Introduction: Acute Respiratory Infections (ARI) is a global public health priority. It is the leading cause of childhood morbidity and mortality, especially in developing countries. The main objective of this work was to look mainly at the bacteriological aspects of ARI in children in Senegalese hospitals. Methodology: We have been conducted a prospective descriptive mono-centric study over a period of one year (from July 1, 2017 to June 30, 2018) at the Albert Royer National child hospital in Dakar. A multiplex PCR (protein chain reaction) has been performed on nasopharyngeal swabs taken from all patients. This technique (viral RNA transcribed into DNA, then the DNA of the different pathogens is simultaneously amplified in the same tube by PCR Fast-tract®, multiplex). Results: During the study period, 109 patients were included. The hospital prevalence of acute respiratory infections was 3.7%. The mean age was 23.7 months with extremes between 1 month and 144 months. Peaks of consultations were found in the months of August, March and April with 22%, 15.6% and 12.8% respectively. Fever, respiratory distress and pulmonary condensation syndrome were the main signs found in our patients. Bacteriology was positive in 82.6% of samples and the most frequently found bacteria were Streptococcus pneumoniae in 38.5%, Haemophilus influenza b in 32.1% and

Fanconi anaemia (FA) is a rare genetic abnormality. Most of FA reported from the sub-Saharan population came from southern Africa, with many patients link.d to a mutation in the FANC G gene. Classic triads of this disorder are: a varied malformation, bone marrow failure, and short stature. This disorder is also associated with predisposition to malignancies. These multiple manifestations, sometimes not uniform, often cause diagnostic delay. We here report a 13-year-old Senegalese boy with FA. He was followed up for polydactyly-repair surgery. Importantly, pre-operative blood check-up revealed severe anaemia, which prompted us to perform bone marrow aspiration: examination revealed dysmyelopoiesis. Being triggered by this, systematic examinations were performed, which revealed other signs indicative of FA;i.e., radial spine abnormalities, triangular face, etc. The diagnosis of FA was strongly suggested, which prompted genetic examination. The chromosomal breakage test detected cellular hypersensitivity to DNA, which confirmed the diagnosis. He is receiving transfusion support, and androgens use is being considered. This case highlights the importance of preoperative examination. Physicians must be aware that FA, although its incidence is low, can be hidden behind infantile anaemia.

Introduction: Cervical cancer is preventable through vaccination against papillomaviruses and screening for precancerous lesions. The objective of this study was to determine the frequency of screening practice and to identify associated factors. Methods: This was a cross-sectional descriptive and analytical study. Data were collected from individual interviews during household surveys. Data collection took.place from (1st) to (30th) September (2021). The following were explored: socio-demographic characteristics, k.owledge, attitudes and practices of women regarding screening. Results: A total of (294) women aged between (18) and (69) years were included. The mean age was (35.79) years (±9.81). The median age was (34) years. The frequency of screening was (16%);that of precancerous lesions was (4.26%). Factors statistically and significantly associated with screening uptak. were cervical cancer information (OR = 5.42 (1.27 - 23.16);p = 0.003), k.owledge of the availability of screening (ORa = 7.73 (3.01 - 18.51);p - 7.31);p = 0.014). Conclusion: Frequency of pre-cancerous lesions of cervical cancer is high in k.umpentoum health district (4.26%), while the practice of screening is low there (16%). The factors associated with this low performance are related to a lack.of adequate k.owledge of women about cervical cancer.

Introduction: Cervical cancer can be prevented by early vaccination of young people against papillomaviruses and screening for precancerous lesions. After a successful pilot phase, vaccination coverage in the generalization phase is low. The aim of this study was to determine papillomavirus vaccination coverage and to identify associated factors. Methods: This was a cross-sectional, descriptive and analytical study conducted from (1st) to (31th) September (2020) in k.umpentoum district. After a literature review, we conducted two-stage cluster sampling and direct structured interviews. Socio-demographic characteristics, k.owledge, attitudes, and practices of mothers or guardians about papillomaviruses vaccination were collected using a standardized questionnaire. Multiple logistic regression was used to estimate odds ratios. Results: A total of (228) mothers or guardians were interviewed. Coverage for the first dose was (44.74%) CI95% (38.17 - 51.44) compared to (25.88%) CI95% (19.52 - 31.17) for the second dose. Factors statistically and significantly associated with coverage of the first dose of papillomaviruses vaccine were instruction of mothers or guardians (OR = 5.62 (3.16 - 9.99);p 0.001), schooling of the young girls (OR = 4.1 (2.23 - 7.53);p 0.001), information on cervical cancer (OR = 18.97 (5.68 - 63.24);p 0.001), k.owledge of risk. factors (OR = 8.04 (4.41 - 14.63);p 0.001), information on papillomaviruses vaccine (ORa = 10.26 (1.69 - 62.23);p = 0.011), k.owledge on vaccine target (OR = 17.11 (8.51 - 34.41);p 0.001), k.owledge of schedule vaccine (ORa = 3.67 (1.2 - 22.51);p = 0.022), k.owledge of prevention methods (OR = 26.86 (12.22 - 59.05);p 0.001), and to be favorable in expanded vaccination program in general (ORa = 18.71 (1.5 - 128.41];p = 0.02). Temporary water bodies’ dynamics play an important role in the epidemiological chain-borne diseases such as Rift Valley fever as they are the main breeding habitats for mosquitoes. During the rainy season, hundreds of these temporary water bodies appear and grow in the Ferlo region (Senegal). The purpose of this research is to generate historical and future time series water levels and areas at three temporary ponds located in the environment and health observatory of Bark.dji. A simple lumped hydrological model was developed for that purpose. It describes each pond watershed as three interconnected reservoirs: canopy, surface storage and soil storage and uses a linear relation to describe infiltration, percolation and baseflow (out of the soil reservoir). Given the depth of the water table in the region, percolation out of the soil surface is considered lost. Evapotraspiration was calculated using the Penman equation and withdraws water from the canopy and surface water reservoirs. Excess runoff from the soil storage is turned into runoff using a triangular unit hydrograph. The calibration was done using two years of hydrological and climatic data collected during the 2011 and 2012 rainy seasons. The calibration was successful and water level in the two ponds was simulated with a Root Mean Square Error (RMSE) of 11.2 to 15 cm. Because of the short duration of the observation, no validation could be done. Given the excellent agreement of the simulated and observed water levels during the calibration phase, the modeling exercise was considered to be successful. The developed models were used to generate historical time series of pond areas and correlate these to mosquitoes’ infestation in the region. Future time series of pond areas were also generated using downscaled outputs of three regional climate models from the AMMA ENSEMBLES experiment. The generated pond levels and areas are being used to assess the evolution of the disease in the next 40 years.

Introduction: Infantile spinal muscular atrophy (ISA) is an autosomal recessive disease caused by primary degeneration of cells in the anterior horn of the spinal cord, leading to muscle weak.ess and hypotonia. Its incidence is estimated at 1 in 6000 births worldwide. In Africa, particularly in Senegal, there are few studies interested on this pathology. We therefore deemed this study necessary, which set itself the objective of describing the diagnostic, therapeutic and progressive aspects of infantile spinal muscular atrophy at the Albert Royer National Children’s Hospital Center in Dak.r (CHNEAR). Methodology: We conducted a retrospective descriptive study over a period of two (2) years from December 2020 to December 2022. Included were all hospitalized patients in whom the diagnosis of spinal muscular atrophy was made with or without genetic confirmation. The data were collected on a pre-established form then entered and analyzed with the following software: Excel 2013 and R version 4.1.3. Results: During our study period, 2100 children were hospitalized, the annual incidence was 0.76%. The average age of our patients was 9 ± 9 months with a range of 3 months to 32 months and the median was 6.5 months. The sex ratio was 7. The notion of family consanguinity was found in 62.5% of cases and the notion of ISA in the family in 25% of cases. Hypotonia and respiratory distress were found at the forefront in equal proportions (50% of cases). Electromyogram (EMG) was performed in 3 patients (37.5%). Symptomatic medical treatment was administered in 100% of patients, 04 patients had benefited from respiratory physiotherapy, i.e. 50% of cases, and genetic counseling was carried out in one patient (12.5%). The evolution was immediately favorable in 2 patients or 25% of cases, unfavorable in 75% of cases with a death rate of 50% and the average age of death was 5.5 months ± 1 with extremes ranging from 3 to 7 months. Rheumatic heart disease is the ultimate consequence of acute articular rheumatism. It remains a public health problem in developing countries. It is a pathology found in all countries of the world, with a clear predominance in developing countries. In Senegal, the hospital prevalence of rheumatic heart disease is 30.5%. Its seriousness lies in the risk.of valvular sequel responsible for cardiac dysfunction that can lead to death. The objective of this study was to describe the epidemiological, diagnostic and evolutionary aspects of rheumatic heart disease at the Albert Royer National Hospital Center of child (CHNEAR). We had carried out a retrospective, descriptive and analytical study lasting 3 years from January 1, 2017 to December 31, 2019. All children aged 4 to 17 hospitalized at CHNEAR for rheumatic heart disease were included. The hospital prevalence was 0.8%. The average age was 10.9 years with a sex ratio of 0.875 in favor of girls. 85.47% of cases came from parents who had a low economic level of life. The history of repeated angina and acute articular rhumatism (AAR) was found respectively in 22.69 and 15.13% of cases. The clinical symptomatology was dominated by heart failure with 58.3% of cases, arthralgia in 7.5% of cases. The biological inflammatory syndrome was found in 86.1%. 73% of the patients had a positive antistreptolysin O antibody (ASLO). Cardiomegaly was found in 89.7%. Heart doppler ultrasound found polyvalvular involvement in more than half of the cases. Mitral insufficiency was the predominant valve disease in 98.20% of cases. The evolution was favorable in the majority of cases but 9.32% of deaths were observed.

The Mak. area, located in eastern Sénégal, constitutes a segment of Paleoproterozoic (Birimian) formations of the Kédougou-Kéniéba Inliers (KKI) in the western part of the West African craton. The basic volcanism of the Birimian formations of the KKI has long been considered to be related to a single magmatic event associated with a Mid Oceanic Ridge Basalts (MORB) setting. The aim of this paper is to demonstrate on the basis of the architectural characteristics of the granitoids dark enclaves, the occurrence of at least two distinct phases of basic Magamtism in the Birimian of the KKI. The methodology consists of a cartography, a petrographic and architectural characterization of the dark.enclaves within the Mak. granitoids, in order to constrain their spatial and temporal relationships with the granitic magma. Acute chest syndrome (ACS) is a serious pulmonary complication of sick.e cell disease. It is estimated to be responsible for a quarter of deaths in the pediatric sick.e cell population. In Senegal, there are not enough pediatric studies in this area. The objective of our study was to determine the epidemiological, diagnostic and evolutionary characteristics of ATS at the Albert Royer National Children’s Hospital (CHNEAR) in Dak.r. This was a retrospective study in patients hospitalized at CHNEAR for ATS from January 1, 2021 to March 31, 2022. We included patients hospitalized and diagnosed with ATS. We had collected 102 patients, i.e. a hospital incidence of 2.96%. The average age of the children was 9 years old;the sex ratio was 1.04. The main symptoms on admission were hypoxemia (97.06%), chest pain (77.45%), dyspnea (77.45%) and fever (65.69%). 52.94% of patients had an associated vaso-occlusive crisis (VOC). The chest x-ray was abnormal in 92 patients, a rate of 90.20% and showed images of pneumonia (71%);bronchitis (17.65%) and pleurisy (0.98%). None of the children benefited from a pulmonary ultrasound. The treatment associated with analgesics (100%), broad-spectrum antibiotics (100%), oxygen therapy (100%), hydration (95.09%), transfusion (73.53%), non-ventilation invasive (6.86%), intubation (2.94%) and beta 2 mimetics (12.75%). No patient benefited from incentive spirometry. Almost all of the patients 95.10% (n = 97) had a favorable clinical evolution. However, five children (4.90%) had an unfavorable outcome including one case of complication such as strok. (0.98%) and four (4) cases of death. The average hospital stay was 8 days. ATS is common in children with sick.e cell disease in Senegal and its etiologies seem to be dominated by infectious causes in our context.

Pneumothorax is defined by the presence of air between the parietal pleura and the visceral pleura. Most of the child’s pneumothorax is traumatic. The spontaneous pneumothorax (PS) of the child is rare, and often associated with a favoring factor. When it is large, it can affect the life expectancy. The objective of our study was to determine the epidemiological, diagnostic and evolutionary aspects of pneumothorax in children aged 0 to 15 years, hospitalized at the Albert Royer National Children’s Hospital in Dak.r. This is a retrospective descriptive study of patient records followed at the Albert Royer National Children’s Hospital (CHNEAR) for the management of a pneumothorax during the period from 2020 to 2022. During our study, 15 patients were hospitalized for pneumothorax, with an average annual incidence of 7.5 cases per year. The sex ratio (H/F) was 2.03. The average age was 3.7 years. Chest pain was present in 27% of patients, dyspnea was found in 93%, and cough in 87% of patients. All patients underwent chest radiography and the location of the pneumothorax was predominant on the right in 60% of cases. Thoracic drainage was performed in 93.3% of patients associated with etiological treatment. The average length of hospitalization was 27.8 days with a cure rate of 86.7% and a recurrence rate of 13.3%.
