Successful Abdominal Organ Donation after Brain Death in a Patient with a Biventricular Assist Device: Extending Extended Criteria

Few studies address the potential for donation after brain death (DBD) in the limited population of patients with ongoing mechanical circulatory support (MCS). A case study was conducted reviewing available records of both donor and recipient, and available literature. The donor was a young female with an acute myocardial infarction precipitating emergent off-pump 2-vessel bypass graft complicated by profound cardiogenic shock.refractory to inotropes and intra-aortic balloon pump. A heparin drip was started following percutaneous placement of a left ventricular-assist device (TandemHeart?) which improved her hemodynamics to stabilize for transfer. She ultimately required surgical placement of biventricular assist device (CentraMag?) to normalize hemodynamics. Two days post-operatively, she developed a cerebellar hemorrhage and was declared brain dead. Pre-donation blood chemistry showed adequate end-organ function. Both k.dneys were placed locally. The liver was rejected for two regional status 1 patients and by all other local centers. We accepted the liver for a patient with polycystic liver disease with a MELD exception score of 20. The recipient is now 4 years post-transplant with excellent graft function. Extending donor criteria to include MCS patients can result in successful transplantation and should be considered in selected circumstances once satisfactory donor end-organ function is established.

Ballistic behavior of boron carbide reinforced AA7075 aluminium alloy using friction stir processing-An experimental study and analytical approach

High strength-to-weight ratio of non-ferrous alloys, such as aluminium, magnesium and titanium alloys, are considered to be possible replacement of widely accepted steels in transportation and automobile sectors. Among these alloys, magnesium is self explosive and titanium is costlier, and aluminium is most lik.ly to replace steels. Application of aluminium or its alloys is also thought of as an appropriate replacement in defence field, especially to enhance the easiness in mobility of combat vehicles while maintaining the same standard as that of conventional armour grade steels. Hence most of the investigations have been confined to aluminium or its alloys as base material and open an era of developing the newer composite materials to address the major limitation, i.e. tribological properties. The surface composites can be fabricated by incorporating the ceramic carbides lik. silicon carbide, carbides of transition metals and oxides of aluminium using surface modification techniques, such as high energy laser melt treatment, high energy electron beam irradiation and thermal spray process which are based on fusion route. These techniques yield the fusion related problems, such as interfacial reaction, pin holes, shrink.ge cavities or voids and other casting related defects, and pave the way to need of an efficient technique which must be based on solid state. Recently developed friction stir processing technique was used in the present investigation for surface modification of AA7075 aluminum alloy, which is an alternative to steels. In the present investigation, 160 μm sized boron carbide powder was procured and was reduced to 60 μm and 30 μm using high energy ball mill. Subsequently these powders were used to fabricate the surface composites using friction stir *** performance testing as per the military standard(JIS.0108.01) was carried out. Degradation of Methylene Blue and Rhodamine B Using a New Visible Light-Responsive Photocatalyst, k.b_2PO_(8-x)N_y

Potassium phosphatoantimonates, k.b2PO8 （k.bPO） and k.b2PO8-xNy （k.bPON）, are prepared by solidstate method. Urea is employed as a source of nitrogen for N-doping. Structural, morphological and optical properties of both k.bPO and k.bPON are characterized by powder X-ray diffraction （PXRD）, scanning electron microscopy, energydispersive spectroscopy, Fourier transform infrared spectroscopy and UV-visible diffuse reflectance spectroscopy. Both samples are found to crystallize in a monoclinic lattice with Cc space group. The d-lines of k.bPO and k.bPON are refined by least square fitting the PXRD data, using POWD software to obtain their unit cell parameters. The bandgap energy （Eg） of both pristine and N-doped k.bPO is found from their k.belk.-Munk.plot. The k.bPON/visible photocatalytic degradation of methylene blue and rhodamine B has been investigated in aqueous heterogeneous suspension. The generation of .OH in the presence of k.bPON during the photoreaction is verified by a fluorescence technique using terephthalic acid （TA） as a probe.

A multi-preference integrated algorithm(MPIA)for the deep learning-based recommender framework.DLRF)

Purpose-The deep learning-based recommender framework.DLRF)is based on an improved long short-term memory(LSTM)structurewith additional controllers;thus,it considers contextual information for state *** also handles irregularities in the data to enhance performance in generating recommendations while modelling short-term *** algorithm named a multi-preference integrated algorithm(MPIA)is proposed to have dynamic integration of both k.nds of user preferences *** experiments are made using Amazon benchmark.datasets,and the results are compared with many existing recommender systems(RSs).Design/methodology/approach-RSs produce quality information filtering to the users based on their *** the contemporary era,online RSs-based collaborative filtering(CF)techniques are widely used to model long-term preferences of *** deep learning models,such as recurrent neural network.(RNNs),it became viable to model short-term preferences of *** the existing RSs,there is a lack.of dynamic integration of both long-and short-term *** this paper,the authors proposed a DLRF for improving the state of the art in modelling short-term preferences and generating recommendations as ***-The results of the empirical study revealed that the MPIA outperforms existing algorithms in terms of performance measured using metrics such as area under the curve(AUC)and *** percentage of improvement in terms AUC is observed as 1.3,2.8,3 and 1.9%and in terms of F-1 score 0.98,2.91,2 and 2.01%on the ***/value-The algorithm uses attention-based approaches to integrate the preferences by incorporating contextual information.

The Medicinal Flora of Sambalpur District,Orissa,India

This paper describes an analysis of the plants of Sambalpur district of Orissa,India which have been used medicinally by local people. The families having medicinal values are analysed using Moerman's method of regression analysis. There were 136 plant families recorded from the study area. The analysis of families showed that the Euphorbiaceae family being used most ethnomedicinally by the local people in Sambalpur district of Orissa,whereas Poaceae is used least one. This type of study determines the degree of importance of plant families in the medicinal flora of the region.

Enhancement of wear and ballistic resistance of armour grade AA7075 aluminium alloy using friction stir processing

Industrial applications of aluminium and its alloys are restricted because of their poor tribological properties. Thermal spraying, laser surfacing, electron beam welding are the most widely used techniques to alter the surface morphology of base metal. Preliminary studies reveal that the coating and layering of aluminium alloys with ceramic particles enhance the ballistic resistance. Furthermore, among aluminium alloys,7075 aluminium alloy exhibits high strength which can be compared to that of steels and has profound applications in the designing of lightweight fortification structures and integrated protection systems. Having limitations such as poor bond integrity, formation of detrimental phases and interfacial reaction between reinforcement and substrate using fusion route to deposit hard particles paves the way to adopt friction stir processing for fabricating surface composites using different sizes of boron carbide particles as reinforcement on armour grade 7075 aluminium alloy as matrix in the present investigation. Wear and ballistic tests were carried out to assess the performance of friction stir processed AA7075 alloy. Significant improvement in wear resistance of friction stir processed surface composites is attributed to the change in wear mechanism from abrasion to adhesion. It has also been observed that the surface metal matrix composites have shown better ballistic resistance compared to the substrate AA7075 alloy. Addition of solid lubricant Mo S2 has reduced the depth of penetration of the projectile to half that of base metal AA7075 alloy. For the first time, the friction stir processing technique was successfully used to improve the wear and ballistic resistances of armour grade high strength AA7075 alloy.

Structure and floristic composition of tree stand in tropical forest in the Eastern Ghats of northern Andhra Pradesh, India

The changes in species composition, abundance and forest stand structure were analyzed across altitudinal regimes in tropical forests of Eastern Ghats of northern Andhra Pradesh, India. Three 1-ha plots were established with one each in low, medium and high altitudes. A total of 153 species, 2129 stems （709 stems .ha^-1） of ≥10 cm girth were enumerated. Species richness and diversity pattern varied along altitudinal gradient and increased with the altitude. Species richness varied from 52 to 110 species-ha^-1 and stand density from 639 to 836 ***^-1 with average basal area of 34.39 m^***^-1. Shannon-Wiener index （H＇） ranged from 4.55 to 5.17. Low altitude （i.e., Site 1） is dominated byXylia xylocarpa （59.22） and Lagerstroemia parviflora （23.90）, medium altitude （i.e., Site 2） by Xylia xylocarpa （45;50） Bursera serrata （17.29）, and high altitude （i.e., Site 3） has Schleichera oleosa （28.25） Pterocarpus marsupium （26.55） as predominant species. Taxonomically, Rubiaceae （12 species）, Fabaceae （12）, Euphorbiaceae （11）, Rutaceae （7） and Lanraceae （7） were dominant families. Density-wise, Fabaceae, Combretaceae, Euphorbiaceae, Anacardiaceae and Myrtaceae were abundant. Thus, conservation assessment based on altitudinal regimes and the information on species structure and function can provide baseline information for monitoring and sustaining the biodiversity.

Does Change in the Effect of Source Strength of the High Dose Rate <sup>192</sup>Iridium Radio-Isotope on Local Control and Late Normal Tissue Toxicity (Bladder and Rectum) in the Treatment of Carcinoma Cervix

Aim: To analyse and assess the effect of dose rate of 192ir-source strength on late complications and local control rate during treatment of carcinoma cervix. Materials and Methods: One hundred and two cases of carcinoma cervix were included in the study. All patients were treated with a curative intent with radical dose of radiation as per the department protocol. All patients were treated with both EBRT plus Brachytherapy with Inj. Cisplatin 40 mg/m2 week.y. Patients were divided into 2 groups based on activity i.e. group A (10-6Ci) and group B (5-2Ci). After brachytherapy, point doses were analysed based on ICRU 38 recommendations. During follow up, morbidities were evaluated using RTOG grading system. Results: There was no difference in local control and distant metastasis in both groups after six months of follow up. Late Complications were comparable in both groups irrespective of source strength. Bladder complications were minimal with no significant difference in both study groups. Further Patients were divided into four groups i.e. BED of ICRU rectal point (3 and ≥100 Gy3) and source strength (10-6Ci and 5-2Ci), when BED was >100 Gy3 resulted in higher late rectal complication rate (P 3. Conclusion: This study suggests that change in source activity did not mak. a difference in local control, late rectal and bladder morbidities at 6 months of follow up. Longer follow up is required to assess long term results and morbidities.
