Gender differences in clinical outcomes of acute myocardial infarction undergoing percutaneous coronary intervention: insights from the KAMIR-NIH Registry

Background There are numerous but conflicting data regarding gender differences in outcomes following percutaneous coronary intervention(PCI). Furthermore, gender differences in clinical outcomes with acute myocardial infarction(AMI) following PCI in Asian population remain uncertain because of the under-representation of Asian in previous trials. Methods A total of 13,104 AMI patients from Korea Acute Myocardial Infarction Registry-National Institute of Health(KAMIR-NIH) between November 2011 and December 2015 were classified into male(n = 8021, 75.9%) and female(n = 2547, 24.1%). We compared the demographic, clinical and angiographic characteristics, 30-days and 1-year major adverse cardiac and cerebrovascular events(MACCE) in women with those in men after AMI by using propensity score(PS) matching. Results Compared with men, women were older, had more comorbidities and more often presented with non-ST segment elevation myocardial infarction(NSTEMI) and reduced left ventricular systolic function. Over the median follow-up of 363 days, gender differences in both 30-days and 1-year MACCE as well as thrombolysis in myocardial infarction minor bleeding risk were not observed in the PS matched population(30-days MACCE: 5.3% vs. 4.7%, log-rank P = 0.494, HR = 1.126, 95% CI: 0.800-1.585;1-year MACCE: 9.3% vs. 9.0%, log-rank P = 0.803, HR = 1.032, 95% CI: 0.802-1.328;TIMI minor bleeding: 4.9% vs. 3.9%, log-rank P = 0.215, HR = 1.255, 95% CI: 0.869-1.814). Conclusions Among Korean AMI population undergoing contemporary PCI, women, as compared with men, had different clinical and angiographic characteristics but showed similar 30-days and 1-year clinical outcomes. The risk of bleeding after PCI was comparable between men and women during one-year follow up.

To examine the effects of Populus tomentiglandulosa(PT) extract on the expressions of antioxidant enzymes and neurotrophic factors in the cornu ammonis 1(CA1) region of the hippocampus at 5 min after inducing transient global cerebral ischemia(TGCI) in gerbils, TGCI was induced by occlusion of common carotid arteries for 5 min. Before ischemic surgery, 200 mg·kg–1 PT extract was orally administrated once daily for 7 d. We performed neuronal nuclear antigen immunohistochemistry and Fluoro-Jade B staining. Furthermore, we determined in situ production of superoxide anion radical, expression levels of SOD1 and SOD2 as antioxidant enzymes and brain-derived neurotrophic factor(BDNF) and insulin-like growth factor I(IGF-I) as neurotrophic factors. Pretreatment with 200 mg·kg–1 PT extract prevented neuronal death(loss). Furthermore, pretreatment with 200 mg·kg–1 PT extract significantly inhibited the production of superoxide anion radical, increased expressions of SODs and maintained expressions of BDNF and IGF-I. Such increased expressions of SODs were maintained in the neurons after IRI. In summary, pretreated PT extract can significantly increase levels of SODs and protect the neurons against TGCI, suggesting that PT can be a useful natural agent to protect against TGCI.

Presumptive case of sparganosis manifesting as a hepatic mass: A case report and literature review

A 60-year-old man was admitted due to rectosigmoid colon cancer, and a hepatic mass was incidentally found during the staging work-up. The mass appeared cystic with a thick wall and contained multiple bizarre cord-like structures on ultrasound, computed tomography and magnetic resonance imaging. The differential diagnoses included organizing abscess/hematoma, foreign body granuloma and parasite infestation. Serologic study revealed anti-sparganum antibodies. Over 4-year followup, the patient did not complain of symptoms, and no changes in the characteristics of the liver mass were observed. Hepatic sparganosis is rare; only two cases have been clinically reported, and no detailed radiologic description was available until now. This case report presents a detailed radiologic description of a hepatic mass that could most likely represent hepatic sparganosis.

本论文旨在研究,新兴经济及经济转变时期,政治环境变化是否严重影响企业价值变化。既有研究已经表明,政治关联在政治经济大环境下对企业净值有积极或消极效应,或者无效应。本人以1998年至2013年韩国庆尚道与全罗道的上市公司为例进行面板数据分析,发现当时与执政党政治网络关系联系紧密的企业比无此密切关系的企业获得更多正面效应,而当时与反对党政治网络关系联系紧密的企业比无此密切关系的企业获得更多负面效应。更重要的是,不可预料的政权更迭致使企业价值逐渐由正效应转化为负效应。当韩国走在民主政治、自由经济的路上,伴随着政权更迭,这种政治关联是决定韩国企业价值的重要因素。各种稳健性检验已经证明了以上结果。

Hip External Rotator Strength and Compensatory Movement in Three Different Positions

Measuring intrinsic hip external rotator strength (ER) without compensatory pelvic motion and activation of the sartorius is important for preventing or rehabilitating lower extremity injuries. However, the optimal method for measuring intrinsic hip ER muscle strength while minimizing compensatory pelvic motions and activation of the sartorius is unclear. The purpose of this study is to compare measurements of hip ER strength, compensatory pelvic motion, and sartorius activation in the sitting, prone, and sidelying positions. Thirty-one healthy subjects (16 males and 15 females) were recruited for this study. Hip ER strength, pelvic kinematics, and sartorius muscle activation were measured during maximal isometric contraction of the hip ER in the sitting, prone, and sidelying positions. Hip ER strength was measured using a load-cell-type strength-measurement sensor. Pelvic kinematics was measured using an electromagnetic motion-tracking sensor. Electromyography was used to measure sartorius muscle activity. Data were analyzed using one-way repeated-measures analysis of variance. The result showed that hip ER strength and sartorius muscle activation were significantly lower in the sidelying compared with the sitting and prone positions (p < 0.01). Pelvic anteroposterior tilting was significantly greater in the sitting compared with the prone and sidelying positions (p < 0.01). Pelvic rotation differed significantly among positions (p < 0.01). Pelvic lateral tilting was significantly greater in the prone compared with the sitting position (p < 0.017). Compensatory pelvic motion and sartorius muscle activation were lower when hip ER strength measurements were made in the sidelying position. Therefore, the sidelying position is effective for measuring selective intrinsic hip ER strength.

Hypothyroidism affects astrocyte and microglial morphology in type 2 diabetes

In the present study, we investigated the effects of hypothyroidism on the morphology of astrocytes and microglia in the hippocampus of Zucker diabetic fatty rats and Zucker lean control rats. To induce hypothyroidism, Zucker lean control and Zucker diabetic fatty rats at 7 weeks of age orally received the vehicle or methimazole, an anti-thyroid drug, treatment for 5 weeks and were sacrificed at 12 weeks of age in all groups for blood chemistry and immunohistochemical staining. In the me- thimazole-treated Zucker lean control and Zucker diabetic fatty rats, the serum circulating triiodo- thyronine （T3） and thyroxine （＇I＂4） levels were significantly decreased compared to levels observed in the vehicle-treated Zucker lean control or Zucker diabetic fatty rats. This reduction was more prominent in the methimazole-treated Zucker diabetic fatty group. Glial fibrillary acidic protein im- munoreactive astrocytes and ionized calcium-binding adapter molecule 1 （Iba-1）-immunoreactive microglia in the Zucker lean control and Zucker diabetic fatty group were diffusely detected in the hippocampal CA1 region and dentate gyrus. There were no significant differences in the glial fibril- lary acidic protein and Iba-1 immunoreactivity in the CA1 region and dentate gyrus between Zucker lean control and Zucker diabetic fatty groups. However, in the methimazole-treated Zucker lean control and Zucker diabetic fatty groups, the processes of glial fibrillary acidic protein immunoreac- tive astrocytes and Iba-1 immunoreactive microglia, were significantly decreased in both the CA1 region and dentate gyrus compared to that in the vehicle-treated Zucker lean control and Zucker diabetic fatty groups. These results suggest that diabetes has no effect on the morphology of as- trocytes and microglia and that hypothyroidism during the onset of diabetes prominently reduces the processes of astrocytes and microglia.
