Angle of covered self-expandable metallic stents after placement is a risk factor for recurrent biliary obstruction

BACkGROUND Studies have shown that covered self-expandable metallic stents(CSEMS)with a low axial forces after placement can cause early recurrent biliary obstruction(RBO)due to precipitating sludge *** To ascertain whether the angle of CSEMS after placement is a risk factor for RBO in unresectable distal malignant biliary obstruction(MBO).METHODS Between January 2010 and March 2019,261 consecutive patients underwent selfexpandable metallic stent insertion by endoscopic retrograde cholangiopancreatography at our facility,and 87 patients were included in this *** evaluated the risk factors for RBO,including the angle of CSEMS after placement as the primary *** measured the obtuse angle of CSEMS after placement on an abdominal radiograph using the SYNAPSE PACS *** also evaluated technical and functional success,adverse events,time to RBO(TRBO),non-RBO rate,survival time,cause of RBO,and reintervention procedure as secondary *** We divided the patients into two cohorts based on the presence or absence of *** angle of CSEMS after placement(per 1°and per 10°)was evaluated using the multivariate Cox proportional hazard analysis,which was an independent risk factor for RBO in unresectable distal MBO[hazard ratio,0.97 and 0.71;95%confidence interval(CI):0.94-0.99 and 0.54-0.92;P=0.01 and 0.01,respectively].For early diagnosis of RBO,the cut-off value of the angle of CSEMS after placement using the receiver operating characteristic curve was 130°[sensitivity,50.0%;specificity 85.5%;area under the curve 0.70(95%CI:0.57-0.84)].TRBO in thek factor for *** novel results provide pertinent information for future stent management.

Electricity Generation System with Piezoelectric Element Using Acoustic Radiation Energy

In this study, a circular plate that is installing a piezoelectric element at its center is adopted as energy-harvesting system and is subjected to a harmonic point force. Because this system cannot avoid the influence of its acoustic radiation, the influence is considered theoretically using the equation of plate motion taking into account its radiation impedance and is estimated by the electricity generation efficiency, which is derived from the ratio of the electric power in the electricity generation and the mechanical power supplied to the plate. As a result, the efficiency is suppressed by the acoustic radiation from the plate, so that the efficiencies are so different in whether to take into consideration the radiation impedance or not. Because those results are verified by the electricity generation experiment and radiation acoustic energy has a hopeful prospect for improving the performance of this system, mechanical-acoustic coupling is used to make the most of the acoustic energy. Therefore, a cylinder that has the above plates at both ends is also adopted as the electricity generation system and mechanical-acoustic coupling is caused between the plate vibrations and an internal sound field into the cylindrical enclosure by subjecting one side of each plate to a harmonic point force. Then, the effect of coupling is evaluated by comparing with the efficiencies in the electricity generation system of only plate. Specifically, because the radiation impedance increases with the plate thickness, i.e., with the natural frequency of the plate, it is demonstrated that the effect of coupling becomes remarkable with increasing the thickness on the electricity generation efficiency.

Vegetation Spatial Heterogeneity of Different Soil Regions in Inner Mongolia, China

The vegetation spatial heterogeneity and ecological characteristics in different soil regions were analyzed by surveying the vegetation in 12 different soil regions of Inner Mongolia, China, including coniferous-broadleaf deciduous forests, shrub, grassland, and desert regions with 1122 large 2 cm × 2 cm quadrats （actual size 30 km × 30 km, referred to as L-quadrat hereafter） in about 1.18 million km2. Each L-quadrat was divided into four small 1 cm × 1 cm quadrats （actual size 15 km × 15 km, S-quadrat）. The vegetation was analyzed based on the beta-binomial distribution to describe the frequency of occurrence and spatial heterogeneity for each kind of vegetation. The weighted average of the heterogeneity of all vegetation in the same soil region provides a measure of the soil regional landscape level heterogeneity which describes the spatial complexity of the regional landscape composition of the existing vegetation. Comparison of the vegetation characteristics in the 12 soil regions shows that, the calcic gray soil has the highest average vegetation type per quadrat. The largest soil region is calcic chestnut soil and has the most vegetation types. Every soil region has its own dominant vegetation sequence which dominates in occurrence and dominant vegetation types which dominates in spatial heterogeneity. For the Inner Mongolian vegetation, the weighted average of the heterogeneity is 0.60 and the vegetation diversity index is 4.47.

Spatial Heterogeneity of Vegetation in China

The spatial heterogeneity and ecological characteristics of 74 vegetation types and the lack of vegetation type within the 9.6 million km2 of Chinese territory were studied using data from the China vegetation map. The analysis used 877 large quadrats 10 mm×10 mm （actual size 100 km×100 km）, which covered about 8.77 million km^2. Each large-quadrat was divided into four small 5 mm×5mm quadrats （actual size 50 km×50 km）. The frequency of occurrence of all vegetation types was recorded in each small quadrat. The survey using the Shiyomi method of vegetation analysis based on the beta-binomial distribution was adopted to describe the frequency of occurrence and spatial heterogeneity for each kind of vegetation. The weighted average heterogeneity of all the vegetation types in the landscape provides a measure of the landscape level heterogeneity which describes the spatial intricacy of the existing vegetation composition. The maximum spatial vegetation heterogeneity was 0.8620 in the frigid-temperate coniferous forest of Larix （V1）, which dominates cold moist northeast China. The minimum spatial heterogeneity with a low occurrence was the Caragana tibetica in the gravel desert in the western Yellow River Hetao area. The minimum occurrence with a low spatial heterogeneity was the Monsoon rainforest on rock in Guangxi and Yunnan Provinces. The vegetation types on the Qing-Zang Plateau were found to be representative of Chinese vegetation because of the very high occurrence and spatial heterogeneity of these vegetation types. The weighted average of the heterogeneity was 0.677, while the vegetation diversity index was 3.29.

Phytoestrogens Inhibiting Androgen Receptor Signal and Prostate Cancer Cell Proliferation

The androgen receptor（AR） signaling activated by dihydrotestosterone（DHT） plays critical roles in pros- tate cancer development and progression. Phytoestrogens, which are diphenolic compounds with estrogen and an- ti-estrogen effects, can bind to estrogen receptors. However, their function on AR signaling has not been fully eluci- dated. In this study, dual-luciferase reporter assay, immunobloting, docking system test, MTT assay, immunofluores- cence and chromatin immunoprecipitation（ChlP） assays were employed to examine the potential effects of three phytoestrogens（genistein, daidzein, flavone） on DHT-activated prostate specific antigen（PSA） activation, cell proli- feration and AR transactivation in lymph node carcinoma of prostate（LNCaP） cells. Phytoestrogens were detected to down-regulate DHT-activated AR-mediated PSA promoter transactivation by dual-luciferase reporter system. Fur- thermore, three phytoestrogens, especially genistein, were demonstrated to significantly decrease AR-activated PSA protein expression by Western blotting analysis. MTT experiment proves that phytoestrogens, especially genistein, remarkably inhibits the DHT-indueed cell proliferation in LNCaP cells. To provide reasonable explanations for expe- rimental phenomena mentioned above, we did docking system test and detected phytoestrogens to share the same AR-binding site with DHT. To further prove the competition between phytoestrogen and DHT on AR binding, we examined the effects of phytoestrogens on DHT-activated AR nuclear translocation and immunofluorescence analysis which confirms that phytoestrogens, especially genistein, inhibit DHT-activated androgen receptor nuclear transloca- tion. Results from ChIP show that phytoestrogens down-regulate DHT-induces AR binding to the androgen response elements（AREs, including AREI, AREII, and AREIII） in PSA promoter. AIM To assess cancer-testis antigens(CTAs) expression in gastric cancer patients and examined their associations with clinicopathological *** Eighty-three gastric cancer patients were evaluated in this study. Gastric cancer specimens were evaluated for the gene expression of CTAs, kitakyushu lung cancer antigen-1(kk-LC-1), melanoma antigen(MAGE)-A1, MAGE-A3 and New York esophageal cancer-1(NYESO-1), by reverse transcription PCR. Clinicopathological background information, such as gender, age, tumor size, macroscopic type, tumor histology, depth of invasion, lymph node metastasis, lymphatic invasion, venous invasion, and pathological stage, was obtained. Statistical comparisons between the expression of each CTA and each clinicopathological background were performed using the χ2 test. RESULTS The expression rates of kk-LC-1, MAGE-A1, MAGE-A3, and NY-ESO-1 were 79.5%, 32.5%, 39.8%, and 15.7%, respectively. In early stage gastric cancer specimens, the expression of kk-LC-1 was 79.4%, which is comparable to the 79.6% observed in advanced stage specimens. The expression of kk-LC-1 was not significantly associated with clinicopathological factors, while there were considerable differences in the expression rates of MAGE-A1 and MAGE-A3 with vs without lymphatic invasion(MAGE-A1, 39.3% vs 13.6%, P = 0.034; MAGE-A3, 47.5% vs 18.2%, P = 0.022) and/or vascular invasion(MAGE-A1, 41.5% vs 16.7%, P = 0.028; MAGE-A3, 49.1% vs 23.3%, P = 0.035) and, particularly, MAGE-A3, in patients with early vs advanced stage(36.5% vs 49.0%, P = 0.044), respectively. Patients expressing MAGE-A3 and NYESO-1 were older than those not expressing MAGE-A3 and NY-ESO-1(MAGE-A3, 73.7 ± 7.1 vs 67.4 ± 12.3, P = 0.009; NY-ESO-1, 75.5 ± 7.2 vs 68.8 ± 11.2, P = 0.042). CONCLUSION The kk-LC-1 expression rate was high even in patients with stage I cancer, suggesting that kk-LC-1 is a useful biomarker for early diagnosis of gastric cancer.

Investigation of Prefrontal Cortex Activity in University Students with Presenteeism: A Near-Infrared Spectroscopy (NIRS) Study

Presenteeism refers to impaired performance attributed to attending work with health problems. There has been no study examining the state of presenteeism with objective measures. We compared cerebral hemodynamic changes, measured by near-infrared spectroscopy (NIRS), during neuropsychological tests conducted by university students with presenteeism and healthy controls. Twenty-two university students participated in the study;11 of them with impaired performance caused by mental health problem were allocated to the presenteeism group and 11 without health problems to the control group. Presenteeism was assessed by the Presenteeism Scale for Students. To evoke hemodynamics changes, the participants completed a Word Fluency Test (WFT) and a Trail Making Test (TMT). The NIRS probes were located over the bilateral prefrontal area. Students with presenteeism had significantly higher incidences of depression than controls. However, there was no significant difference in behavioral performance examinations between the two groups. With regard to hemodynamics changes, the repeated measures analysis of covariance of the NIRS signals revealed significant interactions between group and task activation. Although we observed a significant increase in oxygenated hemoglobin concentration during the WFT among controls (simple main effect;left channel, F(1, 19) = 27.34, P F(1, 19) = 22.05, P < 0.001), no changes were found in students with presenteeism during either the WFT (simple main effect;left channel, F(1, 19) = 0.12, P F(1, 19) = 0.08, P t = ﹣0.94, P with Bonferroni correction = 0.745;right channel, t = ﹣2.19, P with Bonferroni correction < 0.113). This is the first study to reveal differences in activity in the cerebral cortex associated with presenteeism. The fact that students with presenteeism have prefrontal dysfunction might reinforce the concept of presenteeism.

Japanese Community Pharmacists’ Practice Research Literacy

Objectives: Lack of basic practice research literacy could be considered a barrier to conducting or participating in practice research. Therefore, we examined pharmacy practice research literacy in Japanese community pharmacists. Methods: Community pharmacists (n = 478) who delivered presentations at three major pharmacy-related conferences in 2012 and 2013 completed the survey. We selected 10 key terms related to practice research and asked participants whether they knew the meaning of. Questionnaires were sent to and returned by pharmacists via mail. Results: Of the 478 pharmacists who received questionnaires, 230 (47.9%) completed the survey. The response rates indicating that participants were familiar with the 10 key terms were approximately 50% or lower. The highest response rate was 67.0% for “bias”, and the lowest was 17.0% for “PICO/PECO: Patient-Intervention-Comparison-Outcome/Patient-Exposure-Comparison-Outcome”. Younger pharmacists tended to know several key terms, such as “p values”, “bias”, and “outcome”. Conclusion: Japanese community pharmacists were not knowledgeable with respect to conducting and participating in pharmacy practice research. Practice research knowledge was superior in younger pharmacists. Education in pharmacy schools and continuing professional development programs is important for community pharmacists’ practice research development.

Japanese Community Pharmacists’ Barriers to Conducting or Participating in Practice Research

Objectives: This study identified barriers to Japanese community pharmacists’ active conduct or participation in practice research. Methods: Community pharmacists (n = 478) who gave presentations at three major pharmacy-related conferences in 2012 and 2013 were questioned about their difficulties of giving presentations, support for better presentations, and barriers to conducting pharmacy practice research in their practical setting. A questionnaire was mailed to and returned by the pharmacists directly. Results: We obtained 230 responses (47.9%). Presentation difficulties included pharmacists’ time constraints and lack of experience organizing the report’s results or discussion. Many thought statistical analysis support was necessary. The barriers were in sufficient time, community pharmacies’ lack of research supervisors, and other community pharmacists’ lack of understanding practice research’s importance. These were comparable to pharmacists’ barriers in other countries, except for money and funds. Conclusions: Japanese community pharmacists should clarify that practice research in their professional roles improves patients’ outcomes. Barriers were similar to pharmacists in other countries. Publication of pharmacists’ practice research results is important to expand their roles. Collaboration between faculties and pharmacists is a challenge for practice research development in the Japanese community setting.
