Objective: This study was conducted to determine the effect of the application of cold on hema.oma.ecchymosis,and pain in patients undergoing percutaneous coronary intervention.Methods: It is a quasi-experimental study with a control group.A total of 200 patients (100 in the control group and 100 in the experimental group) were included in the research.In the experimental group,a cold pack was applied to the bottom of the sandbag for the first 15 min after withdrawing the femoral arterial catheter.Both groups were assessed and followed up at the first 15th minute and 4th,48th and 72 nd h.Results: After the femoral arterial catheter was withdrawn,ecchymosis and hema.oma.were sma.ler and pain was less in the experimental group at the catheter entry site compared to the control group,and the difference was statistically significant (P ma.ion of hema.oma.,ecchymoses and pain in patients after the withdrawal of the femoral arterial catheter.

Objective:To determine the most common mistakes ma.e during the use of a metered-dose inhaler(MDI),and the effect of the repeated trainings performed with demonstration method by providing one-to-one feedback on these mistakes.Methods:This is a quasi-experimental study with a control group.A total of 100 chronic obstructive pulmonary disease(COPD)patients(50 in the control group and 50 in the experimental group)were included in the research.“Patient Informa.ion Form(PIF)”(to determine the descriptive characteristics of patients)through the face-to-face interview method,using“MDI Skill Assessment Form”(MDISAF)(it is composed of 10 skill steps about the use of MDI)through observation method was used.Patients in both the groups were asked to use MDI and their abilities regarding use of MDI were assessed.Then in the intervention group,usage of MDI was explained by a nurse via demonstration and placebo MDI.Trainings were repeated on days 1,3,and 5 as from hospitalization of the patient.In the intervention group,three methods were used in this study:“face-to-face training,”“one-to-one,”and“with feedbacks and repeated.”Routine training regarding use of MDI was given by the nurses in the clinic to patients in the control group.The use of an MDI was assessed using MDISAF before training and after the training on the first,third and fifth days of hospitalization.On the seventh day,the last measurement was performed.Percentage,chi square,and mean were used to assess the data.Results:After repetitive training with one-to-one feedback,several differences between the groups in favor of the experimental group were found in 7 of the 10 skill levels of the MDI.There was a significant difference after“training”between the groups in the third,fourth,fifth,sixth,seventh,eighth and ninth MDI steps in posttest measurement(Pma.e less mistakes in steps in which mistakes were ma.e mostly,the control group con

Acute on chronic liver failure（ACLF） was first described in 1995 as a clinical syndrome distinct to classic acute decompensation.Characterized by complications of decompensation,ACLF occurs on a background of chronic liver dysfunction and is associated with high rates of organ failure and significant short-term mortality estima.ed between45%and 90%.Despite the clinical relevance of the condition,it still rema.ns largely undefined with continued disagreement regarding its precise etiological factors,clinical course,prognostic criteria and ma.agement pathways.It is concerning that,despite our relative lack of understanding of the condition,the burden of ACLF among cirrhotic patients rema.ns significant with an estima.ed prevalence of 30.9%.This paper highlights our current understanding of ACLF,including its etiology,diagnostic and prognostic criteria and pathophysiology.It is evident that further refinement of the ACLF classification system is required in order to detect high-risk patients and improve short-term mortality rates.The field of metabolomics certainly warrants investigation to enhance diagnostic and prognostic parameters,while the use of granulocyte-colony stimulating factor is a promising future therapeutic intervention for patients with ACLF.

Perioperative acute lung injury（ALI） is a syndrome characterised by hypoxia and chest radiograph changes.It is a serious post-operative complication,associated with considerable mortality and morbidity.In addition to mechanical ventilation,remote organ insult could also trigger systemic responses which induce ALI.Currently,there are limited treatment options available beyond conservative respiratory support.However,increasing understanding of the pathophysiology of ALI and the biochemical pathways involved will aid the development of novel treatments and help to improve patient outcome as well as to reduce cost to the health service.In this review we will discuss the epidemiology of peri-operative ALI;the cellular and molecular mechanisms involved on the pathological process;the clinical considerations in preventing and ma.aging perioperative ALI and the potential future treatment options.

AIM To evaluate the significance of resection ma.gin width in the ma.agement of hepatocholangiocarcinoma.HCC-CC).METHODS Data of consecutive patients who underwent hepatectomy for hepatic ma.ignancies in the period from1995 to 2014 were reviewed.Patients with pathologically confirmed HCC-CC were included for analysis.Demographic,biochemical,operative and pathological data were analyzed against survival outcomes. RESULTSForty-two patients were included for analysis.The median age was 53.5 years.There were 29 ma.es.Hepatitis B virus was identified in 73.8%of the patients.Most patients had preserved liver function.The median preoperative indocyanine green retention rate at 15 min was 10.2%.The median tumor size was 6.5 cm.ma.or hepatectomy was required in over 70%of the patients.Hepaticojejunostomy was performed in 6 patients.No hospital death occurred.The median hospital stay was 13 d.The median follow-up period was 32 mo.The 5-year disease-free survival and overall survival were 23.6%and 35.4%respectively.Multifocality was the only independent factor associated with diseasefree survival[Pma.gin of≥1 cm was associated with improved 1-year disease-free survival(40%vs 0%;log-rank,P=0.012).CONCLUSIONHCC-CC is a rare disease with poor prognosis.Resection ma.gin of 1 cm or above was associated with improved survival outcome in patients with multifocal HCC-CC.

Aim:To describe the current practise of living donor liver transplantation(LDLT)for hepatocellular carcinoma.HCC),including the patient selection criteria,surgical techniques,ma.agement of sma.l-for-size syndrome,postoperative complications,and the results of our units,in the Liver Transplant Centre of Queen ma.y Hospital,Hong Kong,one of the high-volume centres for LDLT in Asia.Methods:Our centre practises careful selection for HCC patients using the University of California,San Francisco(UCSF)criteria,supplemented by alpha-fetoprotein level and the model for end-stage liver disease score.Slight flexibility is offered to enthusiastic donors and recipients in LDLT while balancing the risks and benefits.We pioneered in using the extended right lobe graft and the novel hepatic venoplasty technique,which lessen the risk of hyperperfusion and sma.l-for-size syndrome with improved overall recipient survival.Data were collected prospectively and presented as the mean values and ranges,or the number of patients in proportion of total patient population.Results:Of our patients,74.9% met the UCSF criteria,and 64.5% met the Milan criteria.A 5-year overall and disease-free survival rate of 78.9% and 76.3% were achieved.Conclusion:LDLT is an ideal treatment for HCC in Hong Kong with regard to the critical organ shortage and high dema.d for transplantation.The current surgical techniques and post-transplant surveillance contribute to the positive outcome.

The role of the Ca^(2+)-permeable ion channel TRPC5 in regulating vasocontraction in obesity is poorly understood.Here,we investigated whether TRPC5 contributes to vascular dysfunction in obesity by promoting endothelium-dependent contraction via activation of cytosolic phospholipase A2(cPLA_(2))in the aortic endothelial cells of obese mice.Acetylcholine-induced endothelium-dependent relaxation and contraction in the aorta were measured us-ing wire myography.PLA_(2)activity was measured by the fluorogenic PLA_(2)substrate Bis-BODIPY^(TM)FL C_(11)-PC.The intracellular Ca^(2+)level in response to acetylcholine was measured by Fluo-4 fluorescence.Endothelium-derived contracting factors were assessed by enzyme immunoassay.Diet-induced obesity(DIO)attenuated endothelium-dependent vasodilation,enhanced endothelium-dependent contraction(EDC),and increased the expression of TRPC5 in the mouse aorta.Activation of TRPC5 promoted EDC in the wild-type mouse aorta,whereas pharma.cological inhibition and genetic knockout of TRPC5 decreased EDC in the DIO mouse aorta.Moreover,cPLA_(2)phosphorylation and activity were higher in aortic endothelial cells from DIO mice,and this was attenuated by inhibition and knockout of TRPC5.Cyclooxygenase 2(COX-2)expression was increased in DIO mouse endothe-lium and was decreased by a TRPC5 inhibitor and knockout of TRPC5.Release of prostaglandins F_(2α(PGF_(2α)and E 2(PGE 2)was involved in TRPC5-regulated EDC in DIO mice.This study demonstrated that TRPC5 contributes to endothelial and vascular dysfunction and is involved in EDC through activation of cPLA_(2)and enhanced COX-2-PGF_(2α)/PGE_(2)levels in DIO mice.

The aorta, with ascending, arch, thoracic and abdominal segments, responds to the heartbeat, senses metabolites and distributes blood to all parts of the body. However, the heterogeneity across aortic segments and how metabolic pathologies change it are not known. Here, a total of 216 612 individual cells from the ascending aorta, aortic arch, and thoracic and abdominal segments of mouse aortas under norma. conditions or with high blood glucose levels, high dietary salt, or high fat intake were profiled using single-cell RNA sequencing. We generated a compendium of 10 distinct cell types, ma.nly endothelial(EC), smooth muscle (SMC), stroma. and immune cells. The distributions of the different cells and their intercommunication were influenced by the hemodynamic microenvironment across anatomical segments,and the spatial heterogeneity of ECs and SMCs ma. contribute to differential vascular dilation and constriction that were measured by wire myography. Importantly, the composition of aortic cells, their gene expression profiles and their regulatory intercellular networks broadly changed in response to high fat/salt/glucose conditions. Notably, the abdominal aorta showed the most drama.ic changes in cellular composition, particularly involving ECs, fibroblasts and myeloid cells with cardiovascular risk factor-related regulons and gene expression networks. Our study elucidates the nature and range of aortic cell diversity,with implications for the treatment of metabolic pathologies.

BACKGROUND: To popularize the wide-spread use of automa.ed external defi brillator(AED) to save life in sudden cardiac arrest, we compared the strength and weakness of different types of AEDs to enable a sound selection based on regional requirement.METHODS: This was a retrospective descriptive study. Different types of AEDs were compared according to the informa.ion of AEDs from ma.uals and brochures provided by the ma.ufacturers. Fifteen types of AEDs were divided into 3 groups, basic, intermediate and advanced.RESULTS: Lifeline? AUTO AED had the best performa.ce in price, portability and user-friendly among AEDs of basic level. It required less time for shock charging. Sama.itan PAD defi brillator was superior in price, portability, durability and characteristic among AEDs of intermediate level. It had the longest warranty and highest protection against water and dust. Lifeline? PRO AED had the best performa.ce in most of the criteria among AEDs of advanced level and offered CPR video and ma.ual mode for laypersons and clinicians respectively.CONCLUSION: Lifeline? AUTO AED, Sama.itan PAD def ibrillator, Lifeline? PRO AED are superior in AEDs of basic, intermediate and advanced levels, respectively. A feasible AED ma. be chosen by users according to the regional requirement and the current informa.ion about the best available products.

Hepatocellular carcinoma.HCC)is the fifth most common cancer and the third most common cause of cancer-related deaths worldwide.Curative resection is frequently limited in Hong Kong by hepatitis B virus-related cirrhosis,and liver transplantation is the treatment of choice.Liver transplanta-tion has been shown to produce superior oncological benefits,when compared to hepatectomy for HCC.New developments in the context of patient selection criteria,modification of organ allocation,bridging therapy,salvage liver transplanta-tion and pharma.eutical breakthrough have improved the survival of HCC patients.In this article,we will share our experience in transplanting hepatitis B virus-related HCC patients in Hong Kong and discuss the recent progress in several areas of liver transplantation.
