AIm To review the evidence for the use of different nonsteroidal anti-inflammatory drugs(NSAIDs) in the treatment of biliary *** The strategies employed included an extensive literature review for articles and studies related to biliary colic from electronic databases including Pub med,Science Direct,Wiley Inter Science,medline and Cochrane from last 15 ***: "Biliary colic","management of biliary colic","non-steroidal anti-inflammatory drugs","cholelithiasis" and "biliary colic management".Six randomized control trials,1 non-randomized trial and 1 meta-analysis were included in this *** outcomes of these studies and their significance have been reviewed in this *** Current evidence suggests there are no set protocols for biliary colic pain *** are potent in the management of biliary colic,not only in terms of symptom control but in disease progression as *** from the studies on diclofenac and ketorolac,there are studies which have shown that intravenous tenoxicam and injectable flurbiprofen are equally effective in managing biliary *** efficacy of NSAIDs is superior in terms of lower number of doses and longer duration of action in comparison to other analgesic *** This literature review has found that NSAIDs are safe and effective for pain control in biliary colic,and reduce the likelihood of further complications.

Integration of Renewable Energy Resources in microgrid

microgrid is a new concept in power generation. The microgrid concept assumes a cluster of loads and micro sources operating as a single controllable system that provides both power and heat to its local area. Not much is known about microgrid behavior as a whole system. Some models exist which describe the components of a microgrid. In this paper, model of microgrids with steady state and their transient responses to changing inputs are presented. Current models of a fuel cell, microturbines, wind turbine and solar cell have been discussed. Finally a complete model built of microgrid including the power sources, their power electronics, and a load and mains model in mATLAB/Simulink is presented.

Improving Inter-Domain Fast Handover Using mIH Services in mobile WimAX

Recently, the IP connectivity during the mobile Node (mN) movement between Base Stations (BSs) belonging to different Internet Service Providers (ISPs) is still a key issue to be tackled. In this paper, therefore, we develop a new scheme to improve the performance of inter-domain fast handover over mobile WimAX networks. The framework basically relies on the Fast Handover for mobile IPv6 protocol (FmIPv6) when the media Independent Information Services (mIIS) as defined in IEEE802.21 standard is applied to enable the mobile Node in storing the information of the neighboring networks. A Fully Qualified Domain Name (FQDN) is also used to identify the IP address of the previous network operator and the mN during its movements. Since both mIIS and FQDN can support the node mobility between multiple domains, our proposed scheme can also be called P-FmIPv6. The numerical results show that the latency of IP connectivity of this proposed handover can be significantly reduced in addition to less service disruption time during handovers as compared to the existing FmIPv6 when IEEE802.16e network is considered.

With advances in targeted and personalized treatment for lung cancer, molecular analysis of tumors is routinely performed for sequencing of treatment options in patients with advanced non-small-cell lung cancer (NSCLC). Oncogene addiction due to driver mutations includes EGFR exon 20 insertion mutations, mET amplification, EmL4-AL, kRAS G12C point mutations, RET rearrangements, HER2 amplification and mutations, and FGFR amplification and translocations. A re-biopsy at the time of tumor recurrence or progression after first-line treatment failure is important for further molecular assessment and personalized therapy. However, repeat tumor biopsies are fraught with challenges including access to the tumor, sample inadequacy, patient consent, patient performance status, safety, or physician’s choice or assessment. Cytological specimens are gaining importance but are limited due to validation difficulties. Liquid biopsies, which are minimally invasive have shown promise to assess dynamic biomarkers using ctDNA analysis and are thus frequently considered in routine clinical practice in advanced NSCLC patients to guide further targeted treatment. Here we present a comprehensive review that emphasizes the significance of performing tumor re-biopsy in advanced stage NSCLC patients following resistance to first-line treatment and simultaneously highlights the current challenges in performing the same and the current status and future perspectives of liquid biopsy in NSCLC.

Comparative Study for ECHAm5 and SDSm in Downscaling Temperature for a Geo-Climatically Diversified Region, Pakistan

Pakistan is a country with diversified features in terms of geography and climate. It is an agriculture based country, mainly dependent on Indus water system. In Pakistan, there are loftyplateaus to the north and Arabian Sea in the south, while the interior portion is covered with plateaus or agriculture plains. For such a region, any attempt to monitor/analyze climatic data requires some more specific details. A statistical software “SDSm” is utilized for downscaling daily temperature data of Pakistan and the results generated are compared with the output of a recommended model “ECHAm5”. After analysis, it revealed that comparatively SDSm produced much better results. The outputs from both the approaches were correlated with the observed data;SDSm-observed gave values for correlation coefficient R2 in the range of 81% - 94% whereas ECHAm5-observed produced 73% - 87% for different meteorological stations of Pakistan. On the basis of this study, SDSm can be recommended for future scenario generation of temperature data of Pakistan as well.

Evaluation of Dredging Operations for Tigris River within Baghdad, Iraq

River Tigris divides Baghdad, capital of Iraq, in two parts. The reach of the river within Baghdad is about 60 km long. many islands and bars are obstructing the flow of the river within Baghdad. To overcome this problem, dredging operations started along most of Tigris River inside Baghdad City to remove many islands and side bars, which reduced the flooding capacity and the efficiency of water intakes. An examination for the dredging plan under process and two proposed additional plans was performed using the Hydrologic Engineering Centers River Analysis System software (HEC-RAS) for a 50 km long river reach to investigate whether they can recover the designed flooding capacity of the river or just improving it. Calibration and verification processes were implemented in the model using observed water levels at Sarai Baghdad gauging station and along the last 15 km of the river reach. Comparisons of computed water levels were conducted with those of previous studies and historical data. Some improvement of flood capacity was achieved based on the recorded data of the last three decades. Cautions about the water intakes should be considered to maintain their function with the expected drop in water level due to dredging operations.

Analysis of 1246 Cases of Orbital Lesions: A Study of 17 Years

Purpose: The aim of this study is to present an analysis of orbital lesions by classifying them according to the site of origin in patients of all ages presenting at a tertiary care eye hospital from 1996 till 2012. methods: 1637 patients were initially enrolled in this descriptive case series. Clinical data of 1246 patients who completed the study during 17 years were analyzed. Orbital lesions of the patients were examined and managed medically or surgically as per requirement. The histopathological reports of these patients were used to classify the lesions. Results: Out of all cases 54.57% (n = 680) were neoplastic and 45.42% (n = 566) were nonneoplastic lesions. Amongst the neoplastic lesions 86.17% (n = 586) were malignant and 13.82% (n = 94) were benign. Primary orbital lesions were the most common orbital lesions being 963 (77.29%) followed by secondary orbital lesions being 232 (18.62%), hematopoietic reticuloendothelial being 47 (3.77%) and metastatic lesions being 04 (0.32%). Conclusion: Orbital lesions are more common in adults as compared to children. Neoplastic lesions are more common than nonneoplastic lesions, and amongst the neoplastic lesions malignant lesions are more common than benign ones. Primary orbital lesions are the most common orbital lesions followed by secondary orbital lesions, lesions of the hematopoietic reticuloendothelial system and metastatic lesions.
