Influence of Excessive Weight Gain on Maternal and Perinatal Morbidity and Mortality in a University Hospital Environment in Cotonou

Introduction: The pathologies associated with excessive weight gain during pregnancy are numerous and frequent. They are the cause of significant maternal and perinatal morbidity and mortality and thus pose a real public health problem. Patients and method: The main objective of our work was to study the influence of excessive weight gain on maternal and perinatal morbidity and mortality in a university hospital in Cotonou. Study method: The study took place at the CUGo. This was a descriptive and analytical study with retrospective data collection over a period of 10 years from January 1, 2012 to December 31, 2021. The analysis focused on the description of pregnant women and the search for factors associated with an excessive weight gain during pregnancy and which influence maternal and perinatal prognosis. The different odds ratios and their confidence intervals were calculated. The significance level was 5%. Results: A total of 690 pregnant women were included and divided into four sub-populations according to their pre-gestational or first trimester BMI: 05.94% underweight pregnant women, 41.74% normal BMI pregnant women, 28.55% of overweight pregnant women and 23.77% obese pregnant women. The average age of pregnant women was 30.41 years ± 5.40 years. They were civil servants (35.80%) and resided in Cotonou in 93.91% of cases. Pregnant women had a history of hypertension in nearly 3% of cases. Pregnancy hypertension (07.97%) was the main pathology associated with pregnancy in our study. Weight gain was excessive in overweight pregnant women and obese pregnant women in the same proportion 41.12%. The average weight gain was 9.42 kg for underweight pregnant women, 8.35 kg for pregnant women with a normal BMI, 8.46 kg for overweight pregnant women and 7.45 kg for obese pregnant women. Pregnant women who had gained excessive weight during pregnancy had a 9 times higher risk of having pregnancy-induced hypertension (oR = 9.5 and p ≤ 0.0001). They also presented a 6

Introduction: Polycystic ovarian Syndrome (PCoS), also known as Stein Leventhal Syndrome, is one of the common endocrine diseases affecting women of childbearing age and the leading cause of anovulatory infertility worldwide. The aim of this study was to describe the clinical, paraclinical and therapeutic aspects of PCoS at the University Clinic of obstetrics and Gynaecology of the National Hospital and University Centre Hubert Koutoukou MAGA of Cotonou (CNHU). Patients and Method: The study was doing in the University Clinic of Gynecology and obstetrics of the CNHU-HKM. This was a descriptive and analytical longitudinal study with retrospective and prospective data collection over a period of 7 years from January 2015 to December 2021. It focused on women of childbearing age seen in gynecological consultation. Results: The frequency of PCoS was 2.53%. The mean age of the patients was 27.28 ± 6.55 years. They were educated for the most part with a university level in 43.8% of cases. obstetrically they were nulligest (45.63%) and nulliparous in 60% of cases. The history of spontaneous miscarriages was found in 61.11%. The main reasons for consultation were menstrual disorders (51%) followed by the desire for pregnancy (40.6%). Clinically, obesity (30%), overweight (33.1%), high blood pressure (20.63%), hirsutism (24%) and acne (27%) were noted. All the women had micropolycystic ovaries in ultrasound. Testosterone was elevated in 61% of cases. Phenotype D was found in 45.63% of patients. All these patients were put under lifestyle and dietary measures, Metformin (13.80%) and estrogen-progestogen (18.80%). The outcome of the treatment was assessed by cycle regularity, regression of hirsutism, weight loss, fertility and fertility. Introduction: Breast cancer mortality remains high in most developing countries. Benin Republic does not yet have a technical platform required for the diagnosis and effective management of breast cancer “according to the recommendations”. objective: To study the epidemiological, diagnostic and therapeutic aspects of locally advanced and/or metastatic breast cancers at the Centre Hospitalier Universitaire-de la Mère et de l’Enfant Lagune (CHU-MEL) of Cotonou from 2018 to 2021. Study Methods: This was a retrospective cross-sectional study with a descriptive purpose, carried out from January 1, 2018 to September 30, 2021 in the Gynaecology-obstetrics Section of the CHU-MEL of Cotonou. The study population was patients diagnosed with locally advanced and/or metastatic breast cancers. Results: At the end of the study, 63 cases of locally advanced and/or metastatic breast cancer were recorded in 80 cases of breast cancer;a 74.11% prevalence of locally advanced and metastatic breast cancer. The mean age of the patients was 42.6 years. The main reason for consultation was the discovery of a mass in the breast (65.1%). Clinically, an orange peel was noted in 39.6%. Mammography (47.6%) and breast ultrasonography (42.9%) were the reference imaging workups for breast cancer diagnosis. Histopathologically, infiltrating ductal carcinoma (94.1%) was the predominant type with Scarff-Bloom and Richardson (SBR) grade II found in 35.3%. Progesterone and estrogen receptors were present in 42.9%. Mastectomy with axillary node dissection was the surgical treatment performed (80%) after neoadjuvant chemotherapy (39.6%). only one patient had received radiotherapy. No patient received hormone therapy. Conclusion: Locally advanced and metastatic cancers remain a real public health problem. It is essential that major investments be made in order to improve both their diagnosis and management.

Retrospective View of Two Years of Colposcopy Practice in a Gynecologic Clinic in Cotonou

objective: To analyse the colposcopy results at the Gynaecological Clinic of Houéyiho. Patients and methods: Retrospective, descriptive and analytical studies were conducted by the Clinic of Houeyiho in Cotonou from January 2013 to December 2015. Results: 440 women were received in colposcopy including 251 (57%) referred by health workers for VIA (Visual Inspection with Acétique Acid)/VILI (Visual Inspection with Lugol’s Iodine) abnormalities, forty-seven (47) for abnormal smears (10.7%), thirty-eight (38) for metrorrhagia (8.6%). 10% of cases were mainly concerned with post-therapeutic monitoring (six colposcopies of the vagina fundus after hysterectomy for cervical cancer and four after conization). The average age of patients is 41 years within plus or minus 5 years. Forty (40) cases of Grade 1 Atypical Transformation, twenty-nine (29) cases of Grade 2 Atypical Transformation including two (2) cases of leukoplakia (1 post-conization for CINIII, 1 in a HIV+ patient), ten (10) cases of Grade 1 Atypical Transformation/Grade 2 Atypical Transformation association were recorded. A colposcopy biopsy was performed in 95 patients meaning 21.6% of patients. Five biopsies were realized for a cervical neoplasia suspicion and were sent to Pasteur Cerba laboratory in France for the sake of efficiency and rapidity of results. In Grade 1 Atypical Transformation, 66.6% of CIN1 and condylomata plana were noted. Nevertheless, a case of mucinous adenocarcinoma with metaplasia has been discovered in a TAG1 with a 23-year old patient. In Grade 2 Atypical Transformation, the histology reveals 44.4% of CIN 1, 40.7% of severe dysplasia and 11.1% of carcinomas. Conclusion: The colposcopy may be a good screening method with a good colpo-histology agreement in Cotonou. But it’s relatively expensive (20.000 CFA francs) and the reduced number of colposcopists restricts its use in Benin.

Maternal Hospital Mortality in Cotonou: Incidence, Care-Associated Adverse Events and Causes

Introduction: Despite the many efforts made to combat preventable maternal deaths, these still remain high in Benin. It was therefore important to revisit the causes but especially the adverse events associated with care (EIS) in maternity. objective: Determine the incidence, adverse events associated with care and causes of intra hospital maternal mortality in cotonou. Methods: This was a cross-sectional maternal mortality review study with retrospective data collection. It covered all cases of maternal death recorded between 2017 and 2021 in two (2) reference university maternities in Cotonou. Adverse events associated with care and the patient were analyzed using the maternal death audit grid validated for Benin. SPSS.26 software was used for data analysis. Results: The in-hospital maternal mortality ratio in Cotonou was 2028 maternal deaths per 100,000 live births in 2021. only 7.2% (n = 36) of deaths were audited. The deceased mothers were 29.8 ± 7.4 years old, with no fixed monthly income in 82.7% (n = 420). Serious adverse events associated with care were: delay in decision to refer in 37% (n = 188), non-medical referral in 85.8% (n = 436) of cases, inadequate pre-referral treatment in 25.7% of cases. In receiving maternities, delay in diagnosis and inappropriate treatment at the receiving maternity were noted respectively in 22.9% and 28.6% of cases of maternal death. The direct causes of maternal death were dominated by serious obstetric hemorrhage in 43.9% (n = 223). As for the indirect causes, they were dominated by anemia excluding obstetric hemorrhages in 21.5% (n = 109). Conclusion: The in-hospital maternal mortality ratio was very high in Cotonou. The main cause was severe obstetric hemorrhage. There were several serious healthcare-associated adverse events whose correct management would significantly reduce the incidence of maternal deaths.

Introduction: Gynaecological cancers are the deadliest of the women’s cancers in the Republic of Benin. Late diagnosis is the most common reason. objective: This paper aims to describe the epidemiological characteristics, and clinical and pathological signs of gynaecological cancers treated in the Republic of Benin between 2018 and 2022. Patients and Methods: This was a cross-sectional, descriptive, retrospectively collected study of patient data treated between 2018 and 2022 in two university gynaecology departments in Cotonou. All gynaecological cancers that have histological evidence were included. The epidemiological, clinical and pathological characteristics of the cancers were assessed. Results: Cervical, endometrial and ovarian cancers were the most common in the proportions of 62.0%, 24.1%, 12.0% and 1.8% respectively. The mean age at diagnosis was 54 years. The victims were uneducated and had low economic power in 81% and 85% of cases, respectively. The consultation was late in 82.1% of cases. Metrorrhagia, postmenopausal metrorrhagia and pelvic cluster headache were the common reasons for consultation for cervical, endometrial and ovarian cancer, respectively. Diagnosis was late in 66.7% (n = 71). The most common histological types were squamous cell carcinoma, endometrioid adenocarcinoma, and serous cystadenocarcinoma for cervical, endometrial, and ovarian cancers, respectively. Conclusion: Gynaecological cancers were common and their consultation time was delayed. The diagnosis was made at the advanced stage and there were several reasons for this.

The CoVID-19 outbreak in South Korea: Lessons to Be Learned

The Coronavirus 2019 (CoVID-19) is a novel type of coronavirus-related disease that has over 4 million confirmed cases worldwide as of May 13th, 2020. With over 200 countries impacted by the pandemic, many countries have taken drastic measures such as temporary closure of international borders. The purpose of this thesis is to examine the South Korean response to CoVID-19 and the keys to successful containment of the disease. Real time analysis was performed on data provided by the Centers for Disease Control (CDC). Comparisons of disease containment among countries with the highest confirmed cases were normalized for population size differences by taking the proportion of confirmed cases to population size. We further compared the disease outbreak in Seoul, a very urban environment, to the whole country of South Korea, to compare public health in urban and rural environments. We found that the efficient partnership between the private sector and the state led to rapid development in testing kits, which was integral to the South Korean response to CoVID-19. In addition, the South Koreans’ community spirit, approval of government-led interventions, and societal norm of wearing masks were also efficient social responses to the spreading disease. In this paper, we navigate the impacts of a universal healthcare system and its ability to battle infectious diseases and the efficacy of various governmental actions in response to a public health crisis.
