Monogenic features of urolithiasis: A comprehensive review

Objective: Urolithiasis formation has been attributed to environmental and dietary factors. However, evidence is accumulating that genetic background can contribute to urolithiasis formation. Advancements in the identification of monogenic causes using high-throughput sequencing technologies have shown that urolithiasis has a strong heritable ***: This review describes monogenic factors implicated in a genetic predisposition to urolithiasis. Peer-reviewed journals were evaluated by a PubMed search until July 2023 to summarize disorders associated with monogenic traits, and discuss clinical implications of identification of patients genetically susceptible to urolithiasis ***: Given that more than 80% of urolithiases cases are associated with calcium accumulation, studies have focused mainly on monogenetic contributors to hypercalciuric urolithiases, leading to the identification of receptors, channels, and transporters involved in the regulation of calcium renal tubular reabsorption. Nevertheless, available candidate genes and linkage methods have a low resolution for evaluation of the effects of genetic components versus those of environmental, dietary, and hormonal factors, and genotypes remain undetermined in the majority of urolithiasis ***: The pathophysiology underlying urolithiasis formation is complex and multifactorial, but evidence strongly suggests the existence of numerous monogenic causes of urolithiasis in humans.

提出了基于力模拟法研究受动力荷载作用的结构非线性特性的一种新的算法，与传统改变刚度矩阵的方法不同，该算法是通过改变结构位移场来进行非弹性分析。结构的每一个非弹性变形代表一个自由度，因而结构所有的非弹性特性可通过一个方程表示。这个算法在计算时间上是非常有效的，因为它仅需使用初始刚度矩阵，且当运动动力分析中的状态空间数值积分法时，可以达到很高的数值精度。而且，这个算法在分析结构应变硬化性质、弹塑性性质和应变软化性质时都是稳定的。通过将这种算法与已有的非弹性动力分析软件程序相比较，说明了这个算法是的、高效的。最后，通过对一个6层医院建筑动力响应分析的应用，演示了该算法在分析实际结构中的完整的非弹性分析过程。

汽油机颗粒捕集器(GPf)是1种重要的排放后处理系统,能使汽油缸内直喷(GDI)发动机达到现行的排放标准。现行标准规定的非挥发性颗粒物直径大于23.0nm。然而,随着排放法规的逐渐严格,GPf过滤效率需要进一步提高,并且可能会对直径低至10.0nm的非挥发性颗粒物排放进行限制。GPf过滤效率取决于在发动机运行期间聚集在GPf上的炭烟量。在车辆运行期间,当排气温度足够高且含有足够的氧气时,GPf通常是“被动”再生的。研究了发动机废气颗粒数排放(PN)和GPf再生频率对GPf过滤效率的影响。采用2种GPf技术,分别在2台发动机台架上进行了测试,并匹配2台量产车在转毂台架上进行了测试。试验发动机颗粒物排放数量分布的带宽很广,几乎达到1个数量级,更具实际排放代表性。GPf的过滤效率通过符合规定的颗粒数系统(非挥发性颗粒直径大于23.0nm、下限为2.5nm)的粒子计数器,以及差分迁移率光谱仪进行测量计算获得。结果显示,GPf有规律地达到可再生的条件,并且GPf的平均驾驶循环过滤效率高度依赖于发动机颗粒物排放量;当发动机颗粒物排放量增加约1个数量级时,GPf的过滤效率显著提高。研究表明,根据发动机颗粒物排放量选择合适的GPf技术非常重要。

Objective:To review the risk of prostate cancer(PCa)in men with incidentally reported increased intraprostatic uptake at 2-deoxy-2-[18f]fluoro-d-glucose positron emission tomography/computed tomography(18f-fDG PET/CT)ordered at Department of Urology,The Wesley Hospital,Brisbane,QLD,Australia for non-PCa related ***:Retrospective analysis of consecutive men between August 2014 and August 2019 presenting to a single institution for 18f-fDG PET/CT for non-prostate related conditions was *** were classified as benign,indeterminate,or malignant depending of the results of prostate-specific antigen(PSA),PSA velocity,biopsy histopathology,and three-Tesla(3 T)multiparametric MRI(mpMRI)Prostate Imaging Reporting and Data System score,or gallium-68-prostate-specific membrane antigen(68Ga-PSMA)PET/CT ***:Three percent(273/9122)of men demonstrated 18f-fDG avidity within the ***-five percent(231/273)were further investigated,including with PSA tests(227/231,98.3%),3 T mpMRI(68/231,29.4%),68Ga-PSMA PET/CT(33/231,14.3%),and prostate biopsy(57/231,24.7%).Results were considered benign in 130/231(56.3%),indeterminate in 31/231(13.4%),and malignant in 70/231(30.3%).PCa was identified in 51/57(89.5%)of the men who proceeded to biopsy,including 26/27(96.3%)men with Prostate Imaging Reporting and Data System scores 4-5 mpMRI and six men with a positive 68Ga-PSMA PET/*** most common Gleason score on biopsy was greater than or equal to 4+5(14/51,27.5%).68Ga-PSMA PET/CT was concordant with the 18f-fDG findings in 26/33(78.8%).All 13 men with a positive concordant 18f-fDG,3 T mpMRI,and 68Ga-PSMA PET/CT had PCa on *** was no statistically significant difference in the 18f-fDG maximum standardized uptake value between the benign or malignant groups(5.7 vs.6.1;p=0.580).Conclusion:In this study,after an incidental finding of an avid intraprostatic lesion on 18f-fDG PET/CT,70 of the 231 cases(30.3%;0.8%of the entire cohort)had results c

IgG4 Coronary Arteritis Presenting as a Cardiac Pseudo-Tumour

We report on an unusual case presenting with a cardiac pseudo-tumour on echocardiogram, which corresponded to a large soft tissue mural thickening around the mid-right coronary artery. There were similar but not as thick mural lesions around other parts of the coronary arteries. The so-called “pigs-in-a-blanket” sign on computed tomography (CT) scan was pathognomonic of IgG4 coronary arteritis. The IgG4 level was grossly elevated at more than 10 times the upper limit of normal. Positron emission tomography (PET)-CT scans with 18f-fluoro-deoxy-glucose (fDG) and 68Ga-fibroblast Activation Protein Inhibitor (fAPI) were performed to assess the extent of organ involvement of the IgG4-related disease. The patient was treated with 8 injections of rituximab with good serological response. However, the coronary arteritis findings on CT scan remained unchanged.
