Association of time to prostate-specific antigen nadir and logarithm of prostate-specific antigen velocity after progression in metastatic prostate cancer with prior primary androgen deprivation therapy

We investigated the association of time to prostate-specific antigen nadir （TTPN） and logarithm of prostate-specific antigen velocity after progression Log（PSAVAP） in metastatic prostate cancer with prior primary androgen deprivation therapy （ADT）. All metastatic prostate cancer patients treated with primary ADT from 2000 to 2009 were reviewed. Patients who developed disease progression were included in the subsequent analyses. Patients were categorized into three groups according to their TTPN： TTPN of 〈3 months, 3-17 months, and 〉17 months. We compared the Log（PSAVAP） between the different TTPN groups using Mann-Whitney U-test and Kruskal-Wallis test. Further multiple linear regression analyses on Log（PSAVAP） were performed to adjust for other potential confounding factors. Among 419 patients who were treated with primary ADT, 306 patients developed disease progression with a median follow-up of 28 months, Longer TTPN was associated with lower Log（PSAVAP）（P = 0.008） within all subgroup analyses （TTPN of 〈3 vs 3-17 months, P = 0.020; TTPN of 3-17 vs 〉17 months, P = 0.009; and TTPN of 〈3 vs 〉17 months, P = 0.001）. Upon multiple linear regression analyses, baseline PSA （regression coefficient 0.001, P = 0.045）, PSA nadir （regression coefficient 0.002, P = 0.040）, and TTPN （regression coefficient -0.030, P = 0.001） were the three factors that were significantly associated with Log（PSAVAP）. In conclusion, a longer TTPN was associated with lower Log（PSAVAP） in metastatic prostate cancer patients following primary ADT. TTPN cut-offs at 3 months and 17 months appeared to have prognostic significance in predicting Log（PSAVAP）. TTPN may serve as a good prognostic indicator in deciding the treatment strategy in patients with disease progression.

Objective: To report the clinical maternal and fetal outcomes of pregnant women with coronavirus disease 2019 (COVID-19), along with any associated pregnancy complications, in Hong Kong, China, and to assess the impact of COVID-19 vaccination on these ***: This prospective registry-based observational study included pregnant women who were recruited through convenient sampling and had a laboratory-confirmed severe acute respiratory syndrome coronavirus 2 infection with a cycle threshold (Ct) value result available on admission to eight local hospitals in Hong Kong, China. Data on clinical symptoms, laboratory results, medical treatments, delivery timing and mode, and pregnancy complications were extracted from the Hospital Authority’s electronic medical record system. Maternal, fetal, and pregnancy outcomes were compared between unvaccinated pregnant women with COVID-19 and those who had received at least one dose of COVID-19 vaccine before diagnosis. Nonparametric continuous variables and categorical variables were analyzed using the Mann-WhitneyU test and the Pearson’s chi-squared test respectively. AP value less than 0.05 was considered statistically ***: A total of 164 pregnant women were included, of whom 78 (47.56%) were nulliparous. COVID-19 was diagnosed before 28 weeks’ gestation in 30 (18.29%), while 134 (81.71 %) were diagnosed at or after 28 weeks’ gestation. Sixty-two (37.80%) women received at least one dose of COVID-19 vaccine. There were no significant differences between vaccinated and unvaccinated groups in the time interval between COVID-19 diagnosis and delivery, the Ct value, and the gestational age at infection onset or delivery (P > 0.05). The majority of women were symptomatic at diagnosis regardless of vaccination status (55 (88.71%) in vaccinated groupvs. 78 (76.47%) in unvaccinated group (P = 0.052). The global antibiotic resistance crisis necessitates urgent solutions. One innovative approach involves potentiating antibiotics and non-antibiotic drugs with adjuvants or boosters. A major drawback of these membrane-active boosters is their limited biocompatibility, as they struggle to differentiate between prokaryotic and eukaryotic membranes. This study reports the chemical biology investigation of a dual-action oligoamidine(OA1) booster with a glutathione-triggered decomposition mechanism. OA1, when combined with other antimicrobial molecules, exhibits a triple-targeting mechanism including cell membrane disruption, DNA targeting, and intracellular enzyme inhibition. This multi-targeting mechanism not only enhances the in vitro and in vivo eradication of antibiotic-resistant “ESkaPE” pathogens, but also suppresses the development of bacterial resistance. Furthermore, OA1 maintains its activity in bacterial cells by creating an oxidative environment, while it quickly decomposes in mammalian cells due to high glutathione levels. These mechanistic insights and design principles may provide a feasible approach to develop novel antimicrobial agents and effective anti-resistance combination therapies.

目的用音樂療法作為放鬆的介入方式,以減輕結腸鏡檢查者術中的不適,促進結腸鏡檢查的進程及改善舒適度。方法2021年4月12日~5月18日,於澳門某醫院對104位結腸鏡檢查者進行研究,分為研究組及對照組(每組各52人)。研究組結腸鏡檢查者介入音樂療法,對照組予常規照護,比較兩組疼痛評分(numerical rating scale,NRS)、收縮壓、舒張壓、心率及結腸鏡檢查進鏡時間的差異。結果兩組研究參與者在NRS疼痛評分、術中及術後血壓及心率無差異;兩組結腸鏡檢查的進鏡時間均數沒有顯著差異;但進鏡時間≦10分鐘組(非困難腸鏡組)與>10分鐘組(困難腸鏡組)則存在組間差異,在非困難腸鏡組中,研究組進鏡時間為6.2±1.6分鐘,對照組進鏡時間為7.2±1.6分鐘(T=-2.075,P<0.05);困難腸鏡組則沒有顯著差異。結論音樂放鬆療法對結腸鏡檢查者的疼痛緩解、血壓及心率穩定沒有顯著作用,但仍可縮短“非困難腸鏡”的進鏡時間。

A biosynthetic gene cluster for the bioactive fungal sesterterpenoids variecolin(1)and variecolactone(2)was identified in Aspergillus aculeatus ATCC *** production of 1 and 2 was achieved in Aspergillus oryzae by expressing the sesterterpene synthase VrcA and the cytochrome P450 ***,the replacement of VrcB with homologous P450s from other fungal terpenoid pathways yielded three new variecolin analogues(5-7).Analysis of the compounds’anticancer activity in vitro and in vivo revealed that although 5 and 1 had comparable activities,5 was associated with significantly reduced toxic side effects in cancer-bearing mice,indicating its potentially broader therapeutic *** study describes the first tests of variecolin and its analogues in animals and demonstrates the utility of synthetic biology for creating molecules with improved biological activities.
