BACkGROUND The transanal opening of intersphincteric space(TROPIS)procedure,performed to treat complex anal fistulas,preserves the external anal sphincter(EAS)but involves partial incision of the internal anal sphincter(IAS).AIM To ascertain the incidence of incontinence after the division of the IAS as is done in TROPIS and to evaluate whether regular kegel exercises(kE)in the postoperative period can prevent incontinence due to IAS *** Patients operated on for high complex fistulas and having no preoperative continence problem(score=0)were included in the *** patients were operated on by the TROPIS procedure and were recommended kE(pelvic contraction exercises)50 times/*** were commenced on the 10^(th)postoperative day and continued for 1 *** was evaluated objectively(by modified Vaizey’s scores)in the immediate postoperative period(Pre-kE group)and on long-term follow-up(Post-kE group).The incontinence scores in both groups were compared to evaluate the efficacy of *** Of 102 anal fistula patients operated on between July 2018 and July 2020 were included in this *** were 90 males,the mean age was 42.3±12.8,and the median follow-up was 30 mo(18-42 mo).Three patients were lost to *** were 65 recurrent fistulas,92 had multiple tracts,42 had associated abscess,46 had horseshoe fistula and 34 were supralevator *** were magnetic resonance imaging-documented high fistulas(>1/3 EAS involved).Overall incontinence occurred in 31%patients(Pre-kE group)with urge and gas incontinence accounting for the majority of cases(28.3%).The mean incontinence scores in the Pre-kE group were 1.19±1.96(in 31 patients,solid=0,liquid=7,gas=8,urge=24)and in the Post-kE group were 0.26±0.77(in 13 patients,solid=0,liquid=2,gas=3,urge=10)(P=0.00001,t-test).CONCLUSION Division of the IAS led to incontinence,mainly urge incontinence,and also to a mild degree of gas and liquid ***,regular kE led to a significa

Insights into the divergent evolution of the oceanic squid Sthenoteuthis oualaniensis(Cephalopoda:Ommastrephidae)from the Indian Ocean

Sthenoteuthis oualaniensis is known for its complex population structure with three major transoceanic forms(***-sized,dwarf,and giant forms)whose taxonomic status has been disputed for *** integrated taxonomic study examines these prevenient morphotypes gathered on cruises in the Indian Ocean to ascertain their status in the evolutionary history of the *** analyses employing mitochondrial(COI,ND2)and nuclear(H3)markers revealed four genetically distinct and novel lineages of the species in the Indian Ocean,representing three morphotypes from the Arabian Sea and one from the Southern Indian *** mitochondrial-based phylograms revealed two distinct clades in the species:“dwarf forms+giant form”and“middle-sized forms,”which further branch into geographically structured evolutionary *** delimitation analyses recovered five distinct clades,namely,the Arabian Sea giant and dwarf forms,Equatorial,Eastern Typical,and Other Middle-sized forms,representing the consensus molecular operational taxonomic units.H3 being heterozygous could not resolve the *** network and AMOVA analysis of mtDNA genes indicated explicit phylogeographic structuring of haplotypes,whereas these outputs and PCA results were incongruent with the morphological *** features distinguishing the morphotypes were sometimes plastic and mismatched with the *** giant form was genetically close to the dwarf forms,contradicting the earlier notion that it descended from the middle-sized *** may be assumed that the dwarf form evolved following sympatric speciation and adaptation to warm equatorial waters,while the focal features of the Western Arabian Sea guide toward allopatric speciation of the giant form.

Comparison of TiAlN, AlCrN, and AlCrN/TiAlN coatings for cutting-tool applications

Monolayer and bilayer coatings of TiAlN, AlCrN, and AlCrN/TiAlN were deposited onto tungsten carbide inserts using the plasma enhanced physical vapor deposition process. The microstructures of the coatings were characterized using scanning electron microscopy （SEM） and atomic force microscopy （AFM）. The SEM micrographs revealed that the AlrN and AlCrN/TiAlN coatings were uniform and highly dense and contained only a limited number of microvoids. The TiAIN coating was non-uniform and highly porous and contained more micro droplets. The hardness and scratch resistance of the specimens were measured using a nanoindentation tester and scratch tester, respectively. Different phases formed in the coatings were analyzed by X-ray diffraction （XRD）. The AlCrN/TiAlN coating exhibited a higher hardness （32.75 GPa）, a higher Young＇s modulus （561.97 GPa）, and superior scratch resistance （LcN = 46 N） compared to conventional coatings such as TiAlN, A1CrN, and TiN.

Chemical composition and anti-inflammatory activity of Boswellia ovalifoliolata essential oils from leaf and bark

We isolated the essential oils from Boswellia ovalifoliolata *** and *** using hydrodistillation,identified the obtained compounds using gas chromatography–mass spectrometry(GC–MS) and Fourier transform-infrared spectroscopy(FT-IR),and studied the photophysical and electrochemical properties using UV–visible and fluorescence spectroscopy and cyclic *** the basis of GC–MS spectra,38 compounds were identified in the essential oil from leaves and 26 in the oil from *** bark oil contained 13.44% b-Farnesene(sesquiterpene),10.45% caryophyllene oxide(sesquiterpene) and 6.6% spathulenol,(2 Z,6 E)-(sesquiterpene alcohol); the leaf oil contained 11.1% spathulenol(sesquiterpene alcohol),9.0% caryophyllene oxide(sesquiterpenoids) and 6.3% decyl acetate(ester).FT-IR spectra confirmed the presence of aliphatic aldehydes and ketones,carboxylic acid,alcohols,esters and ethers in both oil ***–visible absorption spectra showed maximum absorbance at 245,290 and 402 nm for bark oil,and 250,285 and 325 nm for *** oil showed strong emission with maximum emission wavelength at 456 nm was higher than that of leaf oil at 414 *** essential oil had significant anti-inflammatory activity,and the reduction potential of the leaf oil was-0.44 and-0.56 e V for bark.

Hydroalcoholic extract of licorice(Glycyrrhiza glabra L.) root attenuates ethanol and cerulein induced pancreatitis in rats

Pyoderma Gangrenosum with Oral Involvement–Case Report and Review of the Literature

Pyoderma gangrenosum （PG） is a rare, noninfectious neutrophilic dermatosis. Clinically, it begins with sterile pustules that rapidly progress into painful ulcers of variable depth and size with undermined violaceous borders. The diagnosis of PG is based on the history of an underlying disease, a typical clinical presentation, histopathology, and exclusion of other diseases. The peak incidence occurs between the ages of 20 to 50 years with women being more often affected than men. There have been very few reports of pyoderma gangrenosum with oral mucosal involvement. Oral lesions in previously reported cases have included ulcers of varying sizes from a few mm to several cm and have been reported to have been found on the tongue, soft and hard palate, buccal mucosa, and gingiva. Some of these oral lesions have been associated with ulcerative colitis, inflammatory bowel disease, and polycythemia rubra vera. A few cases were reported with biopsy findings, the histological picture being nonspecific, showing ulceration, and necrosis with inflam- matory cell infiltrate. A peculiar case of pyoderma gan- grenosum with an oral lesion is presented here, and the differential diagnosis is discussed.
