Desmoid Tumor of the Breast as a Manifestation of Gardner’s Syndrome in an Elderly Woman: A Case Report and Review of Literature

Desmoid tumor of the breast is an extremely rare entity. Histologically, it corresponds to a benign fibroblastic proliferation of a local evolution, with a high recurrence potential. We report the first case of elderly woman presented with desmoid-tumor of the breast as a manifestation of Gardner’s syndrome. A brief literature review was provided. We report the case of a 72-year-old woman who presented with five months history of painful and an exophytic mass of her left breast. She is known to have familial polyposis and had a total colectomy with Gardner’s syndrome. She had a history of osteomas of the maxilla. On clinical examination, there was an exophytic painless mass, on the upper medial quadrant of the breast. She had also lipoma in the left leg and pigmented skin lesions in legs. Her mammograms showed a suspicious stellar image. A computed tomography scan showed an ovoid lesion. Excision of the lesion was performed. Histopathology confirms a desmoid-tumor of the breast. Evolution was marked by lesion recurrence. In conclusion, desmoid tumor of the breast is rare, non-metastatic but locally aggressive. Clinical expression is often nonspecific. Treatment remains controversial;surgical excision is the treatment of choice.

Introduction: The neutrophil to lymphocyte ratio (NLR) can be used as a marker of subclinical inflammation, and may have a predictive power in prognosis and severity of atherosclerosis-related diseases. This study aimed to assess an association between the NLR, and clinical characteristics and one-month outcome in acute ischemic stroke (AIS). Subjects and Methods: This case-control observational prospective study included 75 patients admitted to stroke unit of Ain Shams University hospitals with AIS, sub grouped into 3 equal groups according to subtype of AIS, in addition to 25 healthy individuals. The demographic characteristics of the patients, complete blood picture test results at presentation, National Institutes of Health Stroke Scale (NIHSS) scores and modified Rankin Scale (mRS) scores were recorded. The clinical outcome was assessed by the NIHSS and mRS scores after one month. Results: The total leucocyte count was significantly higher in large artery atherosclerosis (P = 0.004) and cardioembolic (P = 0.020) stroke groups, unlike lacunar stroke group (P = 0.082), when compared to controls. The neutrophils count was higher (P Conclusion: NLR was significantly higher among AIS subtypes compared to controls, but not a good predictor for one month outcome.

Effect of Cooking on Amylose Contents of Mungbean （Phaseolus aureus） and Chickpea （Cicer arietinum L.） Varieties

Keeping in view the importance of amylose for various food industries as well as its importance on domestic level, the current study was conducted to analyze the amylose content of pulses of common use e. g. mungbean (NM-92, NM-98 and Ramazan) and kabuli type chickpea (CMNK10-99, CMNK452-2 and Hassan 2k) varieties and quantifying the influence of cooking (boiling) on the amylose contents of pulses. The data revealed that moisture and amylose content of uncooked mungbean varieties ranged 9.01-9.47% and 20.06-22.26% respectively. The moisture and amylose content of Kabuli type chickpea varieties varied 8.52-8.79% and 20.25-22.83% respectively. Moisture content of mungbean (64.16-66.08%) as well as chickpea (55.56-61.52%) varieties increased after cooking. For all the three varieties of mungbean the observed cooking time was 11 minutes. The maximum value of amylose content was assayed for NM-98 (20.74%) followed by Ramzan (20.24%). The cooking time of Kabuli type chickpea varieties CMNK-452-2 and CMNK-10-99 was 60 min. Hassan 2K observed more cooking time (90 min) and highest value of amylose content in cooked (19.01%) samples. The effect of cooking/varietals on sensory evaluation (appearance, flavor, texture and overall acceptability) of mungbean varieties was statistically non significant attributes. Among chickpea varieties, Hassan 2K was ranked lowest due to its small size and hard texture and the sensory attributes were statistically significant (P < 0.05) as compared to the other two varieties.

Background: Cancers of the small bowel are rare. Diagnosis is late and difficult because of the lack of specific signs. Treatment is surgical. Prognosis is usually poor and depends on the histological type of tumor. Aim of Study: To specify the epidemiological, clinical and therapeutic characteristics of small bowel primary cancers in order to improve their prognosis. Material and Methods: This is a retrospective study about 20 cases of malignant tumors of the small bowel, collected in the department of general surgery of Habib Thameur Hospital in Tunis (Tunisia), from January 1994 through June 2011. Results: Our series involved 11 women and 9 men aged 62 on average (range: 44 - 80 years). In 45% of cases, the diagnosis was made in a patient rushed to hospital with clinical features of acute generalized peritonitis (66% of all surgical emergencies). Intestinal transit was performed in 5 patients only. Ultrasound abdominal examination was performed in 11 patients. Abdominal CT scan was performed in 7 patients, but the results were conclusive in 4 cases only (57%). Small bowel scanning was done in 5 patients only, but led to a positive diagnosis in all of them. All of our patients underwent surgery. Tumors of the small bowel were histologically divided as follows: carcinoid tumor (8 cases), leiomyosarcoma (7 cases), giant B-cell lymphoma (2 cases), malignant stromal tumor (2 cases) and malignant myxoid schwannoma (1 case). Malignant tumors of the small bowel most commonly arise in the ileum (60%) followed by the jejunum (35%). As for the long-term course, there was a recurrence at one year of a leiomyosarcoma and two recurrences of stromal tumors associated with liver metastases. Conclusion: Small bowel cancers are rare. Time to consultation is long and diagnosis is difficult and late due to the absence of typical presentation. Treatment is surgical and progression depends essentially on histological findings.

本文研究了槟榔(Piperbetle,PB)水提取物对癌细胞转移能力的调节作用及其对微管蛋白结构和网络的影响。采用亚毒剂量对PB和5-氟尿嘧啶(5-FU)处理的癌细胞进行抗迁移研究,观察细胞迁移20小时。在所有处理组中,100μg/m L PB处理的癌细胞均显示出最大的抗迁移作用(P=0.016)。总体上,PB对癌细胞的抗迁移作用高于5-FU。通过细胞形态观察发现,PB处理的细胞表现出与标准微管抑制剂(紫杉醇)相似的细胞特征,并在PB和紫杉醇处理的癌细胞中发现M期细胞群。通过对微管结构和网络进一步研究,我们发现PB和紫杉醇处理的癌细胞表现出长期破坏的纺锤体。因此,我们认为PB具有对癌细胞的抗迁移作用,并可能改变蛋白结构和网络。

The discovery of a strong association between hepatitis C virus(HCV) infection and mixed cryoglobulinemia(MC) has led to an increasingly rare diagnosis of idiopathic essential MC(EMC).The incidence of EMC is high in regions where there is a comparatively low HCV infection burden and low in areas of high infection prevalence,including *** diagnosis of EMC requires an extensive laboratory investigation to exclude all possible causes of cryoglobulin *** addition,although cryoglobulin testing is simple,improper testing conditions will result in false negative ***,we present a 46-year-old female patient with a case of EMC with dermatological and renal manifestations,highlighting the importance of extensive investigation to reacha proper *** review the need for appropriate laboratory testing,which is often neglected in clinical practice and which can result in false negative *** review also emphasizes the significance of an extended testing repertoire necessary for better patient *** a strong association of MC with HCV infection and other causes that lead to cryoglobulin formation,EMC remains a separate *** diagnosis requires proper temperature regulation during sample handling,as well as characterization and quantification of the *** of rheumatoid factor activity and complement levels in the cryoglobulin test-panel promotes better patient management and *** guidelines should be developed and implemented for cryoglobulin detection and the diagnosis of cryoglobulinemic syndrome,which will reduce variability in inter-laboratory reporting.
