Nationwide trends and predictors of inpatient mortality in 83884 transjugular intrahepatic portosystemic shunt

AIM: To evaluate and validate the national trends and predictors of in-patient mortality of transjugular intrahepatic portosystemic shunt (TIpS) in 15 ***: Using the National Inpatient Sample which is a part of Health Cost and Utilization project, we identified a discharge-weighted national estimate of 83884 TIpS procedures performed in the United States from 1998 to 2012 using international classification of diseases-9 procedural code 39.1. The demographic, hospital and co-morbility data were analyzed using a multivariant analysis. Using multi-nominal logistic regression analysis, we determined predictive factors related to increases in-hospital mortality. Comorbidity measures are in accordance to the Comorbidity Software designed by the Agency for Healthcare Research and ***: Overall, 12.3% of patients died during hospitalization with downward trend in-hospital mortality with the mean length of stay of 10.8 ± 13.1 d. Notable, African American patients (OR = 1.809 vs Caucasian patients, p patients (OR = 1.347 vs non-transferred, p p patients in the Northeast region (OR = 1.449 vs West, p pital mortality. Number of diagnoses and number of procedures showed positive correlations with in-hospital death (OR = 1.249 per one increase in number of procedures). patients diagnosed with acute respiratory failure (OR = 8.246), acute kidney failure (OR = 4.359), hepatic encephalopathy (OR = 2.217) and esophageal variceal bleeding (OR = 2.187) were at considerably higher odds of in-hospital death compared with ascites (OR = 0.136, p pital death were fluid and electrolyte disorders (OR = 2.823), coagulopathy (OR = 2.016), and lymphoma (OR = 1.842).CONCLUSION: The overall mortality of the TIpS procedure is stea

Demonstrate that the key factor to determine the activity of reconstructed surfaces is the surface chemistry, instead of reconstruction degree using the popular perovskite LaNi1-xFexO3 oxides as model *** content can influence both the surface reconstruction degree, the activation degree, and the activity of reconstructed *** oxygen evolution reaction activity of reconstructed catalysts is primarily governed by the chemistry of the reconstructed surface species. Water splitting hinges crucially on the availability of electrocatalysts for the oxygen evolution reaction. The surface reconstruction has been widely observed in perovskite catalysts, and the reconstruction degree has been often correlated with the activity enhancement. Here, a systematic study on the roles of Fe substitution in activation of perovskite LaNiO3 is reported. The substituting Fe content influences both current change tendency and surface reconstruction degree. LaNi0.9Fe0.1O3 is found exhibiting a volcano-peak intrinsic activity in both pristine and reconstructed among all substituted perovskites in the LaNi1-xFexO3 (x = 0.00, 0.10, 0.25, 0.50, 0.75, 1.00) series. The reconstructed LaNi0.9Fe0.1O3 shows a higher intrinsic activity than most reported NiFe-based catalysts. Besides, density functional theory calculations reveal that Fe substitution can lower the O 2p level, which thus stabilize lattice oxygen in LaNi0.9Fe0.1O3 and ensure its long-term stability. Furthermore, it is vital interesting that activity of the reconstructed catalysts relied more on the surface chemistry rather than the reconstruction degree. The effect of Fe on the degree of surface reconstruction of the perovskite is decoupled from that on its activity enhancement after surface reconstruction. This finding showcases the importance to customize the surface chemistry of reconstructed catalysts for water oxidation.

Isothermal study of effusion cooling flows using a large eddy simulation approach

An isothermal numerical study of effusion cooling flow is conducted using a large eddy simulation(LES) *** main types of cooling are considered,namely tangential film cooling and oblique patch effusion *** represent tangential film cooling,a simplified model of a plane turbulent wall jet along a flat plate in quiescent surrounding fluid is *** contrast to a classic turbulent boundary layer flow,the plane turbulent wall jet possesses an outer free shear flow region,an inner near wall region and an interaction region,characterised by substantial levels of turbulent shear stress *** shear stress characteristics hold significant implications for RANS modelling,implications that also apply to more complex tangential film cooling flows with non-zero free stream *** LES technique used in the current study provides a satisfactory overall prediction of the plane turbulent wall jet flow,including the initial transition region,and the characteristic separation of the zero turbulent shear stress and zero shear strain *** effusion patch cooling is modelled using a staggered array of 12 rows of effusion holes,drilled at 30° to the flat plate *** effusion holes connect two channels separated by the flat ***,these comprise of a channel representing the combustion chamber flow and a cooling air supply channel.A difference in pressure between the two channels forces air from the cooling supply side,through the effusion holes,and into the combustion chamber *** from successive effusion rows coalesces to form an aerodynamic film between the combustion chamber main flow and the flat *** practical applications,this film is used to separate the hot combustion gases from the combustion chamber *** numerical model is shown to be capable of accurately predicting the injection,penetration,downstream decay,and coalescence of the effusion *** addition,the numerical model captures entrainmen

Management and prevention of acute and chronic lateral ankle instability in athletic patient populations

Acute and chronic lateral ankle instability are common in high-demand patient populations. If not managed appropriately, patients may experience recurrent instability, chronic pain, osteochondral lesions of the talus, premature osteoarthritis, and other significantlong-term disability. Certain populations, including young athletes, military personnel and those involved in frequent running, jumping, and cutting motions, are at increased risk. proposed risk factors include prior ankle sprain, elevated body weight or body mass index, female gender, neuromuscular deficits, postural imbalance, foot/ankle malalignment, and exposure to at-risk athletic activity. prompt, accurate diagnosis is crucial, and evidence-based, functional rehabilitation regimens have a proven track record in returning active patients to work and sport. When patients fail to improve with physical therapy and external bracing, multiple surgical techniques have been described with reliable results, including both anatomic and nonanatomic reconstructive methods. Anatomic repair of the lateral ligamentous complex remains the gold standard for recurrent ankle instability, and it effectively restores native ankle anatomy and joint kinematics while preserving physiologic ankle and subtalar motion. Further preventative measures may minimize the risk of ankle instability in athletic cohorts, including prophylactic bracing and combined neuromuscular and proprioceptive training programs. These interventions have demonstrated benefit in patients at heightened risk for lateral ankle sprain and allow active cohorts to return to full activity without adversely affecting athletic performance.

Under the assumption that the underlying measure is a non-negative Radon measure which only satisfies some growth condition and may not be doubling, we define the product of functions in the regular BMO and the atomic block -~1 in the sense of distribution, and show that this product may be split into two parts, one in L1 and the other in some Hardy-Orlicz space.

BACKGROUND Hepatic artery stenosis is a complication of orthotopic liver transplant occurring in 3.1%-7.4%of patients that can result in graft failure and need for *** therapy with angioplasty and stenting has been used with a high degree of technical success and good clinical outcomes,but tortuous hepatic arteries present a unique challenge for *** stents for this application should be maneuverable and conformable while also exerting adequate radial force to maintain a patent *** SUMMARY Herein we report our experience with a neurovascular Wingspan stent system in a challenging case of recurrent hepatic artery stenosis and discuss the literature of stenting in tortuous transplant hepatic *** Wingspan neurovascular stent is self-expanding,has good conformability,and adequate radial resistance and as such it could be added to the armamentarium of interventionalists in the setting of a tortuous and stenotic transplant hepatic artery.

Removal of saxitoxin and anatoxin-a by pAC in the presence and absence of microcystin-LR and/or cyanobacterial cells

Cyanobacteria can produce cyanotoxins such as microcystin-LR(mc),saxitoxin(STX),and anatoxin-a(ANTX-a)which are harmful to humans and other *** removal efficiencies of STX and ANTX-a by powdered activated carbon(pAC)was investigated,aswell as when mc-LR and cyanobacteriawere *** conductedwith distilled water and then source water,using the pAC dosages,rapid mix/flocculation mixing intensities and contact times of two drinking water treatment plants in northeast *** pH 8 and 9,STX removal was 47%-81%in distilled water and 46%-79%in source water,whereas it was 0-28%for pH 6 in distilled water and 31%-52%in source *** 1.6μg/L or 20μg/L mc-LR was present with STX,STX removal was increased with pAC simultaneously removing 45%-65%of the 1.6μg/L mc-LR and 25%-95%of the 20μg/L mc-LR depending on the ***-a removal at pH 6 was 29%-37%for distilled water and 80%for source water,whereas it was 10%-26%for pH 8 in distilled water and 28%for pH 9 in source *** presence of cyanobacteria cells decreased ANTX-a removal by at least 18%.When 20μg/L mc-LR was present with ANTX-a in source water,59%-73%ANTX-a and 48%-77%of mcLR was removed at pH 9 depending on the pAC *** general,a higher pAC dose led to higher cyanotoxin *** study also documented that multiple cyanotoxins can be effectively removed by pAC for water at pH’s between 6 and 9.

Calcium acts as a second messenger for signaling to a variety of stimuli including MAMps （Microbe-Associated Molecular patterns）, such as fig22 and elf18 that are derived from bacterial flagellin and elongation factor Tu, respectively. Here, Arabidopsis thaliana mutants with changed calcium elevation （cce） in response to fig22 treatment were isolated and characterized. Besides novel mutant alleles of the fig22 receptor, FLS2 （Flagellin-Sensitive 2）, and the receptor-associated kinase, BAK1 （Brassinosteroid receptor 1-Associated Kinase 1）, the new cce mutants can be categorized into two main groups--those with a reduced or an enhanced calcium elevation. Moreover, cce mutants from both groups show differ- ential phenotypes to different sets of MAMps. Thus, these mutants will facilitate the discovery of novel components in early MAMp signaling and bridge the gaps in current knowledge of calcium signaling during plant-microbe interactions. Last but not least, the screening method is optimized for speed （covering 384 plants in 3 or 10 h） and can be adapted to genetically dissect any other stimuli that induce a change in calcium levels.
