A review of whole-medical systems and holistic care approach for type 2 diabetes and associated metabolic syndrome

Whole-person care and holistic care approach has been proposed for complementary and integrative health care for type 2 diabetes ***,some doubts still exist on the feasibility of replicating processes followed in clinical trials and observational studies in real-world *** narrative literature review summarized and assessed existing clinical evidence(clinical trials,observational studies,and case reports)describing holistic and integrated care approach in adult and adolescent individuals with type 2 diabetes mellitus in clinical *** goal was to highlight existing evidence for implementation and outcomes of whole-medical systems and holistic integrated care approach for type 2 diabetes mellitus.A nonsystematic literature search was performed on Google Scholar,PubMed,Web of Science,ProQuest and ScienceDirect to identify clinical evidence from different parts of the world,evaluating the use of whole-medical systems and/or holistic care interventions in clinical practice for management of type 2 diabetes *** keywords were used in the *** were analyzed using content analysis and simple descriptive statistics(percentages).Most of the studies(64%)were mainly conducted in Eastern countries(India,China and Israel)while 36%of the studies were conducted in the Western countries(USA,Netherlands,Canada and Mexico).Lifestyle medicine and integrated naturopathy were shown to be the commonly used whole-medical systems for type 2 diabetes mellitus *** improvements in type 2 diabetes parameters,medication use,other symptoms,and overall feeling of wellness were observed in all *** review study revealed limited utilization and/or documentation of whole-medical systems or holistic care treatments for type 2 diabetes mellitus in regions of the world other than eastern *** medicine,naturopathy,yoga,Ayurveda and traditional Chinese medicine were shown to be effective for type 2 diabetes mellitu

Multi-layer perceptron-based data-driven multiscale modelling of granular materials with a novel Frobenius norm-based internal variable

One objective of developing machine learning(ML)-based material models is to integrate them with well-established numerical methods to solve boundary value problems(BVPs).In the family of ML models,recurrent neural networks(RNNs)have been extensively applied to capture history-dependent constitutive responses of granular materials,but these multiple-step-based neural networks are neither sufficiently efficient nor aligned with the standard finite element method(FEM).Single-step-based neural networks like the multi-layer perceptron(MLP)are an alternative to bypass the above issues but have to introduce some internal variables to encode complex loading *** this work,one novel Frobenius norm-based internal variable,together with the Fourier layer and residual architectureenhanced MLP model,is crafted to replicate the history-dependent constitutive features of representative volume element(RVE)for granular *** obtained ML models are then seamlessly embedded into the FEM to solve the BVP of a biaxial compression case and a rigid strip footing *** obtained solutions are comparable to results from the FEM-DEM multiscale modelling but achieve significantly improved *** results demonstrate the applicability of the proposed internal variable in enabling MLP to capture highly nonlinear constitutive responses of granular materials.

Multiple sclerosis(MS)is an autoimmune disorder of the central nervous system(CNS)and is primarily characterized by immune cell infiltration leading to relapses followed by remission phases and a disease course turning progressive over time with neurodegenerative processes taking over(Amin and Hersh,2023).Of note,beyond relapse-associated worsening early in disease progression independent of relapse activity may arise independently of relapse activity and can occur in all ***-mediated damage of myelin sheaths and the subsequent loss of mature oligodendrocytes are resulting in impaired axonal integrity,neurodegeneration and accounts for irreversible neuronal damage(kuhlmann et al.,2023).the current landscape of available disease-modifying therapies comprises mainly immunomodulatory drugs that effectively diminish relapses and slow down progression at the onset form of the disease,namely relapsing MS(RMS).In this regard,a number of drugs have been approved as disease-modifying therapies for MS by US Food and Drug Administration and European Medicines Agencies(Box 1).

Enhanced and asymmetric signatures of hybridization at climatic margins: Evidence from closely related dioecious fig species

Hybridization plays a significant role in biological evolution. However, it is not clear whether ecological contingency differentially influences likelihood of hybridization, particularly at ecological margins where parental species may exhibit reduced fitnesses. Moreover, it is unknown whether future ecosystem change will increase the prevalence of hybridization. Ficus heterostyla and F. squamosa are closely related species co-distributed from southern thailand to southwest China where hybridization, yielding viable seeds, has been documented. As a robust test of ecological factors driving hybridization, we investigated spatial hybridization signatures based on nuclear microsatellites from extensive population sampling across a widespread contact range. Both species showed high population differentiation and strong patterns of isolation by distance. Admixture estimates exposed asymmetric interspecific gene *** of hybridization increase significantly towards higher latitude zones, peaking at the northern climatic margins. Geographic variation in reproductive phenology combined with ecologically challenging marginal habitats may promote this phenomenon. Our work is a first systematic evaluation of such patterns in a comprehensive, latitudinally-based clinal context, and indicates that tendency to hybridize appears strongly influenced by environmental conditions. Moreover, that future climate change scenarios will likely alter and possibly augment cases of hybridization at ecosystem scales.

High/medium entropy alloys(H/MEAs)are generally possible to exhibit chemical short-range order(SRO).However,the complex role of SRO on mechanical properties from nano-scale to meso-scale is still challenging so ***,we study the strengthening mechanism and deformation behavior in a model body-centered-cubic HfNbta MEA by using atomic-scale molecular dynamics,micro-scale dislocation dynamics,and meso-scale crystal plasticity finite *** SRO inhibits dislocation nucleation at the atomic scale,improving the flow *** SRO-induced ultrastrong local stress fluctuation greatly improves the micro-scale dislocation-based strength by the significant dislocation forest ***,the ta-rich locally ordered structure leads to an obvious heterogeneous strain and stress partitioning,which forms a strong strain gradient in the adjacent grain interiors and contributes to the strong back-stress-induced strain hardening.

Phytosomes(phytophospholipid complex) are dosage forms that have recently been introduced to increase the stability and therapeutic effect of herbal medicine. Currently, bioactive herbs and the phytochemicals they contain are considered to be the best remedies for chronic diseases. One promising approach to increase the efficacy of plant-based therapies is to improve the stability and bioavailability of their bio-active ingredients. Phytosomes employ phospholipids as their active ingredients, and use their amphiphilic properties to solubilize and protect herbal extracts. the unique properties of phospholipids in drug delivery and their use in herbal medicines to improve bioavailability results in significantly enhanced health benefits. the introduction of phytosome nanotechnology can alter and revolutionize the current state of drug delivery. the goal of this review is to explain the application of phytosomes, their future prospects in drug delivery, and their advantages over conventional formulations.

Evaluation of the Quality of Life of Patients Suffering from Chronic Rhinosinusitis in a Hospital Setting in kinshasa

Background and objective: Chronic rhinosinusitis (CRS), a common disease worldwide, has a major impact on patients’ quality of life (QoL). In recent years, the measurement of health-related QoL has made it possible to assess the patient’s state of health, the severity of the CRS and the treatment. the aim of this study was to assess the QoL of patients with CRS in a hospital setting in kinshasa. Methods: this was a cross-sectional analytical study conducted from June 2020 to May 2021 in the ENt service of the kinshasa University Hospital and the Monkole Hospital Center. the study involved 113 patients aged at least 18 years, whose QOL was compared with that of a control group consisting of 100 non-patients. QOL was assessed using the Sino-Nasal Outcome test (SNOt-22) Questionnaire. Results: the mean age of the patients was 41.0 years (18 - 74 years) and that of the controls was 39.1 years (19 - 77 years). More than half of the participants were university graduates. the median SNOt-22 score was 44 (14 - 78) in patients and 7 (1 - 40) in controls. Compared with subjects without CRS, patients with CRS had significantly higher scores in all four domains of the SNOt-22. Around 9 out of 10 patients had a severely impaired QoL compared with 2 out of 10 controls. Considering mildly impaired QoL, we found that in the otological and sleep domains, scores were not significantly different between patients and controls. In the case of severely impaired QoL, however, only the otological domain showed a non-significant difference in scores between the two groups. Conclusion: CRS is a real public health problem in our environment and significantly alters the QoL of patients suffering from it, with repercussions on their professional productivity;hence the need for better management.

A rudimentary aspect of human life is the health of an individual,and most commonly the wellbeing is impacted in a colossal manner through the consumption of food. the intake of calories therefore is a crucial aspect that must be meticulously monitored. Various health gremlins can be largely circumvented when there is a substantial balance in the number of calories ingested versus the quantity of calories *** food calorie estimation is a popular domain of research in recent times and is meticulously analyzed through various image processing and machine learning techniques. However,the need to scrutinize and evaluate the calorie estimation through various platforms and algorithmic approaches aids in providing a deeper insight on the bottlenecks involved,and in improvising the bariatric health of an individual. this paper pivots on comprehending a juxtaposed approach of food calorie estimation through the use of employing Convolution Neural Network(CNN)incorporated in Internet of things(Iot),and using the Django framework in Python,along with query rule-based training to analyze the subsequent actions to be followed post the consumption of food calories in the constructed webpage. the comparative analysis of the food calorie estimate implemented in both platforms is analyzed for the swiftness of identification,error rate and classification accuracy to appropriately determine the optimal method of use. the simulation results for Iot are carried out using the Raspberry Pi4B model,while the Anaconda prompt is used to run the server holding the web page.
