Surgical treatment strategy for multiple injury patients in ICU

Objective： To investigate the surgical treatment for patients with multiple injuries in ***： Clinical data of 163 multiple injury patients admitted to ICU of our hospital from January 2006 to January 2009 were retrospectively studied, including 118 males and 45 females, with the mean age of 36.2 years （range, 5-67 years）. The injury regions included head and neck （29 cases）,face （32 cases）, chest （89 cases）, abdomen （77 cases）, pelvis and limbs （91 cases） and body surface （83 cases）. There were 57 cases combined with shock. ISS values varied from 10 to 54, 18.42 on average. Patients received surgical treatments in ICU within respectively 24 hours （10 cases）, 24-48 hours （8 cases）, 3-7 days （7 cases） and 8-14 days （23 cases）.Results： Forthe 163 patients, the duration of ICU stay ranged from 2 to 29 days, with the average value of 7.56 days. Among them, 143 were cured （87.73%）, 11 died in the hospital （6.75%） due to severe hemorrhagic shock （6 cases）,craniocerebral injury （3 cases） and multiple organ failure （2 cases）, and 9 died after voluntarily discharging from hospital （5.52%）. The total mortality rate was 12.27%.Conclusions： The damage control principle should be followed when multiple injury patients are resuscitated in ICU. Surgical treatment strategies include actively controlling hemorrhage, treating the previously missed injuries and related wounds or surgical complications and performing planned staging operations.

Lowering the synthesis temperature of boron nitride nanotubes(BNNTs)is crucial for their *** primary reason for adopting a high temperature is to enable the effective activation of highmelting-point solid *** this study,we developed a novel approach for efficiently activating boron by introducing alkali metal compounds into the conventional MgO–B *** approach can be adopted to form various low-melting-point AM–Mg–B–O growth *** growth systems have improved catalytic capability and reactivity even under low-temperature conditions,facilitating the synthesis of BNNTs at temperatures as low as 850℃.In addition,molecular dynamics simulations based on density functional theory theoretically demonstrate that the systems maintain a liquid state at low temperatures and interact with N atoms to form BN *** findings offer novel insights into the design of boron activation and are expected to facilitate research on the low-temperature synthesis of BNNTs.

Commercial aircraft crews have experienced a trend from five-person crew to dual-pilot *** from both technological and market demands,Single Pilot Operations(SPO)is considered an important development direction in modern aviation *** this paper,starting from Dual-Pilot Operations(DPO),the piloting process,decision-making process and decisionmaking mode of DPO for commercial aircraft are studied to obtain the operational requirements of ***,based on above analysis,the operational mechanism of SPO is studied and the core technology of SPO mode is ***,a new closed frequent bicluster mining algorithm named FsCluster is proposed for the optimization of the SPO model,and the other efficient bicluster mining algorithm named TsCluster is proposed for the analysis and verification of the SPO ***,a typical flight phase scenario is modelled by Magic System of System,and combined with the proposed algorithms for analysis and verification to determine whether the SPO mode can meet the DPO requirements.

Vascular remodeling is a common pathological process in cardiovascular diseases and includes changes in cell proliferation, apoptosis and differentiation as well as vascular homeostasis. Mechanical stresses, such as shear stress and cyclic stretch, play an important role in vascular remodeling. Vascular cells can sense the mechanical factors through cell membrane proteins, cytoskeletons and nuclear envelope proteins to initiate mechanotransduction, which involves intercellular signaling, gene expression, and protein expression to result in functional regulations. Non-coding RNAs, including micro RNAs and long non-coding RNAs, are involved in the regulation of vascular remodeling processes. Mechanotransduction triggers a cascade reaction process through a complicated signaling network in cells. High-throughput technologies in combination with functional studies targeting some key hubs and bridging nodes of the network can enable the prioritization of potential targets for subsequent investigations of clinical translation. Vascular mechanobiology, as a new frontier field of biomechanics, searches for principles of stress-growth in vasculature to elucidate how mechanical factors induce biological effects that lead to vascular remodeling,with the goal of understanding the mechanical basis of the pathological mechanism of cardiovascular diseases at the cellular and molecular levels. Vascular mechanobiology will play a unique role in solving the key scientific problems of human physiology and disease, as well as generating important theoretical and clinical results.

Dimethyl ether(DME)carbonylation is considered as a key step for a promising route to produce ethanol from ***(HPAs)are proved to be efficient catalysts for DME *** this work,the reaction mechanism of DME carbonylation was studied theoretically by using density functional theory calculations on two typical HPA models(HPW,HSiW).The whole process consists of three stages:DME dissociative adsorption,insertion of CO into methoxyl group and formation of product methyl *** activation barriers of all possible elementary steps,especially two possible paths for CO insertion were calculated to obtain the most favorable reaction mechanism and rate-limiting ***,the effect of the acid strength of Brønsted acid sites on reactivity was studied by comparing the activation barriers over HPW and HSiW with different acid strength,which was determined by calculating the deprotonation energy,Mulliken population analyses and adsorption energies of pyridine.
