Introduction: During the application of non-invasive positive pressure ventilation (NPPV) therapy in home mechanical ventilation (HMV), leaks in the NPPV mask may occur owing to the position of the mask due to conditions such as skin disorders. Methods: To investigate whether such a leak affects FiO2 supplied to the alveoli, we simulated a patient with chronic obstructive pulmonary disorder during NPPV in HMV. In addition, FiO2 was measured in the portion assumed to be the mouth and lungs while setting the flow of the oxygen concentrator and leak amount based on a previous study. Results: FiO2 supplied to the lungs increased statistically significantly upon increasing the amount of leak (P Conclusions: We observed that FiO2 supplied to alveoli can be reduced by a leak in the NPPV mask. Because our results differ from those previously reported, we believe that further studies should reassess the selection of respirators and oxygen concentrators.

Intratracheal aspiration in a patient on ventilation is generally performed using a catheter. Of late, closed suction systems have been used more frequently than open systems. To remove tracheal secretions through the intratracheal tube, catheter suction is used, and the suction catheter may be of the closed- or open-type. The catheters are cleaned by flushing with sterile 0.9% saline, resulting in dropping of dew. This phenomenon is caused by some factors: influence of flow, clogging of the suction tube, problem of manipulation of flushing and angle between the intubation tube and the connection port. But this dropping has not yet been investigated. In this study, we focused at this angle and we used the test lung which is simulated machine on behalf of patient. We consider two situations adult and child, also two types of catheters Eco-Cath and Ty-care. In child case we consider elbow-type suction catheters and Y-connector type. The angle was set at 0°, 20°, 40°, 60°, and 90°, and we observed the difference between Eco-Cath and Ty-care. In adults difference is significant (p 0.001), In 90°between Tracheal intubation and Tracheotomy (p ≈ 0.26). In child difference is significant (p 0.001), In 90°between Tracheal intubation and Tracheotomy (p ≈ 0.15). In child case we observed dependency of angle on various conditions too. The relation of the angle and dew was clarified.

Corporations need to improve business processes in order to enhance velocity and service levels while reducing their processing costs and differentiating themselves in the face of competition. The levitation of importance beyond support roles has raised IT investment decisions to high priority in chief executive officers＇ agendas. Corporate planning groups as well as lines of business are increasingly applying techniques of IT applications portfolio management in a more systematic fashion to improve decision-making and resource-allocation processes. Recent advances in software engineering and IT service delivery methodologies have achieved the logical separation of business functions from implementation. This separation has made a new breed of innovative IT project possible with a new project risk structure; the adjustment of portfolio management techniques is appropriate. We present an integrated portfolio management model so that the corporation can focus on organic growth through sources at both the department and top management levels. The research gives clear advice as to how top management can seek economic growth by selecting an entrepreneurial strategic posture, implying a strong risk-taking propensity. By integrating a risk-return model and risk-tolerance paradigm to cope with today＇s risk structure, overall capabilities can improve the decision process and the corporation＇s performance as well. The application of the integrated technique to a Japanese manufacturing firm is described.

Earthquake simulation test of circular reinforced concrete bridge column under multidirectional seismic excitation

Structures behave multi-directionally when subjected to earthquake excitation. Thus, it is essential to evaluate the effect of multidirectional loading on the dynamic response and seismic performance of reinforced concrete bridge columns in order to develop more advanced and reliable design procedures. To investigate such effects, a 1/4 scaled circular reinforced concrete bridge column specimen was tested under two horizontal and one vertical components of a strong motion that has long duration with several strong pulses. Damage progress of reinforced concrete columns subjected to strong excitation was evaluated from the test. The test results demonstrate that the lateral force response in the principal directions become smaller than computed flexural capacity due to the bilateral flexural loading effects, and that the lateral response is not significantly affected by the fluctuation of the axial force because the horizontal response and axial force barely reached the maximum simultaneously due to difference of the predominant natural periods between the vertical and the horizontal directions. Accuracy of fiber analyses is discussed using the test results.

Bcl-2 is an anti-apoptotic oncoprotein and its protein levels are inversely correlated with prognosis in many ***, the role of Bcl-2 in the progression of prostate cancer is not clear. Here we report that Bcl-2 is required for theprogression of LNCaP prostate cancer cells from an androgen-dependent to an androgen-independent growth stage. ThemRNA and protein levels of Bcl-2 are significantly increased in androgen-independent prostate cancer cells. shRNA-medi-ated gene silencing of Bcl-2 in androgen-independent prostate cancer cells promotes UV-induced apoptosis and suppressesthe growth of prostate tumors in vivo. Growing androgen-dependent cells under androgen-deprivation conditions resultsin formation of androgen-independent colonies; and the transition from androgen-dependent to androgen-independentgrowth is blocked by ectopic expression of the Bcl-2 antagonist Bax or Bcl-2 shRNA. Thus, our results demonstratethat Bcl-2 is not only critical for the survival of androgen-independent prostate cancer cells, but is also required for theprogression of prostate cancer cells from an androgen-dependent to an androgen-independent growth stage.

We present a theoretical model for acoustic nonlinearity measurement of dispersive specimens at high frequency. The nonlinear Khokhlov-Zabolotskaya-Kuznetsov （KZK） equation governs the nonlinear propagation in the SiO2/specimen/SiO2 multi-layer medium. The dispersion effect is considered in a special manner by introducing the frequency-dependant sound velocity in the KZK equation. Simple analytic solutions are derived by applying the superposition technique of Gaussian beams. The solutions are used to correct the diffraction and dispersion effects in the measurement of acoustic nonlinearity of cottonseed oil in the frequency range of 33-96 MHz. Re- garding two different ultrasonic devices, the accuracies of the measurements are improved to ±2.0% and ±1.3% in comparison with ±9.8% and ±2.9% obtained from the previous plane wave model.

Oxidative stress is one of the major causes of male infertility; it damages spermatogenic cells, the spermatogenic process and sperm function. Recent advances in redox biology have revealed the signalling role of reactive oxygen species （ROS） that are generated by cells. While highly reactive oxidants, such as the hydroxyl radical, exert largely deleterious effects, hydrogen peroxide can feasibly serve as a signal mediator because it is moderately reactive and membrane permeable and because it can oxidize only limited numbers of functional groups of biological molecules. The amino acid side chain most sensitive to oxidation is cysteine sulphydryl, which is commonly involved in the catalysis of some enzymes. Although the reactivity of cysteine sulphhydryl is not very high in ordinary proteins, some phosphatases possess a highly reactive sulphydryl group at their catalytic centre and are thereby oxidatively inactivated by transiently elevated hydrogen peroxide levels after extracellular stimuli and under certain environmental conditions. Peroxiredoxins, in turn, show moderate hydrogen peroxide-reducing activity, and their role in the modulation of ROS-mediated signal transduction in ordinary cells, mediated by protecting phosphatases from oxidative inactivation, has attracted much attention. Although knowledge of the signalling role of ROS in the male reproductive system is limited at present, its significance is becoming a focal issue. Here, we present a review of the emerging signalling role of hydrogen peroxide in testes.

Recently, a number of environmental problems caused from fossil fuel combustion have been focused on. In addition, with the eventual depletion of fossil energy resources, hydrogen gas is expected to be an alternative energy resource in the near future. It is characterized by high energy per unit weight, high reaction rate, wide range of flammability and the low emission property. On the other hand, many researches have been underway in several countries to improve a propulsion system for an advanced aircraft. The system is required to have higher power, lighter weight and lower emissions than existing ones. In such a future propulsion system, hydrogen gas would be one of the promising fuels for realizing the requirements. Considering these backgrounds, our group has proposed a new cycle concept for hydrogen-fueled aircraft propulsion system. In the present study, we perform 3 dimensional computations of turbulent flow fields with hydrogen-fueled combustion around a turbine blade. The main objective is to clarify the influence of arrangement of hydrogen injector holes. Changing the chordwise and spanwise spacings of the holes, the 3 dimensional nature of the flow and thermal fields is numerically studied.
