Type D personality negatively associated with self-care in Chinese heart failure patients

Background Little is known about the association between type D personality and self-care behaviors in heart failure （HF） patients. We examined the effect of type D personality on self-care behaviors and self-efficacy among Chinese HF patients. Methods A cross-sectional study with a convenience sample was conducted. All participants completed the questionnaires of the self-care of HF index （V6） and type D personality scale. Demographic and clinical variables were obtained from medical records and patient interviews. The me- thods used for data analysis included descriptive analysis, independent-sample t-test, Z2 test, and multiple linear regression. Results A total of 127 HF patients were included and 61.4% of them were male. The average age for this study sample was 64.9± 12.34 years. The majority of the participants were in a New York Heart Association class III or IV （87%）, and the average length of living with HF was 38.24 ± 41.1 months. A total of 33.1% of the participants were identified as having type D personality. No significant differences were determined in the demographic and clinical variables between type D and non-type D patients, except for the mean age and the length of living with HF. Type D patients were younger and had a shorter time of living with HF than their non-type D counterparts. Multiple regression demonstrated significant associations between type D personality and self-care maintenance and self-efficacy after adjusting the demographic and clinical factors. However, type D personality was not significantly associated with self-care management behaviors. Conclusions Type D personality was negatively related to self-care maintenance and self-efficacy in Chinese HF patients. Future study is warranted to develop a tailored intervention to improve engagement in self-care behaviors in HF patients with type D personality.

Ex vivo relaxation effect of Cuscuta chinensis extract on rabbit corpus cavernosum

The effect of Cuscuta chinensis extract on the rabbit penile corpus cavernosum （PCC） was evaluated in the present study. Penises obtained from healthy male New Zealand white rabbits （2.5-3.0 kg） were precontracted with phenylephrine （Phe, 10 pmol I^-1） and then treated with various concentrations of Cuscuta chinensis extract （1, 2, 3, 4 and 5 mg ml^-1）. The change in penile tension was recorded, and cyclic nucleotides in the PCC were measured by radioimmunoassay （RIA）. The interaction between Cuscuta chinensis and sildenafil was also evaluated. The result indicated that the PCC relaxation induced by Cuscuta chinensis extract was concentration-dependent. Pre-treatment with an nitric oxide synthase （NOS） inhibitor （No〉 nitro-L-arginine-methyl ester, L-NAME）, a guanylyl cyclase inhibitor （1H-[1,2,4]oxadiazolo[4,3-a]quinoxalin-l-one, ODQ）, or a protein kinase A inhibitor （KT 5720） did not completely inhibit the relaxation. Incubation of penile cavernous tissue with the Cuscuta chinensis extract significantly increased cyclic guanosine monophosphate （cGMP） and cyclic adenosine monophosphate （cAMP） in the PCC. Moreover, the Cuscuta chinensis extract significantly enhanced sildenafil-induced PCC relaxation. In conclusion, the Cuscuta chinensis extract exerts a relaxing effect on penile cavernous tissue in part by activating the NO-cGMP pathway, and it may improve erectile dysfunction （ED）, which does not completely respond to sildenafil citrate.

Life skill-based training is essential for intellectual disabled individuals to regain autonomy and social *** reality(VR)could deliver a nascent application that is more engaging and *** objective of this study was to evaluate the training effects of the VR-based multiple life skill training program on life skill perfor-mance,self-efficacy,memory,cognitive and behavioral functions via a multicenter randomized controlled trial.A total of 145 intellectual disabled participants were recruited for a randomized controlled trial with three in-tervention arms:VR(n=42);traditional(n=53);and control(n=50).The life skill tasks for the interventions included 1)grocery shopping,2)cooking,and 3)kitchen *** measures were performance scores for the three tasks,self-efficacy scale,digit span score,and Frontal Assessment Battery score,graded by blinded ***-after effects of each group were evaluated using separated Wilcoxon Signed-rank *** sig-nificantly improved cooking,cleaning performance,and memory span,while traditional training significantly improved shopping,cooking,and cleaning *** generalized estimating equation evaluated effects between groups adjusted for age,gender and intelligence quotient(IQ).VR had significantly larger improvement effects in cooking and cleaning than the control group and memory span compared to traditional training and controls *** appeared to be a significant confounder on the training *** work may consider developing artificial intelligence to customize programs depending on IQ level.

LINEAR AND HYPERBRANCHED POLYMERS WITH HIGH THERMAL STABILITY AND LUMINESCENCE EFFICIENCY

New acetylene monomers, 6-{[(1-naphthylethynyl-4-phenyl)carbonyl]oxy}-1-phenyl-1-hexyne (1), 2,5-diethynyl-thiophene (3), and 4,4'-diethynylbiphenyl (6) were synthesized. Homopolymerization of 1 and copolycyclotrimerizations of 3 and 6 with 1-heptyne and 1-octyne have been achieved with WCl6- and TaCl5-Ph4Sn catalysts, respectively, giving soluble linear disubstituted polyacetylene (2) and hyperbranched polyarylenes (5 and 8) with high molecular weights (up to 1.2 x 10(5)) in high yields (up to 98%). The structures and properties of the polymers are characterized and evaluated by R NMR, TGA, UV, photoluminescence (PL), and electroluminescence (EL) analyses. All the polymers possess high thermal stability and emit strong blue light upon photoexcitation. The intensity of the emitted light is greater than that of poly(1-phenyl-1-octyne), a well-known highly luminescent disubstituted polyacetylene. Little aggregation-induced red shift in the PL was observed in the thin films of the polymers. By constructing a multi-layer EL device, high EL quantum yield (0.18%) has been achieved in 2, which are the best results for substituted polyacetylenes attainable so far.

Association between smoking,pancreatic insulin secretion and insulin resistance in Chinese subjects with or without glucose intolerance

Background There are studies suggesting smoking may increase the risk of type 2 diabetes. Effects of smoking on insulin secretion and insulin resistance （IR） are, however, controversial. Methods This is a cross-sectional study. Since there were very few smokers among Hong Kong Chinese women, only men （n=-1068） were analyzed in this report. Fasting and 2-hour plasma glucose and insulin were measured. Insulinogenic index as well as beta-cell function and IR based on homeostatic model assessment （HOMA） by computer model （HOMA Calculator v2.2） were calculated. Results Of the 1068 men, 147 had newly diagnosed diabetes, 131 newly diagnosed impaired glucose tolerance （IGT） and 790 were non-diabetic normal controls. Smokers had similar fasting and 2-hour insulin levels, insulinogenic index and HOMA derived beta-cell function as compared to non-smokers in the groups with diabetes, IGT or normal oral glucose tolerance test （OGTT）. IR was also similar between smokers, ex-smokers and non-smokers in those with normal OGTT. In men with IGT or diabetes, after adjustment for age and body mass index, smokers were more insulin resistant as compared to non-smokers （IR, IGT： 1.59±1.07 vs 1.03±0.54, P〈0.05; diabetes： 1.96±1.36 vs 1.06±0.45, P〈0.01）. With Logistic regression analysis, comparing smokers and non-smokers, IR was independently associated with smoking （odds ratio （95% C/）, IGT： 2.23 （1.05, 4.71）; diabetes： 3.92 （1.22, 12.58））. None of the other insulin parameters enter into the model among those with normal OGTT or comparing ex-smokers and non-smoker or smokers and ex-smokers. Conclusions In Chinese men, smoking did not show any direct association with insulin levels and pancreatic insulin secretion. Smoking men with IGT or diabetes appeared more insulin resistant than their non-smoking counterparts.

Diagnostic challenge of lipomatous uterine tumors in three patients

Lipomatous uterine tumors are uncommon benign neoplasms, with incidence ranging from 0.03% to 0.2%. They can generally be subdivided into two types: pure or mixed lipomas. A third group of malignant neoplasm has been proposed, which is liposarcoma; however, this is very rare. In this article, we report three patients having lipomatous uterine tumors, including one uterine lipoma and two uterine lipoleiomyomas. All our patients are postmenopausal women, which is the typical presenting age group. They did not have any symptoms and the tumors were only found incidentally on imaging. However, in some patients, symptoms may uncommonly occur. If symptoms occur, these are similar to those of leiomyoma. We illustrate the imaging features of the tumors in our patients with ultrasound, computed tomography (CT) scan and magnetic resonance imaging (MRI). The tumor typically appears as a well-defined homogenously hyperechoic lesion on ultrasound. It shows fat density on CT scan and signal intensity of fat on MRI. MRI is the modality of choice because of its multiplanar capability and its ability to demonstrate fat component of the lesion, as illustrated in our cases. We also discuss the importance of differentiating lipomatous uterine tumors from other lesions, especially ovarian teratoma which requires surgical intervention. Despite the rarity and the common asymptomatic nature of the tumors, we believe that this series of three cases demonstrates a review of a rare tumor which provides important knowledge for patient management.

Visible and near-infrared(NIR)light dual-band photodetectors(PDs)have potential applications in signal detection,bioimaging,optical communications and safety ***,we report an ultrafast perovskite/organic heterojunction dual-mode PD with a voltage-modulated photoresponse range in visible and NIR *** PD,comprising a perovskite layer to absorb visible light(500–810 nm)and an organic bulk heterojunction layer for NIR light absorption(810–950 nm),exhibited a switchable spectral response in the visible or NIR *** voltage-modulated visible and NIR photoresponses of the PD were attributable to controlled charge photogeneration in perovskite and organic blend thin films under different bias *** device exhibited peak responsivities of 93.5 and 102.2 mA/W in the visible and NIR bands,respectively;a high detectivity of 4.3×10^(9) Jones(at forward bias of 0.7 V and incident 625 nm light)and 1.6×10^(12) Jones(at reverse bias of–1.5 V and incident 900 nm light);a fast microsecond response time;and a wide dynamic range(>120 dB)both in the visible mode and NIR ***,this voltage-modulated dual-band PD shows promising applications in visible light and NIR imaging,which is proven by demonstrating a PD array with 25 pixels(5×5).

针对汽车底盘关键子系统对侧翻运动影响能力及有效作用区域的差异，综合考虑了车辆运动耦合及轮胎力耦合因素，建立了包含悬架、制动和转向系统的11自由度整车侧翻动力学模型。以主动前轮转向、独立制动及主动悬架等底盘典型控制系统为对象，以系统的控制输入为分析变量，选取各系统对车体产生的侧倾修正力矩为权重评价依据，定量地研究了其对侧翻控制的功能权重及有效作用范围。结果表明，这三个控制子系统都可以在不同程度上有效地控制侧翻运动。分析明确了各系统工作的有效工作区域和控制权重，为车辆侧翻控制策略的制定提供了依据。

Ultrafast low-pump fluence all-optical modulation based on graphene-metal hybrid metasurfaces

Graphene is an attractive material for all-optical modulation because of its ultrafast optical response and broad spectral ***,all-optical graphene modulators reported so far require high pump fluence due to the ultrashort photo-carrier lifetime and limited absorption in *** present modulator designs based on graphene-metal hybrid plasmonic metasurfaces with highly enhanced light-graphene interaction in the nanoscale hot spots at pump and probe(signal)*** on this design concept,we have demonstrated high-speed all-optical modulators at near and mid-infrared wavelengths(1.56μm and above 6μm)with significantly reduced pump fluence(1–2 orders of magnitude)and enhanced optical *** near-infrared pump-probe measurement results suggest that the modulators’response times are ultimately determined by graphene’s ultrafast photocarrier relaxation times on the picosecond *** proposed designs hold the promise to address the challenges in the realization of ultrafast all-optical modulators for mid-and far-infrared wavelengths.

Outcomes of emergency and interval hepatectomy for ruptured resectable hepatocellular carcinoma: a single tertiary referral centre experience

Aim: The short and long term outcomes of patients who underwent emergency and interval hepatectomy for ruptured and resectable hepatocellular carcinoma (HCC) were analysed. Methods: The data of patients with ruptured HCC presenting between April 2004 and October 2015 were analysed. Emergency hepatectomy was defined as hepatectomy within 48 h of the clinico-radiological diagnosis of HCC rupture. Results: Thirty patients underwent hepatectomy for ruptured HCC. Nine (30%) patients underwent emergency hepatectomy. The median age was 56 and 54 years (P = 0.13) with a similar gender distribution. The mean HCC size (10.5 vs. 8.3 cm, P = 0.17), total blood loss (3,000 vs. 850 mL, P = 0.002) and total units of red blood cell transfusion (1.9 vs. 0.5 units, P = 0.27) were greater in the emergency hepatectomy group. The complication rate was 44% and 38% (P = 0.53), with median length of hospital stay of 10 and 12 days (P = 0.07) in the emergency and interval hepatectomy groups, respectively, and no 30-day mortality in both groups. The median overall survival was 29 and 15.7 months (P = 0.25), with survival rates of 78%, 45%, 0% and 85%, 43% and 5% at 1, 3 and 5 years in the emergency and interval hepatectomy groups, respectively. Conclusion: Hepatectomy should be considered for ruptured HCC provided the patient could tolerate curative resection.
