Diabetic retinopathy(DR)is a form of *** oxidative stress in the mitochondria and cell membranes decreases ischemic injury and end-organ damage to the *** approaches are needed,which reduce the risk and improve the outcomes of DR while complementing current therapeutic ***(hcy)elevation and oxidative stress are potential therapeutic targets in *** genetic polymorphisms such as those of methylenetetrahydrofolate reductase(MThFR),increase hcy and DR risk and *** with DR have high incidences of deficiencies of crucial vitamins,minerals,and related compounds,which also lead to elevation of hcy and oxidative *** the effects of the MThFR polymorphism and addressing comorbid deficiencies and insufficiencies reduce the impact and severity of the *** approach provides safe and simple strategies that support conventional care and improve *** vitamin co-factor availability also impairs the release of neurotrophic and neuroprotective growth ***,this accounts for variability in presentation and response of DR to conventional ***,there are straightforward recommendations for addressing these issues and supporting traditional treatment *** have reviewed the literature for nutritional interventions that support conventional therapies to reduce disease risk and *** combinations of vitamins B1,B2,B6,L-methylfolate,methylcobalamin(B12),C,D,natural vitamin E complex,lutein,zeaxanthin,alpha-lipoic acid,and n-acetylcysteine are identified for protecting the retina and *** medical foods have been successfully used as therapy for *** based on this review and our clinical experience are developed for clinicians to use to support conventional therapy for *** from both type 1 diabetes mellitus(T1DM)and type 2 diabetes mellitus(T2DM)have similar retinal findings and responses to nutritional therapies.

Laparoscopic pancreaticoduodenectomy: a descriptive and comparative review

Laparoscopic pancreaticoduodenectomy（LPD） is an extremely challenging surgery. First described in 1994, it has been slow to gain in popularity. Recently, however, we have seen an increase in the number of centers performing this operation, including our own institution, as well as an increase in the quantity of published data. The purpose of this review is to describe the current status of LPD as described in the literature. We performed a literature search in the Pub Med database using Me Sh terms ＂laparoscopy＂ and ＂pancreaticoduodenectomy＂. We then identified articles in the English language with over 20 patients that focused on LPD only. Review articles were excluded and only one article per institution was used for descriptive analysis in order to avoid overlap. There were a total of eight articles meeting review criteria, consisting of 492 patients. On descriptive analysis we found that percent of LPD due to high-grade malignancy averaged 47% over all articles. Average operative time was 452 minutes, blood loss 369 cc＇s, pancreatic leak rate 15%, delayed gastric emptying 8.6%, length of hospital stay 9.4 days, and short term mortality 2.3%. Comparison studies between open pancreaticoduodenectomy（OPD） and LPD suggested decreased blood loss, longer operative time, similar post-operative complication rate, decreased pain, and shorter hospital length of stay for LPD. There was also increased number of lymph nodes harvested and similar margin free resections with LPD in the majority of studies. LPD is a safe surgery, providing many of the advantages typically associated with laparoscopic procedures. We expect this operation to continue to gain in popularity as well as be offered in increasingly more complex cases. In future studies, it will be beneficial to look further at the oncologic outcome data of LPD including survival.

BACKGROUND:Three types of progressive familial intrahepatic cholestasis(PFIC)have been identified,but their etiologies include unknown mechanisms. DATA SOURCES:A PubMed search on'progressive familial intrahepatic cholestasis'and'PFIC'was performed on the topic,and the relevant articles were reviewed. RESULTS:The etiologies of the three PFIC types still include unknown *** in PFIC type 1,enterohepatic circulation of bile acid should be *** acid,partial external biliary diversion and liver transplantation have been used for the treatment of PFIC patients according to disease course. CONCLUSIONS:Since the etiologies and disease mechanisms of PFIC are still unclear,detailed studies are urgently required. Strategies for more advanced therapies are also *** developments in the future are indispensable,especially for PFIC type 1 patients.

Immunotherapy for pancreatic ductal adenocarcinoma: an overview of clinical trials

Pancreatic ductal adenocarcinoma（PDAC） is the fourth leading cause of cancer-related death and current therapeutic strategies are often unsatisfactory. Identification and development of more efficacious therapies is urgently needed. Immunotherapy offered encouraging results in preclinical models during the last decades, and several clinical trials have explored its therapeutic application in PDAC. The aim of this review is to summarize the results of clinical trials conducted to evaluate the future perspective of immunotherapy in the treatment of PDAC.

Enemy release at range edges:do invasive species escape their herbivores as they expand into new areas?

Under the assumption that the underlying measure is a non-negative Radon measure which only satisfies some growth condition and may not be doubling, we define the product of functions in the regular BMO and the atomic block -~1 in the sense of distribution, and show that this product may be split into two parts, one in L1 and the other in some hardy-Orlicz space.

Distinguishing between parenchymal and anastomotic leakage at duct-to-mucosa pancreatic reconstruction in pancreaticoduodenectomy

AIM： To distinguish anastomotic from parenchymal leakage at duct-to-mucosa reconstruction of the pancreatic remnant. METhODS： We reviewed the charts of 68 pancreaticoduodenectomies performed between 5/2000 and 12/2005 with end-to-side duct-to-mucosa pancreatojejunostomy （PJ）. The results of pancreatography, as well as peripancreatic drain volumes, and amylase levels were analyzed. RESULTS： Of 68 pancreatojejunostomies, 48 had no leak by pancreatography and had low-drain amylase （normal）; eight had no pancreatographic leak but had elevated drain amylase （parenchymal leak）; and 12 had pancreatographic leak and elevated drain amylase （anastomotic leak）. Although drain volumes in the parenchymal leak group were significantly elevated at postoperative day （POD） 4, no difference was found at POD 7. Drain amylase level was not significantly different at POD 4. In contrast, at POD 7, the anastomotic-leak group had significantly elevated drain amylase level compared with normal and parenchymalleak groups （14158 ＋ 24083 IU/L vs 89 ＋ 139 IU/L and 1707 ＋ 1515 IU/L, respectively, P = 0.012）. CONCLUSION： For pancreatic remnant reconstruction after pancreaticoduodenectomy, a combination of pancreatogram and peripancreatic drain amylase levels can be used to distinguish between parenchymal and anastomotic leakage at pancreatic remnant reconstruction.

The ghosts of parasitism past:lingering frontline anti-brood parasite defenses in a former host

Coevolutionary arms races between brood parasites and hosts provide tractable systems for understanding antagonistic coevolution in nature;however,little is known about the fate of frontline antiparasite defenses when the host“wins”the coevolutionary arms *** recreating bygone species interactions,using artificial parasitism experiments,lingering defensive behaviors that evolved in the context of parasitism can be understood and may even be used to identify the unknown agent of parasitism *** we present the first study of this type by evaluating lingering“frontline”nest defenses that have evolved to prevent egg laying in a former brood parasite *** Australian reed warbler Acrocephalus australis is currently not parasitized but is known to exhibit fine-tuned egg discrimination—a defensive behavior indicative of a past brood parasite–host arms race and common in closely related parasitized ***,using 3D-printed models of adult brood parasites,we examined whether the Australian reed warbler also exhibits frontline defenses to adult brood parasites,and whether we could use these defenses to identify the warbler’s“ghost of parasitism past.”Our findings provide evidence that the Australian reed warbler readily engages in frontline defenses that are considered adaptive specifically in the context of brood ***,individuals were unable to discriminate between adults of different brood parasite species at their ***,our results demonstrate that despite a relaxation in selection,defenses against brood parasitism can be maintained across multiple stages of the host’s nesting cycle,and further suggest that,in accordance with previous findings,that learning may be important for fine-tuning frontline defense.

Rapid on-site identification of volatile organic compounds （VOCs） in ambient air is an important first step in remediation efforts. This study describes modification of a commercially available, portable GC/MS system and development of an analysis protocol for rapid （〈 3 min） sampling and identification of VOCs typically found at contaminated sites at the low ppbv level.

BACKGROUND:A reliable model of fulminant liver failure (FLF) is urgently required in this research *** study aimed to develop a murine FLF ***:We used three groups of male C57BL/6 mice:control,with azoxymethane treatment (AOM group),and with galactosamine and tumor necrosis factor-alpha treatment (Gal+TNF-α group).The effects of body temperature (BT) control on survival in all three groups were investigated Using BT control,we compared the survival,histopathological findings and biochemical/coagulation profiles between the two experimental *** effects of hydration on international normalized ratios of prothrombin time (PT INRs) were also ***-dependent survival curves were constructed for both experimental *** behavior was assessed using a coma ***:No unexpected BT effects were seen in the control *** AOM group,but not the Gal+TNF-α group showed a significant difference in survival curves between those with and without BT *** assessment showed consistent FLF findings in both experimental groups with BT *** were significant differences between the experimental groups in aspartate aminotransferase levels and PT-INRs,and significant differences in PT-INRs between the sufficiently and insufficiently hydrated *** were significant differences between FLF models in the duration of each coma stage,with significant differences in stages 1 and 3 as percentages of the disease state (stages 1-4).The two FLF models with BT care showed different survival curves in the dose-dependent survival ***:AOM provides a good FLF model,but requires a specialized environment and careful BT *** FLF models may also be useful,depending on the research *** attention to caregiving and close observation are indispensable for successful FLF models.
