Disparities in Prevalence and Determinants of Hypertension amongst Bamiléké Adults Residing in Two Different Agroecological Zones of Cameroon

Background: Cardiovascular diseases such as hypertension (HTN) are one of the main causes of death in Cameroon. This study aimed at assessing prevalence disparities and determinants of hypertension amongst Bamiléké adults residing in two different agroecological zones of Cameroon. Methods: A cross-sectional and descriptive survey was conducted among Bamiléké population living in the Highlands zone (Western region) and in the Monomodal Rainforest zone (Littoral region) of Cameroon from August 2016 to August 2017. Participants (962) were aged at least 20 years old. Data on sociodemographic, hemodynamic, anthropometric, and biochemical parameters and lifestyle of the participants were collected. Results: Results obtained revealed that 34.2% were hypertensive and those residing in the highland zone were more affected than those living in the monomodal rainforest zone (44.5% vs 22.9%). The different subtypes of HTN (Isolated systolic hypertension (14.1%), isolated diastolic hypertension (7.2%) and Systo-diastolic hypertension (23.3%)) were also more prevalent in the Highlands Zone. The most prevalent stage of HTN was pre-HTN (31.5%). However, people living in the monomodal rainforest zone were more affected by pre-HTN compared to Bamiléké living in the highland zone (33.6% vs. 29.6%). Results also showed that high consumption (≥ 3 times/week) of carbohydrate- and fat-rich foods, ageing, obesity, and marital status were associated with high blood pressure in both agroecological zones. Besides, secondary education (OR = 0.68;95% CI: 0.42 - 0.99) in the Highlands Zone and high (≥3 times/week) vegetable consumption (OR = 0.66;95% CI: 0.44 - 0.98) in the Monomodal Rainforest Zone had a protective effect on elevated blood pressure of population. Conclusion: There is a disparity in the prevalence of hypertension and some of its determinants among Bamiléké adults residing in different agroecological zones. A 105-day feeding study was conducted to evaluate the effect of partial （0%, 20%, 40%, 60% and 80%） and complete （100%） substitution of dietary fish meal （FM） with frozen Artemia on growth performance and feed utilization of Giant freshwater prawn （Macrobrachium rosenbergii） post larvae （PL） reared in fiberglass tanks under controlled conditions. A total of 1080 M. rosenbergii PL, average weight of 6.10 ± 0.4 mg, were equally distributed over 18 fiberglass tanks, （1 m^3 each, and triplicate per treatment）. Six experimental isocaloric （14 MJ/kg/digestible energy） and isonitrogenous （35% crude protein） diets were lbrmulated. Control diet （D1, 100% FM） was formulated with FM as the main protein source （0% Artemia）. Diets D2-D6 were substituted with five dietary protein levels of frozen Artemia （20%, 40%, 60%, 80% and 100%）, respectively. Results showed that growth performance of M. rosenbergii PL fed different Artemia based diets was either equal or superior to prawn fed control FM diet. In addition, increasing dietary frozen Artemia replacement level from 20% up to 100% led to a significant （P 〈 0.05） increase in survival, growth and feed utilization, which indicate that the essential nutrient requirements needed for M. rosenbergii PL was covering with improving feed satiation of PLs. It could be concluded that frozen Artemia biomass instead of fishmeal in M. rosenbergii PL diets can improve survival rate, growth performance, feed utilization, contributing to reducing the use of FM in practical diets and the nursery phase could be shorter and more profitable.

A 90-day experiment was conducted to investigate the effects of different dietary linoleic acid （LA; 18：2n-6） and linolenic acid ratios （LNA; 18：3n-3） on growth induces, feed utilization and tissue fatty acid profile of freshwater prawn, Macrobrachium rosenbergii post-larvae （PL）. The experiment was conducted in cubic indoor fiberglass tanks, each holding 700 L in triplicate. Post-larvae with an average weight of 20.8 ± 0.20 mg were stocked at 80 PL m2. Five experimental isocaloric （15.06 MJ kgl digestible energy）, and isonitrogenous （30.45% digestible protein） diets were formulated by blending of soybean oil and linseed oil to containing five dietary LA/LNA ratios （7.80, 2.75, 1.28, 0.65 and 0.30）. The highest survival values were recorded for prawn PL fed diet containing 0.65 LA/LAN ratios. Growth indices of PL significantly increased （P 〈 0.05） with decreased dietary LA/LAN ratios to 0.65. The same trend was observed for the highest （P ≤ 0.05） protein efficiency ratio, protein productive value, fat retention, energy retention and best feed conversion ratio. The total whole tissue polyunsaturated fatty acid composition of M. rosenbergii PL was dominated by LA followed by LAN. Post larvae fed the diets containing higher LA/LNA ratios showed a higher tissue LA/LNA ratio. The obtained findings revealed that fatty acid patterns ofM. rosenbergii PL were influenced by fatty acid profiles of diets. The diet containing 0.65 LA/LNA ratio is recommended to obtaining optimum growth performance and feed utilization for M. rosenbergii PL.

PURPOSE: To analyze clinical features of the tubulointerstitial nephritis and uveitis (TINU) syndrome in Japan, especially exacerbations or recurrences of uveitis. DESIGN: Retrospective observational study. METHODS: We reviewed the clinical features in 12 patients with TINU syndrome diagnosed by renal biopsy, who were evaluated and treated at the Department of Ophthalmology, Hokkaido University Hospital. Visual acuity was measured using decimal visual acuity. RESULTS: Patient age ranged from 10 to 33 years (mean 21 years), and females accounted for 83% of the cases. Both eyes were affected in 11 patients (92% ). All 12 patients initially experienced ocular symptoms, with hyperemia being the chief complaint. We diagnosed patients as having uveitis and conducted blood tests and urinalysis. Urinalysis did show characteristic findings, most notably glucosuria, increased β 2 microglobulin that were <10 times that of normal levels, and increased N-acetylglucosaminidose levels in a high percentage of patients. Recurrent or exacerbating uveitis was seen in six patients (50% ). In exacerbated or recurrent uveitis, in flammation was more severe (< 3+ cells of the anterior eye segment). The ocular inflammation generally responded well to treatment with oral corticosteroids, and the number of recurrences was lower at corticosteroid doses of < prednisolone 40 mg. CONCLUSIONS: Ophthalmologists play an important role in the initial discovery of patients with TINU syndrome. Because inflammation was more severe in exacerbated or recurrent uveitis, in instances where uveitis is refractory to local therapy, oral corticosteroids should be considered as early as possible.

基于2008年4月22—26日吕宋海峡调查航次的下放式声学多普勒流速剖面仪(LADCP)和船载ADCP(SADCP)等观测资料,并采用潮波模式模拟结果去除潮流对观测资料的影响,观测结果表明:调查期间黑潮入侵南海的位置与1992年春季比较接近,其分支位于调查海区中部C2、C7、C8和C9站,表层黑潮在C8站分离为两支,分别流向C9和C2站,C9站北向流明显比C8站减弱。在C2站,黑潮分支位于400m层以浅,其最大西向流速为77cm/s,而在C7、C8和C9站黑潮分支位于500m层以浅,黑潮在入侵南海的过程中其核心深度逐渐变浅。上层黑潮明显作反气旋弯曲。本调查航次的观测结果在定性上支持吕宋海峡水交换有"三明治"垂直结构的特性。
