AIM:To examine the efficacy of telbivudine(LdT)+adefovir(ADV)vs continuation of lamivudine(LAM)+ADV in patients with LAM-resistant chronic hepatitis B(CHB)who show a suboptimal response to LAM+***:This was a randomized,active-control,open-label,single-center,parallel *** eligible patients were enrolled in this study in Severance Hospital,Yonsei University College of Medicine,Seoul,South Korea,between March 2010 and March *** Be antigen(HBeAg)-positive CHB patients whose serum hepatitis B virus(HBV)DNA remained detectable despite at least 6 mo of LAM+ADV therapy were *** patients were randomized to either switching to LdT(600 mg/d orally)plus ADV(10 mg/d orally)(LdT+ADV group)or to continuation with LAM(100 mg/d orally)plus ADV(10 mg/d orally)(LAM+ADV group),and were followed for 48 *** hundred and six patients completed the 48-wk treatment *** HBV DNA,HBeAg status,liver biochemistry and safety were monitored at baseline and week 12,24,36 and ***:The duration of prior LAM+ADV treatment was 18.3(LdT+ADV)and 14.9 mo(LAM+ADV),respectively(P=0.131).No difference was seen in baseline serum HBV DNA between the two groups[3.66(LdT+ADV)vs 3.76(LAM+ADV)log10IU/mL,P=0.729].At week 48,although there was no significant difference in the mean reduction of serum HBV DNA from baseline between LdT+ADV group and LAM+ADV group(-0.81 vs-0.47 log10IU/mL,P=0.167),more patients in the LdT+ADV group had undetectable HBV DNA levels compared to those in the LAM+ADV group(30.2%vs 11.5%,P=0.019).Three patients with LdT+ADV treatment and 2 patients with LAM+ADV treatment achieved HBeAg *** patients in both groups tolerated the treatment well without serious adverse *** proportion of patients with estimated glomerular filtration rate≥90 mL/min per 1.73 m2in the LdT+ADV group increased from 49.1%(26/53)at baseline to 58.5%(31/53)at week 48,while that in the LAM+ADV group decreased from 37.7%(20/53)at baseline to 30.2%(16/53)at

To the Editor： A lumbar epidural block is used to treat patients with a herniated intervertebral disc, spinal stenosis, or lumbosacral radiculopathy. This approach reduces radicular pain by decreasing inflammation of nerve roots through epidural corticosteroid injection. Patients with moderate-to-severe spinal stenosis who taking various anticoagulant agents have more risk for epidural hematoma.

Prevalence of extracardiac findings in the evaluation of ischemic heart disease by multidetector computed tomography

Objective Multidector computed tomography (MDCT) is now commonly used for the evaluation of coronary artery disease. Because MDCT images include many non-cardiac organs and the patient population evaluated is highly susceptible to extracardiac diseases, this study was designed to evaluate the prevalence of extracardiac findings in the MDCT evaluation of ischemic heart disease. Methods From March 2007 to March 2008, a total of six-hundred twenty patients, who underwent 64-slice MDCT evaluations for chest pain, or dyspnea, were enrolled in this study. Cardiac and non-cardiac findings were comprehensively evaluated by a radiologist. Results Enrolled patients included 306 men (49.4%), with a mean age of 66 years. Significant coronary artery stenosis was found in 41.6%of the patients. A total of 158 extracardiac findings were observed in 110 (17.7%) patients. Commonly involved extracardiac organs were lung (36.7%), hepatobiliary system (21.5%), thyroid (19.6%), kidney (10.8%), spine (9.7%) and breast (0.6%). Of those 110 patients, 50 (45.5%) patients underwent further diagnostic investigations. Malignant disease was detected in three (2.7%) patients (lung cancer, pancreatic cancer, and thyroid cancer). Conclusions Extracardiac findings are frequently present and should be a concern in the MDCT evaluation of chest pain syndrome.

Objective： Cheongseoikki-tang （CIT, Korean）, also called Qingshu Yiqi decoction（清署益气汤）and Seisho-ekki-to （Japanese）, is well known as an effective traditional combination of herbs for treating cardiovascular diseases. This study was to research its effects on bone marrow-derived mast cell （BMMC）-mediated allergy and inflammation mechanisms. Methods： In this study, the biological effect of Cheongseoikki-tang ethanol extract （CITE） was evaluated, focusing on its effects on the production of allergic mediators by phorbol 12-myfistate 13-acetate （PMA） plus calcium ionophore A23187 （A23187）-stimulated BMMCs. These allergic mediators included intedeukin-6 （IL-6）, prostaglandin D2 （PGD2）, leukotfiene （34 （LTC4）, and β-hexosaminidase （13-hex）. Results： Our data revealed that CITE inhibited the production of IL-6, PGD2,/TC4, and 15-hex induced by PMA plus A23187 （P〈0.05）. Conclusion： These findings indicate that CITE has the potential for use in the treatment of allergy.

The histopathological diagnosis of gastric mucosal biopsy and endoscopic mucosal resection/endoscopic submucosal dissection specimens is important,but the diagnostic criteria,terminology,and grading system are not the same in the East and West.A structurally invasive focus is necessary to diagnose carcinoma for most Western pathologists,but Japanese pathologists make a diagnosis of cancer based on severe dysplastic cytologic atypia irrespective of the presence of *** the Vienna classification was introduced to reduce diagnostic discrepancies,it has been difficult to adopt due to different concepts for gastric epithelial neoplastic *** pathologists experience much difficulty making a diagnosis because we are influenced by Japanese pathologists as well as Western *** is geographically close to Korea,and academic exchanges are ***,Korean doctors are familiar with Western style medical *** a result,the terminology,definitions,and diagnostic criteria for gastric intraepithelial neoplasia are very heterogeneous in *** solve this problem,the Gastrointestinal Pathology Study Group of the Korean Society of Pathologists has made an effort and has suggested guidelines for differential diagnosis:(1) a diagnosis of carcinoma is based on invasion;(2) the most important characteristic of low grade dysplasia is the architectural pattern such as regular distribution of crypts without severe branching,budding,or marked glandular crowding;(3) if nuclear pseudostratification occupies more than the basal half of the cryptal cells in three or more adjacent crypts,the lesion is considered high grade dysplasia;(4) if severe cytologic atypia is present,careful inspection for invasive foci is necessary,because the risk for invasion is very high;and(5) other structural or nuclear atypia should be evaluated to make a final decision such as cribriform pattern,papillae,ridges,vesicular nuclei,high nuclear/cytoplasmic ratio,loss

Low-intensity light detection necessitates high-responsivity *** achieve this,we report In_(0.53)Ga_(0.47)As∕In As∕In_(0.53)Ga_(0.47)As quantum well(InAs QW)photo-field-effect-transistors(photo-FETs)integrated on a Si substrate using direct wafer *** of the In As QW channel was carefully designed to achieve higher effective mobility and a narrower bandgap compared with a bulk In_(0.53)Ga_(0.47)As,while suppressing the generation of defects due to lattice ***-performance 2.6 nm In As QW photo-FETs were successfully demonstrated with a high on/off ratio of 10~5 and a high effective mobility of 2370 cm^(2)∕(V·s).The outstanding transport characteristics in the InAs QW channel result in an optical responsivity 1.8 times greater than InGaAs photo-FETs and the fast rising/falling ***,we experimentally confirmed that the InAs QW photo-FET can detect light in the short-wavelength infrared(SWIR;1.0–2.5μm)near 2μm thanks to bandgap engineering through In As QW *** result suggests that the InAs QW photo-FET is promising for high-responsivity and extended-range SWIR photodetector applications.

Electronic nose(e-nose)technology for selectively identifying a target gas through chemoresistive sensors has gained much attention for various applications,such as smart factory and personal health *** overcome the crossreactivity problem of chemoresistive sensors to various gas species,herein,we propose a novel sensing strategy based on a single micro-LED(μLED)-embedded photoactivated(μLP)gas sensor,utilizing the time-variant illumination for identifying the species and concentrations of various target gases.A fast-changing pseudorandom voltage input is applied to the μLED to generate forced transient sensor responses.A deep neural network is employed to analyze the obtained complex transient signals for gas detection and concentration *** proposed sensor system achieves high classification(~96.99%)and quantification(mean absolute percentage error~31.99%)accuracies for various toxic gases(methanol,ethanol,acetone,and nitrogen dioxide)with a single gas sensor consuming 0.53 *** proposed method may significantly improve the efficiency of e-nose technology in terms of cost,space,and power consumption.

Influence of tool plunge depth and welding distance on friction stir lap welding of AA5454-O aluminum alloy plates with different thicknesses

AA5454-O aluminum alloy plates with thicknesses of 1.4 and 1.0 mm were friction-stir-lap-welded (FSLWed).The influences of the tool plunge depth and welding distance on surface appearance,macrostructure and mechanical properties of the FSLWed plates were experimentally *** tensile shear load of the FSLWed plates was compared with that of the adhesive-bonded ***-free FSLWed zones were successfully obtained in all the tool plunge depths and the welding *** FSLWed zones exhibited the relatively smooth surface *** all the FSLWed conditions,the FSLWed zone exhibited higher average hardness than the base *** addition,the upper plate exhibited a higher average hardness than the lower plate,although there was no special tendency in spite of the change in the tool plunge depth and the welding *** maximum tensile shear load of the FSLWed plates was much higher than that of the adhesive-bonded aluminum alloy ***,under the FSLW condition of the plunge depth of 1.8 mm and the welding distance of 40 mm,the tensile shear load of the FSLWed plate reached a level about 41% greater than that of the adhesive-bonded aluminum alloy *** addition,the maximum tensile shear load of the FSLWed plate was increased with the increase of the welding distance.
