Presence of columnar-lined esophagus is negatively associated with the presence of esophageal varices in Japanese alcoholic men

AIM To determine whether the presence of columnar-lined esophagus(CLE) is associated with the presence of esophageal varices(EVs) in male Japanese alcoholics. METHODS The subjects were 1614 Japanese alcohol-dependent men(≥ 40 years of age) who had undergone upper gastrointestinal endoscopic screening. Digitalized records of high-quality endoscopic images that included the squamocolumnar junction and esophagogastric junction were retrospectively jointly reviewed by four expert endoscopists for the purpose of diagnosing CLE. The authors investigated whether and to what extent there were associations between the presence of CLE and the presence of EVs, especially in the group with liver cirrhosis(LC).RESULTS CLE ≥ 5 mm in length was found in 355 subjects(≥ 30 mm in 6 of them), LC without EVs in 152 subjects, LC with EVs in 174 subjects, and EVs without LC in 6 subjects. Advanced EVs, i.e., nodular, large or coiled forms, red color sign, or post-treatment, were found in 88 subjects. The incidence of CLE ≥ 5 mm decreased in the following order(P < 0.0001): 23.3% in the group without EVs, 17.4% in the group with small and straight EVs, and 5.7% in the group with advanced EVs. The multivariate ORs(95%CI) for EVs and advanced EVs in the group with LC were lower when CLE ≥ 5 mm was present [0.46(0.23-0.93) and 0.24(0.08-0.74), respectively, vs 0-4 mm CLE]. CONCLUSION The presence of CLE in male Japanese alcoholics was negatively associated with the presence of EVs.

表示理由的接续助词「から」「ので」历来是日语学习者容易发生偏误的语法点。除了明显的偏误,像「明日用事がありますから、ちょっと難しいかもしれません」这种虽然达不到偏误但日本人不会使用的说法在学习者身上也经常可以见到。两者的使用区别,对日语学习者来说难度很高。有关「から」「ので」的前人研究,有从“主观·客观”的角度进行的研究。后来因“主观·客观”说不能解释许多反例,“礼貌理论”、“关联理论”、“与前项语气的共现”等观点便应运而生。根据与听话人的亲密度进行区别使用,是“礼貌理论”的观点。认为「から」的重点在后项,「ので」则强调前项,着眼于「からだ→から」「のだ→ので」的性质所得出的结论是“关联理论”的主张。然而,这些观点所进行的分析,因只是从一个单一角度进行,故即便比“主观·客观”说的反例稍少,但还是出现了许多不能解释的例外。有鉴于此,本文拟在上述前人研究的基础上,再次尝试对二者用法进行辨析,最终达到构建一套完整的,在教学中具有可操作性的区别使用的判断基准的目标。进而通过对学生的偏误例以及学习者语料库的分析,找出偏误的原因,确立适合学习者水平的教学法。研究认为,判断「から」「ので」的区别使用中有阶段性,为此而设定了三级判断基准即顺序1、2、3。判断基准顺序1是由说话人和听话人的关系判定,判断基准顺序2、3则是由后项的内容来决定的。如是“判断”、“行动”、“事实”中任何一个时,适用判断基准顺序2;如不能判断是其中哪个时,可适用判断基准顺序3。学习者的偏误有多种原因,其中与教材、日语教育现场的指导有着很大的关系。「から」「ので」虽然属于初级的语法点,但由于在中高级学习者中偏误率仍居高不下的状况,在中高级教学中仍需进行指导。在中级以上的会话、写作、阅读、论文写作等课程中,是否教「から」「ので」两者的使用区别,很多时候都是取决于老师个人,而没有统一的教学法。笔者认为,采用本研究的三级判断基准,通过教师们共享按学习者的水平,并且扎根于「から」「ので」本质的教学法,对防止学习者的偏误必将起到一定的帮助。

Gender differences in recognising depression in a case vignette in a university student population: Interaction of participant and vignette subject gender with depressive symptomatology

Objective: Gender difference in depression prevalence may be explained by variation in the recognition of depression due to differences in gender identity. Method: We distributed one of four questionnaires describing fictional cases of Major Depressive Episode (MDE) (2 predominant symptom types × 2 case vignette subject genders) to 72 students. Participants were asked whether and how much they thought the subject in the case was emotionally as well as somatically ill. They were also administered the Center for Epidemiologic Studies-Depression Scale and the Ito Sex Role Scale. Results: Participants scoring high in gender identity communion and ideal gender roles communion and delicacy were more likely to identify the vignette subject as ill. The severity of somatic illness was rated more highly for the case of predominantly somatic symptomatology. The recognition of severity of psychological illness was influenced by two interactive terms: the gender of the vignette subjects x gender of the participants and predominant symptoms x gender of the vignette subjects. Recognition of MDE case vignettes as indicating illness, particularly psychological illness, was more likely when the participant was female, scored highly in femininity, or was of the gender as the case subject. Conclusion: These findings may be concordant with the self-schema theory.

Concept Development and Implementation of Family Care/Caring Theory in Concentric Sphere Family Environment Theory

Caring is directed toward a variety of things. One of them is thought to be the concept of “family caring” aimed at families. This study attempts to clarify family caring and develop Family Care/ Caring Theory (FCCT), with the aim of implementing it in conjunction with an existing family nursing theory, the Concentric Sphere Family Environment Theory (CSFET). In Japan and in Hong Kong, family ethnography (including formal interviews) was conducted. As a result, the item “family health care nurses and their colleagues” was added to the family external environment of the CSFET. In the family environment, evidence was obtained to the effect that the family system unit is cared for by the nursing professional, and conversely the family system unit cares for the nursing professional, in a circular transaction. Observing the two-dimensional plane formed by the structural distance and functional distance, family caring assumes a structure of concentric circles, and according to transactions, the structural distance and functional distance between the nursing professional and family system unit are gradually approached, and through deepening of mutual trust maintain an appropriate distance. Moreover observing the three-dimensional space-time continuum which is created through addition of the temporal distance, family caring forms a helical structure. As transactions are repeated along the temporal axis, the family system unit’s self-actualization of other individuals and the self-actualization of the nursing professional are realized. Through these processes, a family care/caring relationship is reinforced and established. This is the concept of FCCT. Through future utilization in clinical settings this will be empirically substantiated, and it will be necessary to continue making creative corrections and revisions.

Glycosylation and secretion of human α-amylases

Three human α-amylases exist: Amy1 (salivary amylase), Amy2A (pancreatic amylase), and Amy2B (expressed in various tissues). These amylases share a 97% - 99% amino acid sequence identity, and two potential N-glycosylation sites (N427 and N476) are commonly found in the C-terminal region. In general, salivary amylase is more frequently glycosylated than pancreatic amylase, and it is still uncertain why differences in the glycosylation pattern among human amylase iso-zymes occur. In this study, we found that there was no significant change of ratio of glycosylated molecules among isozymes produced by the same cultured cells, indicating that glycosylation of amylase is influenced by the type of cell producing the enzyme rather than being an inherent property of the amylase isozymes. We analyzed the glycosylation efficiency of N-glycosylation sites in recombinant Amy2A mutants produced by HEK293 cells and found that glycosylation efficiencies of N427 and N476 were 3% - 18% and 40% - 52%, respectively, indicating that the major N-glycosylation site of glycosylated Amy2A produced by HEK293 cells is N476. The difference in the glycosylation efficiency of each N-glycosylation site also seemed to contribute in part to generate different glycosylation patterns of human amylases. We also confirmed that the C-terminal region of human amylase plays a critical role in secretion, although glycosylation does not play a part in this effect.

Case of clear-cell hepatocellular carcinoma that developed in the normal liver of a middle-aged woman

A 36-year-old woman was admitted to our department for close examination of a liver tumor that was found during a medical checkup. Abdominal US, CT and MRI showed a tumor in segment 7 (S7) of the liver. Although imaging suggested hepatocellular carcinoma, laboratory tests showed no abnormality in liver function, hepatitis virus markers were negative, and tumor markers including protein induced by vitamin K absence or antagonist Ⅱ (PIVKA-Ⅱ), α-fetoprotein (AFP), carbohydrate antigen 19-9 (CA19-9), and carcinoembryonic antigen (CEA) were all within normal ranges. Upon aspiration biopsy of the liver, the histopathological diagnosis was moderately differentiated hepatocellular carcinoma. Therefore, right hepatectomy was performed. Although a part of the tumor was necrotic, about 60% of the viable part showed a clear-cell variant. Consequently, it was diagnosed as clear-cell hepatocellular carcinoma. It was noted that the background liver tissue was normal. This case is worthy of reporting because development of clear-cell hepatocellular carcinoma in the normal liver of a middle-aged woman is rarely seen.

A case of primary biliary cirrhosis complicated by Behcet's disease and palmoplantar pustulosis

A 46-year-old woman was diagnosed with palmoplantar pustulosis （PPP） at the Department of Dermatology, Fukushima Medical University Hospital in 2000, and was treated with ointment. However, because liver dysfunction developed in 2003, she was referred to our department, where primary biliary cirrhosis （PBC） was also diagnosed on the basis of clinical findings. One year later, at the age of 49, she developed manifestations of Behcet＇s disease （BD）, including erythema nodosum in the lower extremities. Because she had a history of uveitis, recurrent oral ulceration was present, and the HLA typing was positive for B51, BD was additionally diagnosed. Liver function normalized within three months of the start of treatment with ursodesoxycholic acid （UDCA）. This is the first case of PBC associated with BD and PPP.
