylation writer and DNA methylation reader, which is important for beterochromatin organization during differentiation.

Finger pulp reconstruction with thenar flap: Aesthetic and functional outcome

Purpose:Fingertip injuries are common in industrial production activities as well as in domestic *** of pulp hampers daily life *** and aesthetic aspects are important in fingertip *** bone is usually exposed along with soft tissue *** to reconstruct the pulp flap with adequate bulk is ***:We reported a case series of 12 patients with the injury over the volar aspect of distal phalanx of the index or middle *** all cases,laterally based thenar flap was *** flap donor site was closed primarily in most of cases,while 4 patients required skin *** flap was detached between 2e3 *** assessment was done using static and dynamic 2-point discrimination and range of motion at each *** aesthetic outcome was assessed through *** results were analyzed using the unpaired t-test(SPSS version 21).Results:Partial necrosis occurred in 2 cases while rest of flaps survived *** 2-point discrimination ranged from 6e10 mm,mean 8.6 mm;and dynamic 2-point discrimination ranged from 8e10 mm,mean 8.9 *** mean satisfaction score was(4.0±0.55).Conclusion:Thenar flap is a good choice for reconstruction of the finger pulp as it provides the bulk with good functional and aesthetic outcome.

Ayurvedic concept of Shatkriyakala:a traditional knowledge of cancer pathogenesis and therapy

The term Kriyakala refers to the recognition of the stage of a disease＇s progress, which helps to determine appropriate measure to correct the imbalance in Doshas （biological factors）. It is a compound expression, comprised of Kriya and kala, where Kriya means the choice to treatment （medicine, food and daily-routine） used to improve the disturbance in Doshas, and kala refers to the stage of progress of a disease. Sushruta, an ancient Indian surgeon, has described the concept of Kriyakala in Vamaprashnadhyaya, an ancient Vedic Sanskrit text, which seeks to explain the incidence of Vamas in terms of Doshic disturbances. Vama, in modern parlance, may be described as an inflammatory process that may lead ulceration and chronic inflammation, promoting all stages of carcinogenesis. Abnormal interactions between Prakriti （genotype） and environmental factors vitiate the Doshas and impair immunity, which can lead to aberrant cell growth and cancer. Moreover, the interaction between vitiated Doshas and weak Dhatus （body tissues） manifests as cancers of a specific organ. Shatkriyakala （six stages of progress of a disease）, on the other hand, provides a framework to assess the cancer and its pathogenesis in different stages. According to Ayurvedic concepts, all cancer therapies treat the affected tissues indirectly by eliminating vitiated Doshas, rejuvenating Dhatus and restoring immunity in cancer patients. The present review describes the six stages of Shatkriyakala in detail, with an emphasis on research areas to validate the concept of Shatkriyakala. This traditional knowledge can be utilized with modern technologies to detect predisposition for cancer or diagnose cancer in its early stages.
