A 5-Year Retrospective Study on the Use of Manual Vacuum Aspiration in the Federal Teaching Hospital Abakaliki

Introduction: Since the inception of our hospital in 2011, manual vacuum aspiration has been in use for the treatment first trimester miscarriages. Hence there is a need for operational review on its use. Aim: The aim of this study is to evaluate the determinants and outcome of Manual Vacuum Aspiration (MVA) use in our hospital. Method: This was a retrospective study on the use of MVA for various indications in our facility over a 5-year period. Results: There were 625 (19.7%) manual vacuum aspirations among 3179 gynaecological patients seen during the period. The age range of the women was from 15 to 48 years and the mean age was 28.5 ± 5.3 years. Incomplete abortion was the commonest indication for the use of MVA and accounted for 89.9% of cases. Other indications for the use of MVA include missed miscarriage (1.8%), and blighted ovum (1.8%). The complications were uterine perforation (0.3%), infection (3.7%) and severe anaemia (10.7%). The mean total hospital stay was 1.6 ± 0.6 days. Overall, 267 (42.7%) patients were managed as a day case. Conclusion: Manual vacuum aspiration is an effective tool in the management of early pregnancy complications. It is a safe, easily performed and possibly cost-effective procedure, with advantages for both the patient and the health care system.

Background: The application of invasive obstetric procedures has an end point of reducing the Caesarean section rate. The declining rate of use of these procedures is one of the reasons for increasing Caesarean section rates in our environment. Objective: The aim of the study was to determine the practice of operative vaginal deliveries among obstetricians practicing in Nigeria and to evaluate the reasons for non-use of these procedures. Methodology: It was a questionnaire based study. The questionnaires were administered to practitioners of Obstetrics in the various centres in Nigeria. Data was collated and analyzed with Epi-Info statistical software version 7.0 (Center for Disease Control and Prevention, USA), and conclusions were drawn by means of descriptive statistics. Results: A total of 1200 questionnaire were distributed but 1104 were returned and used for analysis. This gave a response rate of 92%. The age distribution of the respondents showed that the 20 - 30 year age group had the least number of respondents 22 (2%) while 41 - 50 age group had the highest number of respondents 486 (4%). The majority of the respondents were males 839 (76%). In terms of duration of practice, 449 (40.7%) of the respondents had more than 6 years duration of practice while 256 (23.2) had practiced for less than 3 years. Tertiary centres had highest number of respondents 71.8%. The distribution of the respondents according to the geopolitical zones in Nigeria are;South-East (57.6%), South-South (19.6%), South West (11.4%), North-West (4.9%), North-East (3.8%), North-Central (2.7%). The practice of these procedures among respondents were: External cephalic version (68.0%), Symphysiotomy (41.7), Forceps delivery (68.8%), Destructive delivery (60.1%), Vaginal breech delivery (85.5%), Vacuum extraction (84.8%). In tropical countries, malaria and preeclampsia/eclampsia are common diseases of pregnancy;and placenta have been implicated in the pathophysiology of both disease processes. The two diseases have pathophysiologic similarities in the placenta such as placenta ischaemia, endothelial dysfunction and production of pro-inflammatory cytokine. Yet, there is paucity of studies on the association of these two disease processes. Determining the association between the two disease processes may help to unravel the pathogenesis of preeclampsia and also help in its prevention and patient management. Objective: Determined the association between placenta malaria parasitemia and preeclampsia/eclampsia among parturients at Alex Ekwueme Federal University Teaching Hospital Abakaliki. Materials and Methods: This was a case control study that was conducted in the Labour wards of department of Obstetrics and Gynaecology, Alex Ekwueme Federal University Teaching Hospital Abakaliki (AEFUTHA) and Mile 4 Missionary Hospital Abakaliki, a comprehensive health care centre in Abakaliki, Ebonyi state. It was conducted over a period of 6 months between 1st October 2021 and 31st March, 2022. The cases in this study were parturients that developed preeclampsia/eclampsia in the course of pregnancy, while the controls were parturient without preeclampsia/eclampsia. Interviewer-administered questionnaires were used to collect data on socio-demographic characteristics, obstetrics and medical histories. Histological examinations were conducted to isolate plasmodium falciparum parasites from placenta samples obtained from the maternal surface of the placenta. The data was processed using Epi Info software. Categorical variables were analyzed using Mc Nemar X2 test, with a p-value of 0.05 considered statistically significant. The density functional theory (DFT) with a minimal number of parameters allows a very successful phenomenological description of ground state properties of nuclei all over the periodic table. The recent developments on the application of the covariant density functional theory as well as its extensions by the group in Beijing for a series of interests and hot topics in nuclear astrophysics and nuclear structure are reviewed, including the rapid neutron-capture process, Th/U chronometer, and isospin corrections for superallowed β transitions.

Changes in Soil C and N Contents and Mineralization Across a Cultivation Chronosequence of Paddy Fields in Subtropical China

Dynamics of soil organic matter in a cultivation chronosequence of paddy fields were studied in subtropical *** of soil organic matter was determined by measuring CO2 evolution from soil during 20 days of laboratoryincubation. In the first 30 years of cultivation, soil organic C and N contents increased rapidly. After 30 years, 0-10 cmsoil contained 19.6 g kg-1 organic C and 1.62 g kg-1 total N, with the corresponding values of 18.1 g kg-1 and 1.50g kg-1 for 10-20 cm, and then remained stable even after 80 years of rice cultivation. During 20 days incubation themineralization rates of organic C and N in surface soil (0-10 cm) ranged from 2.2% to 3.3% and from 2.8% to 6.7%,respectively, of organic C and total N contents. Biologically active C size generally increased with increasing soil organicC and N contents. Soil dissolved organic C decreased after cultivation of wasteland to 10 years paddy field and thenincreased. Soil microbial biomass C increased with number of years under cultivation, while soil microbial biomass Nincreased during the first 30 years of cultivation and then stabilized. After 30 years of cultivation surface soil (0-10 cm)contained 332.8 mg kg-1 of microbial biomass C and 23.85 mg kg-1 of microbial biomass N, which were 111% and 47%higher than those in soil cultivated for 3 years. It was suggested that surface soil with 30 years of rice cultivation insubtropical China would have attained a steady state of organic C content, being about 19 g kg-1.

The cucumber scab caused by Cladosporium cucumerinum Ell. is one of the most important diseases of cucumber in the PR. of China and elsewhere. Recently, wider acceptance of under glas culture exacerbates the problem. Both in greenhouses and in free fields in the Northern China pose a challenge in the control of this disease, because there are no effective fungicide to be used. Induced resistance with chemicals may be an ideal way to protect cucumber against this disease , if there are no resistant cultivars to be used. This paper reports the effect of BION, a new activator of SAR, on the induction of resistance to cucumber scab using two treatment methods (seed treatment and foliar spray) at different concentrations (10,20,40, 80, 160 mg. mL- 1). The results showed that the number of diseased plants was decreased drastically in BION - treated plants, the disease incidence was 35 %, whereas it was 75 % in control plants. The disease severity of plant with BION treatment by foliar spray was 2 and it was 1.5 if used as seed treatment, whereas it was 3 and 3.1 in control plants respectively. Moreover it has no phytotoxic effect to be observed at concentrations used in the experiment.
