Porta hepatis tuberculous lymphadenopathy(TL)is rare,and the clinical and imaging manifestations often mimic *** offer a bet-ter understanding of porta hepatis TL and thus improve its diagnosis and treatment,here,we retrospectively reviewed and analyzed 10 porta hepatis TL cases between May 2017 and November *** this study,porta hepatis TL occurred predominantly in women(70%),with a mean age of 43.1 years(range,16-70 years).Among the patients,60%were initially suspected of malignancy(metastatic lymphadenopathy and lymphoma).The clinical manifestations were nonspecific,such as fever and weight loss,without overt abdominal *** patients showed high concentrations of serum C-reactive protein,erythrocyte sedimentation rate,alkaline phosphatase,γ-glutamyl transpeptidase and carbohydrate antigen *** rim-like enhancement was a characteristic finding on contrast-enhanced CT for all the *** examination confirmed the diagnosis in 90%of patients with caseous granulo-matous inflammation and positive results of Xpert MTB/RIF,quantitative real-time PCR and/or metagenomic next-generation sequenc-ing *** patients underwent antituberculosis therapy with a median treatment duration of 13.5 months(range,1-43 months).They all showed satisfactory therapeutic responses with improved symptoms and resolution on imaging after *** examination by biopsy remains the primary diagnostic method.A combination of the characteristic epidemiologic,clinical,imaging fea-tures and lesion biopsy for histopathology are essential for the diagnosis and treatment of TL.

Understanding the immunological characteristics of monocytes-including the characteristics associated with fibrosis-in severe coronavirus disease 2019(COVID-19)is crucial for understanding the pathogenic mechanism of the disease and preventing disease *** this study,we performed single-cell transcriptomic sequencing of peripheral blood samples collected from six healthy controls and 14 COVID-19samples including severe,moderate,and convalescent samples from three severely/critically ill and four moderately ill *** found that the monocytes were strongly remodeled in the severely/critically ill patients with COVID-19,with an increased proportion of monocytes and seriously reduced *** addition,we discovered two novel severe-disease-specific monocyte subsets:Mono 0 and Mono *** subsets expressed amphiregulin(AREG),epiregulin(EREG),and cytokine interleukin-18(IL-18)gene,exhibited an enriched erythroblastic leukemia viral oncogene homolog(ErbB)signaling pathway,and appeared to exhibit pro-fibrogenic and pro-inflammation *** also found metabolic changes in Mono 0 and Mono 5,including increased glycolysis/gluconeogenesis and an increased hypoxia inducible factor-1(HIF-1)signaling ***,one pre-severe sample displayed a monocyte atlas similar to that of the severe/critical *** conclusion,our study discovered two novel severedisease-specific monocyte subsets as potential predictors and therapeutic targets for severe ***,this study provides potential predictors for severe disease and therapeutic targets for COVID-19 and thus provides a resource for further studies on COVID-19.

The pandemic of coronavirus disease 2019(COVID-19),a respiratory disease caused by a novel severe acute respiratory syndrome coronavirus-2,is causing substantial morbidity and *** with the respiratory symptoms,underlying diseases in senior patients,such as diabetes,hypertension,and coronary heart disease,are the most common comorbidities,which cause more severe outcomes and even *** cellular attachment and entry of severe acute respiratory syndrome coronavirus-2,the key protein involved is the angiotensin I converting enzyme 2(ACE2),which is located on the membrane of host ***,we aim to curate an expression profile of Ace2 and other COVID-19 related genes across the available diabetes murine *** on strictly manual curation and bioinformatics analysis of the publicly deposited expression datasets,Ace2 and other potentially involved genes such as Furin,Tmprss2,Ang,and Ang2 were *** found that Ace2 expression is rather ubiquitous in three selected diabetes prone strains(db/db,ob/ob and diet-induced obese).With the most abundant datasets present,the liver shows a medium Ace2 expression level compared with the lungs,pancreatic islets,brain and even T *** is a more critical factor for Ace2 expression in db/db compared with the other two *** Ace2,the other four host genes showed varied levels of correlation to each *** accelerate research on the interaction between COVID-19 and underlying diseases,the Murine4Covid transcriptomics database(***/Murine4Covid)will facilitate the design of research on COVID-19 and comorbidities.

Influenza viruses continue to cause epidemics worldwide every ***,due to the lack of an effective assessment for the severity of influenza epidemics,it was extremely difficult to take preventative *** were extracted from infectious diseases reports from 2011–*** regression model and susceptible-exposed-infectious-recovered model were built to understand the characteristics and processes of the *** reported incidence of influenza was 1,913,698 from January 2011 to February 2018,with an average-yearly-reported-incidence-rate of 19.21 per 100,***,there had been a substantial nationwide epidemic of influenza after September 2017,when the average yearly reported incidence rate was 87.29 per 100,000 and an annual percentage change of 48.1%.The hemagglutinin genes of most influenza A(H1N1 and H3N2)viruses from the period of the epidemic had lower homology to those before August *** the hemagglutinin of the recommended A(H3N2,H1N1)and B(Victoria)viruses for vaccines 2017/2018 had low matches with the epidemic *** basic reproduction number was *** vaccination benefit was linearly related to vaccination coverage,while the quarantine measure had only significantly benefited when over 60%of the quarantined *** most severe epidemic of influenza in China since 2011 occurred during the period from September 2017 to February *** to quarantine,influenza vaccination is more effective way to prevent influenza,and strategies to increase vaccination coverage should be taken for the prevention of severe epidemics of influenza.

“吕贝克灾难”、双胞胎研究及收养研究等流行病学观察性研究结果都充分表明,宿主基因是决定机体对结核分枝杆菌是否易感和感染后是否发病的重要因素。从连锁分析到全基因组关联研究已发现,人白细胞抗原(HLA)类基因和非HLA类基因(SLC11A1、VDR、ASAP1、细胞因子类基因及模式识别受体类基因)与结核病易感性相关。本文对近年涉及结核病易感性基因的研究进行回顾,重点叙述HLA类基因和非HLA类基因与结核病发病的关联,并对文中涉及的基因进行富集分析,结果提示大部分基因参与免疫炎症调控,并与自身免疫性疾病关系密切,可为后续结核感染风险预测和结核病诊治提供参考。
