Type 1 diabetes(T1D)is an autoimmune disease that usually strikes early in life,but can affect individuals at almost any *** is caused by autoreactive T cells that destroy insulin-producing beta cells in the *** studies estimate a prevalence of 1 in 300 children in the United States with an increasing incidence of 2%-5%annually *** daily responsibility,clinical management,and vigilance required to maintain blood sugar levels within normal range and avoid acute complications(hypoglycemic episodes and diabetic ketoacidosis)and long term micro-and macro-vascular complications significantly affects quality of life and public health care *** the expansive impact of T1D,research work has accelerated and T1D has been intensively investigated with the focus to better understand,manage and cure this *** advances have been made in the past decades in this regard,but key questions remain as to why certain people develop T1D,but not others,with the glaring example of discordant disease incidence among monozygotic *** this review,we discuss the field’s current understanding of its pathophysiology and the role of genetics and environment on the development of *** examine the potential implications of these findings with an emphasis on T1D inheritance patterns,twin studies,and disease *** a better understanding of this process,interventions can be developed to prevent or halt it at early stages.

Radiofrequency ablation versus microwave ablation for colorectal liver metastases:long-term results of a retrospective cohort surgical experience

Background:Ablation is an alternative treatment modality for selected patients with colorectal liver metastases(CRLMs).Although initially widely performed via radiofrequency ablation(RFA),more recently,microwave ablation(MWA)is being preferred due to its perceived superiority in creating the ablation *** aim of this study is to compare the long-term efficacy of these two modalities performed ***:Patients undergoing surgical liver ablation from 2005-2023 at a tertiary center by a single surgeon for CRLM were included in a retrospective institutional review board-approved *** were compared using Wilcoxon,Chi-square,Kaplan-Meier,and Cox multivariate regression *** data are presented as median(interquartile range).Results:There were a total of 242 *** RFA was done in 121 patients with 303 lesions and laparoscopic MWA in 121 patients with 300 *** was no difference between the groups regarding operative time(161 vs.147 minutes,respectively,P=0.4),perioperative morbidity(3%vs.8%,respectively,P=0.2)or hospital stay(1 vs.1 day,P=0.05).Local recurrence(LR)per lesion with at least 1 year of imaging follow-up was 29%in the RFA and 13%in the MWA group(P<0.001).Based on univariate survival analysis,tumor size,blood vessel proximity,ablation margin,and ablation modality were independent predictors of *** control these variables,direct matching was *** cohort included 189 ***-Meier analysis of these cohorts showed increased LR-free survival in the MWA group *** RFA group(P=0.005).Conclusions:This large study confirms our initial observation that local tumor control rate is better after MWA ***.

Feynman Path Integral Using Operator Integration in Banach Space

Feynman-Path Integral in Banach Space: In 1940, R.P. Feynman attempted to find a mathematical representation to express quantum dynamics of the general form for a double-slit experiment. His intuition on several slits with several walls in terms of Lagrangian instead of Hamiltonian resulted in a magnificent work. It was known as Feynman Path Integrals in quantum physics, and a large part of the scientific community still considers them a heuristic tool that lacks a sound mathematical definition. This paper aims to refute this prejudice, by providing an extensive and self-contained description of the mathematical theory of Feynman Path Integration, from the earlier attempts to the latest developments, as well as its applications to quantum mechanics. About a hundred years after the beginning of modern physics, it was realized that light could in fact show behavioral characteristics of both waves and particles. In 1927, Davisson and Germer demonstrated that electrons show the same dual behavior, which was later extended to atoms and molecules. We shall follow the method of integration with some modifications to construct a generalized Lebesgue-Bochner-Stieltjes (LBS) integral of the form , where u is a bilinear operator acting in the product of Banach spaces, f is a Bochner summable function, and μ is a vector-valued measure. We will demonstrate that the Feynman Path Integral is consistent and can be justified mathematically with LBS integration approach.

Diabetic neuropathy results in vasomotor dysfunction of medium sized peripheral arteries

BACKGROUND The effect of the sympathetic nervous system on peripheral arteries causes vasoconstriction when smooth muscle cells in the walls of blood vessels contract,which leads to narrowing of arteries and reduction of the blood *** To compare sympathetic vasomotor activation of the brachial arteries in healthy subjects and patients with painful diabetic neuropathy;and therefore,to assess whether there is significant vasomotor dysfunction of medium sized arteries in diabetic *** The study included 41 diabetic neuropathy patients and 41 healthy *** diameter and flow rate of the brachial arteries were ***,using a bipolar stimulus electrode,a 10 mA,1 Hz electrical stimulus was administered to the median nerve at the wrist level for 5 *** brachial artery diameter and blood flow rate were re-measured after *** In the control group,the median flow rate was 70.0 mL/min prior to stimulation and 35.0 mL/min after stimulation,with a statistically significant decrease(P<0.001),which is consistent with sympathetic nervous system functioning(vasoconstriction).In the diabetic neuropathy group,median flow rate before the stimulation was 35.0 mL/*** stimulation,the median flow rate was 77.0 mL/min;thus,no significant decrease in the flow rate was *** the control group,the median brachial artery diameter,which was 3.6 mm prior to stimulation,decreased to 3.4 mm after stimulation,and this decrease was also statistically significant(P=0.046).In the diabetic neuropathy group,the median brachial artery diameter increased from 3.4 mm to 3.6 mm following nerve *** again,no narrowing was *** Our research suggests that diabetic neuropathy results in significant vasomotor dysfunction of medium sized peripheral *** vasoconstriction in response to sympathetic activation is impaired in diabetic neuropathy.

New insights on premature ejaculation： a review of ：lefinition, classification, prevalence and treatmeni

There are ongoing debates about the definition, classification and prevalence of premature ejaculation （PE）. The first evidence-based definition of PE was limited to heterosexual men with lifelong PE who engage in vaginal intercourse. Unfortunately, many patients with the complaint of PE do not meet these criteria. However, these men can be diagnosed as one of the PE subtypes, namely acquired PE, natural variable PE or premature-like ejaculatory dysfunction. Nevertheless, the validity of these subtypes has not yet been supported by evidence. The absence of a universally accepted PE definition and lack of standards for data acquisition have resulted in prevalence studies that have reported conflicting rates. The very high prevalence of 20%-30% is probably due to the vague terminology used in the definitions at the time when such surveys were conducted. Although many men may complain of PE when questioned for a population-based prevalence study, only a few of them will actively seek treatment for their complaint, even though most of these patients would define symptoms congruent with PE. The complaints of acquired PE patients may be more severe, whereas complaints of patients experiencing premature-like ejaculatory dysfunction seem to be least severe among men with various forms of PE. Although numerous treatment modalities have been proposed for management of PE, only antidepressants and topical anaesthetic creams have currently been proven to be effective. However, as none of the treatment modalities have been approved by the regulatory agencies, further studies must be carried to develop a beneficial treatment strategy for PE.

Comparison of Congenital Tooth Deficiencies Seen in Permanent Teeth in Individuals with Unilateral Cleft Lip and Palate to Those without Cleft Lip and Palate

Objective: To compare the congenital tooth deficiencies seen in permanent dentition in individuals with unilateral cleft lip and palate (UCLP) to non-cleft individuals with Angle Class I malocclusion. Method: The study was performed on orthopantomograph films of 50 individuals with UCLP aged between 12 - 16 years and 50 individuals with Angle Class I malocclusion individuals aged between 13 - 15 years. Individuals with UCLP;32 clefts were on the left side and 18 clefts were on the right side. Permanent third molar teeth deficiency was excluded from the study. Results: In 50 individuals with UCLP;35 (70%) upper lateral incisors were congenitally deficient in the cleft area, while 15 (30%) missing lateral teeth were found in the non-cleft side. In control group, 12 (24%) of 50 patients had congenital lateral incisor deficiency. Congenital deficiency of upper lateral incisor in UCLP;the cleft area was higher than the non-cleft side (p Conclusion: The deficiency of the lateral incisors in the cleft side is more often congenitally deficient than upper second premolar teeth and this should be considered in the treatment planning from an early age.

Different typologies of workplace conflict and their connections with post traumatic embitterment disorder (PTED)

Reported is an empirical study which shows that Post-Traumatic Embitterment Disorder (PTED) is the most appropriate psychological diagnosis for victims of workplace conflicts, particularly bullying. A group of 118 people all reporting conflict at work were evaluated with the LIPT questionnaire, the PTED self-rating scale and a guided psychological interview. 91.5% proved to be affected by a PTED, the slight majority males, aged between 31 and 40 years and subjected to bullying. The evidence suggests that some workplace conflict victims who are presently diagnosed and treated as depression or phobia can be in fact cases of PTED. The treatment may be adjusted and the PTED scale may be used as a screening in-strument similar to scales for anxiety and depressive disorders.

Trust Management for Mobile Media Distribution

Multimedia content delivery to capable smart phones with high-speed next-generation Internet connectivity is becoming commonplace. However, the openness of delivery demands adaptive and robust management of intellectual property rights. The purpose of this article is to describe a framework to address the central issues in content delivery: a scalable peer-to-peer-based content delivery model. Our method pairs the delivery with a secure access control model that enables data providers to secure a return from making their original content available. Our work resulted in a prototype implementation written in Java that includes a client for the Android mobile platform. Adding robust trust management to scalable peer-to-peer content delivery is the major significance of our work.

Imaging through diffusers presents a challenging problem with various digital image reconstruction solutions demonstrated to date using ***,we present a computer-free,all-optical image reconstruction method to see through random diffusers at the speed of *** deep learning,a set of transmissive diffractive surfaces are trained to all-optically reconstruct images of arbitrary objects that are completely covered by unknown,random phase *** the training stage,which is a one-time effort,the resulting diffractive surfaces are fabricated and form a passive optical network that is physically positioned between the unknown object and the image plane to all-optically reconstruct the object pattern through an unknown,new phase *** experimentally demonstrated this concept using coherent THz illumination and all-optically reconstructed objects distorted by unknown,random diffusers,never used during *** digital methods,all-optical diffractive reconstructions do not require power except for the illumination *** diffractive solution to see through diffusers can be extended to other wavelengths,and might fuel various applications in biomedical imaging,astronomy,atmospheric sciences,oceanography,security,robotics,autonomous vehicles,among many others.

Solution of the finite slab criticality problem using an alternative phase function with the second kind of Chebyshev polynomials

The critical size of a finite homogenous slab is investigated for one-speed neutrons using the alternative phase function(AG, Anli-Gungor) in place of the scattering function of the transport equation. First of all, the neutron angular flux expanded in terms of the Chebyshev polynomials of second kind(UN approximation) together with the AG phase function is applied to the transport equation to obtain a criticality condition for the ***, using various values of the scattering parameters, the numerical results for the critical half-thickness of the slab are calculated and they are tabulated in the tables together with the ones obtained from the conventional spherical harmonic(PN) method for comparison. They can be said to be in good accordance with each other.
