Background: Mario Bertolotti, (1917) described LSTV-Lumbar spinal transitional vertebra as Bertolotti Syndrome a century ago and associated low back pain with it. Yet, it needs to be given significance in general orthopaedic practice even now, and radiologists underreport it. LSTV is a congenital anatomical anomaly that Castellvi classified into four varying types. Purpose: I titled this phenomenon “kiran Nandivada’s stuck blade in a grinder jar degenerative cascade by LSTV at L5-S1” to clearly explain how an abnormal mega-transverse apophysis with its various variations affects the weight-bearing mechanics as the L5-S1 which is a vital junction where the maximum body weight is directed into both the sacroiliac joints and if a transitional vertebra occurs it becomes detrimental and abnormally redirects the load-bearing forces and leads to a progressive degenerative cascade both proximally and distally. As the L5 vertebral rotations and other movements of flexion, extensions, lateral flexion are stuck like a bent grinder blade, the other areas of the region are damaged progressively as the pelvis, just like the grinder motor tries to move it, resulting in overheating and maybe even a burnout results in the form of facet arthritis, disc degeneration in the normal disc above, the transitional disc at L5-S1, foraminal osteophytes causing radiculopathy, sacroiliac joint arthritis. Material and Methods: Around 200 X-rays of children and adults with this congenital anomaly have been studied between 2020 and 2023. This is a retrospective study. Results: 1) Our study showed an Increased incidence of LSTV at 15%. 2) Patients ranged from asymptomatic, atypical lumbago to classical lumbago with sciatica and claudication. Findings supported facet and sacroiliac joint arthritis and the pain, relieved with physiotherapy, posture corrections, weight reduction, and lifestyle precautions, negating the need for local steroid injections, radiofrequency ablation or surgical excision as

Efficacy of a Diabetes Specific Nutritional Supplement (DSNS) on Glycemic Response in Prediabetic Adults: A Two-Armed, Open-Labelled Randomized Controlled Study

It is well known that Diabetes Specific Nutritional Supplements (DSNSs) are linked to improved glycemic control in individuals with diabetes. However, data on efficacy of DSNSs in prediabetics is limited. This was a two-armed, open-labelled, randomized controlled six-week study on 199 prediabetics [30 - 65 years;Glycosylated Hemoglobin (HbA1c) 5.7% - 6.4% and/or Fasting Blood Glucose (FBG) 100-125 mg/dl]. Two parallel phases were conducted: Acute Blood Glucose Response (ABGR) and Intervention phase. Prediabetic participants were randomized into test (n = 100) and control (n = 99). The primary objective was to assess the ABGR of DSNS versus an isocaloric snack, measured by incremental Area under the Curve (iAUC). Test and control received 60 g of DSNS and 56 g of isocaloric snack (cornflakes) respectively, both in 250 ml double-toned milk on visit days 1, 15, 29 and 43. Postprandial Blood Glucose (PPG) was estimated at 30, 60, 90, 120, 150 and 180 minutes. During the 4 weeks intervention phase, the test group received DSNS with lifestyle counselling (DSNS + LC) and was compared with the control receiving lifestyle counselling alone (LC alone). Impact was studied on FBG, HbA1C, anthropometry, body composition, blood pressure, nutrient intake, and physical activity. The impact of DSNS was also studied using CGM between two 14-day phases: CGM1 baseline (days 1 - 14) and CGM2 endline (days 28 - 42). DSNS showed significantly lower PPG versus isocaloric snack at 30 (p 12, and chromium were reported by DSNS + LC versus LC alone. No other significant changes were reported between groups. It may be concluded that DSNS may be considered as a snack for prediabetic or hyperglycemic individuals requiring nutritional support for improved glycemic control.

Reproductive Performance and Fiber Quality Responses of Cotton to Potassium Nutrition

Potassium (k) deficiency affects cotton growth and development and fiber properties. An experiment was conducted in an outdoor pot culture facility by imposing four potassium stress treatments (100%, 40%, 20% and 0% of optimum k level) prior to flowering during 2010 and 2011 growing season. Upland cotton cultivar, TM-1, was seeded in the pots comprised of fine sand as rooting medium. Flowers and bolls were tagged daily to estimate boll maturation period (BMP). Leaf samples were collected every four days from flowering to maturity to estimate leaf k content. Plant height and node numbers were recorded from emergence to 21 days after treatment. Photosynthesis and stomatal conductance were measured weekly from day of treatment imposition to physiological maturity at an interval of seven days. Stem, leaf, and boll dry-component weights, and boll numbers were recorded at the end of the experiment in each year. From each boll, the lint samples were collected and grouped based on average leaf potassium concentration during BMP, and fiber quality parameters were recorded for each group in each treatment. At high k deficient (0 k) condition, total biomass declined by 27% and 28% in years 2010 and 2011, respectively. Significantly, lower numbers of bolls were retained per plant at 0 k stress treatment during both the years. Leaf photosynthesis (r2 = 0.92) and stomatal conductance (r2 = 0.80) declined with declining leaf k levels. Fiber length, strength, micronaire, and uniformity declined linearly with decrease in leaf k content. Weaker fibers with medium length were produced under k-deficient conditions with micronaire values in the discount range. Fiber uniformity, however, did not decline with decrease in leaf k. The effects of annealing temperatures and chelating agents on the structural and optical properties of ZnO nanoparticles were *** average particle size of ZnO nanoparticles increased with increase of annealing *** decrease of the full width at half maximum(FWHM)with increasing annealing temperatures inferred increase of particle/grain *** grain sizes were also observed to be increased with increase of annealing *** the absorption spectra of the samples,the absorption was red-shifted and the energy band gap was blue-shifted with increase of annealing temperatures.A sharp UV emission peak was observed and the intensity of this peak increased with annealing temperatures corresponding to the high crystallinity in the *** high annealing temperature of 700℃,ZnO exhibited a less intense deep level *** negligible deep level emission was attributed to the oxygen vacancies created at higher annealing temperatures.

Hormonal contraception for human males:prospects

Development of an ideal hormonal contraceptive for man has been the goal of several research workers during thepast few decades. Suppression of pituitary gonadotropic hormones, which in turn would inhibit spermatogenesis whilemaintaining normal libido and potentia has been the approach for a contraceptive agent. Intramuscularly administeredand orally active testosterone or testosterone in combination with progesterone have been shown to cause inhibition ofspermatogenesis resulting in azoospermia in normal men. Similarly testosterone has been used in combination with go-nadotropin releasing hormone antagonists and agonists to inhibit pituitary gonadotropic hormone release. Immunologicalapproach to neutralize the circulating levels of follicle stimulating hormone has also been shown to cause inhibition ofspermatogenesis. The available literature shows that testosterone causes reversible azoospermia without any significantside effects in Asian population effectively and appears to be a promising chemical for control of fertility in man.

Background: Duodenoscope-related multidrug-resistant organism(MDRO) infections raise concerns. Disposable duodenoscopes have been recently introduced in the market and approved by regulatory agencies with the aim to reduce the risk of endoscopic retrograde cholangiopancreatography(ERCP) associated infections. The aim of this study was to evaluate the outcome of procedures performed with single-use duodenoscopes in patients with clinical indications to single-operator cholangiopancreatoscopy. Methods: This is a multicenter international, retrospective study combining all patients who underwent complex biliopancreatic interventions using the combination of a single-use duodenoscope and a single-use cholangioscope. The primary outcome was technical success defined as ERCP completion for the intended clinical indication. Secondary outcomes were procedural duration, rate of cross-over to reusable duodenoscope, operator-reported satisfaction score(1 to 10) on performance rating of the single-use duodenoscope, and adverse event(AE) rate. Results: A total of 66 patients(26, 39.4% female) were included in the study. ERCP was categorized according to ASGE ERCP grading system as 47(71.2%) grade 3 and 19(28.8%) grade 4. The technical success rate was 98.5%(65/66). Procedural duration was 64(interquartile range 15-189) min, cross-over rate to reusable duodenoscope was 1/66(1.5%). The satisfaction score of the single-use duodenoscope classified by the operators was 8.6 ± 1.3 points. Four patients(6.1%) experienced AEs not directly related to the single-use duodenoscope, namely 2 post-ERCP pancreatitis(PEP), 1 cholangitis and 1 ***: Single-use duodenoscope is effective, reliable and safe even in technically challenging procedures with a non-inferiority to reusable duodenoscope, making these devices a viable alternative to standard reusable equipment.

CFD modeling of pressure drop and drag coefficient in fixed beds:Wall effects

Simulations of fixed beds having column to particle diameter ratio （D/dp） of 3, 5 and 10 were performed in the creeping, transition and turbulent flow regimes, where Reynolds number （dpVLρL/μL） was varied from 0.1 to 10,000. The deviations from Ergun＇s equation due to the wall effects, which are important in D/dp 〈 15 beds were well explained by the CFD simulations. Thus, an increase in the pressure drop was observed due to the wall friction in the creeping flow, whereas, in turbulent regime a decrease in the pressure drop was observed due to the channeling near the wall. It was observed that, with an increase in the D/dp ratio, the effect of wall on drag coefficient decreases and drag coefficient nearly approaches to Ergun＇s equation. The predicted drag coefficient values were in agreement with the experimental results reported in the literature, in creeping flow regime, whereas in turbulent flow the difference was within 10-15%.

Effect of organo-modified montmorillonite nanoclay on mechanical,thermal and ablation behavior of carbon fiber/phenolic resin composites

The mechanical,thermal and ablation properties of carbon phenolic(C-Ph)composites(Type-I)reinforced with different weight percentages of organo-modified montmorillonite(o-MMT)nanoclay have been studied *** milling was used to disperse different weight(wt)percentages(0,1,2,4,6 wt.%)of nanoclay into phenolic *** changes to resin due to nanoclay was *** the other hand,nanoclay added phenolic matrix composites(Type-II)were prepared to study the dispersion of nanoclay in phenolic matrix by small angle X-ray scattering and thermal stability changes to the matrix by thermogravimetric analyser(TGA).This data was used to understand the mechanical,thermal and ablation properties of Type-I *** laminar shear strength(ILSS),flexural strength and flexural modulus of Type I composites increased by about 29%,12%and 7%respectively at2 wt.%addition of nanoclay beyond which these properties *** was attributed to reduced fiber volume fraction(%Vf)of Type-I composites due to nanoclay addition at such high *** ablation rate of Type-I composites was evaluated using oxy acetylene torch test at low heat flux(125 W/cm^(2))and high heat flux levels(500 W/cm^(2)).Mass ablation rates have increased at both flux levels marginally up to 2 wt.%addition of nanoclay beyond which it has increased *** is in contrast to increased thermal stability observed for Type-I and Type-II composites up to 2 wt.%addition of *** ablation rates due to nanoclay addition was attributed to higher insulation efficiency of nanolcay,which accumulates more heat energy in limited area behind the ablation front and self-propagating ablation mechanisms triggered by thermal decomposition of organic part of nanoclay.

The myth of “minima” and “maxima”,the species of Physalis in the Indian Subcontinent

The status of the names, Physalis minima L. and P. maxima Mill. (Solanaceae), and their alleged presence on the Indian subcontinent are discussed. The issues of nativity and identity of Linnaean Physalis minima are long-debated while the use of the name P. maxima Mill. and its report from India are recent. The available evidence indicates that the name "P. minima L." is misapplied to two different elements, viz., P. angulata L. and P. lagascae Roem. & Schult. The name Physalis minima L. may be rejected as nomen confusum, for which the paper provides the primary information. As on today, it is submerged under the synonymy of P. angulata L. The correct name for the widely known P. minima is P. lagascae. The name "P. maxima Mill." applied to the escape and naturalized weed in the Indian subcontinent and elsewhere is to be substituted by P. pruinosa L., a name misapplied to P. grisea (Waterf.) M. Martínez.
