Floods affect more people globally than any other type of natural hazard. Great potential exists for new technologies to support flood disaster risk reduction. In addition to existing expert-based data collection and analysis, direct input from communities and citizens across the globe may also be used to monitor, validate, and reduce flood risk. New technologies have already been proven to effectively aid in humanitarian response and recovery. However, while ex-ante technologies are increasingly utilized to collect information on exposure, efforts directed towards assessing and monitoring hazards and vulnerability remain limited. Hazard model validation and social vulnerability assessment deserve particular attention. New technologies offer great potential for engaging people and facilitating the coproduction of knowledge.

Gas-forming liver abscess associated with rapid hemolysis in a diabetic patient

We experienced a case of liver abscess due to Clostridium perfringens(CP) complicated with massive hemolysis and rapid death in an adequately controlled type2 diabetic patient. The patient died 6 h after his first visit to the hospital. CP was later detected in a blood culture. We searched for case reports of CP septicemia and found 124 cases. Fifty patients survived, and 74died. Of the 30 patients with liver abscess, only 3 cases survived following treatment with emergency surgica drainage. For the early detection of CP infection, detection of Gram-positive rods in the blood or drainage fluid is important. Spherocytes and ghost cells indicate intravascular hemolysis. The prognosis is very poor once massive hemolysis occurs. The major causative organisms of gas-forming liver abscess in diabetic patients are Klebsiella pneumoniae(K. pneumoniae) and Escherichia coli(E. coli). Although CP is relatively rare,the survival rate is very poor compared with those of K. pneumoniae and E. coli. Therefore, for every case that presents with a gas-forming liver abscess, the possibility of CP should be considered, and immediate aspiration of the abscess and Gram staining are important.

目的调查护士对有喂养困难的唇腭裂患儿所采用的临床奶瓶喂养技术现状。方法采用质性描述性研究设计。于2021年12月至2022年1月,向日本1109所设有产科、新生儿科或小儿牙科病房的医院分别发放匿名问卷。参与者是具有5年以上唇腭裂患儿护理工作经验的护士。问卷包括喂养技术4个方面的开放式问题,包括:奶瓶喂养前的准备、乳头插入方法、吸吮帮助和停止奶瓶喂养的标准。根据意义相似性对资料进行分类和分析。结果共获得410份有效问卷。资料分析结果显示,奶瓶喂养前的准备工作分为7个类别(如,改善患儿的口腔运动、保持患儿平稳呼吸),27个子类别;在乳头插入方法中,分为4个类别(如,用乳头在口腔内形成负压封闭裂口、插入乳头时不接触裂口),11个子类别;在吸吮帮助中,分为5个类别(如,促进觉醒、口腔产生负压),13个子类别;在停止奶瓶喂养的标准中,分为5个类别(如,觉醒水平降低、生命体征变差),16个子类别。96.0%护士希望学习唇腭裂患儿的奶瓶喂养技术。结论本研究确定了多项用于唇腭裂喂养困难患儿的奶瓶喂养技术。然而,这些技术之间存在相互矛盾,方法的有效性也未得到评估。未来需要进行干预研究,以确定各项技术的益处或潜在的危害。

Peripheral Nerve Block Combined with Epidural Anesthesia for Incarcerated Inguinal Hernia Repair in a Patient with Severe Chronic Obstructive Pulmonary Disease: A Case Report

Background: Invasive mechanical ventilation worsens prognosis in patients with severe chronic obstructive pulmonary disease (COPD). To avoid complications in these patients, anesthesia should be carefully considered. Case presentation: A 78-year-old man with COPD presented with dyspnea and pain from the epigastric to the umbilical regions. He was diagnosed with left incarcerated inguinal hernia and underwent radical inguinal hernia repair and surgical ileus treatment. To avoid general anesthesia with tracheal intubation, epidural anesthesia was combined with peripheral nerve blocks. An epidural catheter was inserted from T12/L1, and ilioinguinal-iliohypogastric and genitofemoral nerve blocks were performed under ultrasound guidance. No post-surgery complications or pain symptoms were noted. Conclusions: General anesthesia would likely have been challenging due to the patient’s COPD;however, management of peritoneal traction pain is difficult using peripheral nerve block alone. By combining epidural anesthesia with peripheral nerve blocks, we safely performed a procedure in a patient with severe COPD while avoiding invasive positive pressure ventilation.

Efficacy of an Abduction Brace in Preventing Initial Dislocation in the Early Postoperative Period after Primary Total Hip Arthroplasty

We hypothesized that a hip brace may prevent the initial dislocation in the early postoperative period after total hip arthroplasty (THA). We performed a prospective evaluation of the efficacy of a brace in preventing dislocation in 54 primary THAs in 43 patients. All of the patients wore braces for 3 weeks postoperatively. The occurrence of dislocation was evaluated six months postoperatively. There were no cases of dislocation while wearing a brace, while two posterior dislocations occurred 8 and 12 days postoperatively while picking something up from the floor without a brace in so-called provocative positions. The results of this study suggest that a hip brace helps patients to recognize careless provocative positions and prevents the initial hip dislocation in this period instead of usual postoperative management for an average of 6 weeks after discharge, such as a high toilet seat, restricted hip flexion in the activities of daily living, use of a reacher or grabber, an abduction pillow, and a high chair.

Relationships between Tongue and Lip Movement and Physical Fitness among Independent Community-Dwelling Elderly in Japan

Aim: The present study aims to examine the association between tongue and lip movement and physical fitness with a sample of independent community-dwelling elderly. Methods: A total of 119 older adults living in a city of Japan participated in this study. Tongue and lip movement was evaluated with oral diadochokinesis (ODK) rate for the syllable “pa”, “ta”, and “ka” measured by counting syllable repetitions within one second. Physical fitness was objectively evaluated with the Timed-Up and Go Test (TUG) and the 30-Second Chair Stand Test (CS-30). Subjective physical fitness was also assessed with a Physical Component Summary score (PCS) of the 36-item Short-Form Health Survey (SF-36). Partial correlation coefficients between ODK rates and physical fitness measures were obtained after adjusting the effects of age and the number of present teeth. Results: There were significant differences in ODK rates for the syllable “ta” and “ka” between the participants aged 65 - 74 and 75 or older. Significant differences in all the physical fitness measures were also observed in the two age groups. Partial correlation analysis with age and number of present teeth as covariates yielded significant correlations of ODK rates to TUG and PCS, but not with CS-30. Conclusion: Older adults whose functional mobility declined tended to maintain the movement of tongue and lips. Moreover, those who perceived declining physical fitness also tended to show a decline of tongue and lip movement. Thus, the present study added some knowledge on the relationships between general physical fitness and tongue and lip movement. It also made several suggestions for future interventions based on the findings.

Objective: We present descriptive epidemiology of Mycoplasma infection using the (Nursery) School Absenteeism Surveillance System ((N)SASSy) in addition to national official sentinel surveillance for infectious diseases (NOSSID). It is expected to be helpful for early detection and response to outbreak of Mycoplasma infection and also to be useful for antimicrobial resistance measures. Method: The study period was 2010-2014 seasons. The study area was the whole of Ibaraki prefecture, Japan. NOSSID reports the number of patients with Mycoplasma pneumonia from 13 sentinel hospitals. We compare the number of patients and the incidence rate of NOSSID with the same information in (N)SASSy. Result: In NOSSID, the largest number of patients by age was 27 patients of one year old in 2012. (N)SASSy showed that the incidence rate in the elementary schools is high in 2011 and 2012. Especially, the second grade students were the highest. Discussion: Even though Mycoplasma infection is well known as a pediatric disease, we showed that second grade was the highest in incidence at first. Because (N)SASSy is timely and real-time information collection, evaluation and sharing countermeasures with (nursery) schools, public health centers, and physicians are useful for students and nursery school children, and communities. Conclusion: Because mycoplasma infection is common pediatric infectious diseases and because some patients rarely develop severe infections, we must prevent larger outbreaks. (N)SASSy can provide timely intervention at the initial phase of outbreak by monitoring situations in (nursery) schools and comparing data to baseline information.

Outbreak of Human Metapneumovirus in Ibaraki, Japan and Its Descriptive Epidemiology

Introduction: The Infection Control Law in Japan does not cover monitoring of human metapneumovirus (HMPV). Therefore, its epidemiology is not well known. However, rapid diagnostic testing for this virus has been reimbursed by public health insurance in Japan since 2014. One case of acute encephalitis caused by HMPV was reported in April 2017 in Ibaraki. All schools in Ibaraki prefecture started to participate in the (Nursery) School Absenteeism Surveillance System ((N)SASSy) in 2009. All nursery schools started to use it in 2012. Methods: We specifically examine its seasonality and incidence by age. The study period extends from October 1, 2014 through May 31, 2017. Results: In total, (N)SASSy received 187 cases during the period. Spring and early summer were apparently the high season. The highest incidence was 36 cases in April 2015. The incidence among zero and one year olds was higher in 2017 than in the same season among 2014-2016. The necessary effort to download data from (N)SASSy, show these figures, and confirm the situation and background was a few minutes. Discussion: Rapid diagnostic testing for HMPV elucidated its epidemiology for the first time. Moreover, timely information sharing through (N)SASSy led the Ibaraki prefectural office to publish a reminder in July 2017 about the HMPV outbreak situation and to encourage precautions against it by nursery schools.
