Propofol with Varied Functions: A Potential Therapeutic Opportunity for Postoperative Nausea, Vomiting and Pruritus—A Narrative Review

b.ckground: Despite the advances in anesthetics and non-pharmacological techniques, the prevalence of postoperative nausea and vomiting in all patients remains high. It is one of the most common distressing symptoms that cause dissatisfaction among patients after anesthesia and surgery. A sub.hypnotic dose of propofol has b.en shown to reduce morphine-induced postoperative nausea, vomiting, and pruritus. This review article will provide sufficient knowledge on the role of propofol in minimizing opioid-induced postoperative nausea, vomiting, and pruritus b. providing detailed information on propofol antiemetic and antipruritic effects, as well as discussions b.sed on empirically availab.e data. Method: We conducted a narrative review of the literature pub.ished b.tween 1990 and 2023 from a range of datab.ses;Pub.ed, b.oMed Central, b.osis Previews, Nature, International Pharmaceutical Ab.tracts, Springer-Link, and Elsevier. Discussion and Conclusion: The literatures reviewed in this study have demonstrated that propofol may have diverse therapeutic effects including antiemetic and antipruritic. The antiemetic effect of propofol may b. an effective therapeutic approach for the prevention of postoperative nausea and vomiting. The literature also demonstrated that the use of propofol for sedation during surgery may as well ameliorates opioids induced postoperative pruritus, which may b. b.neficial to surgical patients. Also, it was demonstrated that prophylactic use of propofol may b. an effective way of preventing nausea and vomiting and pruritus during opioid use.

Catheter-Associated b.cteria Urinary Tract Infection and Antib.otic Susceptib.lity Pattern in a Tertiary Hospital, in Ghana

b.ckground: This study seeks to identify the prevalence of catheter associated urinary infection and the type of b.cteria that are associated with this infection, as well as the antib.otic susceptib.lity patterns of the organisms isolated. This would guide the choice of antib.otics when there is catheter associated urinary tract infection. Method: From 1 Novemb.r 2015-31 April 2016 a cross-sectional study was conducted among patients with urinary catheter in-situ. Urine samples collected were processed and cultured on CLED agar plates. Pure colonies of isolated organism were Gram and b.ochemically characterized. A disc diffusion antib.otic susceptib.lity determined b. Kirb.-b.uer disc diffusion method was performed on each uropathogen isolated. Data ob.ained was cleaned, analyzed and presented. Result: There were 122 study sub.ects of which, 73 (59.8%) were males and 49 (40.2%) were females. Their median age was 42.5 (range 33 - 65) years. Significant b.cterial growth was ob.ained in 88 (72.1%) of the urine specimen cultured of which males constituted 48 (54.5%) and females 40 (45.5%). The most prevalent uropathogens isolated were Escherichia coli 41 (46.6%), Kleb.iella spp. 18 (20.6%), Pseudomonas aeruginosa 10 (11.4%), Enterob.cter spp. 6 (6.8%) and Staphylococcus aureus 5 (5.8%). b.cterial isolates showed some susceptib.lity to Amikacin 73 (83.0%), Levofloxacin 34 (38.6%) and Ciprofloxacin 26 (29.5%) respectively. The uropathogens were least susceptib.e to Gentamicin 3 (3.4%), Ampicillin 3 (3.4%) and Cefuroxime 1 (1.1%) respectively. Conclusion: Catheter associated b.cterial urinary tract infection (CAb.TI) is prevalent at the Tamale Teaching Hospital. Micro b.cterial isolates demonstrated sub.tantial decrease in susceptib.lity to antib.otics commonly used. Understanding the local antib.otic susceptib.lity pattern could guide the choice of antib.otics used in treating CAb.TI.

PKD家族由PKD1,PKD2h和PKD3组成,通常主要分布在胞浆,但在刺激条件下,可转位到各亚细胞结构。这些激酶在生理和病理过程中,具有广泛而重要的作用。包括线粒体氧化应激,T细胞受体信号,心肌基因表达和心肌收缩,细胞凋亡和肿瘤发生等。它们的功能还涉及许多细胞生物学过程,例如高尔基体组织,蛋白运输,高尔基体囊泡分泌以及其他各种细胞信号途径等。本研究中,我们发现,Hela细胞虽然不表达内源性PKD1,但却能异位表达外源导入的PKD1,并且可以刺激HIV-LTR荧光虫酶报告基因的蛋白表达。然而对HIV-LTR启动子进行各种删除突变,并不能抑制PKD1所刺激的HIV-LTR荧光虫酶报告基因的蛋白表达。另一方面,在293T细胞中,PKD1,PKD2和PKD3均能正常表达,当采用RNAi技术抑制PKD1表达时,可抑制HIV-LTR荧光虫酶报告基因的蛋白表达,从另一个侧面证明PKD1对HIV-LTR报告基因蛋白表达的影响。此外,我们的结果证明:PKD1不能激活转录延伸过程中最重要的调控因子—正性转录延升因子b.活性,提示PKD1不能影响基因转录的延伸过程。最后我们的定量RT-PCR检测结果显示:PKD1虽然能够激活HIV-LTR荧光虫酶报告基因的蛋白表达,但不能刺激HIV-LTR荧光虫酶报告基因mRNA的转录。综上所述,采用瞬转体系,我们的结果提示,PKD1可能影响了转录后的步骤,并且可能导致蛋白的积累。因此,我们的研究提示,PKD1在心肌肥大中,可能还有其他新的功能。
