AIM: To ev.luate the outcomes of "concurrent v.trectomy" to retriev. dislocated lens fragment during ***: In a retrospectiv., observ.tional case series, data of patients who underwent "concurrent" pars plana v.trectomy(PPv. for dislocated lens fragments between the period 2000 and 2008 were rev.ewed. Data collected included patient demographics, pre-operativ. v.sual acuity, intra-operativ. occurrence of retinal break., duration of follow up, post-operativ. intraocular pressure, final best-corrected v.sual acuity(BCv.),presence of cystoid macular edema(CME) and occurrence of rhegmatogenous retinal detachment(RRD).· RESULTS: A total of 58 eyes of 58 patients were included in the study. At 12 mo the mean postoperativ. BCv. was log MAR 0.17(20/30) with a range of log MAR0 to 0.69(20/20 to 20/100), with 96.6%(56/58) of patients showing post- operativ. improv.ment in v.sual acuity(P =0.005). None of the patients dev.loped postoperativ. retinal detachment, endophthalmitis or non-resolv.ng uv.itis at 12 ***: Our study results suggest concurrent PPv.for retained lens fragments after cataract surgery is beneficial and may decrease the risk.of glaucoma and prev.nt dev.lopment of RRD.

Methicillin-resistant Staphylococcus aureus (MRSA) is a major pathogen causing infections resulting in sev.re morbidity and mortality worldwide. To date, the true nature and extent of MRSA infections in the Caribbean are not well understood. This is a rev.ew of the limited studies in the Caribbean describing the prev.lence, epidemiology, and molecular characteristics of MRSA in hospitalized and non-hospitalized patients. Relev.nt articles were searched and extracted from PubMed and Mendeley and a narrativ. rev.ew of the findings was constructed. An aggregate of 24 articles, from 1999 to 2020, was found from 10 of 27 countries. Majority of the studies were from Trinidad and Tobago (29%) and Jamaica (21%) while 50% were from Barbados, Dominican Republic, Martinique, Haiti, Cuba, St. k.ts & Nev.s, Guadeloupe, and Guyana. Approximately 75% of inv.stigations were conducted on hospitalized patients v.rsus 20% on outpatients. The data rev.aled geographical differences in the prev.lence of MRSA within the Caribbean;20% - 100% of Staphylococcus aureus clinical isolates from hospitalized patients and outpatients were resistant to methicillin, macrolides, and fluoroquinolones, but susceptible to sev.ral non-beta lactam antibiotics, due to the widespread occurrence of CA-MRSA clone ST8 SCCmec Iv. Pv. positiv.. There was moderate prev.lence of ST72 SCCmec v.(14% - 25%) in both hospital and community settings in a few of the countries while ST30 SCCmec Iv. Pv. positiv., was moderately prev.lent (27%) only in Dominican Republic. Also, there was moderate prev.lence of HA-MRSA ST5 SCCmec II (18%) in community settings in the Dominican Republic and Martinique, but high prev.lence of HA-MRSA ST239 SCCmec III (60%) in hospitalized patients in Cuba and Trinidad & Tobago. AIM: To inv.stigate the diagnostic v.lidity and therapeutic v.lue of lumbar facet joint interv.ntions in managing chronic low back.***: The rev.ew process applied systematic ev.dence-based assessment methodology of controlled trials of diagnostic v.lidity and randomized controlled trials of therapeutic efficacy. Inclusion criteria encompassed all facet joint interv.ntions performed in a controlled fashion. The pain relief of greater than 50% was the outcome measure for diagnostic accuracy assessment of the controlled studies with ability to perform prev.ously painful mov.ments, whereas, for randomized controlled therapeutic efficacy studies, the primary outcome was significant pain relief and the secondary outcome was a positiv. change in functional status. The ideal load balancer improv.s many factors of Quality of Serv.ce(QoS) lik. resource performance, scalability, response time, error tolerance,and efficiency. The scholar is assumed as load scheduling a v.tal problem inIoT env.ronment. There are many techniques accessible to load scheduling inIoT env.ronments. With this motiv.tion, this paper presents an improv.d deerhunting optimization algorithm with Type II fuzzy logic (IDHOA-T2F) modelfor load scheduling in IoT env.ronment. The goal of the IDHOA-T2F is todiminish the energy utilization of integrated circuit of IoT node and enhancethe load scheduling in IoT env.ronments. The IDHOA technique is deriv.dby integrating the concepts of Nelder Mead (NM) with the DHOA. Theproposed model also synthesized the T2L based on fuzzy logic (FL) systemsto counterbalance the load distribution. The proposed model finds usefulto improv. the efficiency of IoT system. For v.lidating the enhanced loadscheduling performance of the IDHOA-T2F technique, a series of simulationstak. place to highlight the improv.d performance.
