BACKGROUND Acute carpal tunnel syndrome(ACTS)is commonly caused by repetitive strain,trauma,or inflammatory ***,ACTS due to tophaceous gout is a clinical event that remains poorly understood and *** rare manifestation necessitates prompt diagnosis and intervention to prevent irreversible *** SUMMARY A 51-year-old man who had poorly controlled hyperuricemia presented with ACTS secondary to tophaceous *** of rapid symptom progression symptoms and severe median nerve compression within 3 mo,the patient underwent emergency decompression surgery for both wrists at different time ***,he exhibited complete recovery of sensory and motor functions,with no recurrence at long-term *** outcomes were achieved through immediate decompression surgery,anti-inflammatory medications,postoperative active and passive range-of-motion exercises,and intermittent wrist *** diagnosis and surgical intervention,when necessary,are crucial for preventing long-term complications and obtaining favorable outcomes in patients with *** optimal gout management strategy involving pharmacologic therapy and lifestyle modifications may help minimize ACTS recurrence and improve clinical *** Prompt surgical intervention and optimal gout management are crucial for preventing irreversible nerve damage and ACTS recurrence.

Biogeochemistry of sedimentary organic matter in the Yongjiang River estuary in the southern part of Hangzhou Bay,china,since the Late Pleistocene

A sediment core(YJK19-02)collected from the southern outlet of Hangzhou Bay near the Yongjiang River estuary in East china was analyzed for grain size,lignin,bulk elemental composition,stable carbon isotope,and rare earth elements(REEs)to determine the sources and diagenesis of sedimentary organic matter(OM)of the estuary and adjacent areas since the Late Pleistocene.δ^(13)C values(-24.80‰–-23.60‰),total organic carbon/total nitrogen(TOC/TN)molar ratios(8.00–12.14),and light rare earth element/heavy rare earth element ratios(LREE/HREE=8.34–8.91)revealed the predominance of terrestrial sources of OM,mainly from the Changjiang(Yangtze)*** lignin parameters of syringyl/vanillyl(S/V=0.20–0.73)and cinnamyl/vanillyl(C/V=0.03–0.19)ratios indicate the predominance of nonwoody angiosperms,and the vanillic acid/vanillin ratios[(Ad/Al)_(V)=0.32–1.57]indicate medium to high degrees of lignin *** increasing trend ofΛ(total lignin in mg/100-mg OC)values from ca.14500 a BP to ca.11000 a BP reflected the increase in temperature during the Late ***,a time lag effect of temperature on vegetation abundance was also *** relatively higher and stableΛvalues correspond to the higher temperature during the mid-Holocene from ca.8500 a BP to ca.4500 a BP.Λvalues decreased from ca.4000 a BP to the present,corresponding to historical temperature fluctuations during this *** results show that the vegetation abundance in the Yongjiang River Basin since the Late Pleistocene was related to the temperature fluctuation duo to climate change.

BACKGROUND Simple bone cysts(SBC)are benign tumor-like bone lesions typically identified in *** SBC may lead to growth disturbances or growth arrest,such cases are *** mechanisms behind these observations remain ***,research on the etiology of SBC remains inconclusive,and there has been no consensus on the appropriate timing and methodology for *** SUMMARY Here,we present our experience in the successful surgical management of a 10-year-old girl with SBC,who presented with a pathological fracture complicated by malunion of the displaced fracture,varus deformity,and limb length *** hypothesized two possible etiologies for the patient’s growth arrest and subsequent humerus varus deformity:(1)Direct disruption of the physis by fluid from the cyst itself;and(2)damage to the epiphysis due to repetitive pathological fractures associated with *** addressing this case,surgical intervention was undertaken to correct the proximal humerus varus *** approach offered the advantages of simultaneously correcting angular abnormalities,achieving mild limb lengthening,providing definitive SBC treatment,and reducing the overall treatment *** As per current literature,acute correction of acute angular deformity in proximal humeral SBC is not well ***,in this specific case,acute correction was considered an optimal solution.

Thermodynamic theory of flotation for a complex multiphase solid -liquid system and high-entropy flotation

The flotation of complex solid–liquid multiphase systems involve interactions among multiple components,the core problem facing flotation ***,the combined use of multicomponent flotation reagents to improve mineral flotation has become an important issue in studies on the efficient use of refractory mineral ***,studying the flotation of complex solid–liquid systems is extremely difficult,and no systematic theory has been developed to *** addition,the physical mechanism associated with combining reagents to improve the flotation effect has not been unified,which limits the development of flotation theory and the progress of flotation *** this study,we applied theoretical thermodynamics to a solid–liquid flotation system and used changes in the entropy and Gibbs free energy of the reagents adsorbed on the mineral surface to establish thermodynamic equilibrium equations that de-scribe interactions among various material components while also introducing adsorption equilibrium constants for the flotation reagents adsorbed on the mineral *** homogenization effect on the mineral surface in pulp solution was determined using the chemical potentials of the material components of the various mineral surfaces required to maintain *** flotation effect can be improved through synergy among multicomponent flotation reagents;its physical essence is the thermodynamic law that as the number of compon-ents of flotation reagents on the mineral surface increases,the surface adsorption entropy change increases,and the Gibbs free energy change of adsorption *** to the results obtained using flotation thermodynamics theory,we established high-entropy flotation theory and a technical method in which increasing the types of flotation reagents adsorbed on the mineral surface,increasing the adsorption entropy change of the flotation reagents,decreasing the Gibbs free energy change,and improving the adsorption efficiency

High glucose reduces Nrf2-dependent cRAGE release and enhances inflammasome-dependent IL-1βproduction in monocytes:the modulatory effects of EGCG

Soluble receptor for advanced glycation end products(sRAGE)acts as a decoy sequestering of RAGE ligands,thus preventing the activation of the ligand-RAGE axis linking human ***,the molecular mechanisms underlying sRAGE remain *** this study,THP-1 monocytes were cultured in normal glucose(NG,5.5 mmol/L)and high glucose(HG,15 mmol/L)to investigate the effects of diabetesrelevant glucose concentrations on sRAGE and interleukin-1β(IL-1β)*** modulatory effects of epigallocatechin gallate(EGCG)in response to HG challenge were also *** enhanced intracellular reactive oxygen species(ROS)generation and RAGE *** secretion of sRAGE,including esRAGE and cRAGE,was reduced under HG conditions,together with the downregulation of a disintegrin and metallopeptidase 10(ADAM10)and nuclear factor erythroid 2-related factor 2(Nrf2)nuclear ***,the HG effects were counteracted by siRAGE and exacerbated by *** immunoprecipitation results showed that Nrf2 binding to the ADAM10 promoter and HG interfered with this *** data reinforce the notion that RAGE and Nrf2 might be sRAGE-regulating *** HG conditions,the treatment of EGCG reduced ROS generation and RAGE ***-stimulated cRAGE release was likely caused by the upregulation of the Nrf2-ADAM10 *** inhibited HG-mediated NLRP3 inflammasome activation at least partly by stimulating sRAGE,thereby reducing IL-1βrelease.

Observation of gapless corner modes of photonic crystal slabs in synthetic translation dimensions

Second-order topological photonic crystals support localized corner modes that deviate from the conventional bulk-edge ***,the frequency shift of corner modes spanning the photonic band gap has not been experimentally ***,we observe the gapless corner modes of photonic crystal slabs within a parameter space by considering translation as an additional synthetic *** corner modes,protected by topological pumping in synthetic translation dimensions,are found to exist independently of the specific corner *** gapless corner modes are experimentally imaged via the near-field scanning measurement and validated numerically by full-wave *** propose a topological rainbow with gradient translation,demonstrating the ability to extract and separate specific frequency components of light into different spatial *** work contributes to the advancement of topological photonics and provides valuable insights into the exploration of gapless corner modes in synthetic dimensions.

BACKGROUND Gallbladder rupture is common in laparoscopic cholecystectomy because the gallbladder is usually in acute or chronic inflammation *** gallstones may sometime be spilled into the peritoneal cavity,resulting in intra-abdominal ab-scess if the gallstones were not *** diagnosis of intra-abdominal ab-scess caused by unretrieved gallstone can usually be correctly identified in the routine imaging studies,such as abdominal ultrasonography or computed tomo-graphy(CT).Here we present a case of abscess formation from unretrieved gall-stone following laparoscopic cholecystectomy,which mimics the imaging findings of metastatic gallbladder *** SUMMARY This case described a 78-year-old man who received laparoscopic cholecystectomy and gallbladder adenocarcinoma was diagnosed after *** adjuvant chemotherapy,the following up abdominal CT showed several small nodules at right upper abdomen and peritoneal carcinomatosis is *** laparoscopic surgery for the excision of seeding tumor was conducted and the pathological diagnosis of the nodules and mass was inflammatory tissues and gallbladder *** Spilled gallstones are a common complication during laparoscopic cholecystectomy and some gallstones fail to be retrieved due to the size or the restricted view of laparoscopic *** spilled gall bladder stones,surgeons may consider regular computerized tomography follow-up,and if necessary,laparoscopic examination can be used as a means of confirming the diagnostic and treatment.

Isotopic insights on quantitative assessments of interaction of eco-hydrological processes in multi-scale karst watersheds

The dynamics of hydrological processes and the storage mechanisms of karst water resources are the most important issues in karst *** impact of environmental changes on water quantity,and the evaluation and quantification of eco-hydrological processes remain poorly *** this study,high-frequency continuous monitoring in multi-scale karst watersheds in Southwest china combined the approaches of water isotopes and the hybrid single-particle lagrangian integrated trajectory(HYSPLIT)model to identify the recharge mechanisms between atmospheric vapor,rainfall,surface water,and groundwater,and to reveal the interaction of eco-hydrological *** dominant moisture sources in Puding(PD)County were the Indian Ocean(43-69%)and local moisture(24-33%).Theδ^(18)O and deuterium excess(d-excess)values showed a positive correlation indicating that secondary or sub-cloud evaporation was prominent in the wet *** water line-conditioned excess(lc-excess)indicated that karst water interacted with recent precipitation,groundwater,and evaporation across *** to its specific hydrogeological structure,surface water and rainwater have a higher contribution rate to groundwater *** Chenqi stream replenished the Houzhai River mainly in the form of groundwater,with percentages ranging from 38.1 to 93.5%in the wet season,and 47.8-80.1%in the dry *** the Houzhai outlet,surface water and groundwater interconverted frequently with a percentage of 45.6-49.1%.We believe this is the first systematic study to quantify the supply relationship between water vapor transport,rainfall,surface water and groundwater in the chinese karst zone,making a significant move forward in the field of karst hydrological processes and improving the efficiency of water resource evaluation and management.
