Pedicled abdominal flap using deep inferior epigastric artery perforators for forearm reconstruction: A case report

BACKGROUND Pedicled abdominal flaps are a widely used surgical technique for forearm reconstruction in patients with soft tissue ***,some drawbacks include restricted flap size,partial flap loss,and donor-site *** address these concerns,we present a case of a pedicled abdominal flap using the deep inferior epigastric artery perforators(DIEP)for forearm reconstruction in a patient with a large soft tissue *** SUMMARY A 46-year-old male patient was admitted to our hospital with forearm injury caused by a pressing machine.A 15 cm×10 cm soft tissue defect with complete rupture of the ulnar side structures of the forearm was *** week after orthopedic management of the neurovascular injury and fractures using the first stage of Masquelet technique,the patient was referred to the plastic and recon-structive surgery department for wound *** debridement and negative-pressure wound therapy revealed a 20 cm×15 cm soft tissue defect.A pedicle abdominal flap with the DIEP was used to cover the *** weeks later,the flap was detached from the abdomen,and the abdominal defect was directly ***,the second stage of Masquelet technique was performed at the fracture site at week ***,all donor and recipient sites healed without complications,such as flap dehiscence,infection,hematoma,or *** site osteosynthesis was achieved without *** Pedicled abdominal flap using the DIEP provides a reliable option for forearm reconstruction in patients with large soft tissue defects.

Design and compensation of second-order sub-sampling digital frontend

The problem of designing a digital frontend (DFE) was considered which can dynamically access or sense dual bands in any radio frequency (RF) regions without requiring hardware changes. In particular, second-order bandpass sampling (BPS) as a technique that enables to realize the multiband reception function was discussed. In a second-order BPS system, digital reconstruction filters were utilized to eliminate the interferences generated while down converting arbitrarily positioned RF-band signals by using the direct digitization method. However, the inaccuracy in the phase shift or the amplitude mismatch between the two sample streams may cause insufficient rejection of interference. Practical problems were studied, such as performance degradation in signal-to-interference ratio (SIR) and compensation methods to overcome them. In order to demonstrate the second- order BPS as a flexible DFE suitable for software-defined radio (SDR) or cognitive radio (CR), a DFE testbed with a reconfigurable structure was implemented. Moreover, with a view to further demonstrate the proposed compensation algorithms, experimental results show that dual bands are received simultaneously.

Label-free Detection for a DNA Methylation Assay Using Raman Spectroscopy

Background： DNA methylation has been suggested as a biomarker for early cancer detection and treatment. Varieties of technologies for detecting DNA methylation have been developed, but they are not sufficiently sensitive for use in diagnostic devices. The aim of this study was to determine the suitability of Raman spectroscopy for label-free detection of methylated DNA. Methods： The methylated promoter regions of cancer-related genes cadherin 1 （CDH1） and retinoic acid receptor beta （RARB） served as target DNA sequences. Based on bisulfite conversion, oligonucleotides ofmethylated or nonmethylated probes and targets were synthesized for the DNA methylation assay. Principal component analysis with linear discriminant analysis （PCA-DA） was used to discriminate the hybridization between probes and targets （methylated probe and methylated target or nonmethylated probe and nonmethylated target） of CDH! and RARB from nonhybridization between the probe and targets （methylated probe and nonmethylated target or nonmethylated probe and methylated target）. Results： This study revealed that the CDH1 and RARB oligo sets and their hybridization data could be classified using PCA-DA. The classification results for CDH1 methylated probe ＋ CDH1 methylated target versus CDH! methylated probe ＋ CDHI unmethylated target showed sensitivity, specificity, and error rates of 92%, 100%, and 8%, respectively. The classification results for the RARB methylated probe ＋ RARB methylated target versus RARB methylated probe ＋ RARB unmethylated target showed sensitivity, specificity, and error rates of 92%, 93%, and 11%, respectively. Conclusions： Label-free detection ofDNA methylation could be achieved using Raman spectroscopy with discriminant analysis.

Introduction Antibiotic use for acute otitis media(AOM)is one of the major sources of antimicrobial ***,the effective minimal antibiotic duration for AOM remains ***,guidelines often recommend broad ranges(5-10 days)of antibiotic use,yet the clinical impact of such a wide window has not been *** We systematically searched PubMed/MEDLINE,Embase,Scopus,Web of Science,and Cochrane Library from database inception to 6 October *** meta-analysis was conducted on randomized controlled trials that assessed antibiotic treatment for AOM in children(PROSPERO CRD42020196107).Results For amoxicillin and amoxicillin-clavulanate,7-day regimens were noninferior to 10-day regimens in clinical responses[amoxicillin:risk ratio(RR)0.919(95%CI 0.820-1.031),amoxicillin-clavulanate:RR 1.108(0.957-1.282)],except for≤2 *** the third-generation cephalosporins,7-day and 10-day regimens had similar clinical responses compared to placebo[7-day:RR 1.420(1.190-1.694),10-day:RR 1.238(1.125-1.362)compared to placebo].However,5-day regimens of amoxicillin-clavulanate and third-generation cephalosporins were inferior to 10-day *** to amoxicillin,a shorter treatment duration was tolerable with *** Our findings indicated that 1O days of antibiotic use may be unnecessarily long,while the treatment duration should be longer than 5 ***,5-day regimens would be sufficient for a modest treatment *** findings revealed that the current wide range of recommended antibiotic durations may have influenced the clinical outcome of AOM,and a narrower antibiotic duration window should be re-established.
