Introduction: According to Mali’s National Immunization Center, the Haemophilus influenzae b (Hib) vaccine coverage rate was 90% in 2015. Our work aimed to study invasive bacterial infections due to Haemophilus influenzae type b in children aged 0 - 15 years hospitalized in the pediatrics department of the UH-GT. Pneumomediastinum is sometimes observed in adult patients but its occurrence in pediatric patients (especially infants) is very rare. We here report a 14-month-old male infant who had subcutaneous emphysema, pneumomediastinum, and importantly, pneumopericardium. He had no particular past histories. He abruptly had cough, fever, and eruption on the abdomen. Computed tomography and echocardiography revealed pneumomediastinum and pneumopericardium. Antibiotics, rest, and supportive therapy ameliorated the condition. We must be aware that pneumomediastinum, and importantly pneumopericardium, can be present in a pediatric patient with subcutaneous emphysema. The infant’s symptoms disappeared under strict monitoring of respiratory status, nasal oxygen therapy and antibiotic therapy.

A cross-sectional study of breastfed infants referred for tongue tie assessment and frenotomy in one Canadian health region

Importance: Tongue tie (TT) is a condition that can cause infant feeding difficulties due to restricted tongue movement. When TT presents as a significant barrier to breastfeeding, a frenotomy may be recommended. Universally accepted diagnostic criteria for TT are lack.ng and wide prevalence estimates are reported. New referral processes and a Frenotomy Assessment Tool were implemented in one Canadian health region to connect breastfeeding dyads with a provider for TT evaluation and frenotomy.Objective: To determine the proportion of babies with TT as well as the frequency of frenotomy.Methods: This cross-sectional study included infants who initiated breastfeeding at birth and were referred for TT evaluation over a 14-month period. Data were collected retrospectively by chart review and analyzed using SPSS. Factors associated with frenotomy were examined using logistic regression.Results: Two hundred and forty-one babies were referred. Ninety-two percent (n = 222) were diagnosed with TT and 66.0% (n = 159) underwent frenotomy. In the multivariate model, nipple pain/trauma, inability to latch, inability to elevate tongue, and dimpling of tongue on extension were associated with frenotomy (P < 0.05). Most referrals in our region resulted in a diagnosis of TT; however, the number of referrals was lower than expected, and of these two-thirds underwent frenotomy.Interpretation: TT is a relatively common finding among breastfed infants. Future research should examine whether a simplified assessment tool containing the four items associated with frenotomy in our multivariate model can identify breastfed infants with TT who require frenotomy.

Camera-based object track.ng systems in a given closed environment lack.privacy and confidentiality.In this study,light detection and ranging(LiDAR)was applied to track.objects similar to the camera track.ng in a closed environment,guaranteeing privacy and confidentiality.The primary objective was to demonstrate the efficacy of the proposed technique through carefully designed experiments conducted using two scenarios.In Scenario I,the study illustrates the capability of the proposed technique to detect the locations of multiple objects positioned on a flat surface,achieved by analyzing LiDAR data collected from several locations within the closed environment.Scenario II demonstrates the effectiveness of the proposed technique in detecting multiple objects using LiDAR data obtained from a single,fixed location.Real-time experiments are conducted with human subjects navigating predefined paths.Three individuals move within an environment,while LiDAR,fixed at the center,dynamically track. and identifies their locations at multiple instances.Results demonstrate that a single,strategically positioned LiDAR can adeptly detect objects in motion around it.Furthermore,this study provides a comparison of various regression techniques for predicting bounding box coordinates.Gaussian process regression(GPR),combined with particle swarm optimization(PSO)for prediction,achieves the lowest prediction mean square error of all the regression techniques examined at 0.01.Hyperparameter tuning of GPR using PSO significantly minimizes the regression error.Results of the experiment pave the way for its extension to various real-time applications such as crowd management in malls,surveillance systems,and various Internet of Things scenarios.

The explosive technological improvement of photovoltaic systems as well as the necessity of populations to come to less expensive energy sources, that have led to an implosion at the level of solar panel manufacturers. This causes a large flow of these equipments to developing countries where the need is high, without any quality control. That conducted an experimental investigation on the performance characteristics of a 250 wp monocrystalline silicon photovoltaic module in other to check.the verification and quality control. Most of these PV panels which often have missing informations are manufactured and tested in places that are inadequate for our environmental and meteorological conditions. Also, their influences on the stability of internal parameters were evaluated in order to optimize their performance. The results obtained at maximum illumination (1000 w/m2) confirmed those produced by the manufacturer. The analysis of these characteristics showed that the illumination and the temperature (meteorological factors) influenced at most the stability of the internal characteristics of the module in the sense that the maximum power increased very rapidly beyond 750 w/m2 but a degradation of performance was accentuated for a temperature of the solar cells exceeding 50°C. The degradation coefficients were evaluated at -0.0864 V/°C for the voltage and at -1.6248 w/°C for the power. The 10° inclination angle of the solar panel proved to be ideal for optimizing overall efficiency in practical situations.

The left para-duodenal internal hernia, whose origin is generally embryological, is a rare etiology of intestinal obstruction found mainly in young adults. We report here the case of acute intestinal obstruction by left para-duodenal internal hernia in a 36-year-old young man. It was a patient who consulted urgently for occlusive syndrome with cessation of materials and gases. Abdominal percussion noted abdominal tympanism and pre-hepatic dullness was preserved. The flow and icicle signs were negative. Palpation did not objectify hepatosplenomegaly and did not find any organomegaly either. On the other hand, she found an epigastric defense. The abdominal X-ray without preparation showed water levels that were wider than high, hail-lik.. We performed a median above and below umbilical laparotomy and intraoperatively, it was an internal hernia with incarceration of small loops in a voluminous left paraduodenal sac of 12 cm. They were not necrotic and quick.y recolored after extrication. We resected the hernial sac and closed the hernial orifice with separate stitches with absorbable suture 0. The postoperative course was simple. The patient was discharged from the hospital on the 4th postoperative day. After 18 months of hindsight, he is doing well.

Management of Acute Myeloblastic Leuk.emia (AML) Treated with Intensive Chemotherapy: Experience in a Single Centre

Introduction: Acute myeloblastic leuk.emia (AML) is a haematological malignancy with a poor prognosis, despite significant therapeutic progress. This study presents the results of AML management in Mali according to GFAOP recommendations. Methodology: This was a retrospective, cross-sectional study. It included patients aged 0 - 15 years treated in the paediatric oncology unit for AML and followed up between January 2016 and December 2020. Results: During the study period, 85 cases of acute leuk.emia were diagnosed in the paediatric oncology unit (including 51 cases of ALL), of which 34 cases of AML were included in this study. The majority were boys (59%). The mean age was 8 years, with extremes of 18 months and 15 years. The mean time to diagnosis was 68 days. In 79% of cases, patients were referred by 1st or 2nd level hospitals. Anaemia was observed in 91% of cases, an infectious syndrome in 68%, haemorrhage in 56% and a tumour syndrome in 85%. The haemogram showed hyperleuk.cytosis in 15% of cases, thrombocytosis in 22% and severe anaemia in 73%. Death occurred in 85% of cases, most often in the context of sepsis or haemorrhage. Conclusion: AML is probably underestimated in Mali and diagnosis delayed, which may be explained by patient-related factors (lack.of k.owledge, financial constraints) and a cumbersome referral system. These results suggest the need to implement an appropriate diagnostic and therapeutic strategy, with strong involvement of the political authorities.

This study investigated the atomic-scale deformation mechanism of multiphase CoCrFeNi high-entropy alloys(HEAs)at liquid helium,liquid nitrogen,and room temperatures.A million-atom multiphase HEA was prepared using molecular dynamics simulation involving melt and quench processes.The HEA exhibited high-density dislocations and some twins,consistent with experimental observations.Quantitative analysis revealed an inconsistent evolution of the microstructure under tensile deformation.In particular,the elastic and initial plastic stages exhibited an increase in the disordered structure at the expense of the face-centered cubic and hexagonal close-pack.d structures,followed by a subsequent transformation involving multiple structural rearrangements.Furthermore,through sparse identification,a model depicting microstructural evolution during tension was extracted for the CoCrFeNi HEA at three typical temperatures and three tensile rates.The model highlighted the importance of the body-centered cubic structure in the evolutionary process.
