Endovascular treatment of a ruptured pseudoaneurysm of the internal carotid artery in a patient with nasopharyngeal cancer:A case report

BACKGROUND Radiation therapy(RT)for nasopharyngeal cancer can cause several complications.In rare cases,an internal carotid artery pseudoaneurysm can occur,which can be fatal.We report the experience of a nasopharyngeal cancer patient who underwent radiation therapy and subsequently developed a fatal pseudoaneurysm of the petrous internal carotid artery.CASE SUMMARY A 39-year-old man was diagnosed with nasopharyngeal cancer 2 years ago(American Joint Committee on Cancer Stage T3N2M0)and received concurrent chemoradiation therapy.He subsequently relapsed and received chemotherapy.One week after the 4th cycle of chemotherapy,he was admitted to the emergency room of our hospital because of massive epistaxis accompanied by a headache.A pseudoaneurysm of the petrous internal carotid artery was confirmed by digital subtraction angiography(DSA).Stent-assisted endovascular coil embolization was performed and complete occlusion was achieved.No pseudoaneurysm was observed on DSA after coil embolization;however,intermittent epistaxis was maintained even after coil embolization.After seven days,a diagnostic laryngoscopy was performed.Massive bleeding occurred after aspiration of the blood clot during the laryngoscopy and the patient died of hypovolemic shock.In this case,epistaxis may have been a sign of pseudoaneurysm;therefore,treatment such as embolization should be performed promptly,and careful management should be undertaken after treatment.CONCLUSION This case highlights a rare,serious complication of RT in nasopharyngeal cancer and how it should be recognized and treated.

Al2O3-TiO2/ZrO2-SiO2 based porous ceramics from particle-stabilized wet foam

The porous ceramics based on Al2O3–TiO2/ZrO2–SiO2 from particle-stabilized wet foam by direct foaming were discussed. The initial Al2O3–TiO2 suspension was prepared by adding TiO2 suspension to partially hydrophobized colloidal Al2O3 suspension with equimolar amount, to form Al2TiO5 on sintering. The secondary ZrO2–SiO2 suspension was prepared using the equimolar composition, and to obtain ZrSiO4, ZrTiO4, and mullite phases in the sintered samples, the secondary suspension was blended into the initial suspension at 0, 10, 20, 30, and 50 vol%. The wet foam exhibited an air content up to 87%, Laplace pressure from 1.38 to 2.23 mPa, and higher adsorption free energy at the interface of approximately 5.8×108 to 7.5×108 J resulting an outstanding foam stability of 87%. The final suspension was foamed, and the wet foam was sintered from 1400 to 1600 °C for 1 h. The porous ceramics with pore size from 150 to 400 μm on average were obtained. The phase identification was accomplished using X-ray diffraction (XRD), differential thermal analysis (DTA), and thermogravimetric analysis (TGA), and microstructural analysis was performed using field emission scanning electron microscopy (FESEM).

Solar-harvesting lead halide perovskite for artificial photosynthesis

Facing the upcoming energy and environmental crisis, artificial photosynthesis for producing various solar fuels (e.g., hydrogen or carbon products) via a solar-to-chemical energy conversion is receiving increasing attention;however, its low conversion efficiency is a challenge for commercialization. To resolve low-efficiency issues, lead halide perovskite (LHP) with outstanding optoelectronic properties compared to conventional semiconductors can be a promising approach to improve the solar-to-fuel conversion reactions and solar fuel production efficiency. The tunable energy band structure and charge transport properties of LHP have promoted their extensive use in the production of solar fuels. This study summarizes the recent advancements of LHP-mediated solar-to-fuel conversions, classified by their redox reactions, namely solar water splitting, hydrohalic acid splitting, and CO_(2) reduction. Advanced approaches for achieving high conversion efficiency and long-term durability are discussed, including the configuration of devices, the composition of LHP, and the protection strategy of LHP. Moreover, the reaction mechanisms of LHP-mediated solar-to-chemical energy conversions and obstacles for enhancing the conversion efficiency are discussed. Finally, we present the perspectives on the development of LHP-incorporated solar-to-fuel conversion systems, which might open a new era of energy harvesting and storage.

Sequential transcatheter arterial chemoembolization and portal vein embolization before right hemihepatectomy in patients with hepatocellular carcinoma

Background:Recent studies showed that sequential selective transcatheter arterial chemoembolization(TACE)and portal vein embolization(PVE)provided better future liver remnant(FLR)regeneration rate and disease-free survival following surgery compared with PVE alone.The present study aimed to clarify whether preoperative sequential TACE and PVE before right hemihepatectomy can reduce postoperative hepatocellular carcinoma(HCC)recurrence and improve long-term disease-free and overall survival.Methods:Recurrence and survival outcomes were retrospectively evaluated in 205 patients with HCC who underwent right hemihepatectomy by a single surgeon from November 1993 to November 2017.Patients were divided into four groups according to the procedure performed before the surgery:sequential TACE and PVE(TACE-PVE),PVE-only,TACE-only,or na?ve control groups.The baseline patient and tumor characteristics,postoperative outcomes,recurrence-free survival and overall survival were analyzed.Results:Baseline patient and tumor characteristics upon diagnosis were similar in all four groups,while sequential TACE and PVE were well tolerated.The TACE-PVE group had a higher mean increase in percentage FLR volume compared with that of the PVE-only group(17.46%±6.63%vs.12.14%±5.93%;P=0.001).The TACE-PVE group had significantly better overall and disease-free survival rates compared with the other groups(both P<0.001).Conclusions:Sequential TACE and PVE prior to surgery can be an effective therapeutic strategy for patients with HCC scheduled for major hepatic resection.The active application of preoperative sequential TACE and PVE for HCC would allow more patients with marginal FLR volume to become candidates for major hepatic resection by promoting compensatory FLR hypertrophy without the deterioration of basal hepatic functional reserve or tumor progression.

Improving the capacitance and energy density is a significant challenge while developing practical and flexible energy storage system(ESS).Redox mediators(RMs),as redox-active electrolyte additives,can provide additional energy storing capability via electrochemical faradaic contribution on electrodes for high-performance flexible ESSs.Particularly,determining effective material combinations between electrodes and RMs is essential for maximizing surface faradaic redox reactions for energy-storage performance.In this study,an electrode-RM system comprising heterostructured hybrid(carbon fiber(CF)/MnO_(2)) faradaic electrodes and iodine RMs(I-RMs) in a redox-active electrolyte is investigated.The CF/MnO_(2)with the 1-RMs(CF/MnO_(2)-I) induces dominant catalytic faradaic interaction with the I-RMs,significantly enhancing the surface faradaic kinetics and increasing the overall energy-storage performance.The CF/MnO_(2)-I ESSs show a 12.6-fold(or higher) increased volumetric energy density of 793.81 mWh L^(-1)at a current of 10 μA relative to ESSs using CF/MnO_(2)without I-RMs(CF/MnO_(2)).Moreover,the CF/MnO_(2)-I retains 93.1% of its initial capacitance after 10,000 cycles,validating the excellent cyclability.Finally,the flexibility of the ESSs is tested at different bending angles(180° to 0°),demonstrating its feasibility for flexible and high-wear environments.Therefore,CF/MnO_(2)electrodes present a practical material combination for high-performance flexible energy-storage devices owing to the catalytic faradaic interaction with I-RMs.

Prognostic significance of the neutrophil-to-lymphocyte ratio and platelet-to-lymphocyte ratio in patients with stage Ⅲ and Ⅳcolorectal cancer

AIM To evaluate the prognostic value of the neutrophil-tolymphocyte ratio(NLR) and platelet-to-lymphocyte ratio(PLR) in patients with colorectal cancer(CRC).METHODS Between April 1996 and December 2010, medical records from a total of 1868 patients with CRC were retrospectively reviewed. The values of simple inflammatory markers including NLR and PLR in predicting the long-term outcomes of these patients were evaluated using Kaplan-Meier curves and Cox regression models.RESULTS The median follow-up duration was 46 mo(interquartile range, 22-73). The estimation of NLR and PLR was based on the time of diagnosis. In multivariate Cox regression analysis, high NLR (≥ 3.0) and high PLR(≥ 160) were independent risk factors predicting poor long-term outcomes in patients with stage Ⅲ and Ⅳ CRC. However, high NLR and high PLR were not prognostic factors in patients with stage Ⅰ and Ⅱ CRC.CONCLUSION In this study, we identified that high NLR (≥ 3.0) and high PLR (≥ 160) are useful prognostic factors to predict long-term outcomes in patients with stage Ⅲ and Ⅳ CRC.

Salvage living donor liver transplantation for recurrent hepatocellular carcinoma after prior laparoscopic hepatectomy

Salvage liver transplantation(LT)is frequently performed on patients who experience recurrent hepatocellular carcinoma(HCC)after primary hepatectomy for HCC[1,2].The main concern in these patients is the technical feasibility of salvage LT,especially as prior hepatectomy may result in heavy adhesions[3,4].Salvage living donor LT(LDLT)is a more demanding procedure than salvage deceased donor LT(DDLT)using an entire donor graft with a long vascular pedicle[5,6].Because less than optimal dissection of perihepatic adhesions could result in uncontrollable pinpoint bleedings at the dissection surface[1,5],many transplant surgeons avoid performing salvage LDLT.Minimally invasive laparoscopic hepatectomy(LH)results in fewer intraperitoneal adhesions than the open method,reducing the difficulty of surgical dissection during future LT[7–10].To date,however,no study has compared salvage LDLT for recurrent HCC after LH to that after open hepatectomy(OH).

Nanograin network memory with reconfigurable percolation paths for synaptic interactions

The development of memory devices with functions that simultaneously process and store data is required for efficient computation.To achieve this,artificial synaptic devices have been proposed because they can construct hybrid networks with biological neurons and perform neuromorphic computation.However,irreversible aging of these electrical devices causes unavoidable performance degradation.Although several photonic approaches to controlling currents have been suggested,suppression of current levels and switching of analog conductance in a simple photonic manner remain challenging.Here,we demonstrated a nanograin network memory using reconfigurable percolation paths in a single Si nanowire with solid core/porous shell and pure solid core segments.The electrical and photonic control of current percolation paths enabled the analog and reversible adjustment of the persistent current level,exhibiting memory behavior and current suppression in this single nanowire device.In addition,the synaptic behaviors of memory and erasure were demonstrated through potentiation and habituation processes.Photonic habituation was achieved using laser illumination on the porous nanowire shell,with a linear decrease in the postsynaptic current.Furthermore,synaptic elimination was emulated using two adjacent devices interconnected on a single nanowire.Therefore,electrical and photonic reconfiguration of the conductive paths in Si nanograin networks will pave the way for next-generation nanodevice technologies.

Choroidal and skin metastases from colorectal cancer

Choroidal and skin metastasis of colon cancer is rare. In women, the frequency of cutaneous metastasis from colon cancer as the primary lesion in is 9% and skin metastasis occurs in 0.81% of all colorectal cancers. We report a patient with colonic adenocarcinoma who presented with visual disorder in her right eye and scalp pain as her initial symptoms. Contrast-enhance orbital magnetic resonance imaging with fat suppression revealed an infrabulbar mass, and skin biopsy of the posterior parietal scalp confirmed adenocarcinoma. These symptoms were diagnosed as being caused by choroidal and skin metastases of colonic adenocarcinoma. We started palliative chemotherapy with oral capecitabine(1000 mg/m2, twice a day, on days 1-14) every 3 wk, which was effective at shrinking the brain masses and improving the visual disorder. This is the first report that capecitabine is effective at reducing a choroidal and cutaneous metastatic lesion from rightsided colorectal cancer.

Classification of Precipitation Types Using Fall Velocity–Diameter Relationships from 2D-Video Distrometer Measurements

Fall velocity–diameter relationships for four different snowflake types（dendrite,plate,needle,and graupel） were investigated in northeastern South Korea,and a new algorithm for classifying hydrometeors is proposed for distrometric measurements based on the new relationships.Falling ice crystals（approximately 40 000 particles） were measured with a two-dimensional video disdrometer（2DVD） during a winter experiment from 15 January to 9 April 2010.The fall velocity–diameter relationships were derived for the four types of snowflakes based on manual classification by experts using snow photos and 2DVD measurements：the coefficients（exponents） for different snowflake types were 0.82（0.24） for dendrite,0.74（0.35） for plate,1.03（0.71） for needle,and 1.30（0.94） for graupel,respectively.These new relationships established in the present study（PS） were compared with those from two previous studies.Hydrometeor types were classified with the derived fall velocity–diameter relationships,and the classification algorithm was evaluated using 3 × 3 contingency tables for one rain–snow transition event and three snowfall events.The algorithm showed good performance for the transition event：the critical success indices（CSIs） were 0.89,0.61 and 0.71 for snow,wet-snow and rain,respectively.For snow events,the algorithm performance for dendrite and plate（CSIs = 1.0 and 1.0,respectively） was better than for needle and graupel（CSIs = 0.67 and 0.50,respectively）.
